Purpose: The aim of this study was to compare the functional and radiographic outcomes of dorsally displaced distal radius fractures treated by closed reduction plaster cast fixation (CRPCF) and external fixation (EF) in patients 65 years and older., Methods: This retrospective and nonrandomized study comprised 46 consecutive patients older than 65 years who had distal radial fractures. Patients were divided into 2 groups according to treatment: a CRPCF group and an EF group. All the fractures were dorsally displaced and AO/ASIF type A or C, without articular stepoff or gap. Cases were evaluated based on the criteria of Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaire score, wrist range of motion, and radiologic results., Results: The mean follow-up period was 25.1 months. Union was achieved in all cases. Although it was not a statistically significant difference, posttreatment complications were more common in the CRPCF group (10 patients) than in the EF group (7 patients). In the CRPCF group, most complications were discomfort from the cast, whereas in the EF group, most were pin site infections. The average wrist extension and ulnar deviation (clinically) and palmar tilt and radial height (radiologically) were statistically better in the EF group at the final follow-up. The mean DASH scores were 20.3 in the CRPCF group and 21.9 in the EF group. There was no statistically significant difference in the DASH scores; in wrist flexion, radial deviation, pronation, supination, grip strength, or pinch strength (clinically); or in ulnar variance or radial inclination (radiologically). There was no correlation between the DASH scores and palmar tilt and ulnar variance., Conclusions: We concluded that both CRPCF and EF are useful methods for distal radius fractures in elderly patients. Ninety-two patients with displaced Colles' fractures were followed prospectively after manipulation under regional anaesthesia. Radiographs were taken before and after manipulation and after 3 months when a functional assessment was also made. A correlation was sought between outcome and radiological measurements. Final radial shortening of 3mm or more was associated with a significantly worse functional outcome after 3 months (P < 0.001). Where the initial radial shortening was 3 mm or more, the probability of malunion was 65% whereas with less than 3mm, the probability of malunion was 28% (P < 0.001). With 5 mm or more of radial shortening at presentation, the probability of malunion was 73% (P < 0.01). The decision to manipulate remains a matter of judgement but a high failure rate renders simple manipulation and plaster cast fixation a poor treatment option in fractures with 5mm or more of radial shortening at presentation.

In the author's current area of practice the use of either the Bier's block or haematoma block for manipulating distal radial fractures seems to have been based mostly on the A&E consultant's preference. The purpose of this literature review was to determine if there was sufficient evidence on the advantages and disadvantages of each method of regional anaesthesia to advocate the general use of one and the exclusion of the other. When reviewing the literature the author specifically looked in to three key areas to answer this question: 1) the patient's perception of pain experienced during and after the manipulation; 2) patient safety i.e. the documented risks of the anaesthetic type used; and 3) the success of the manipulation as determined by repeat radiographs immediately after the application of a plaster of Paris cast.

Twenty-five sedentary, low-demand patients older than 60 years were retrospectively evaluated for function and radiographic results following nonoperative treatment of displaced distal radius fractures. The mean age was 72 years and the average follow-up period was 34 months. All patients had radiographic and functional evaluations. The radiographic assessment included a scoring system based on measurements of the final dorsal angle, loss of radial length, and the final radial inclination. The development of radiocarpal and distal radioulnar joint arthrosis was also investigated. The functional assessment included subjective and objective criteria. Overall satisfaction, ability to return to previous activity level or occupation, concern over wrist appearance, and a functional task questionnaire were part of the subjective assessment. Objective assessment included neuromuscular evaluation and measurements of range of motion and grip strength. The final radiographic scores revealed that 6 patients (24%) had excellent results, 11 (44%) had good results, 2 (8%) had fair results, and 6 (24%) had poor results. Six of 10 patients with intra-articular fractures developed progressive wrist arthrosis. Two of 6 patients with radiographic arthrosis had unsatisfactory functional outcome. Five of 8 patients with intra-articular fractures that healed with a residual stepoff >/=2 mm had satisfactory functional outcome; 3 of these 8 patients had unsatisfactory functional outcome. The functional assessment revealed that 22 patients (88%) had excellent or good results and 3 (12%) had fair or poor results. The radiographic outcome did not correlate with the functional outcome. Twenty-three of 25 patients (92%) were satisfied with the overall outcome of the treatment and 22 (88%) were able to return to their previous activity level or occupation. Despite an obvious clinical deformity in over half of the patients, none were unhappy with the clinical appearance of the wrist. According to the functional task questionnaire, jar opening was by far the most difficult task to perform after fracture healing. Functional outcome was satisfactory in most cases; a high level of personal satisfaction and return to previous activity level was observed, regardless of the radiographic result. Nonoperative treatment of distal radius fractures yields satisfactory outcome, especially in those with low functional demands. It also is indicated in poor operative candidates. Twenty-five wrists with comminuted, displaced, intra-articular fractures of the distal radius were prospectively treated with an adjustable external fixator for an average of 51 days. Twenty fractures (80% [20 of 25] were available for follow-up) in 16 adults (mean age, 34 years) were treated with the Wrist Jack external fixator system (Hand Biomechanics Lab, Sacramento, CA) and evaluated at a mean follow-up period of 25 months. Ten patients (12 fractures) sustained high-energy trauma with multiple injuries, while 6 patients (8 fractures) sustained isolated distal radius fractures. Percutaneous pins supplemented the fixation in 6 fractures. All fractures were reduced to restore articular congruity to within 1 mm. At follow-up, 5% were excellent, 75% good, 20% fair, and none as poor using the demerit point system of Gartland and Werley as modified by Sarmiento. Grip strength averaged 80% of the unaffected limb. Seventeen of the 20 fractures showed some evidence of articular incongruity at follow-up evaluation. Restoration of palmar tilt, radial inclination, radial length, and range of motion were at acceptable values. Subjective analysis confirmed 85% of the patients to have only occasional pain or none at all and 15% to have some pain with weakness or limitation of motion. Two patients required additional surgery: 1 underwent a Darrach procedure and the other a tendon transfer for a rupture of the extensor pollicis longus tendon. Results suggest that an external fixator system provides an additional alternative to the surgical armamentarium for an otherwise difficult fracture fixation problem.

Thirty elderly patients with moderately displaced Colles' fractures were randomly assigned to manipulation under Bier's block or plaster immobilization alone. Moderately displaced was defined as 10 degrees to 30 degrees of dorsal angulation and less than 5 mm of radial shortening compared with the uninjured side. The groups were well matched for age, sex, fracture type and displacement; immobilization time and rehabilitation were standardized. The outcome measures were: radiological position at union, the functional score of Gartland and Werley, grip strength, cosmesis and algodystrophy assessment. There was no detectable difference between the groups in any of the outcome measures. Two-thirds of the correction of dorsal angulation achieved by manipulation was lost by 5 weeks. We conclude that up to 30 degrees of dorsal angulation and 5 mm of radial shortening may be accepted in selected elderly patients.

Clinical evaluation of outcome after Colles fracture has not been standardized. To assess the relative responsiveness of various clinical and questionnaire measures for the assessment of outcome after Colles fracture, 21 patients were surveyed on the day fracture immobilization was discontinued and again 3 months after that date with the following measures: a short form general health survey (SF-36), the Arthritis Impact Measurement Scale (AIMS2), the Brigham and Women's Hospital carpal tunnel questionnaire; pinch strength, grip strength, pressure sensibility, range of motion, and dexterity. Significant changes, all in the direction of improved health status, occurred in the following scales or measures: AIMS2 mobility, hand and finger function, arm function, household tasks, "arthritis" (fracture) pain, self-care, satisfaction, physical health, affect, and tension; Brigham function; SF-36 physical role and mental health; and grip, pinch, dexterity, and range of motion. The impairments that occur after Colles fracture are multidimensional and are only partially captured by traditional physical measures. Questionnaires such as the SF-36, AIMS2, and Brigham and Women's instruments provide a mechanism to capture the function and symptom dimensions objectively.

A new prefabricated brace for the functional treatment of Colles' fractures has been developed. It is applied at fracture reduction and maintains fracture position by the application of three-point loading. In a prospective randomized clinical trial treating 85 displaced Colles' fractures, with blind independent follow-up, the brace gave better functional results than conventional plaster treatment. The improved function was apparent up to 6 months after injury. Finger function and pinch strength were also better in the brace-treated patients. Anatomical results were similar in the two groups.

The treatment of Colles' fractures in the elderly comprises a heavy workload for both accident and orthopaedic departments. The initial management has important clinical and financial implications for patient and hospital. The demand is variable and the ability to respond must also be flexible. The choice of anaesthetic technique is therefore most important. In our experience, intravenous sedation with midazolam (a water soluble benzodiazepine) has proven to be safe and effective in providing good conditions for anatomical reduction of Colles' fractures on an outpatient basis.

In all, 85 per cent of Colles' fractures are clinically satisfactory after 10 years. Clinical results improve little after 3 months. Displaced intraarticular fracture, wrist deformity and algodystrophy are associated with an unsatisfactory outcome.

Intracast pressures were recorded in conventional plaster-of-Paris below-elbow casts and compared with a new prefabricated brace for the functional treatment of Colles' fractures. Interface pressures were consistently higher in the brace than in the conventional plaster cast throughout the treatment period, and demonstrated more specific loading to the fracture site.

Functional disability after a Colles' fracture was correlated to the degree of axial shortening of the radius measured at the site of distal radioulnar joint. According to the sum of demerit points, the functional end result was unsatisfactory (fair or poor) in 4% of the patients with an acceptable anatomic result, in 25% of the patients with grade 1 shortening (corresponding to a shortening of 3 to 5 mm), and in 31% of the patients with grade 2 shortening (shortening of more than 5 mm). The results suggest that even a minor axial shortening of the radius, without concomitant malalignment of the articular surface, carries an increased risk of permanent disability.

A prospective radiological and functional assessment has been performed on 235 consecutively treated displaced Colles' fractures. The age and sex of the patients, as well as the initial displacement, involvement of articular surface and the adequacy of reduction of the fracture, were all analysed. Because of their greater potential for collapse, the better reduced fractures tended to lose more position during treatment. Despite this, in general, the better the reduction, the better was the final anatomical position of the fractures. The functional result at 3 months was related to the severity of initial displacement of the fracture and, to a lesser extent, to the anatomical result. Involvement of the radiocarpal and the distal radioulnar joints did not seem to affect adversely the patients' rehabilitation, but this was slower and less complete in the elderly. At 6 months, 60 per cent of patients had good or excellent function in the hand and wrist.

This paper reports a prospective study of the treatment of Colles' fractures, either by bipolar fixation or a forearm cast. Bipolar fixation reduced the degree of secondary displacement and no patient required a repeat manipulation. The functional results at 6 months, as assessed by the Scheck system, showed a strong trend in favour of bipolar fixation, but did not reach 95 per cent confidence limits. The technique of bipolar fixation proved to be safe and suitable for use in a busy district general hospital, without the necessity of elaborate equipment.

A total of 136 patients required an anaesthetic to enable distal fractures of the forearm to be reduced. There were no significant differences in the numbers requiring remanipulation between the three methods employed: haematoma block, Bier's block or general anaesthesia. The advantages of the haematoma block are discussed.

The functional recovery from Colles' fracture was studied prospectively over a 1-year period in 106 patients randomized to treatment by plaster or external fixation. Although external fixation allows immediate mobilization of the wrist and leads to a much better anatomical result, these factors did not permit any improvement in the wrist's early functional recovery. One year following injury the grip strength of the fixator-treated group was significantly superior to that of the plaster-treated patients.

A simple modification to the method of regional anaesthesia known as Bier's block is presented. This was applied to patients with Colles' fractures. In a controlled trial the technique produced anaesthesia more quickly than the other method.

Two techniques for reduction of Colles' fractures were compared in a prospective, randomized study. With use of a newly designed dynamic bone alignment device, reduction manoeuvres were performed on 62 patients with Colles' fractures (group A) without any form of anaesthesia. In a control group of 54 patients (group B) Colles' fractures were reduced manually in the traditional way using local infiltration anaesthesia. In each group the pain experienced by the patients was recorded. Severe pain during the reduction was reported by 8 per cent of the patients in group A and by 35 per cent of those in group B. No complications occurred during or after dynamic reduction without anaesthesia. This method of treatment seems to be a very gentle one for the patients, with the advantage of causing less pain than treatment under local anaesthesia.

One hundred forty-five patients with fractures of the distal end of the radius (age range, 13 to 76 years; means, 61 years) were treated conservatively with a Chinese system of external fixation. Most of the fractures were of the extension and extraarticular type. The fixation system consists of four splints used in conjunction with three pressure pads and three or four slings. The system was used after successful manipulative reduction, and the patients were instructed to move their joints as soon as possible. The results, especially in elderly patients, were highly encouraging. We believe that this Chinese system of external fixation serves as a kind of functional brace.

Nine patients undergoing manipulation of Colles' fractures with varying doses of local anaesthetic injected into the fracture haematoma, were investigated by monitoring the arterial levels of lignocaine. Although the procedure was considered satisfactory by all patients, potentially toxic concentrations of lignocaine were found in the arterial circulation, reaching a maximum after manipulation of the fracture. A mean peak level of 2625 micrograms/l was found when 10 ml of 2 per cent lignocaine was used and a mean peak level of 846 micrograms/l in patients receiving 10 ml of 1 per cent lignocaine.
