In-hospital sudden death and cardiopulmonary cerebral resuscitation problems could have a common structured response. However, most hospitals lack a solution- oriented strategy. This paper provides detailed update on the main alternatives to reduce lethality of in-hospital sudden death and presents a proposal from the "Hospital General de Cienfuegos". The institutional approach for greater survival and lesser disability when facing sudden death will be based on multidisciplinary initiatives primarily centered on quality and prevention in the different stages of cardiopulmonary cerebral resuscitation. A cardio-protected hospital comprises management, organizational, assistance, academic and logistical processes for every single link of the survival chain in a medical emergency event, mainly sudden cardiac arrest. The proposal of the "Hospital General de Cienfuegos", a cardioprotected facility, brings together all elements of an integrated cardiopulmonary cerebral resuscitation system aimed at quality, safety and satisfaction of patients and caregivers. Introduction: The coronary artery disease is the most common cause of sudden cardiac death. In up to 50% of cases it can occur suddenly, with cardiorespiratory arrest, sometimes as the first and last symptom. Objective: To evaluate the knowledge about cardiopulmonary resuscitation and the conditions of the crash carts. Method: A descriptive and transversal study was carried out, with 63 health professionals, in the Internal Medicine Department of the «Hospital Celia Sánchez Manduley», in Manzanillo, Cuba, in April 2016; they were given a knowledge questionnaire and a form to evaluate the crash carts. Results: The 75% of participants demonstrated low knowledge. The 69.9% of doctors have 1 to 5 years of experience and of them, 50% showed a low level of knowledge and the rest, average. In the three groups of the analyzed graduated nurses, more than 85% achieved a low rating. The 80% of doctors and 83.3% of graduates who received a course on resuscitation showed low knowledge. Of the five rooms that the Internal Medicine Department has, only one has a crash cart and its condition is not tolerable. Conclusions: There is a lack of knowledge concerning cardiopulmonary resuscitation in the Internal Medicine Department. The proposal of a modular, optional or own course for nurses concerning issues on basic and advanced cardiopulmonary resuscitation is presented, taking into account the learning needs of graduates of this specialty, from a prospective descriptive exploratory type of qualitative study, as a human resources development and training project. The analysis of the specialization's curriculum, an extensive literature review and consultation with experts was made to endorse the proposed research. Introducción: La muerte súbita (MS) constituye un problema de salud de primer orden. Los estudios patológicos post mortem son esenciales para descartar la presencia de una enfermedad hereditaria. Objetivo: Describir las características y el diagnóstico de las causas de MS durante 21 años (1995-2015). Método: Se estudiaron 564 casos consecutivos (44±18 años; 76,6% hombres) con MS (479; 84,9%) o muerte súbita resucitada (MSR) (85; 15,1%) en el Instituto de Medicina Legal y el Hospital Virgen de la Arrixaca de Murcia, España. Se excluyeron las cardiopatías isquémica, valvular, y otras condiciones no hereditarias. Resultados: En 274 fallecidos de MS (57%) se realizó un estudio post mortem, que fue diagnóstico en 189 (69%) casos; 101 individuos (17%) permanecieron sin diagnóstico. Se evidenció un mayor número de pacientes incluidos en la última década con respecto a la primera (375 [66%] frente a 189[34%], p<0,001), igual que el número de estudios post mortem (59% vs. 41%; p<0,001), y el diagnóstico definitivo (69% vs. 10%). El reconocimiento de miocardiopatía arritmogénica aumentó en el último período (relación miocardiopatía hipertrófica/arritmogénica de 3,5 en los últimos 5 años). A partir de 2006 hubo un incremento en la tasa de MSR frente a la total (la relación de MS/MSR pasó de 11,6 a 5,1). Las canalo patías fueron significativamente más frecuentes en la MSR (38% vs. 11%; p <0,001) (Figura). Conclusiones: Se objetiva la importancia de la asistencia extrahospitalaria y la relevancia de los estudios post mortem para llegar a un diagnóstico definitivo.
