Abstract: Background: Adoption of neoadjuvant chemotherapy (NACT) in the “real world” has been poorly investigated. Aim of this study was to examine the rate of NACT in Italy, trends over time and determinants of therapeutic choices. Methods: Senonetwork, the recognized network of Breast Centers in Italy, has developed a voluntary national data warehouse with the aim to monitor and improve treatments quality. A retrospective analysis was conducted among 58,661 breast cancer (BC) patients treated between 2017 and 2022 by 24 high-volume Breast Centers participating in the project. Results: After subset exclusion, 37,215 primary BC patients were analysed, 32,933 underwent primary-breast-surgery and 4,282 underwent NACT. 2. Towards a collaborative stakeholder engagement pathway to increase ocean sustainability related to marine spatial planning in developing coastal states

Abstract: With the increased emphasis on charting ocean sustainability narratives, marine spatial planning (MSP) is envisioned as a key tenet. MSPs emphasize the systematic and collaborative planning and management of ocean space (resources and activities) for the benefit of all users. Regions that have implemented MSP based on collaborative stakeholder engagement are progressively realizing better ocean sustainability outcomes. Unfortunately, in developing coastal states, progress toward MSP is largely pedestrian and has attracted less interest. This is partly due to archaic coastal/marine resource models that are dominated by few powerful stakeholders. This is worsened by increasing human-environmental shocks, which are creating bleak futures. Our study systematically sourced 12,316 documents from Scopus that were analyzed using bibliometrics to (i) conduct a performance analysis, (ii) conduct a scientific mapping analysis and (iii) identify game-changing developments that can drive ocean sustainability. A performance analysis revealed that even though scholarship and publications on MSP have increased globally, scholarship among or led by researchers from the global south are limited. Scientific mapping analysis revealed emerging positive trends in multi-country collaborations as well as the recognition of threats to the marine environment. Reversing this requires increased stakeholder engagement. However, how to achieve this goal in most developing coastal states has been less studied. Building on this, we developed a novel Collaborative Stakeholder Engagement Pathway (CoSEP) involving eight (8) interrelated steps that can help build collaborative engagements for MSP development and ocean sustainability. A notable takeaway from the CoSEP is that; since research on MSP development is limited or in its infancy, knowledge of how and when to engage which stakeholders is key in creating collaborative mechanisms for positive ocean sustainability, including ocean justice. This can help localize sustainable ocean development pillars and build avenues for integrated coastal resource management. Using participatory approaches that bring forward microlevel stakeholder perspectives could be a future driver in designing effective interventions and cultures to create MSPs that meet ocean sustainability targets.

3. Co-designing transformative ocean sustainability narratives to address complex human-environmental challenges facing coastal fisherwomen: An evidence-based study

Abstract: Marine fishing is paramount to the provision of sustainable coastal livelihood welfare; especially in the global South. To coastal women, marine fishing avails multiplier social-economic benefits; notably employment, food, and income. Sustaining the innumerable benefits of marine fishing is a conduit towards progressive fisherwomen empowerment and the attainment of transformative ocean sustainability targets. However, in most coastal regions, increasing evidence of human-environmental challenges is blanketing the ability of fisherwomen to benefit from marine fishing and, this is worsened by the perpetuation of social barriers to fisherwomen's inclusion in sustainable coastal fisheries value chains. In addition, there is limited research on how to co-design transformative actions to reduce these challenges partly due to the use of top-down approaches and frameworks that are largely complex and less inclusive. Our study aims to co-design transformative ocean sustainability narratives to address the complex human-environmental challenges by using local perspectives of coastal fisherwomen. We systematically reviewed 38 documents mainly focusing on marine fisherwomen in Kerala and used a micro-level community participatory approach; involving Focus Group Discussions with 43 fisherwomen in Alappad in Kerala, India. Review findings revealed that a combination of human-environmental challenges is still prevalent. However, in Alappad, three human-induced systemic barriers predominate: lack of support from people in their own Panchayat, seasonal jobs, and road problem. Through co-designing, fisherwomen in Alappad proposed a novel pathway involving ten steps. This revealed that coastal fisherwomen can identify key systemic challenges that hinder their empowerment and promotion of sustainable ocean practices. Using perspectives and narratives of local fisherwomen could be key in breaking systemic barriers to fisherwomen inclusion in not only fishing but also, in other coastal areas and/or sectors and thus could help in developing collaborative ocean sustainability and governance interventions.

4. Outcomes of patients with advanced liver disease undergoing cardiac surgeryCentral MessagePerspective

Abstract: Objective: Liver disease (LD) is considered a risk factor for inferior outcomes in general and cardiac surgery, yet current cardiac surgery risk estimators exclude LD, and literature on the topic remains scant. We sought to evaluate whether the presence of advanced LD is associated with inferior outcomes following cardiac surgery. Methods: This single-center, retrospective, observational study included 285 patients diagnosed with LD who underwent cardiac surgery in 2010 to 2020. The cohort contained 3 groups, Child-Turcotte-Pugh (CTP) class A (n = 219), CTP early-class B (n = 34), and CTP advanced-class B (n = 32). A model for end-stage liver disease score of 12.7 points (determined using a receiver-operating characteristic curve analysis on 30-day mortality) dichotomized class B into early- and advanced-groups. Univariate and multivariate logistic regression analyses were performed to identify predictors of 30-day mortality. Results: Patients in CTP advanced-class B had the longest length of stay (14 days), highest incidence of prolonged ventilation (46.9%), renal failure (21.9%), 30-day mortality (18.8%), and in-hospital mortality (18.8%). Incidence of ≥1 postoperative complication was higher in CTP advanced-class B (59.4%), compared with CTP class A (37.9%) and CTP early-class B (38.2%). Multivariate logistic regression analysis demonstrated that female sex (odds ratio, 3.01; 95% CI, 1.07-8.77; P = .037) and peripheral vascular disease (odds ratio, 4.01; 95% CI, 1.33-12.2; P = .013) were independent predictors of 30-day mortality in patients with advanced LD. Conclusions: Severity of LD influences perioperative outcomes following cardiac surgery. Abstract: Objective: Tricuspid annuloplasty is associated with increased risk of atrioventricular block and subsequent implantation of a permanent pacemaker. However, the exact incidence of permanent pacemaker, associated risk factors, and outcomes in this frame remain debated. The aim of the study was to report permanent pacemaker incidence, risk factors, and outcomes after tricuspid annuloplasty from nationwide databases. Methods: By using data from multiple Swedish mandatory national registries, all patients (n = 1502) who underwent tricuspid annuloplasty in Sweden from 2006 to 2020 were identified. Patients who needed permanent pacemaker within 30 days from surgery were compared with those who did not. The cumulative incidence of permanent pacemaker implantation was estimated. A multivariable logistic regression model was fit to identify risk factors of 30-day permanent pacemaker implantation. The association between permanent pacemaker implantation and long-term survival was evaluated with multivariable Cox regression. Results: The 30-day permanent pacemaker rate was 14.2% (214/1502). Patients with permanent pacemakers were older (69.8 ± 10.3 years vs 67.5 ± 12.4 years, P = .012). Independent risk factors of permanent pacemaker implantation were concomitant mitral valve surgery (odds ratio, 2.07; 95% CI, 1.34-3.27), ablation surgery (odds ratio, 1.59; 95% CI, 1.12-2.23), and surgery performed in a low-volume center (odds ratio, 1.85; 95% CI, 1.17-2.83). Permanent pacemaker implantation was not associated with increased long-term mortality risk (adjusted hazard ratio, 0.74; 95% CI, 0.53-1.03). Conclusions: This nationwide study demonstrated a high risk of permanent pacemaker implantation within 30 days of tricuspid annuloplasty. However, patients who needed a permanent pacemaker did not have worse long-term survival, and the cumulative incidence of heart failure and major adverse cardiovascular events was similar to patients who did not receive a permanent pacemaker.

Abstract: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M–protein, skin changes syndrome) is a rare condition due to an underlying plasma cell neoplasm whose clinical presentation can be various so it could lead to delayed diagnosis and treatment. The pathogenesis of the syndrome is not well understood, and its therapy is adapted from other plasma cell disorders with the aim of alleviating symptoms, decreasing end-organ damage, improving quality of life and prolonging overall survival. We report a case of a 71 years-old woman who has been treated with continuous DRd (daratumumab, lenalidomide and dexamethasone) scheme.

7. Correlation Between Co Levels in Hair and Blood of Patients Who Underwent Metal-on-metal Hip Arthroplasty

Abstract: Background: The purpose of this paper is to study the dependence of Co levels in hair on Co levels in blood after metal-on-metal (MoM) hip replacement and prove the suitability of hair analysis coupled to blood analysis in the decision process regarding implant revision evaluation. Methods: Hair samples of 19 MoM patients having both well-functioning and malfunctioning implants and Co mass concentration levels in blood between 0.2 μg L-1 and 221.0 μg L-1 were included. A method based on inductively coupled plasma mass spectrometry was validated and used to measure the Co level in hair. Results: The Co mass fraction in the hair of patients ranged between 0.011 mg kg-1 and 0.712 mg kg-1. A correlation analysis showed a statistically significant positive correlation (r = 0.932, P < .001) between Co in the hair and that in the blood in the full-level range and a statistically nonsignificant positive correlation (r = 0.595, P = .091) in the low-level range. Conclusions: A correlation between the Co level in the hair and that in the blood exists when the latter is clearly above the 7 μg L-1 mass concentration threshold suggested for implant revision evaluation. The correlation disappears when the Co level in blood approaches or falls down the mass concentration threshold and that in the hair approaches or falls within the normal population range of 0.004-0.14 mg kg-1. Accordingly, clinicians could consider a hair analysis coupled to a blood analysis to assess the revision of malfunctioning MoM implants that release metals in patient’s body.

Abstract: The two-stage elephant trunk (ET) and thoracic endovascular aortic repair technique for type A and B aortic dissection can result in complications between the two stages. We have presented the case of a patient with an acute-on-chronic type B aortic dissection complicated by ET kinking and migration into the false lumen. We used a hybrid approach consisting of a first stage (retrograde thoracic endovascular aortic repair) and a second stage (“body floss” with antegrade thoracic endovascular aortic repair) to successfully reposition the ET back into the true lumen.

Abstract: Replacing fossil plastic with green and biobased alternatives is one of the most significant challenges of this century. Polyhydroxyalkanoates, more specifically, poly(3-hydroxybutyrate) (PHB), have been found as a very promising alternative to traditional plastics, but currently, they are about 2% of the bioplastics produced. In this work, the growth of a methanotrophic strain belonging to the Methylocystis species, a natural producer of poly(3-hydroxybutyrate), was modelled in a bubble column bioreactor fed with methane, considering a discontinuous feeding strategy. Three-dimensional simulations were performed by using a CFD approach. The biomass growth strategy based on the variation of the methane bubble diameter, in a range of 0.003–0.005 m, and the aspect ratio, in the range 5–16, was simulated. Their effects were assessed on the process efficiency and performance in terms of methane fed, biomass and PHB yields. When the bubble diameter was varied, the best performance was achieved at a diameter of 0.005 m with methane fed of 34.2 g, biomass yield of 0.534 gbiomass/gCH4 and PHB yield of 0.327 gPHB/gCH4. When the aspect ratio was varied, the best performance was achieved at an aspect ratio of 10 with methane fed of 34.9 g, biomass yield of 0.523 gbiomass/gCH4 and PHB yield of 0.321 gPHB/gCH4. Finally, the estimation of the PHB yield was performed. CFD analysis allowed to verify the absence of death or stagnant zones inside the reactor.

Abstract: The aim of this study was to evaluate the potential use of textile sludge and its pyrolysis by-products in the energy sector and the potential use of the oily phase fraction (OPF) as an antibacterial against selected microorganisms. Characterization assays were carried out, for the raw textile sludge (TS) and pyrolysis by-products, as pH variation, elemental analysis, metal content, FTIR, and higher heating value. The OPF was also fractionated using a column chromatography with different solvents (n-hexane, dichloromethane, and methanol) as mobile phases, and then each fraction was analyzed by gas chromatography coupled with mass spectrometry (GC-MS). Due to the biocide activity of the OPF, antibacterial assays were carried out against Escherichia coli (ATCC 700336), Staphylococcus aureus (ATCC 25923), Pseudomonas aeruginosa (ATCC BAA-47), and Klebsiella pneumoniae (ATCC 10031). From the results, it was observed that: the obtained OPF was able to high inhibition performance in the bacterial growth at concentrations >17 mg mL-1; and the TS, OPF, and tar (TRF) presented higher heating value > 20 MJ kg-1. The results show the potential use of OPF as a low-cost antibacterial agent and the feasibility of using textile sludge and pyrolytic oil fractions as an alternative source of fuel for energy generation.

Abstract: An energy management system optimizes the self-consumption of a residential photovoltaic installation, and the performance losses due to production uncertainties are evaluated. The specific case under study is an individual home equipped with photovoltaic (PV) panels where only an Electric Water Heater (EWH) is manipulated, and the rest of the appliances represent a fixed load. By formulating the problem of maximizing self-consumption as an unconstrained optimization problem, a novel and computationally efficient optimization algorithm has been proposed. The next step was to numerically evaluate the performance of this EWH management strategy under various PV power production scenarios, generated through a presented methodology. The reference baseline is a rule-based controller using a most likely forecast of PV production. Simulations performed in Dymola over 10 months demonstrate that, at a 30-minute timestep, the impact of a “perfect” PV production forecast is negligible compared with the impact of the choice of the control algorithm. Besides, a most likely forecast is good enough for the proposed algorithm to reach high self-consumption levels. Indeed, although the proposed optimization based on a most likely forecast yields an increase of 10 points of self-consumption compared to the baseline, only an additional 2 points of increase can be reached using “perfect” production information.

In this study, the potential of Methylocystis hirsuta and Methylocystis parvus to support poly(3-hydroxybutyrate) (PHB) production in co-cultivation with Rhodococcus opacus and Pseudomonas putida under a CH4:O2 atmosphere was assessed batchwise. The metabolic activation of almost all co-cultures studied was faster than that of single strain cultures, bringing higher methane and oxygen consumption rates. Higher PHB yields were achieved when coupling M. hirsuta with R. opacus (63 % w w−1) or with R. opacus and P. putida (64.4 % w w−1) compared to M. hirsuta alone (38.5 % w w−1). Abstract: Producción Científica, In this work, the potential of Methylocystis hirsuta to simultaneously use methane and volatile fatty acids mixtures for triggering PHBV accumulation was assessed for the first time batchwise. Biotic controls carried out with CH4 alone confirmed the inability of Methylocystis hirsuta to produce PHBV and achieved 71.2 ± 7 g m−3d−1 of PHB. Pure valeric acid and two synthetic mixtures simulating VFAs effluents from the anaerobic digestion of food waste at 35 °C (M1) and 55 °C (M2) were supplied to promote 3-HV inclusion. Results showed that pure valeric acid supported the highest polymer yields of 105.8 ± 9 g m−3d−1 (3-HB:3-HV=70:30). M1 mixtures led to a maximum of 103 ± 4 g m−3d−1 of PHBV (3-HB:3-HV=85:15), while M2 mixtures, which did not include valeric acid, showed no PHV synthesis. 28. Impact of aortic aneurysms in trans-catheter aortic valve replacement: A single center experience

Abstract: Background: Patients who undergo trans-catheter aortic valve replacement (TAVR) may have concomitant aortic aneurysms. We sought to clarify the incidence of aortic aneurysms and its impact on clinical outcomes among patients undergoing TAVR. Methods: We performed a retrospective analysis of patients with severe symptomatic aortic stenosis who underwent TAVR from January 2012 to June 2016. Multi-detector computer tomography (MDCT) was performed on all patients and images were reviewed to identify thoracic and abdominal aortic aneurysms (TAA and AAA). The incidence of vascular complications (VARC-2 definition), and in-hospital and 6-month major adverse cardiac events (MACE) defined as a composite of all-cause mortality, cardiac arrest, myocardial infarction, and stroke were recorded. Results: Among 232 patients included in the analysis, 22 patients (9.5%) had aortic aneurysms (11 had AAA, 8 had TAA, and 3 had both). Patients with aortic aneurysms had a higher, albeit statistically insignificant, rate of smoking history (63.6% vs. 42.9%, p = 0.062). Both groups of patient predominantly underwent TAVR via trans-femoral access (72.7% vs. 71.4%, p = 0.90). The incidence of vascular complications was similar between the two groups (9.1% vs. 10.5%, p = 1.0). Patients with aortic aneurysms had a similar in-hospital MACE (4.5% vs. 6.2%, p = 1.0) and 6-month MACE (9.1% vs. 9.0%, p = 1.0) compared to those without aneurysms. Conclusions: In our patient cohort, 9.5% of patients who underwent TAVR had concomitant aortic aneurysms. Patients with aortic aneurysms had similar incidence of vascular complications as well as in-hospital and 6-month MACE compared to those without. Abstract: Bisphosphonates are a class of drugs that are widely used to inhibit loss of bone mass in patients. We show here that the administration of clinically relevant doses of bisphosphonates in mice increases antibody responses to live and inactive viruses, proteins, haptens, and existing commercial vaccine formulations. Bisphosphonates exert this adjuvant-like activity in the absence of CD4+ and γδ T cells, neutrophils, or dendritic cells, and their effect does not rely on local macrophage depletion, Toll-like receptor signaling, or the inflammasome. Rather, bisphosphonates target directly B cells and enhance B cell expansion and antibody production upon antigen encounter. These data establish bisphosphonates as an additional class of adjuvants that boost humoral immune responses.

Abstract: OBJETIVO: Verificar o grau de concordância entre uma escala de triagem e uma de diagnóstico do desenvolvimento motor de lactentes no sexto mês de vida, estabelecendo o ponto de corte mais apropriado para triagem. MÉTODOS: Estudo seccional, incluindo recém-nascidos a termo, com idade gestacional entre 37 e 41 semanas, assintomáticos, que receberam alta da maternidade 2 dias após o nascimento, residentes na região de Campinas. Foram excluídas síndromes genéticas, malformações, infecções congênitas, internações em unidade de terapia intensiva e baixo peso ao nascimento. Os instrumentos de avaliação foram: Alberta Infant Motor Scale (AIMS) e Bayley Scales of Infant Development II (BSID-II). Para a AIMS, foram utilizados dois pontos de corte, percentil 5 ou 10 e, para as BSID-II, foi utilizada a classificação dos lactentes na escala motora conforme a pontuação do index score (IS): desempenho inadequado (IS < 85, abaixo de menos 1 desvio padrão da média) ou desempenho adequado (IS > 85, maior ou igual a menos 1 desvio padrão da média). RESULTADOS: A amostra foi constituída por 43 lactentes. Seis lactentes (14,00%) apresentaram desempenho motor inadequado. Considerando a classificação motora das BSID-II e o percentil 5 da AIMS, obteve-se sensibilidade = 100%, especificidade = 78,37%, acurácia = 81,39%, índice kappa = 0,50 e p < 0,001; considerando a classificação motora das BSID-II e o percentil 10 da AIMS, obteve-se sensibilidade = 100%, especificidade = 48,64%, acurácia = 55,81%, índice kappa = 0,20 e p = 0,025. CONCLUSÕES: Os resultados sugerem boa concordância entre os instrumentos de avaliação no sexto mês. A melhor combinação para os parâmetros analisados é a utilização do percentil 5 da AIMS.OBJECTIVE: To ascertain the degree of agreement between a score for screening and another for diagnosis of motor development in 6-month old infants and to define the most appropriate cutoff point for screening. METHODS: A sectional study, enrolling asymptomatic full term newborns with gestational ages from 37 to 41 weeks, who were discharged from the maternity unit 2 days after birth and are resident in the Campinas area. Infants were excluded if they presented genetic syndromes, malformations, congenital infections, intensive care admission or low birth weight. The assessment instruments investigated were the Alberta Infant Motor Scale (AIMS) and the Bayley Scales of Infant Development II (BSID-II). Two cutoff points were evaluated for the AIMS, the 5th and 10th percentiles, and for the BSID-II infants were classified according to its motor index score (IS) as having inadequate (IS < 85, at least 1 standard deviation below the mean) or adequate performance (IS > 85, above the mean minus 1 standard deviation). RESULTS: The study sample comprised 43 infants. Six infants (14.00%) exhibited inadequate motor performance. Using the BSID-II motor classification and the 5th percentile AIMS cutoff, sensitivity was 100%, specificity 78.37%, accuracy 81.39%, kappa index 0.50 and p < 0.001; whereas, using the BSID-II motor classification and the 10th percentile AIMS cutoff, sensitivity was 100%, specificity 48.64%, accuracy 55.81%, kappa index 0.20 and p 0.025. CONCLUSIONS: The results suggest that concordance between the two 6-month assessment scales is good. The parameters employed are best combined using the 5th percentile AIMS cutoff point.

Abstract: International audience; An energy management system optimizes the self-consumption of a residential photovoltaic installation, and the performance losses due to production uncertainties are evaluated. The specific case under study is an individual home equipped with photovoltaic (PV) panels where only an Electric Water Heater (EWH) is manipulated, and the rest of the appliances represent a fixed load. By formulating the problem of maximizing self-consumption as an unconstrained optimization problem, a novel and computationally efficient optimization algorithm has been proposed. The next step was to numerically evaluate the performance of this EWH management strategy under various PV power production scenarios, generated through a presented methodology. The reference baseline is a rule-based controller using a most likely forecast of PV production. Simulations performed in Dymola over 10 months demonstrate that, at a 30-minute timestep, the impact of a “perfect” PV production forecast is negligible compared with the impact of the choice of the control algorithm. Besides, a most likely forecast is good enough for the proposed algorithm to reach high self-consumption levels. Indeed, although the proposed optimization based on a most likely forecast yields an increase of 10 points of self-consumption compared to the baseline, only an additional 2 points of increase can be reached using “perfect” production information.

Abstract: Background Frailty is a state of increased vulnerability to stressors, associated to poor health outcomes. The aim of this study was to design and introduce a Frailty Index (FI; according to the age-related accumulation of deficit model) in a large cohort of hospitalized older persons, in order to benefit from its capacity to comprehensively weight the risk profile of the individual. Methods Patients aged 65 and older enrolled in the REPOSI register from 2010 to 2016 were considered in the present analyses. Variables recorded at the hospital admission (including socio-demographic, physical, cognitive, functional and clinical factors) were used to compute the FI. The prognostic impact of the FI on in-hospital and 12-month mortality was assessed. Results Among the 4488 patients of the REPOSI register, 3847 were considered eligible for a 34-item FI computation. The median FI in the sample was 0.27 (interquartile range 0.21–0.37). The FI was significantly predictive of both in-hospital (OR 1.61, 95%CI 1.38–1.87) and overall (HR 1.46, 95%CI 1.32–1.62) mortality, also after adjustment for age and sex. Conclusions The FI confirms its strong predictive value for negative outcomes. Its implementation in cohort studies (including those conducted in the hospital setting) may provide useful information for better weighting the complexity of the older person and accordingly design personalized interventions.

Abstract: Endothelial microparticles (EMPs) are markers of endothelial injury and activation. The role of EMPs in arterial hypertension is not well understood and EMPs are increased both in arterial hypertension and coronary artery disease (CAD). The data presented here show EMPs as defined by CD31+/41−, CD62e+, and CD144+ surface markers and vascular hemodynamic parameters including office and central blood pressure, heart rate, aortic augmentation index, pulse wave velocity, flow-mediated dilation, nitroglycerin-mediated dilation, brachial artery diameter, hyperemic wall shear stress, and laser Doppler perfusion of the cutaneous microcirculation of normotensives and hypertensives with and without CAD.

Abstract: Background: Patients who undergo trans-catheter aortic valve replacement (TAVR) may have concomitant aortic aneurysms. We sought to clarify the incidence of aortic aneurysms and its impact on clinical outcomes among patients undergoing TAVR. Methods: We performed a retrospective analysis of patients with severe symptomatic aortic stenosis who underwent TAVR from January 2012 to June 2016. Multi-detector computer tomography (MDCT) was performed on all patients and images were reviewed to identify thoracic and abdominal aortic aneurysms (TAA and AAA). The incidence of vascular complications (VARC-2 definition), and in-hospital and 6-month major adverse cardiac events (MACE) defined as a composite of all-cause mortality, cardiac arrest, myocardial infarction, and stroke were recorded. Results: Among 232 patients included in the analysis, 22 patients (9.5%) had aortic aneurysms (11 had AAA, 8 had TAA, and 3 had both). Patients with aortic aneurysms had a higher, albeit statistically insignificant, rate of smoking history (63.6% vs. 42.9%, p = 0.062). Both groups of patient predominantly underwent TAVR via trans-femoral access (72.7% vs. 71.4%, p = 0.90). The incidence of vascular complications was similar between the two groups (9.1% vs. 10.5%, p = 1.0). Patients with aortic aneurysms had a similar in-hospital MACE (4.5% vs. 6.2%, p = 1.0) and 6-month MACE (9.1% vs. 9.0%, p = 1.0) compared to those without aneurysms. Conclusions: In our patient cohort, 9.5% of patients who underwent TAVR had concomitant aortic aneurysms. Patients with aortic aneurysms had similar incidence of vascular complications as well as in-hospital and 6-month MACE compared to those without. Abstract: Prior theory on creativity argues that people can be highly creative when they begin with a well-defined problem and then engage in a range of creative activities as they develop novel and useful solutions to the problem. However, there is a growing body of research supporting an alternative model to creativity, in which people can also be highly creative by first generating ideas without a clear problem in mind, and then evaluating those ideas across numerous problem domains in an effort to discover a problem that can be solved by the ideas. In this paper, we resolve the apparent contradiction between these problem-first and idea-first models of the creative process by examining the role that constraints play in shaping the creative process. We begin by developing a typology of constraints that differentiates between two types of constraint and two sources of constraint. Then, we integrate the two disparate models of the creative process into a new dynamic problem-solving model. Finally, we show how particular confluences of constraint systematically shape the activities that people engage in during the dynamic problem-solving process, thereby facilitating two effective modes of problem solving, which we call deliberate problem solving and emergent problem solving.

Abstract: Background: Pneumonia causes more deaths than any other infectious disease, especially in older patients with multiple chronic diseases. Recent studies identified a low functional status as prognostic factor for mortality in elderly patients with pneumonia while contrasting data are available about the role of diabetes. The aim of this study was to evaluate the in-hospital, 3-month and 1-year mortality in elderly subjects affected by pneumonia enrolled in the RePoSi register.Methods: We retrospectively analyzed the data collected on hospitalized elderly patients in the frame of the REPOSI project. We analyzed the socio-demographic, laboratory and clinical characteristics of subjects with pneumonia. Multivariate logistic analysis was used to explore the relationship between variables and mortality.Results: Among 4714 patients 284 had pneumonia. 52.8% were males and the mean age was 80 years old. 19.8% of these patients had a Barthel Index <= 40 (p < 0.0001), as well as 43.2% had a short blessed test >= 10 (p < 0.0117). In these subjects a significant CIRS for the evaluation of severity and comorbidity indexes (p < 0.0001) were present. Although a higher fasting glucose level was identified in people with pneumonia, in the multivariate logistic analysis diabetes was not independently associated with in-hospital, 3-month and 1-year mortality, whereas patients with lower Barthel Index had a higher mortality risk (odds ratio being 9.45, 6.84, 19.55 in hospital, at 3 and 12 months).Conclusion: Elderly hospitalized patients affected by pneumonia with a clinically significant disability had a higher mortality risk while diabetes does not represent an important determinant of short and long-term outcome.

Abstract: Background: Chronic pain is a frequent characteristic of elderly people and represents an actual and still poorly debated topic.Objective: We investigated pain prevalence and intensity, and its pharmacological therapy in elderly patients hospitalized in 101 internal medicine wards.Methods: Taking advantage of the "REgistro POliterapie Societa Italiana Medicina Interna" (REPOSI), we collected 2535 patients of whom almost a quarter was older than 85 years old. Among them, 582 patients were affected by pain (either chronic or acute) and 296 were diagnosed with chronic pain.Results: Patients with pain showed worse cognitive status, higher depression and comorbidities, and a longer duration of hospital stay compared to those without pain (all p<.0366). Patients with chronic pain revealed lower level of independency in their daily life, worse cognitive status and higher level of depression compared to acute pain patients (all p<.0156). Moreover, most of them were not treated for pain at admission (73.4%) and half of them was not treated with any analgesic drug at discharge (50.5%). This difference affected also the reported levels of pain intensity. Patients who received analgesics at both admission and discharge remained stable (p=.172). Conversely, those not treated at admission who received an analgesic treatment during the hospital stay decreased their perceived pain (p<.0001).Conclusions: Our results show the need to focus more attention on the pharmacological treatment of chronic pain, especially in hospitalized elderly patients, in order to support them and facilitate their daily life after hospital discharge.

Abstract: International audience; Body segment parameters (BSP) for each body's segment are needed for biomechanical analysis. To provide population-speciﬁc BSP, precise estimation of body's segments volume and density are needed. Widely used uniform densities, provided by cadavers' studies, did not consider the air present in the lungs when determining the thorax density. The purpose of this study was to propose a new uniform thorax density representative of the living population from 3D external body shape modeling. Bi-planar X-ray radiographies were acquired on 58 participants allowing 3D reconstructions of the spine, rib cage and human body shape. Three methods of computing the thorax mass were compared for 48 subjects: (1) the Dempster Uniform Density Method, currently in use for BSPs calculation, using Dempster density data, (2) the Personalized Method using full-description of the thorax based on 3D reconstruction of the rib cage and spine and (3) the Improved Uniform Density Method using a uniform thorax density resulting from the Personalized Method. For 10 participants, comparison was made between the body mass obtained from a force-plate and the body mass computed with each of the three methods. The Dempster Uniform Density Method presented a mean error of 4.8% in the total body mass compared to the force-plate vs 0.2% for the Personalized Method and 0.4% for the Improved Uniform Density Method. The adjusted thorax density found from the 3D reconstruction was 0.74 g/cm 3 for men and 0.73 g/cm for women instead of the one provided by Dempster (0.92 g/cm 3 ), leading to a better estimate of the thorax mass and body mass.
