Objective To investigate the incidence and related factors of postoperative complications in acoustic neuroma patients. Methods The clinical data of 96 patients with acoustic neuroma underwent microsurgery in the First Affiliated Hospital of Nanjing Medical University from January 2018 to September 2019 were analyzed retrospectively. Results In this data, dizziness(66.7%), hearing impairment(58.3%), and facial paralysis(32.3%) were the most common postoperative complications. Cerebrospinal fluid leakage(10.4%) was a serious complication after acoustic neuroma surgery. Tumor size, operation time, tumor recurrence, and eradicate the mastoid process were independent risk factors of the cerebrospinal fluid leakage(all P＜0.05). The incidence of intracranial infections(8.3%) was related to operative time and cerebrospinal fluid leakage(all P＜0.05). Conclusion Microsurgery is effective in the treatment of acoustic schwannoma. The prevention and treatment of these risk factors in perioperative period is efficient to the prognosis of patients. [ABSTRACT FROM AUTHOR]

Objective To explore the effect of ultrasound scanning in ventriculoperitoneal shunt in patients with hydrocephalus with skull defect after craniotomy. Methods The clinical data of 75 patients with skull defect and hydrocephalus after craniotomy underwent ventriculoperitoneal shunt were analyzed retrospectively. The number of intraoperative puncture, intraoperative blood loss, operative time, related complications, and follow-up recovery were compared between the ultrasound group and the control group groups. Results The number of puncture, intraoperative blood loss and operative time in the ultrasound group were significantly less than those in the control group(P<0.05-0.005). There was no statistically significant difference in the incidence of infection and bleeding complications between the two groups within 2 weeks after surgery(χ2=2.163,P=0.141). The complications of tube blockage and infection in the ultrasound group were less than those in the control group follow up for more than 1 year , but there was no statistical significance. Conclusion VP shunt surgery for hydrocephalus patientswith skull defect, real-time guidance of ultrasound technology can be used to obtain a better and ideal position of the ventricle end of shunt, and the puncture injury, operation time and bleeding were reduced. ObjectiveTo explore the microsurgical approach for craniopharyngioma and the surgical outcomes. MethodsClinical data of 71 patients with craniopharyngioma underwent microsurgical resection were analyzed retrospectively. The operations were performed via pterional approach in 40 cases， frontobasal interhemispheric approach in 14， transsphenoidal approach in 7， lateral supraorbital approach in 5， interhemispheric-transcallosal-interforniceal approach in 3 and supraorbital keyhole in 2. ResultsThe total resection of tumor were achieved in 47 cases, subtotal resection in 23 and partial resection in 1. In 30 patients with vision loss as the first symptom, 21 cases had improved visual acuity after surgery. There was postoperatively transient diabetes insipidus in 52 cases， electrolyte disturbance in 34， abnormal endocrine functions in 40， central high fever in 13， ipsilateral oculomotor palsy in 6 and mild disturbance of consciousness in 6. The patients were followed up for mean period of 23.5 months, ranged from 3 to 35 months. No patients died of the operation. Tumor recurrence occurred in 13 cases. The postoperative endocrine function of patients with pituitary stalk preservation is better than that of patients with pituitary stalk resection（P＜0.05）, and the preservation of pituitary stalk is independent of tumor recurrence（P＞0.05）. Patients with tumor calcification have a lower probability of pituitary stalk preservation than those without calcification（P＜0.05）. ConclusionAn appropriate surgical approach according to the characteristics of the tumor，meticulous microsurgical manipulation and good protection of the pituitary stalk during operation is of great significance for improving the surgical efficacy of patients with craniopharyngioma.
