Fatty infiltration of the liver at postmortem examination has been recommended as a criterion for selection of infants who have died suddenly and unexpectedly for further biochemical investigation for disorders of fatty acid oxidation. We describe a boy with medium chain acyl CoA dehydrogenase deficiency who died four months after diagnosis and in whom only minimal hepatic fatty infiltration was found.

Twelve hundred and fifty-one pregnant women were contacted and their infants followed to age 5 years. Nine hundred and fifty-one (76%) children completed the trial. Measurements were made of birthweight, length, head circumference, and triceps skinfold at 10 days, and of weight, height, head circumference, and triceps at 5 years. Throughout the entire period of the study half the families, selected at random, were supplied with milk tokens entitling the mother, while pregnant, and all children under age 5 years, to 1 pint of milk a day at half its current price. The tokens led to a small increase in milk purchases by the families and to a small increase in milk drunk by the children. However no effect was detected in any growth measurement either in the total group, or in a more "vulnerable" group of children from the largest families.

A simple, rapid, and accurate method for measuring oxygen consumption in infants is described. Expired gas is entrained in a stream of air drawn through a gently applied face mask. Oxygen consumption is derived from (1) gas flow rate, measured with a flowmeter, and (2) the fall in oxygen concentration of the gas mixture, measured with a paramagnetic analyser. A measurement can be made at the bedside within 2 minutes without disturbing the infant. In 17 mature 2-day-old neonates mean oxygen consumption was 6.52 ml/kg per min, SD 0.90, similar to that previously reported using less simple methods,

A care by parent scheme was established in the children's department of a university hospital. It was seen as the natural extension of the increased involvement of parents in the care of their children in hospital. A structured observational study was carried out to monitor its effect on the lives of child patients. Children in the scheme spent far less time awake alone, cried less, and slept less than those nursed unaccompanied. They had far more social interaction with a smaller number of adults, most of their contacts being with family members rather than hospital staff. Children with a resident parent but outside the scheme were generally in an intermediate position on these factors.

Four patients with salt-losing congenital adrenal hyperplasia (CAH) who had stopped mineralocorticoid therapy for several years, showed raised plasma concentrations of 17OH-progesterone and plasma renin activity, despite adequate glucoticoid therapy. One patient was able to reduce urinary sodium excretion when the sodium intake was restricted. Another patient who was a salt-loser, developed signs of an adrenal crisis when salt deprived. In comparison, one nonsalt-loser and 2 normal subjects decreased urinary sodium excretion in response to sodium restriction. The addition of fludrocortisone (100 micrograms) to usual maintenance doses of glucocorticoid, resulted in normal levels of plasma 17OH-progesterone and plasma renin activity in all 4 salt-losers. Two female salt-losers, with raised plasma testosterone concentrations, began menstruating when their plasma testosterone concentrations returned to normal after treatment with fludrocortisone. It is recommended that salt-losing CAH patients should be given mineralocorticoid, in addition to glucocorticoid therapy, at least until adult life.

A care by parent option was introduced into a general paediatric ward without any additional finance or facilities. Most parents coped successfully and were grateful for the opportunity of caring for their children. All believed that their children benefited from their active involvement. The nurses believed that their role was enhanced and their job satisfaction increased. This system offers advantages and could become more generally used in paediatric wards in Britain.

The Prematometer is a new instrument for the accurate measurement of the length of small neonates inside incubators. It is capable of giving precise results.

Flectalon, web of aluminised polyvinylchloride fibres, has been formulated to minimise radiant heat losses and to provide conventional insulation. Critical temperature determinations were used to assess the insulating efficacy of this and other swaddling materials in infants. The critical temperature for a baby 2 to 10 days old was 31 degrees C when naked and 23 degrees C when wrapped in a Silver Swaddler or a sheet and two blankets. The use of a quilt made with Thinsulate or Hollofil with a mass per unit area of 160 to 180 g/m2 reduced the critical temperature to 19.5 degrees C, while Flectalon of comparable weight reduced the critical temperature to 13.8 degrees C: Flectalon is thus an efficient insulator. The risk of overheating was studied by monitoring swaddled babies, rectal temperatures at various ambient temperatures. Some forms of swaddling caused increases in rectal temperatures at "normal' hospital temperatures, implying risks from warmer environments and assessments of swaddling materials should, therefore, include medical evaluation of efficiency and safety. Flectalon merits assessment in other groups at risk from hypothermia.

This study reports the value of behavioural treatment of hyperactive children. Twelve children with multiple behavioural problems, including concentration difficulties, were identified and treated during a 3-month period. Six of the children began treatment immediately; the other 6 children received similar treatment but after a delay of between 4 and 6 weeks. Children were seen at the clinic or in the home, depending upon the individual difficulties and geographical distances. Initially the families were seen weekly, but the frequency later was determined by the severity of the problems and the response to treatment. There was a significant improvement in the children's behaviour as reported by the parents, the home visitor, and videotaped recordings. During the initial period of beteen 4 and 6 weeks the treated children improved significantly, whereas the others, as yet untreated, showed no change. As soon as the treatment was introduced to the second group these children's behaviour also improved and to about same extent in 6 weeks as the treated children had in up to 12 weeks' treatment. This interesting observation suggests that a short period of treatment, only 6 weeks, may be sufficient for most children with hyperactivity. This is more feasible with the present-day resources of many departments.

The application and interpretation of computerised spectral analysis of the neonatal electroencephalogram (EEG) using the Neuroscience Berg Fourier Analyser (BFA) is described. Recordings are immediately available at the cotside. Electrophysiological changes can be recognised by individuals with no previous experience in EEG technology. The compact nature of the analysis allows long periods of recording to be viewed within minutes. In addition to the unequivocal demonstration of both clinical and subclinical seizures, the BFA is useful in the evaluation of interseizure activity--that is, disturbance of sleep patterns, electrical output, and hemisphere asymmetry.

Clinical and haematological findings at the nadir of the refractory, early anaemia of prematurity were compared in a study of 95 preterm infants. 53% of 30 babies less than 32 weeks' gestational age at birth had abnormal clinical features resulting from anaemia at its nadir, with a combination of tachycardia, tachypnoea, dyspnoea and feeding difficulties, diminished activity, and pallor. The expression 'available oxygen', derived from the Hb concentration and Hb-O2 affinity, correlated more closely with clinical features of anaemia that did the Hb concentration alone. A formula is presented that predicts the 'available oxygen', provided the Hb concentration and post-conceptual age are known; this avoids the need for direct measurement of Hb-O2 affinity. Clinical anaemia is common in preterm infants with Hb concentrations of up to 10.5 g/dl, consequent on the high O2 affinity of fetal Hb. This is the first description of any common clinical consequence of high Hb-O2 affinity.

We report an infant who developed cranial diabetes insipidus after septicaemic shock. This condition should be considered in any child who suffers an acute collapse and it may be more common in the high risk neonate than has previously been recognised.

This study investigated the hypothesis that training of mothers with Down's syndrome children would be beneficial both to the child and parents. The mothers were taught behaviour modification techniques based on learning theory and were given group discussions on dealing with their family or personal problems. The subjects were 16 mothers with a Down's syndrome child, divided into two groups on the basis of their child's sex and chronological and mental ages. The Griffiths Scale was used for assessment. The mothers in the treatment group received 12 sessions of training and group counseling over a 6-month period, whereas the control mothers received no additional attention except the usual routine from the general practitioner and health visitor. The result show clear gains to both the child and mother in the treatment group. The child improved, especially in language development as well as in the other areas, and the mother-gained more confidence and competence in her daily management of the child.
