Objectives: To apply the ROBIS tool for assessment of risk of bias (RoB) in systematic reviews (SRs) in a meta-review on effective doses (EDs) in dental and maxillofacial cone beam CT. Methods: Three electronic databases and reference lists of included SRs were searched. Eligible SRs were classified as having low, high or unclear RoB. Findings of SRs were synthesised and data from primary studies combined to relate ED to field of view (FOV) and operating potential (kV). Results: Seven SRs were included: three displayed low RoB, three high and one had unclear RoB. Only one SR related ED to image quality. Deficiencies in reporting of eligibility criteria, study selection and synthesis of results in SRs were identified. FOV height had a significant relationship with ED, explaining 27.2% of its variability. Median ED for three FOV categories differed significantly. Operating potential had a weak relationship with ED, with no significant difference in median ED between three operating potential groups. Conclusion: The ROBIS tool should have a role for meta-reviews of different aspects of radiology. The disappointing results for RoB might be remedied by developing standards to improve the quality of reporting of primary dosimetry studies and of SRs. Future dosimetry studies should always relate ED to image quality or diagnostic accuracy. Advances in knowledge: This meta-review is the first to implement ROBIS for SRs of ED and identified that trustworthiness of some SRs is questionable. The percentage change in average ED per cm increase in FOV height could be calculated, emphasizing the importance of FOV as a determinant of ED in CBCT.

OBJECTIVES To analyse the possible correlation between the trabecular bone pattern of the edentulous mandible visualized by panoramic radiography and the bone mineral density (BMD) measured by computed tomography (CT). METHODS Panoramic radiography and CT were performed on 28 patients with edentulous mandibles. The trabecular bone pattern of 47 regions on the panoramic radiographs was classified into one of five grades. BMD was measured within circular regions of interest on the axial CT images in the corresponding trabecular area. The gradings were correlated with the CT values using one-way analysis of variance. RESULTS There was a significant correlation between the grade of the trabecular bone pattern and the BMD (P < 0.0001). CONCLUSION The proposed classification for the trabecular bone pattern on panoramic radiographs might be a useful aid for the evaluation of the edentulous mandible prior to implant placement.

To elucidate the significance of the radiographic finding of a concavity of the posterior surface of the temporomandibular condyle.Three clinical cases with a concavity were followed up clinically and radiographically. A total of 39 temporomandibular joint (TMJ) autopsy specimens were examined by corrected sagittal tomography and seven condyles with a concavity of the posterior surface found. They were analysed microscopically together with three joints with a normal, rounded condyle.At follow-up tomography, two of the clinical cases revealed no change while the third patient had erosive changes after 7 months. The microscopic findings of the autopsy specimens varied. The articular cartilage in three of the seven joints with a concavity was similar to that in the normal condyles. The outer surface of the articular cartilage was sometimes uneven and the cartilage thickened, with an irregular junction between the articular cartilage and subchondral bone. In three joints the cartilage in the concavity had been replaced by granulation tissue. All seven joints had increased cellular activity in the concavity.The radiographic finding of a concavity in the posterior surface of the condyle is not specific. When small and well-defined it is indicative of remodelling and of osteoarthrosis when larger and with a diffuse outline.

Observer performance in tooth-length measurements in panoramic radiography has been examined. Sixty-four teeth, evenly distributed between maxillary first molars, second premolars and mandibular first and second premolars, were fixed in plastic moulds. Each cast was radiographed with an Orthopantomograph, twice with steel balls indicating the cusps and apices of the teeth and once without them. One observer measured the radiographic tooth length twice in both radiographs with indicators in order to estimate the true radiographic tooth length and the repositioning error, and twice in the radiographs without indicators. Seven other observers measured the tooth length on the radiographs without indicators, twice applying their own criteria and twice using defined criteria. The accuracy of the tooth-length measurements was calculated by comparing the true radiographic tooth length with the measurements of the seven observers. The intra-observer variation was defined as the difference between two measurements on the same radiograph. The accuracy and the precision of the tooth-length measurements were highly influenced by the observer performance. The mean radiographic tooth length of the seven observers was closer to the true radiographic tooth length when the observers applied the defined criteria. The inter- and intra-observer variation was higher for the measurements of the maxillary teeth (0.3-1.9 mm) compared with the mandibular teeth (0.3-0.9 mm) for most observers. The highest intra-observer variation was found for the palatal root of the maxillary first molar. The intra-observer variation of four observers decreased somewhat when the defined criteria were used; for the other three observers, however, the variation increased.(ABSTRACT TRUNCATED AT 250 WORDS)

The aim of this study was to investigate both the actual and the radiographic tooth lengths of the maxillary first molar and second premolar and the mandibular premolars in panoramic radiographs. The actual length of 64 extracted teeth was measured. Steel balls were then attached to the cusp and apex and the teeth embedded in plastic moulds. Each pair of plaster casts with their set of teeth was radiographed with an Orthopantomograph twice at an interval of 1 month. The actual and the radiographic tooth length was measured twice by one observer. The mean tooth length of the molar was shorter than that of the premolars. The mean difference between the repeated measurements of the actual tooth length was small and ranged between 0.47 and 1.16% of the tooth length. The mean difference between repeated measurements of the radiographic tooth length was also small, with a method error of 0.13-0.21 mm. The vertical magnification in panoramic radiography was lower for mandibular premolars (13-15%) than for the maxillary second premolar and first molar (17-28%). The palatal root of the maxillary first molar had the highest vertical magnification (28%). Following the second set of radiographs, the mean difference between the measurements was small, except for the palatal root of the maxillary first molar (P0.001). Radiographic measurements of this root should therefore be used with caution. The results for the other roots examined indicate that these could be measured with high reproducibility.

Observer performance in the assessment of the periapical pathology from panoramic and periapical radiography was examined. Five endodontists, five general practitioners and five oral radiologists were asked to assess the periapical status of 117 teeth. The observers assessed the panoramic and periapical radiographs of the teeth, which were evenly distributed throughout the jaws with a 50% probability that either an osteolytic or sclerotic lesion was present. The results of the comparison between panoramic and periapical radiography were influenced by the selection of observers. When the oral radiologists acted as observers, the mean P(A) value for periapical radiography was higher than for panoramic radiography (P less than 0.001), resulting in periapical radiography presenting a higher overall diagnostic accuracy than panoramic radiography for all 15 observers (P less than 0.01). There was, however, no difference between panoramic and periapical radiography when the two groups of endodontists and general practitioners acted as observers. The comparison of the three groups of observers showed no difference between their diagnostic accuracy when assessing panoramic radiographs. With periapical radiography, the oral radiologists demonstrated a higher diagnostic accuracy than the endodontists (P less than 0.05). The observers in each group with the highest diagnostic accuracy also had the highest intra-observer agreement. The mean intra-observer agreement of the five general practitioners was higher than those of the other two groups of observers for panoramic radiography. For periapical radiography, the mean agreement rates of the groups were comparable.

The doses absorbed and energy imparted to facial tissues in hypocyloidal tomography of the maxillary sinuses were measured during a multi-film cassette exposure. Most absorbed doses were below 0.2 mGy, except for frontal tomography in an antero-posterior projection where the dose to the eye lens was 29 mGy. This dose was reduced by a factor of 24 by the use of leaded eyeglasses and by about 100-fold by a postero-anterior projection. Lateral tomography resulted in lower absorbed doses than frontal: the energy imparted was about 10 mJ in the latter compared to 4 mJ in the former.

The diagnostic accuracy of panoramic and periapical radiography was compared by five oral radiologists who assessed the periapical status of 117 teeth evenly distributed throughout the jaws with a 50% probability that either an osteolytic or sclerotic lesion was present. Receiver operating characteristic (ROC) analysis demonstrated no overall significant difference between panoramic and periapical radiography. However, for sclerotic lesions and for all lesions on maxillary premolars and mandibular molars periapical radiography was significantly superior (P less than 0.001); it was also superior for osteolytic lesions in the maxilla as well as for the lesions on mandibular premolars but with a smaller significant difference (P less than 0.05).

In student-directed education the students have a decisive influence on both course content and teaching methods. Two groups of students were taught oral radiology by student-directed and conventional means and the results compared by tests taken at the end of the course and 2.5 years later. There was no significant difference between the two groups in the results of knowledge-based questions. All students performed well immediately after the course but showed a weaker performance 2.5 years later. Student-directed education may be a worthwhile alternative to more conventional teaching methods, not only in giving the same level of knowledge, but also stimulating teachers and students to discuss and solve educational problems.

A case of an impacted deciduous first maxillary molar situated apical to the permanent first premolar is described and illustrated with roentgenograms. A brief discussion of the findings and therapeutic considerations is presented.
