Pyoderma gangrenosum is a rare but significant cause of ulcerations. It is a diagnosis of exclusion. Herein, we suggest diagnostic criteria and some historical perspectives on the diagnosis of pyoderma gangrenosum.

Background: The clinical mucocutaneous manifestations of glucagonoma syndrome are recognized easily when they occur in the classic pattern of acral or periorificial lesions evolving in recurrent crops, with an annular and migratory distribution, in a patient with diabetes mellitus who has had recent weight loss and anemia. Not infrequently, noncharacteristic clinical and histopathologic features are observed and, in these cases, the diagnosis of pancreatic neoplasm may be delayed., Aim: To review the clinical and histopathologic features of cutaneous manifestations of glucagonoma syndrome., Methods: The clinicopathologic features of 13 patients (eight women) with widespread or localized cutaneous eruption as a manifestation of islet cell pancreatic carcinoma with marked glucagon secretion (glucagonoma) were reviewed., Results: The definitive diagnosis of the cutaneous eruption was established at the time of diagnosis of the pancreatic neoplasm (three patients) or afterwards (10 patients). In nine patients, the mucocutaneous manifestations preceded the diagnosis of the pancreatic neoplasm by 1 month to 3 years (mean, 12 months). In only eight biopsy specimens were the histopathologic features considered to be suggestive or characteristic of necrolytic migratory erythema. Diffuse parakeratosis, that occasionally arose abruptly from normal epidermis, was observed in 12 biopsy specimens. By the time necrolytic migratory erythema was diagnosed, the pancreatic carcinoma had metastasized to the liver, regional lymph nodes, or bone in 12 patients., Conclusion: Increased awareness of the polymorphic mucocutaneous and nonspecific histopathologic features of glucagonoma syndrome is needed to avoid unnecessary delay in the diagnosis of this syndrome.

Background: Some workers in paraquat manufacturing, exposed to bipyridines, have developed pigmentation and keratosis on sun-exposed skin. This condition has been described as skin-malignancy or premalignancy. This study was designed to clarify the pathologic features of these lesions and to explore the etiologic role played by bipyridine., Methods: Twenty-three biopsy specimens, obtained from the affected skin of 10 workers, were scrutinized by a dermatopathologist. A total of 242 exposed workers from 28 paraquat factories were examined and interviewed during the period from 1983 to 1991. The severity of the characteristic skin lesions was graded from the lowest to the highest response to analyze the data by Mantel extension for a trend that focused on the heavy exposure to bipyridines as risk factor., Results: All pathology specimens showed various degrees of solar damage: early actinic change, solar lentigo, actinic keratosis (AK), AK coexisting with squamous cell carcinoma (scc), and scc. Six specimens from four workers were scc or scc in situ. Three of six scc showed the coexistence of AK. Of the workers, 133 had skin lesions ranging in severity from grade 1 to grade 3 on sun-exposed areas. The severity of skin changes is strongly associated with heavy exposure to bipyridines (P < 0.0001)., Conclusion: This pathologic study proves that all the lesions showed either photodamage or skin cancer. The strong trend in the correlation between severity of photodamage and exposure to bipyridine leads to the speculation about the synergistic role of bipyridine exposure and the solar effect in causing these malignant and premalignant skin lesions.

A 79-year-old man had generalized humping of the nails and subungual tumors involving several fingers and the left big toe. Findings on histologic study of the tumor, together with the pattern on the peripheral blood smear, established the diagnosis of chronic lymphocytic leukemia. To the best of our knowledge, this is the first reported case of chronic lymphocytic leukemia involving the subungual area. The authors' observation emphasizes the importance of taking a biopsy specimen in the differential diagnosis of subungual tumors.

Eight patients with skin tumor lesions composed of dense, predominantly plasma cell infiltrates were studied. Primary cutaneous plasmacytoma can be reactive (polyclonal) or neoplastic (monoclonal). In four of the patients skin lesions were associated with multiple myeloma. Specific skin lesions usually consisted of reddish or purple nodules located on the trunk. In one case the cutaneous lesions developed at the site of previous herpes zoster. Histologically, the cutaneous plasmacytic infiltrate was mainly diffuse and monomorphous. Most of the plasma cells were mature, but in some cases immature immunoblasts and mitoses were observed. Serum immunoelectrophoresis findings correlated with the monoclonality or polyclonality of the plasmacytoma. Presence or absence of systemic involvement cannot be predicted from the appearance of clinical lesions or from maturity of plasma cell infiltration in the skin.

Multiple edematous plaques and nodules suddenly developed on the face and neck of a 26-year-old man whose physical examination was said to be normal Because the patient's condition fulfilled the diagnostic criteria of Sweet's syndrome, a further general medical examination and hematologic evaluation were performed. Extensive myelofibrosis was revealed by bone marrow biopsy. This case supports the belief that Sweet's syndrome is a reactive phenomenon. Because myelofibrosis was discovered in this patient after Sweet's syndrome was diagnosed, the importance of having the diagnostic criteria of Sweet's syndrome is emphasized. Careful systemic evaluation is indicated, especially when cutaneous lesions are severe or hematologic values are abnormal.

An 11-year-old girl who at 9 1/2 months was diagnosed as having keratoderma of the palms and soles, present since birth, developed typical granuloma annulare at the age of 11 years. To the authors' knowledge, this is the first report of epidermolytic palmo-plantar keratoderma associated with granuloma annulare. There was no known family history of keratoderma. A review of the literature revealed three other patients without family history, although most of the patients have had multiple family members with the same problem. Many skin conditions can be associated with epidermolytic hyperkeratosis. The authors believe that epidermolytic palmo-planter keratoderma is a distinct form of palmo-plantar keratoderma rather than a type of ichthyosis.
