A central issue in the study of the patient with Alzheimer’s disease (AD) is the measurement of the severity of the associated dementia. Determination of the degree of impairment of cognitive function is essential not just for diagnosis, but for monitoring the progression of the disease, for confirmation of diagnosis, and assisting with caregiver planning, as well as for research, including the assessment of the benefit of pharmacologic agents. Numerous scales have been developed for measuring dementia severity (see Ashford et al., 1995). However, all such scales are composites of a diverse variety of queries, which give numeric values that are added together, some using complex algorithms, to give a score which represents the severity of the dementia. While some scales have a relationship with neuropathological features, all of these scales are ordinal in nature, with the numeric values having no direct relationship with any physical factors.
