Introduction:Mental health disorders constitute a growing disease burden in France. In particular, hospitalized patients with depressive or bipolar disorders suffer from weight gain impairing adherence to treatment and the quality of life. The objective of this preliminary study is to better understand determinants of weight gain and their synergetic effects, with the ultimate aim to develop a screening tool identifying at risk patients.Material and methods:Subjects were recruited at the private psychiatric clinic of Littoral, Rang-du-Fliers in the North of France according to pre-definite inclusion criteria. Data assessment has been based on 1) a retrospective case-control study using electronic medical records of depressive or bipolar patients (n = 207); 2) a prospective observational study (n = 20) using the medical records, and additional questionnaires at T0 (interview-administered) and T1 (self-administered) after an average length of hospital stay of 24.8 days (± 6.3) and 3) a cross-sectional study in depressive patients (n = 40) under psychotropic treatment at the day hospital, using the medical records and an interview-administered questionnaire. Descriptive and analytical statistics (Spearman rank correlations, binary logistic regression) were performed with SPSS statistics 17.0.Results:The mean age of all patients was 49.3 years (± 10.7). Patients’ weight change after hospital stay varied in the retrospective study from + 0.5 kg (± 1.98) [min: - 5.4 kg; max: + 8,4kg]. Patients at the day hospital had an mean psychotropic treatment duration of 7.8 years (± 7.8) and gained on average 17.6 kg (± 10,01). Weight gain was particularly related to the use of tricyclic antidepressants, classic antipsychotics and atypical antipsychotics. All psychotropic drugs were significantly associated with a change in eating behavior (rs = 0.37) which was significantly associated with weight gain (rs = 0.40). All studies observed significant associations between weight gain and a lower socio-economic profile (low financial resources, few social contacts, low level of education), polymedication and a sedentary lifestyle with a higher tobacco consumption and less sleeping hours.Discussion:This preliminary study confirms the complex interaction between psychotropic treatments, the modification of eating behavior which can be triggered by other determinants of weight gain in patients with depressive or bipolar disorders, such as tobacco consumption. The results provide new perspectives to create an innovative screening tool to improve the nutritional care in at risk patients. The development of such a tool is ongoing.

IntroductionThe loss of autonomy in elderly varies according to the individual health status, but also to the social and psychological environment. Multiple risk factors play a role, including limited access to a balanced diet, physical inactivity or a poor social network. Prevention programs must therefore be based on a multidimensional approach, but are rarely studied with regard to their effectiveness.Materials and methodsThe objective of the Auton'Al 60 program is to prevent the loss of autonomy related to diet, physical activity and mental health in elderly, aged 60 to 89, in the county of Oise, North of France. Between February and July 2018, 7 different prevention workshops were conducted in 5 geographical areas (urban and rural). A blog and a monthly newsletter have been created to limit the loss to follow- up. Evaluation was based on an in-depth interview at T0 inquiring about the level of autonomy, the dietary habits and the health status, as well as on 2 self-administered questionnaires during and at the end of the program. In addition, each workshop has been evaluated separately according to key indicators. Statistical analysis was performed using SPSS 17.Results69 workshops have been completed during 5 months. In total, 91 subjects (27.8% men - 71.9% women) participated in at least one workshop, with a mean age of 69.1 years (± 6.3). 72.6% of the subjects were present at 6 or 7 workshops, and 49.5% participated in all activities. A state of loss of autonomy was reported by 6.7% of the participants. 32.6% of the population were chronic-disease-free and 43.8% had a normal BMI (kg / m2). The prevalence of obesity was 15.7%. The program showed effectiveness to decrease the consumption of high-fat products in 52.9% as well as the consumption of sugary products in 60.9% of the participants. Furthermore, 23.2% of the subjects started a new social activity during the program. However, compared to other age groups observed fragility seemed to be particularly high in elderly aged from 66 to 69 years.ConclusionThe Auton'Al 60 program confirms the heterogeneity in the level of autonomy, health status and health behavior in elderly. The multidimensional approach has shown satisfactory effectiveness. Prevention strategies have been developed for isolated or low-income subjects and are under current evaluation.

ObjectiveTo assess the feasibility of combining short-term and long-term dietary assessment instruments as new concept for improving usual dietary intake assessment on the individual level.DesignFeasibility study of completing three 24 h dietary recalls (24-HDR) and a self-administered food propensity questionnaire (FPQ). The 24-HDR was conducted by monthly telephone interviews, using EPIC-SOFT software. The FPQ was completely standardized across cohorts and offered either as a web-based tool or in paper format.SettingRandom sample derived from five ongoing European cohort studies (EPIC-San Sebastian, EPIC-Florence, EPIC-Potsdam, Estonia Genome Center (EGC) and Norwegian Women and Cancer study (NOWAC)).SubjectsA total of 400 participants.ResultsOverall, the total participation rate for the present study was 65·3 % (n 261). On average, completion of the 24-HDR was highest for the first 24-HDR (63·0 %) and decreased slightly for the second (60·3 %) and third 24-HDR (56·3 %). The proportions of selecting the web-based FPQ varied among the study centres, with the highest in EGC (92·9 %) and NOWAC (70·0 %) and the lowest in EPIC-San Sebastian (25·5 %) and EPIC-Potsdam (33·9 %). Web users rarely requested support and were younger and more highly educated than those who completed the paper format.ConclusionsThe present study supports the feasibility of a combined application of three 24-HDR and an FPQ in culturally different populations. The varying acceptance of the web-based instrument across populations requires a flexible application of assessment instruments.
