Background Incidents of self-harm are common on psychiatric wards. There are a wide variety of therapeutic, social and environmental interventions that have shown some promise in reducing self-harm in in-patient settings, but there is no consensus on the most appropriate means of reducing and managing self-harm during in-patient admissions. Aims To review interventions used to reduce self-harm and suicide attempts on adolescent and adult psychiatric in-patient wards. Method A systematic literature search was conducted between 14 March 2019 and 25 January 2021 using PsycINFO and Medline (PROSPERO ID: CRD42019129046). A total of 23 papers were identified for full review. Results Interventions fell into two categories, therapeutic interventions given to individual patients and organisational interventions aimed at improving patient–staff communication and the overall ward milieu. Dialectical behaviour therapy was the most frequently implemented and effective therapeutic intervention, with seven of eight studies showing some benefit. Three of the six ward-based interventions reduced self-harm. Two studies that used a combined therapeutic and ward-based approach significantly reduced self-harm on the wards. The quality of the studies was highly variable, and some interventions were poorly described. There was no indication of harmful impact of any of the approaches reported in this review. Conclusions A number of approaches show some promise in reducing self-harm, but the evidence is not strong enough to recommend any particular approach. Current evidence remains weak overall but provides a foundation for a more robust programme of research aimed at providing a more substantial evidence base for this neglected problem on wards.

The severe acute respiratory syndrome coronavirus disease-2 (SARS-CoV-2) pandemic of 2020-2021 created unprecedented challenges for clinicians in critical care transport (CCT). These CCT services had to rapidly adjust their clinical approaches to evolving patient demographics, a preponderance of respiratory failure, and transport utilization stratagem. Organizations had to develop and implement new protocols and guidelines in rapid succession, often without the education and training that would have been involved pre-coronavirus disease 2019 (COVID-19). These changes were complicated by the need to protect crew members as well as to optimize patient care. Clinical initiatives included developing an awake proning transport protocol and a protocol to transport intubated proned patients. One service developed a protocol for helmet ventilation to minimize aerosolization risks for patients on noninvasive positive pressure ventilation (NIPPV). While these clinical protocols were developed specifically for COVID-19, the growth in practice will enhance the care of patients with other causes of respiratory failure. Additionally, these processes will apply to future respiratory epidemics and pandemics.

BackgroundCoronavirus disease 2019 is an international pandemic. One of the cardinal features is acute respiratory distress syndrome, and proning has been identified as beneficial for a subset of patients. However, proning is associated with pressure-related side effects, including injury to the nose and face.MethodThis paper describes a pressure-relieving technique using surgical scrub sponges. This technique was derived based on previous methods used in patients following rhinectomy.ConclusionThe increased use of prone ventilation has resulted in a number of referrals to the ENT team with concerns regarding nasal pressure damage. The described technique, which is straightforward and uses readily available materials, has proven effective in relieving pressure in a small number of patients.

Aim:The coronavirus disease (COVID-19) pandemic is bound to put tremendous pressure on the existing healthcare system. This aim of this technical note is to help in triaging patients with brain tumours who are sent for radiotherapy during this pandemic and to provide safe and evidence-based care.Materials and Methods:Published data for this review were identified by systematically searching PubMed database from November 2007 onwards with the following Medical Subject Heading (Mesh) terms ‘Brain tumours’, ‘COVID-19’, ‘coronavirus’, ‘SARS-nCoV-2’, ‘Radiotherapy’, ‘Guidelines’ ‘hypofractionation’ using Boolean search algorithm. Articles in English language were reviewed.Results:We tried to apply the as low as reasonable achievable (ALARA) principle in triaging and management of patients for radiotherapy. We identified protocols which have hypofractionated regimens (reducing patient visits to hospital, time spent in treatment console) with similar outcomes when compared to conventional fractionated regimens and not overburdening the healthcare facility. We also identified the tumours for which we could safely avoid or delay the initiation of radiotherapy.Conclusion:Treatment decisions made during the COVID-19 pandemic rely on the safety first/do no harm principle and evidence-based prioritisation of cases for triage. This article is a tool to aid in triaging and prioritising brain tumour patient management. This is for consideration during the pandemic only and certainly not as a strategy for permanent practice change.

The article outlines the impact of the COVID-19 pandemic on nationalism around the world. Starting from the premise that nationalism is a global and ubiquitous idea in the contemporary world, it explores whether exclusionary tendencies have been reinforced by the pandemic. The pandemic and government responses will not necessarily trigger the increase in exclusionary nationalism that both far-right politicians and observers have noted. However, there are 4 aspects, examined in the article, that might be shaped by the pandemic. These include the recent trajectory of nationalism and its social relevance prior to the pandemic,the rise of authoritarianism as governments suspend or reduce democratic freedoms and civil liberties, the rise of biases against some groups associated with the pandemic, the rise of borders and deglobalization, and the politics of fear. Thus, while the rise of exclusionary nationalism might not be the inevitable consequence of the pandemic, it risks reinforcing preexisting nationalist dynamics.

We quantified the potential impact of different social distancing and self-isolation scenarios on the coronavirus disease 2019 (COVID-19) pandemic trajectory in Saudi Arabia and compared the modelling results to the confirmed epidemic trajectory. Using the susceptible, exposed, infected, quarantined and self-isolated, requiring hospitalisation, recovered/immune individuals, fatalities model, we assessed the impact of a non-pharmacological interventions’ subset. An unmitigated scenario (baseline), mitigation scenarios (25% reduction in social contact/twofold increase in self-isolation) and enhanced mitigation scenarios (50% reduction in social contact/twofold increase in self-isolation) were assessed and compared to the actual epidemic trajectory. For the unmitigated scenario, mitigation scenarios, enhanced mitigation scenarios and actual observed epidemic, the peak daily incidence rates (per 10 000 population) were 77.00, 16.00, 9.00 and 1.14 on days 71, 54, 35 and 136, respectively. The peak fatality rates were 35.00, 13.00, 5.00 and 0.016 on days 150, 125, 60 and 155, respectively. The R0 was 1.15, 1.14, 1.22 and 2.50, respectively. Aggressive implementation of social distancing and self-isolation contributed to the downward trend of the disease. We recommend using extensive models that comprehensively consider the natural history of COVID-19, social and behavioural patterns, age-specific data, actual network topology and population to elucidate the epidemic's magnitude and trajectory.

Evaluation researchers at Clinical and Translational Science Award (CTSA) hubs are conducting retrospective case studies to evaluate the translational research process. The objective of this study was to deepen knowledge of the translational process and identify contributors to successful translation. We investigated the successful translation of the HemeChip, a low-cost point-of-care diagnostic device for sickle cell disease, using a protocol for retrospective translational science case studies of health interventions developed by evaluators at the National Health Institutes (NIH) and CTSA hubs. Development of the HemeChip began in 2013 and evidence of device use and impact on public health is growing. Data collection methods included five interviews and a review of press, publications, patents, and grants. Barriers to translation included proving novelty, manufacturing costs, fundraising, and academic-industry relations. Facilitators to translation were CTSA pilot program funding, university resources, entrepreneurship training, due diligence, and collaborations. The barriers to translation, how they were overcome, and the key facilitators identified in this case study pinpoint areas for consideration in future funding mechanisms and the infrastructure required to enable successful translation.

Exercise and spirometry testing are associated with coughing, exhaled respiratory droplets and aerosol generation. The risk of SARS-CoV2 transmission associated with these procedures are unknown. We developed a SARS-CoV2-specific consensus guidance document involving key stakeholders for restoring pulmonary diagnostic services, incorporating a patient and community-level risk stratified approach.

Rapid diagnostic testing (RDT) can provide prompt, accurate identification of infectious organisms and be a key component of antimicrobial stewardship (AMS) programs. However, their use is less widespread in Asia Pacific than western countries. Cost can be prohibitive, particularly in less resource-replete settings. A selective approach is required, possibly focusing on the initiation of antimicrobials, for differentiating bacterial versus viral infections and identifying locally relevant tropical diseases. Across Asia Pacific, more data are needed on RDT use within AMS, focusing on the impact on antimicrobial usage, patient morbidity and mortality, and cost effectiveness. Moreover, in the absence of formal guidelines, regional consensus statements to guide clinical practice are warranted. These will provide a regionally relevant definition for RDT; greater consensus on its role in managing infections; advice on implementation and overcoming barriers; and guidance on optimizing human resource capacity. By addressing these issues, the outcomes of AMS programs should improve.

Background:Vocal cord palsy is one of the recognised complications of complex cardiac surgery in the paediatric population. While there is an abundance of literature highlighting the presence of this complication, there is a scarcity of research focusing on the pathophysiology, presentation, diagnosis, and treatment options available for children affected by vocal cord palsy.Materials and methods:Electronic searches were conducted using the search terms: “Vocal Cord Palsy,” “VCP,” “Vocal Cord Injury,” “Paediatric Heart Surgery,” “Congenital Heart Surgery,” “Pediatric Heart Surgery,” “Vocal Fold Movement Impairment,” “VFMI,” “Vocal Fold Palsy,” “PDA Ligation.” The inclusion criteria were any articles discussing the outcomes of vocal cord palsy following paediatric cardiac surgery.Results:The two main populations affected by vocal cord palsy are children undergoing aortic arch surgery or those undergoing PDA ligation. There is paucity of prospective follow-up studies; it is therefore difficult to reliably assess the current approaches and the long-term implications of management options.Conclusion:Vocal cord palsy can be a devastating complication following cardiac surgery, which if left untreated, could potentially result in debilitation of quality of life and in severe circumstances could even lead to death. Currently, there is not enough high-quality evidence in the literature to aid recognition, diagnosis, and management leaving clinicians to extrapolate evidence from adult studies to make clinical judgements. Future research with a focus on the paediatric perspective is necessary in providing evidence for good standards of care.

The COVID-19 pandemic has forced governments around the world into drastic measures without the normal evidence base or analyses of consequences. We present a quantitative model that can be used to rapidly assess the introduction and interaction of nonpharmaceutical infection prevention measures (NPI) both in rapid a priori predictions and in real-world a posteriori evaluations. Two of the most popular NPIs are imposing minimum physical interpersonal distancing and the use of face coverings. The success of both measures is highly dependent on the behavior of the public. However, there is very little published information about the interactions between distance, mask wearing, and the behavioral adaptations that they are likely to generate. We explore the relation between these two fundamental NPIs and the behavioral responses that they may induce, considering both risk compensation and social norms enhancement. At present, we do not have the necessary information to parameterize our model to a sufficient degree to generate quantitative, immediately applicable, advice, but we explore a vast parameter space and illustrate how the consequences of such measures can range from highly beneficial to paradoxically harmful in plausible real situations.

Introduction Global public health is challenged by two concurrent epidemics; COVID-19 and obesity. Considering the global prevalence of obesity, exploring relationships with COVID-19 are of clinical importance. The aim was to provide a comprehensive summary and recommendations on this relationship between COVID-19 and obesity. Method A literature search was performed to prepare a narrative review of COVID-19 and obesity. Results An obesity state promotes chronic inflammation, vitamin D deficiency, hinders immunity and causes mechanical lung compression. These increase susceptibilities to COVID-19 infection, complications including the requirement of invasive ventilation. Existing co-morbidities enhances these complications. Preventive measures of social distancing and self-isolation may increase stigmatisation and psychological deterrents. Hence, special recommendations targeting this vulnerable population are required. Conclusion The obese population is a COVID-19 vulnerable group, requiring special attention during this pandemic to avoid complications and healthcare systems burden. Lacking COVID-19 vaccination, regular physical activity and a healthy diet are recommended with attention to mental health. A prolonged quarantine duration and administration of prophylactic vitamin D may be considered.
