Abstract

On December 20, 1893, 3 years after von Mering and Minkowski showed that total pancreatectomy in dogs resulted in diabetes mellitus,1 Dr. P. Watson Williams in Bristol, England, grafted three fragments of a pancreas obtained from a freshly slaughtered sheep into the subcutaneous tissue of a 15-year-old boy in extremis, 5 months after clinical onset of diabetes.2 The recipient died 3 days later, not of complications from the unsuccessful transplant but of unrelenting acidosis, a sequela of basically untreated diabetes. At autopsy, the recipient’s own pancreas was shriveled and sections showed little but fibrous stroma. According to Williams, the history and the postmortem examination left little doubt that the patient had “pancreatic diabetes” a case that “presented all the conditions that might lead one to hope for beneficial results from successful grafting of the pancreas, if anything can be hoped for in this direction at all.” He was not discouraged, and further stated that “failure was possibly due to obtaining the graft from a sheep that had been killed by bleeding....If ever I felt justified again in resorting to pancreatic grafts in a similar case, I should obtain them from a living animal anesthetized or dispense with the anesthetic altogether.”

Abstract

Pancreas transplants using living donors (LDs) can be done safely and successfully in carefully selected donors and recipients. Worldwide, since 1979, more than 150 LD pancreas transplants have been done; over the last decade, the overall 1-year graft survival rates have exceeded 85%. A meticulous donor workup, per standard evaluation criteria, remains key to a low rate of metabolic and surgical complications in LDs. Such transplants should be considered for (i) candidates with an identical twin or a human leukocyte antigen (HLA)-identical sibling or (ii) candidates with high panel-reactive antibody levels and a suitable LD. The use of LDs decreases the number of deaths of diabetic patients on the waiting list, helps to overcome the organ shortage, reduces mortality and morbidity posttransplant, and improves the quality of life for recipients who, pretransplant, suffered debilitating side effects of diabetes. In addition, the use of LDs (vs. deceased donors) reduces the risk of graft loss from rejection. An LD pancreas transplant remains relatively rare, done under very specific circumstances.

Abstract

A characteristic critical to the success of cell-based regenerative medicine approaches to treatment of disease is the ability of cells or tissue to engraft. Successful islet autotransplantation (IAT) following near-total or total pancreatectomy (TP) was the first clear proof that human islet tissue could engraft at an ectopic site and maintain insulin independence. This clinical observation holds promise for other regenerative medicine strategies aimed at treating diabetes. Follow-up observations have documented that if early insulin independence is achieved, then long-term function, 10 years or longer, is possible, indicating that intrahepatic β-cell residence is durable. Clinically, if pancreatectomy is to be done for benign diseases of the pancreas, IAT should accompany it, as the risks of surgical diabetes outweigh the risks of IAT. Surmounting the differences between IAT and allogeneic islet transplant may improve the therapeutic potential of cellular based replacement treatments for type 1 diabetes.

Abstract

International audience; Sediment core ER07 from Sermilik Fjord by Helheim Glacier in Southeast Greenland was analysedfor alkenones to document sea surface temperature (SST) changes over the past 100 years. Thealkenone SST values, ranging from 8 to 12°C, contrasts with colder values (0-4°C) obtained fromrecent hydrographic surveys inside the fjord. We suggest that advection of allochtonous alkenonesproduced in the warm Irminger Current waters circulating on the shelf likely accounts for thisdifference. The temperature range of the alkenone-derived record is similar to in situ observationsof 8-11°C on the shelf just outside Sermilik Fjord, and its variability over the past 100 yearsresembles the constructed variability over the shelf using remote instrumental data. This suggeststhat oceanographic changes on the adjacent shelf are linked to regional changes of the IrmingerCurrent and the East Greenland Current. The subsurface water heat content has previously beensuggested as an important control on Greenland outlet glacier stability and underlined by an episodeof warm subsurface waters ~ 1940 concurrent with markedly increased calving and retreat ofHelheim Glacier. Our results therefore suggest that alkenone-derived SST time series from highsedimentation rate glacial fjords may provide a new approach for reconstruction of past changes ofshelf water properties and variability around Greenland.

Abstract

Structured Systems Analysis and Design (SSAD) is a methodology currently used by many data processing professionals in the analysis, design and implementation of information systems (IS). The purpose of this paper is to present a methodology to model and simulate dynamic system requirements and specifications, and to suggest the necessary enhancements to the systems notation of SSAD. The methodology and enhancements to the tools of SSAD are referred to as dynamic systems, analysis, design and evaluation (DSADE). The enhanced methodology includes experimental design and analysis of simulation runs of the models. In addition, the use of the enhancements to one of the graphical tools of SSAD, data flow diagrams (DFD), is explained. An example analysis of a banking information system illustrates the methodology and techniques.
