Objective: Previous evidence in epileptic subjects has shown that theta (about 4-7Hz) and gamma rhythms (about 40-45Hz) of hippocampus, amygdala, and neocortex were temporally synchronized during the listening of repeated words successfully remembered (Babiloni et al., 2009). Here we re-analyzed those electroencephalographic (EEG) data to test whether a parallel increase in amplitude of late positive event-related potentials takes place., Methods: Intracerebral electroencephalographic (EEG) activity had been recorded in five subjects with drug-resistant temporal lobe epilepsy, undergoing pre-surgical evaluation. During the recording of the intracerebral EEG activity, the subjects performed a computerized version of the Rey auditory verbal learning test (RAVLT). They heard the same list of 15 common words for five times. Each time, immediately after the listening of the list, the subjects were required to repeat as many words as they could recall., Results: We found that late positive event-related potentials (ERPs) peaking at about 350ms post-stimulus in amygdala, hippocampus, and occipital-temporal cortex had a higher amplitude during the listening of the repeated words that were subsequently recalled than for those that were not recalled., Conclusions: Late positive ERPs reflect a functional mechanism implemented in a human brain network spanning amygdala, hippocampus, and occipital-temporal cortex which is at the basis of the memorization processes of verbal materials., Significance: This ERP component is a promising neuromarker of successful memorization of repeated words in humans., (Copyright 2010 International Federation of Clinical Neurophysiology. Objective: The "neural efficiency" hypothesis posits that neural activity is reduced in experts. Here we tested the hypothesis that compared with non-athletes, elite athletes are characterized by a reduced cortical activation during simple voluntary movement and that this is reflected by the modulation of dominant alpha rhythms (8-12 Hz)., Methods: EEG data (56 channels; EB-Neuro) were continuously recorded in the following right-handed subjects: 10 elite karate athletes and 12 non-athletes. During the EEG recordings, they performed brisk voluntary wrist extensions of the right or left hand (right movement and left movement). The EEG cortical sources were estimated by standardized low-resolution brain electromagnetic tomography (sLORETA) freeware. With reference to a baseline period, the power decrease of alpha rhythms during the motor preparation and execution indexed the cortical activation (event-related desynchronization, ERD)., Results: During both preparation and execution of the right movements, the low- (about 8-10 Hz) and high-frequency alpha ERD (about 10-12 Hz) was lower in amplitude in primary motor area, in lateral and medial premotor areas in the elite karate athletes than in the non-athletes. For the left movement, only the high-frequency alpha ERD during the motor execution was lower in the elite karate athletes than in the non-athletes., Conclusions: These results confirmed that compared with non-athletes, elite athletes are characterized by a reduced cortical activation during simple voluntary movement., Significance: Cortical alpha rhythms are implicated in the "neural efficiency" of athletes' motor systems., (2009 International Federation of Clinical Neurophysiology. Objective: Are obese subjects characterized by a reduction of attentional cortical responses to the enlargement of food or body images?, Methods: Electroencephalographic data were recorded in 19 obese and 15 normal-weight adults during an "oddball" paradigm. The subjects were given frequent (70%) and rare (30%) stimuli depicting faces (FACE), food (FOOD), and landscapes (CONTROL), and clicked the mouse after the rare stimuli. These stimuli depicted the same frequent stimuli graphically dilated by 25% along the horizontal axis. Bioelectrical impedance indexed subjects' body fat percentage. Cortical attentional responses were probed by the difference between positive event-related potentials peaking around 400-500ms post-stimulus for the rare minus frequent stimuli (P300). Low resolution electromagnetic source tomography (LORETA) estimated P300 sources., Results: In the FOOD condition, the amplitude of medial prefrontal P300 sources (Brodmann area 9) was lower in the obese than normal-weight subjects, and there was a negative correlation between the body fat percentage and the amplitude of these sources in all subjects as a single group., Conclusions: These results disclose that prefrontal attentional processes to food size are abnormal in obese subjects., Significance: The present study motivates future research evaluating the effects of cognitive rehabilitation in obese subjects.

Objective: Here we tested the hypothesis that in normal weight subjects, attentional cortical responses to the enlargement of faces are related to features of body weight, as a basis for future studies on the role of neurocognitive mechanisms in eating and weight disorders., Methods: Electroencephalographic data were recorded in 15 normal weight adults during a visual "oddball" paradigm. The subjects were given frequent (70%) and rare (30%) stimuli depicting faces (FACE), food (FOOD), and landscapes (CONTROL). The task was to click the mouse after the rare stimuli. These stimuli depicted the same frequent stimuli graphically dilated by 25% along the horizontal axis. Analysis of bioelectrical impedance indexed subjects' body fat percentage. Cortical attentional responses were probed by the difference between positive event-related potentials peaking around 200-600 ms post-stimulus for the frequent minus rare stimuli (P300). LORETA estimated P300 cortical sources., Results: Main results showed that in the FACE condition, there was a negative correlation between the body fat percentage and the reaction time to the rare stimuli, and a positive correlation between the body fat percentage and the amplitude of prefrontal P300 sources (p<0.05)., Conclusions: These results disclose a relationship between body fat and prefrontal attentional processes to body image in normal weight adults., Significance: The present study motivates future research testing the hypothesis that this relationship might be altered in patients with eating and weight disorders.

Objective: Non-steroidal anti-inflammatory drugs such as ibuprofen have a protective role on risk of Alzheimer's disease (AD). Here we evaluated the hypothesis that long-term ibuprofen treatment affects cortical sources of resting electroencephalographic (EEG) rhythms in mild AD patients., Methods: Twenty-three AD patients (13 treated AD IBUPROFEN; 10 untreated AD PLACEBO) were enrolled. Resting EEG data were recorded before and 1 year after the ibuprofen/placebo treatment. EEG rhythms were delta (2-4 Hz), theta (4-8 Hz), alpha 1 (8-10.5 Hz), alpha 2 (10.5-13 Hz), beta 1 (13-20 Hz), and beta 2 (20-30 Hz). LORETA was used for EEG source analysis., Results: In the AD PLACEBO group, amplitude of delta sources was globally greater at follow-up than baseline. Instead, amplitude of delta sources remained stable or decreased in the majority of the AD IBUPROFEN patients. Clinical (CDR) but not global cognitive status (MMSE) reflected EEG results., Conclusions: These results suggest that in mild AD patients, a long-term ibuprofen treatment slightly slows down the progressive increment of delta rhythms as a sign of contrast against the neurodegenerative processes., Significance: They motivate future investigations with larger population and extended neuropsychological testing, to study the relationships among ibuprofen treatment, delta cortical sources, and higher order functions.

Is directionality of electroencephalographic (EEG) synchronization abnormal in amnesic mild cognitive impairment (MCI) and Alzheimer's disease (AD)? EEG data were recorded in 64 normal elderly (Nold), 69 amnesic MCI, and 73 mild AD subjects at rest condition (closed eyes). Direction of information flux within EEG functional coupling at electrode pairs was performed by directed transfer function (DTF) at delta (2-4 Hz), theta (4-8 Hz), alpha 1 (8-10 Hz), alpha 2 (10-12 Hz), beta 1 (13-20 Hz), beta 2 (20-30 Hz), and gamma (30-40 Hz). Parietal to frontal direction of the information flux within EEG functional coupling was stronger in Nold than in MCI and/or AD subjects, namely for alpha and beta rhythms. In contrast, the directional flow within inter-hemispheric EEG functional coupling did not discriminate among the three groups. These results suggest that directionality of parieto-to-frontal EEG synchronization is abnormal not only in AD but also in amnesic MCI.

Objective: In this study, we tested the hypothesis that a pre-stimulus brief (1 min) 10-Hz audio-visual flickering stimulation modulates alpha EEG rhythms and cognitive-motor performance in elite athletes and in non-athletes during visuo-spatial demands., Methods: Electroencephalographic (EEG) data were recorded (56 channels; EB-Neuro) in 14 elite fencing athletes and in 14 non-athletes during visuo-spatial-motor demands (i.e. subjects had to react to pictures of fencing and karate attacks). The task was performed after pre-stimulus 15- (placebo) or 10-Hz (experimental) flickering audio-visual stimulation lasting 1 min and after no stimulation (baseline)., Results: With reference to the baseline condition, only the 10-Hz stimulation induced a negative correlation between pre-stimulus alpha power and reaction time in the fencing athletes and non-athletes as a single group. The higher the enhancement of alpha power before the pictures, the stronger the improvement of the reaction time. The maximum effects were observed in right posterior parietal area (P4 electrode) overlying sensorimotor integrative cortex. Similar results were obtained in a control experiment in which eight elite karate subjects had to react to pictures of karate and basket attacks., Conclusions: The present results suggest that a preliminary 10-Hz sensory stimulation can modulate EEG alpha rhythms and sensorimotor performance in both elite athletes and non-athletes engaged in visuo-spatial-motor demands., Significance: Identification of the EEG state of sporting experts prior to their performance provides a plausible rationale for the modulation of alpha rhythms to enhance sporting performance in athletes and sensorimotor performance in patients to be rehabilitated.

We investigated the functional properties of rat alpha9 and alpha9alpha10 nicotinic acetylcholine receptors (nAChRs) expressed by transient transfection in the rat GH4C1 cell line, using both Ca(2+) imaging and whole-cell recording. Acute applications of ACh generated short-delay fast-rising and quick-decaying Ca(2+) transients, suppressed in Ca(2+)-free medium and invariably accompanied by the activation of whole-cell inward currents. The mean amplitude of ACh-induced currents was as small as -16 pA in alpha9 subunit cDNA-transfected GH4C1 cells (alpha9-GH4C1), while they were much larger (range: -150 to -300 pA) in alpha9alpha10 subunit cDNAs-transfected GH4C1 cells (alpha9alpha10-GH4C1). Currents were not activated by nicotine, were blocked by methyllycaconitine and were ACh concentration-dependent. Because the Ca(2+) permeability of alpha9-containing nAChRs has been estimated in immortalized cochlear UB/OC-2 mouse cells, we also characterized the ACh-induced responses in these cells. Unlike alpha9- and alpha9alpha10-GH4C1 cells, UB/OC-2 cells responded to ACh with both long-delay methyllycaconitine-insensitive whole-cell currents and long-lasting Ca(2+) transients, the latter being detected in the absence of Ca(2+) in the extracellular medium and being suppressed by the Ca(2+)-ATPase inhibitor thapsigargin, known to deplete IP(3)-sensitive stores. These results indicated the involvement of muscarinic nAChRs and the lack of functional ACh-gated receptor channels in UB/OC-2 cells. Thus, we measured the fractional Ca(2+) current (P(f), i.e. the percentage of total current carried by Ca(2+) ions) in alpha9alpha10-GH4C1, obtaining a P(f) value of 22 +/- 4%; this is the largest value estimated to date for a ligand-gated receptor channel. The physiological role played by Ca(2+) entry through alpha9-containing nAChRs gated by ACh is discussed.

Objective: To assess the prognostic value of scalp electroencephalogram (EEG) after epilepsy surgery, we investigated whether postoperative EEG abnormalities (interictal epileptiform discharges, IED; interictal slow activity, ISA) were associated with seizure outcome and other patient characteristics after resective surgery in patients with temporal lobe epilepsy (TLE)., Methods: Sixty-two patients with medically refractory TLE who underwent surgery were studied. Patients were categorized according to etiology (mesiotemporal sclerosis vs. tumors/cortical dysplasias); extent of surgical resection (extensive vs. limited); and amount of preoperative IED on wake EEG (oligospikers, <1 IED/h, vs. spikers). Patients were also classified as seizure-free (SF) or having persistent seizures/auras (not-SF) during follow up visits 1 month and 1 year after surgery. Preoperative 60-min interictal EEGs were evaluated for IED and ISA, and compared to postoperative wake EEGs., Results: Seizures/auras persisted in 16/62 (25.8%) patients at 1 month and in 8/62 (12.9%) at 1 year follow up. ISA was not significantly related to outcome. Of 42 patients with EEG negative for IED at 1 month, 4 were not-SF; at 1 year, one of 44 such patients was not-SF. IED was significantly associated with seizure/aura persistence in patients categorized as mesiotemporal sclerosis and with extensive surgery. Oligospikers and spikers on preoperative EEG showed no differences in the postoperative seizure outcome, excellent in both cases; moreover, the presence of postoperative IEDs indicated auras/seizures persistence apart from the preoperative EEG spike frequency., Conclusions: Our study showed that the presence of IED of postoperatve EEG strongly indicates seizure/aura persistence. Therefore, serial EEGs should be included in postoperative follow up schedules as a crucial tool in evaluating seizure outcome.

Objective: Heart rate (HR) changes, mainly tachycardia, are often observed during seizures originating from the temporal lobe. The aim of this study was to analyze the role of ictal HR changes in localizing both mesial and lateral temporal lobe epilepsy (TLE) in a group of 68 patients. The influence of the gender and the side of epilepsy on HR modulation was also evaluated., Methods: Ictal HR was recorded during prolonged Video-EEG monitoring performed in 68 patients affected by drug-resistant TLE during a non-invasive pre-surgical protocol. According to the electro-clinical correlation, obtained by video-EEG monitoring, one hundred-thirteen seizures (n=113) and one hundred-forty-four auras (n=144) were identified and included in the study. Furthermore, the electro-clinical correlation allowed the classification of all the epileptic events (seizures and auras) as having mesial or lateral origin, based on the temporal lobe seizure onset zone. Ictal HR was calculated with respect to the R-R waves, and assessed from 15 sec (s) before (T(- 15)) to 15 s after (T(+15)) the time of EEG seizure onset (T(0))., Results: We observed a high incidence (92%) of ictal HR increase in TLE seizures. When the ictal EEG indicated a seizure onset from the mesial temporal structures, the onset of ictal HR increase preceded by about 5 s the EEG ictal onset (SD+/-18.4), whereas the onset of HR increase coincided with the onset of EEG discharges (SD+/-14.8) when the ictal EEG indicated the onset of seizures from the lateral temporal structures. No significant differences were found between male and female patients; and between right and left TLE., Conclusions: Our findings show that ictal HR increase, preceding the onset of the EEG discharge, is associated with ictal EEG seizure pattern defining temporal lobe seizures originating from the mesial temporal lobe structures; this association suggests that the HR changes may be coupled to the functional impairment of neural circuits involved in sympathetic cardiovascular regulation, in the mesial temporal lobe structures. Further studies investigating the relationship between intracranial EEG monitoring and ECG recording are worthwhile, to confirm our results and to give further indications on the pathogenesis of ictal HR abnormalities.

The neuronal alpha7 nicotinic acetylcholine (ACh) receptor is believed to be a highly Ca(2+) permeable ligand-gated receptor-channel. However, the contribution of Ca(2+) to cationic current generated by ACh has not yet been directly measured to date. Simultaneous fluorescence and whole-cell current measurements using the Ca(2+) indicator dye fura-2 were made in GH4C1 pituitary cells stably expressing human alpha7 receptors and the fractional Ca(2+) current (the proportion of whole-cell current carried by Ca(2+); P(f)) was determined. We report that the P(f) value was 11.4+/-1.3%. This value was significantly larger than P(f) of human L248Talpha7 receptor mutant (P(f)=6.3+/-1.0%) and of rat alpha7 receptor (P(f)=8.8+/-1.5%) both determined in transiently transfected GH4C1 cells. In our knowledge, the findings here reported indicate the human alpha7 receptors are the most Ca(2+) conductive homomeric ligand-gated receptor-channels expressed in a heterologous cell system.
