Perception has long been envisioned to use an internal model of the world to explain the causes of sensory signals. However, such accounts have historically not been testable, typically requiring intractable search through the space of possible explanations. Using auditory scenes as a case study, we leveraged contemporary computational tools to infer explanations of sounds in a candidate internal generative model of the auditory world (ecologically inspired audio synthesizers). Model inferences accounted for many classic illusions. Unlike traditional accounts of auditory illusions, the model is applicable to any sound, and exhibited human-like perceptual organization for real-world sound mixtures. The combination of stimulus-computability and interpretable model structure enabled 'rich falsification', revealing additional assumptions about sound generation needed to account for perception. The results show how generative models can account for the perception of both classic illusions and everyday sensory signals, and illustrate the opportunities and challenges involved in incorporating them into theories of perception., (Copyright © 2024 The Authors. Objective: To determine the effect of recreational cannabis legalization (RCL) and/or recreational cannabis commercialization (RCC) on emergency department (ED) visits, hospitalizations, and deaths due to substance use, injury, and mental health among those aged 11 years and older., Methods: A systematic review of six electronic databases up to February 1, 2023. Original, peer-reviewed articles with interrupted time series or before and after designs were included. Four independent reviewers screened articles and assessed risk of bias. Outcomes with 'critical' risk of bias were excluded. Protocol registered on PROSPERO (# CRD42021265183)., Results: After screening and risk of bias assessment, 29 studies were included which examined ED visits or hospitalizations for cannabis use or alcohol (N = 10), opioid mortality (N = 3), motor vehicle fatalities or injury (N = 11), and intentional injury/mental health (N = 5). Rates or number of cannabis-related hospitalizations increased after RCL in Canada and the USA. Immediate increases in rates of cannabis-related ED visits were found after both RCL and RCC in Canada. Rates of traffic fatalities increased after RCL and RCC in certain jurisdictions in the USA., Conclusions: RCL was associated with increased rates of cannabis-related hospitalizations. RCL and/or RCC was associated with increased rates of cannabis-related ED visits, consistently shown across sex and age groups. The effect on fatal motor vehicle incidents was mixed, with observed increases found after RCL and/or RCC. The effect of RCL or RCC on opioids, alcohol, intentional injury, and mental health is not clear. We examined the effect of age on listeners' ability to orient attention to an item in auditory short-term memory (ASTM) using high-density electroencephalography, while participants completed a delayed match-to-sample task. During the retention interval, an uninformative or an informative visual retro-cue guided attention to an item in ASTM. Informative cues speeded response times, but only for young adults. In young adults, informative retro-cues generated greater event-related potential amplitude between 450 and 650 ms at parietal sites, and an increased sustained potential over the left central scalp region, thought to index the deployment of attention and maintenance of the cued item in ASTM, respectively. Both modulations were reduced in older adults. Alpha and low beta oscillatory power suppression was greater when the retro-cue was informative than uninformative, especially in young adults. Our results point toward an age-related decline in orienting attention to the cued item in ASTM. Older adults may be dividing their attention between all items in working memory rather than selectively focusing attention on a single cued item., (Copyright © 2018 Elsevier Inc. Background: Osteomalacia-inducing tumors (OIT) are mesenchymal tumors that characteristically secrete fibroblast growth factor 23, resulting in a paraneoplastic syndrome of hypophosphatemic osteomalacia. These tumors are known to occur in soft tissues and bones in various sites. It is very unusual for OITs to occur intracranially, with only 10 reported intracranial cases since their discovery in 1959. The most common intracrainal OITs are phosphaturic mesenchymal tumors and hemangiopericytomas. We report a case of hypophosphatemic osteomalacia caused by a tumor in the right anterior cranial fossa. We also hypothesize, based on our review of the literature, that this entity is underdiagnosed., Case Description: A 49-year-old woman had a history of a nonhealing ankle fracture that required repeated surgery over 3 years. She subsequently was found to have severe hypophosphatemia and evidence of osteomalacia together with multiple occult fractures. A diagnosis of tumor-induced osteomalacia was suspected. An elevated serum fibroblast growth factor 23 level confirmed the diagnosis. An octreotide scan that was performed to locate the responsible tumor revealed an area of avid uptake in the right frontal lobe. Magnetic resonance imaging showed a large right anterior fossa extra-axial mass. The patient was referred for surgical intervention and was cured clinically after surgical removal of the tumor. Pathologic examination revealed a phosphaturic mesenchymal OIT. Her phosphate levels returned to normal 3 weeks after surgery., Conclusions: The diagnosis of OIT should be considered in a case of severe hypophosphatemia and metabolic bone disease that is not explained by any other metabolic or hereditary disease. These tumors can occur intracranially and may be confused with a meningioma or a hemangiopericytoma. Patients with nonaneurysmal perimesencephalic subarachnoid hemorrhage (NAPSAH) have no discernible source for the bleeding and generally are considered to have a benign condition. Correctly diagnosing these patients is essential because a missed aneurysm can have catastrophic consequences. Those presenting with NAPSAH have a low risk of complications and better outcome than patients presenting with aneurysmal subarachnoid hemorrhage; however, a limited body of literature suggests that not all of these patients are able to return to their premorbid functional status. Clinical screens of cognitive status, such as the mini-mental status examination, suggest good recovery of these patients, although these tests may lack sensitivity for identifying deficits in this patient population. More comprehensive neuropsychologic testing in some studies has identified deficits in a wide range of cognitive domains at long-term follow-up in patients with NAPSAH. Because these patients often do not lose consciousness (and thus do not have substantial transient global ischemia) and they do not undergo a procedure for aneurysm repair, the cognitive sequelae can be explained by the presence of blood in the subarachnoid space. Working memory impairments are prevalent among survivors of aneurysmal subarachnoid hemorrhage (SAH), but few studies have examined specifically these impairments. Such an examination is important because working memory processes are vital for daily cognitive functioning. In the current study, patients with SAH and healthy control participants were administered the word-span and alpha-span test - experimental tests of working memory. In the word-span test, participants recalled increasingly longer word-lists, requiring the maintenance of information in mind. In the alpha-span test, participants recalled the word-lists in alphabetical order, requiring both the maintenance and manipulation of information. Patients with SAH were no different from healthy controls on a battery of standard neuropsychological measures or on the word-span test. They were, however, significantly impaired on the alpha-span test, suggesting a deficit in the manipulation components of working memory. That is, impairment resulting from SAH is present when a working memory task requires additional executive processing demands. This deficit in patients with SAH does not appear to be influenced by aneurysm location, suggesting that some of the effects of SAH on cognition are from diffuse rather than focal pathology; however, a larger sample size is needed to reinforce this claim., (© 2013. Background: Metastasis of a neuroendocrine tumor to the anterior pituitary is extremely rare., Methods: A 55-year-old woman presented with features suggestive of acromegaly and bone and joint pain. A bone scan suggested metastatic disease. A biopsy of the lumbar spinal lesions revealed a neuroendocrine tumor. Magnetic resonance imaging of the sella demonstrated a lesion with parasellar and suprasellar extensions. Given the need for tissue diagnosis and optic compression, the sellar lesion was removed via transsphenoidal hypophysectomy., Results: Histology of the biopsy indicated two different tumors in close association. One showed histological features of a pituitary adenoma, whereas the other was a neuroendocrine carcinoma. The pituitary adenoma was immunopositive for growth hormone (GH), whereas the neuroendocrine carcinoma was immunopositive for serotonin and bombesin and immunonegative for GH., Conclusions: This is the first report of a serotonin and bombesin immunopositive neuroendocrine tumor of unknown primary origin metastatic to a GH-secreting pituitary adenoma, resulting in acromegaly., (Copyright © 2012 Elsevier Inc.
