Abstract

Approximately 6000 men die every year from ruptured abdominal aortic aneurysm in England and Wales. Randomised clinical trials and a large pilot study have shown that ultrasound screening of men aged 65 years can prevent about half of these deaths. However, there is a significant perioperative morbidity and mortality from interventions to repair the detected aneurysm. This paper explores the ethical issues of screening men for abdominal aortic aneurysm. It is concluded that a population screening programme for abdominal aortic aneurysm offers a clear balance of good over harm. It is therefore ethically justified, as long as men are given adequate information at every stage of the process. Each man has the right to be properly informed, regardless of whether he accepts the invitation to be screened and, if an aneurysm is detected, whether or not he accepts treatment. doi: 10.1136/jme.2010.035519

Abstract

Primary chronic open angle glaucoma is characterized by elevated intraocular pressure, changes in the optic disc, and visual field defects. Sensitivity to contrasts is an additional method of clinical evaluation. A study was undertaken to determine if the difference in perception of brightness is abnormal in patients with open angle glaucoma. Twenty-eight patients diagnosed with chronic open angle glaucoma were evaluated for visual acuity, difference in the perception of brightness between the two eyes (brightness ratio), and contrast sensitivity. These results were compared with results obtained from the evaluation of 41 normal subjects. Assessment of visual fields was also performed. There was a significant difference in brightness ratio between the two eyes of 24 of the 28 glaucoma patients (86 percent). This finding correlated with the frequency of visual field loss. Differences in sense of brightness were also related to differences in degree of visual field loss between the two eyes. Thirty-nine percent of the glaucoma patients had contrast sensitivities that were outside the normal range. It is concluded that when visual acuity is normal, the brightness ratio test may be a potential screening method for chronic open angle glaucoma. This test may be especially useful in developing countries because it is fast, simple to perform, and does not require trained personnel to administer. Abstract

This paper examines a proposal to make use of IVF techniques to provide an alternative to therapeutic abortion of fetuses with genetic abnormalities. We begin by describing the proposed procedure, and then show that, considered in itself, it is morally on a par with therapeutic abortion. However, once the wider practical implications are brought into view, the proposed new procedure loses its initial appeal. The pros and cons are not sufficiently clear-cut entirely to rule out the IVF procedure, so the paper concludes by indicating some further complications which may follow, should the procedure come to be adopted.
