Introduction: Hand injuries are one of the conditions most frequently treated in the Plastic and Reconstructive Surgery Service of the Hospital Central Militar, within the wide variety of these injuries are injuries to the flexor system. Objective: To estimate the incidence, functionality and labor reintegration time in patients with hand flexor system injuries treated at the Hospital Central Militar in the period from March 1, 2020 to March 1, 2022. Methodology: Observational, retrospective, longitudinal, descriptive study in adult patients with injury to the flexor system of the hand, in whom the cause of the injury did not result in complex trauma to the hand. The Strickland and Goglovac functional recovery criteria were used as reference. Results: Despite the fact that flexor zone II lesions in the literature present a worse prognosis for the recovery of normal hand function, the results show that most of the patients are in a good functional scale. Limitations of the study: The study was carried out during the period covered by the COVID pandemic, so that subsequent consultations with longer times and the late start of rehabilitation consultations could have contributed to a poorer recovery of functional recovery. from the patients. Absence of the thumb diminishes de function of the hand in a 40%. The pollicization procedure is the gold standard in treatment in these patients; it involves forming a thumb with the finger most proximal to it. We present the case of a 2-year-old girl with transversal deficit of left arm with a type IV hypoplasia of the right thumb. A pollicization of the index fi nger was realized, in the postoperative follow up she had complete passive range of movements of the thumb, six months postsurgery she had M4 active movement. The pollicization procedure provides good sensitivity, range of movement, and esthetic results, besides having less morbidity in the donor site. Even tough it is a high-risk procedure, most of the times satisfactory results are obtained. [ABSTRACT FROM AUTHOR]

Background. The forehead flap for nasal reconstruction was first described in India and has been the cornerstone for nasal coverage of wide defects. More recently, this forehead flap for nasal reconstruction has been through a great variety of modifications and can be done as a single, two or three-stage procedure. Nasal reconstruction requires a critical analysis of the involved tissues, a thorough anatomical and surgical knowledge from the plastic surgeon, to be able to offer a better esthetic result. The aim of this paper is to describe an alternative of the forehead flap for a single-stage nasal reconstruction to cover a wide nasal defect. Conventional technique. The forehead flap is a composite flap consisting of skin, subcutaneous tissue, frontalis muscle and a thin layer of areolar connective tissue. The flap is harvested by dissecting the paramedian pedicle based on the supratrochlear vessels, which are located superficial to the periosteum and exit the medial superior orbital rim then travelling vertically along the muscle. The forehead flap is elevated and transferred as an island in a single-stage procedure, restoring the esthetic subunits of dorsum and lateral sidewall. Clinical case. We describe the case of a patient to whom wide resection of a basocellular carcinoma in the nasal dorsum was performed in the Instituto Nacional de Cancerologia. We performed immediate reconstruction with a single-stage forehead flap, achieving adequate functional and esthetic results. Conclusions. Nasal reconstruction can be performed with a single-stage procedure using the forehead flap achieving adequate functional and esthetic results. [ABSTRACT FROM AUTHOR]
