Introduction: This study aimed to assess the relationship between the healthy eating index (HEI) and depression symptoms in an older adult population using the Chinese Longevity Health Survey. Methods: The HEI was constructed based on the frequency of intake of 13 dietary variables, with higher scores indicating better diet quality. Depression symptoms was assessed by the Center for Epidemiologic Studies Depression-10 scale. We used multivariate binary logistic regression to explore the association between the HEI and depression symptoms and reported as odds ratio (OR) and 95 % confidence interval (95 % CI). Results: A total of 12,078 older adults were included. The average HEI for this group of older adults was 26.4 ± 7.1. After adjusting for covariates, compared with participants in the lowest quartile of a healthy eating index, those in the second to the fourth quartile group had a 0.93- (95 % CI: 0.83 to 1.03), 0.84- (95 % CI: 0.75 to 0.95) and 0.69- (95 % CI: 0.62 to 0.77) fold risk of depression symptoms, respectively. Restricted cubic spline curves showed that the risk of depression symptoms decreased with increasing HEI, especially above 26, and for each unit increase in the HEI, the risk of depression symptoms in older adults decreased by 3 % (OR = 0.97; 95 % CI: 0.96 to 0.99). Conclusions: This study found an association between higher levels of the HEI and reduced risk of depression symptoms in older Chinese adults. This result emphasizes the need to focus on dietary diversity in older adults to promote healthy aging.

Introduction: Given that few studies have examined the relationship between healthy eating and anxiety in older adults, this study aimed to assess the relationship between the healthy eating index and anxiety in an older adult population using the Chinese Longevity Health Survey (CLHLS). Methods: A national cross-sectional study from the CLHLS in 2018. The healthy eating index was constructed based on the frequency of intake of 13 dietary variables. Anxiety was assessed by the Generalized Anxiety Disorder scale. We used multivariate binary logistic regression to explore the association between the healthy eating index and anxiety and reported as odds ratio (OR) and 95 % confidence interval (95 % CI). Results: A total of 13,873 older adults were included in the analysis after excluding participants with missing key variables. After adjusting for potential confounders, compared with participants in the lowest quartile of a healthy eating index, those in the second to the fourth quartile group had 4.7 %, 20.7 %, and 28.4 % lower odds of anxiety compared with those in the first quartile, respectively. Restricted cubic spline curves showed that anxiety risk decreased with increasing healthy eating index, and for each unit increase in the healthy eating index, the risk of anxiety in older adults decreased by 2.3 % (OR = 0.977; 95 % CI 0.970–0.985). Discussion: In Chinese older adults, a healthy diet was associated with lower anxiety. Although prospective studies are still needed to confirm these associations in older populations, this result emphasizes the need to focus on dietary diversity in older adults to promote healthy aging.

Summary: Biological age (BA) has been proposed to evaluate the aging status instead of chronological age (CA). Our study shows evidence that there might be multiple “clocks” within the whole-body system: systemic aging drivers/clocks overlaid with organ/tissue-specific counterparts. We utilize multi-omics data, including clinical tests, immune repertoire, targeted metabolomic molecules, gut microbiomes, physical fitness examinations, and facial skin examinations, to estimate the BA of different organs (e.g., liver, kidney) and systems (immune and metabolic system). The aging rates of organs/systems are diverse. People’s aging patterns are different. We also demonstrate several applications of organs/systems BA in two independent datasets. Mortality predictions are compared among organs' BA in the dataset of the United States National Health and Nutrition Examination Survey. Polygenic risk score of BAs constructed in the Chinese Longitudinal Healthy Longevity Survey cohort can predict the possibility of becoming centenarian.

There has been a plethora of studies on urbanization and older adults, and more recent ones on how older adults adapt to this process with their cognitive competence. Yet it has been unclear about the relationship between them, like how the level and rate of urbanization affect the cognitive function among older adults. This study sourced, formed, and analyzed a set of geospatial big datasets from different sources, such as the Chinese Longitudinal Healthy Longevity Survey (CLHLS) data, and the NPP/VIIRS nighttime light (NTL) data. Results showed a generally negative linear association between the rate of urbanization and cognitive performance among older adults in China. The "U" shaped non-linear relationship between urbanization level and cognitive function, as well as the tipping point, were identified. At the same time, it should be noted that mediators such as education, physical activity, social activity, and community elderly service might be able to mitigate these negative associations. Furthermore, older adults living in eastern regions or urban areas appeared to have better cognitive function than those living in mid-western regions or rural areas in China. Background: Cohort evidence linking long-term survival with exposure to multiple air pollutants (e.g., fine particulate matter [PM 2.5 ] and ozone) was extensively sparse in low- and middle-income countries, especially among older adults. This study aimed to investigate potential associations of long-term exposures to PM 2.5 and ozone with all-cause mortality in Chinese older adults., Methods: A dynamic nationwide prospective cohort comprising 20,352 adults aged ≥65 years were enrolled from the Chinese Longitudinal Healthy Longevity Study and followed up through 2005-2018. Participants' annual exposures to warm-season ozone and year-round PM 2.5 were assigned using satellite-derived spatiotemporal estimates. A directed acyclic graph (DAG) was developed to identify confounding variables. Associations of annual mean exposures to PM 2.5 and ozone with mortality were evaluated using single- and two-pollutant Cox proportional hazards models, adjusting for time-dependent individual risk factors and ambient temperature., Results: During 100 thousand person-years of follow-up (median: 3.6 years), a total of 14,313 death events occurred. The participants were averagely aged 87.1 years at baseline and exposed to a wide range of annual average concentrations of warm-season maximum 8-hour ozone (mean, 54.4 ppb; range, 23.3-81.6 ppb) and year-round PM 2.5 (mean, 65.5 μg/m 3 ; range, 10.1-162.9 μg/m 3 ). Approximately linear concentration-response relationship was identified for ozone, whereas significant increases in PM 2.5 -associated mortality risks were observed only when concentrations were above 60 μg/m 3 . Rises of 10 ppb in ozone and 10 µg/m 3 in PM 2.5 above 60 µg/m 3 were associated with increases in all-cause mortality of 13.2% (95% confidence interval [CI]: 10.2-16.2%) and 6.2% (95% CI: 4.6-7.7%) in DAG-based single-pollutant model, and of 9.7% (95% CI: 6.6-13.0%) and 5.3% (95% CI: 3.7-6.9%) in DAG-based two-pollutant model, respectively. Background: Previous studies have described an association between single time point calf circumference (CC) and mortality. Evidence of associations between CC change and mortality is lacking. We aimed to determine the relationship between the CC change over time and all-cause mortality., Methods: We conducted a retrospective cohort study of 906 participants in the 2014-2018 Chinese Longitudinal Healthy Longevity Survey (CLHLS). Restricted cubic splines were used to analyze associations between the relative CC change (exposure) and all-cause mortality (primary outcome). When there was evidence of non-linearity, a piecewise Cox regression model was next fitted, adjusting for sociodemographic characteristics, health behaviours, health status and baseline CC., Results: The mean (SD) age was 83.8 (12.2) years old, and 50.2% (455/906) of participants were male. We observed a U-shaped association between the relative CC change (%) and all-cause mortality (P for non-linearity <0.001). Participants with stable CC over time had the lowest risk of death. After adjusting for covariates, when CC decreased over time, the hazard ratio per 10% higher in CC change was 1.32 (1.01-1.69). When CC increased over time, the hazard ratio per 10% higher in CC change was 1.35 (1.10-1.66)., Conclusion: Stable CC over time was associated with the lowest mortality risk. Objectives: The aim of this study was to estimate and project the trend in long-term care (LTC) costs for home and community-based services as a percentage of gross domestic product in China between 2005 and 2050., Design: Longitudinal., Setting and Participants: We used 61,249 observations from 37,702 adults age ≥65 years from waves 2005, 2008, 2011, 2014, and 2018 of the Chinese Longitudinal Healthy Longevity Surveys., Methods: LTC costs for home and community-based services consisted of the monetary value of time spent on LTC and the direct LTC cost. We used the age-sex-residence-specific weights provided by the Chinese Longitudinal Healthy Longevity Surveys to estimate the LTC costs from 2005 to 2018. We used a component-based model to project LTC costs, in which the 2-part model was used to estimate the average LTC costs and the multi-state Markov model approach was used to project the future population by age, sex, and disability state., Results: The percentage of older adults with disabilities was projected to increase from 6.1% in 2005 to 7.5% in 2020 and 9.6% in 2050. The total LTC cost for home and community-based services were projected to increase from 0.3% of gross domestic product in 2005 to 0.7% in 2020 and to 6.4% in 2050., Conclusions and Implications: Policymakers in China should take urgent actions to delay the onset of disabilities among older adults, which would curb the increasing LTC costs and maintain the sustainability of the LTC policies., (Copyright © 2022 AMDA – The Society for Post-Acute and Long-Term Care Medicine. Objectives: This study aimed to identify the heterogeneous disability trajectories among older Chinese adults and examine the association between disability trajectories and health care service utilization., Design: Prospective cohort study., Setting and Participants: A community-based study including older adults aged ≥65 years from the Chinese Longitudinal Healthy Longevity Survey., Methods: Disability was assessed by the difficulties in activities of daily living and instrumental activities of daily living between 2002 and 2018. Health care utilization was measured by the expenditures on outpatient and inpatient services in 2018. Growth mixture modeling was conducted to estimate heterogeneous disability trajectories. A 2-part model was used to analyze the association of disability trajectories and health care utilization. Covariates were included based on Andersen's behavioral model., Results: Three classes of disability trajectories were identified: the progressive (7.9%), late-onset (13.7%), and normal classes (78.4%). Older adults who followed the late-onset trajectory of disability were more likely to use inpatient services compared with the normal class (odds ratio = 1.47, P < .010), after controlling potential confounders. Compared with the normal class, the progressive class on average spent US$145.94 more annually (45.2% higher) on outpatient services (P < .010) and $738.99 more annually (72.6% higher) on inpatient services (P < .001); the late-onset class reported higher annual expenditures on outpatient and inpatient services of $215.94 (66.9% higher) and $1405.00 (138.0% higher), respectively (all P < .001)., Conclusions and Implications: Heterogeneous disability trajectories exhibited distinct health care service utilization patterns among older Chinese adults. Older adults affected by late-onset disability incurred the highest health care needs. Emerging evidence in North America and Europe suggests that traffic-related air pollution (TRAP) adversely affects cognition. However, little is known about this relationship in people living in low- and middle-income countries (LMICs). It is also unknown whether indoor air pollution can modify the effect of TRAP. We derived data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS), a prospective cohort study, including 11,187 individuals of 82.0 years old (53.9% female). We ascertained residential proximity to major roadways based on self-reports and assessed cognitive function using the Mini-Mental State Examination (MMSE). We used cooking fuel type and home ventilation as proxies for indoor air pollution. We examined the associations between major road proximity and cognitive impairment using multivariable logistic regressions, controlling for demographic, lifestyle, socioeconomic status, and chronic conditions. We did subgroup analyses and assessed the potential interaction effect. The prevalence of cognitive impairment was 33.4%. The adjusted odds ratios of cognitive impairment were 1.20 (1.05, 1.35), 1.26 (1.09, 1.46), 0.99 (0.84, 1.17), and 1.05 (0.88, 1.25) for individuals living <50 m, 50-100 m, 101-200 m, and 201-300 m compared to those living >300 m from a major roadway. In dichotomized analyses, the risk (OR) associated with living closer to roadways was greater in participants who did not implement any ventilation during cooking (compared to those using natural or mechanical ventilation, 1.86 [1.31-2.65] vs. 1.16 [1.03-1.26], P for interaction = 0.001) and in solid fuel users (compared to clean fuel users, 1.37 [1.13-1.67] vs. 1.13 [1.04-1.21], P for interaction = 0.028). The associations were robust in a set of sensitivity analyses. The results suggested that living closer to major roadways was associated with an increased risk for cognitive impairment in older adults in China, indicating an adverse TRAP effect. A growing body of research suggests that air pollution negatively affects specific health outcomes, but how long- and short-term exposure to air pollution are associated with frailty is unclear. Using longitudinal data from adults aged 65 and over from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) linked with air quality index data, we model a frailty score according to the city-level of air pollution exposure, adjusting for individual socio-demographic factors and city-level indicators. All models show increased frailty with higher exposure to air pollution in one year prior to the interview, when controlling for short-term fluctuations. Moreover, elderly people living in areas where air pollution increased over the follow-up had larger increases in frailty scores than those where air pollution was relatively constant.
