[EN] Introduction and objectives: To analyze the evolution of patients with atrial fibrillation (AF) and diabetes in the mid-term follow-up during the COVID-19 pandemic and to describe its impact on this population. Methods: Multicenter and prospective registry that included patients with AF and diabetes attended in cardiology clinics. A multivariate analysis was performed to determine the variables associated with the occurrence of clinical events and mortality. Recruitment was performed in February-December 2019. Results: The evolution of 633 patients, 96,2% of those included in the REFADI registry with a median follow-up of 835 days was analyzed (mean age 73.8 ± 8.5 years, 54.3% male, CHA2DS2-VASc 4,34 ± 1,4, HAS-BLED 2,47 ± 0,96) were analyzed. The proportion of anticoagulated patients remained constant (95.6% vs 94.5%; P = .24). There was a decrease in the prescription of vitamin K antagonists (from 31.4% to 19.7%; P < .01), and an increase in the use of direct anticoagulants (from 62.0% to 70.3%; P < .01). During the follow-up there was an increase in the prescription of SGLT2 inhibitors (from 20.0% to 25.5%; P < .01) and GLP1 agonists (from 4.2% to 9.1%; P < .01). During this period, 17.2% of patients died, the majority from cardiovascular causes, 6.4% from COVID-19, 2.8% from stroke, and 1.8% from hemorrhage. Older age, lower ejection fraction, lower hemoglobin levels, and especially lower direct anticoagulants prescription were associated with mortality. Métodos: Registro multicéntrico y prospectivo que incluyó a pacientes con FA y diabetes atendidos en consultas de cardiología. Se realizó un análisis multivariante para determinar las variables asociadas a la aparición de eventos clínicos y mortalidad. La inclusión se realizó en febrero-diciembre de 2019. Resultados: Se analizó la evolución de 633 pacientes, 96,2% de los incluidos en el registro REFADI durante un seguimiento medio de 835 días (edad media 73,8 ± 8,5 años, 54,3% varones, CHA2DS2-VASc 4,34 ± 1,4, HAS-BLED 2,47 ± 0,96). La proporción de pacientes anticoagulados se mantuvo constante (95,6 frente a 94,5%; p = 0,24). Hubo un descenso de antagonistas de la vitamina K (del 31,4 al 19,7%; p < 0,01), y un aumento de los anticoagulantes directos (del 62,0 al 70,3%; p < 0,01). Durante el seguimiento hubo un aumento en la prescripción de inhibidores SGLT2 (del 20,0 al 25,5%; p < 0,01) y agonistas GLP1 (del 4,2 al 9,1%; p < 0,01). Falleció el 17,2% de los pacientes, la mayoría de causa cardiovascular, 6,4% por COVID-19, 2,8% por ictus y 1,8% por hemorragia. La mayor edad, la menor fracción de eyección, los niveles más bajos de hemoglobina y especialmente la menor prescripción de anticoagulantes directos se asociaron con la mortalidad. Conclusiones: Los pacientes con FA y diabetes tienen un elevado riesgo tromboembólico y de sufrir complicaciones, sobre todo de origen cardiovascular.

Introducción y objetivos: La coexistencia de la fibrilación auricular y la diabetes mellitus es frecuente. Nuestro objetivo es analizar la manera como los cardiólogos tratamos a los pacientes que padecen ambas enfermedades en 2019. Métodos: Dise ̃namos un registro multicéntrico y prospectivo en el que incluimos todos los pacientes atendidos en consultas externas en los que coexistían ambas entidades. Se recogen parámetros clínicos, electrocardiográficos, ecocardiográficos y analíticos, el tratamiento que venían tomando los pacientes y la actitud terapéutica de los cardiólogos. Resultados: Durante 11 meses incluimos 658 pacientes, 55% mujeres, de 73,8 ± 8,5 a ̃nos de edad. Encontra- mos una elevadísima prevalencia de otros factores de riesgo con diferencias significativas entre géneros. No se utiliza el ácido acetilsalicílico y se anticoagula al 96% de aquellos que lo precisan según las guías. Aquellos que siguen tratados con antivitamina K tienen un tiempo en rango terapéutico de Rosendaal de 59,8 ± 31, pero solo se optimiza el tratamiento en el 57,4% (rango de variabilidad entre cardiólogos 10-93%, p = 0,001) de los que tenían un tiempo en rango terapéutico < 65%. La hemoglobina glucosilada era de 7 ± 1,2, y el 37,5% presentaban cifras de hemoglobina glucosilada ≥ 7. Los cardiólogos optimizaron el tratamiento en el 35,2% de ellos (rango de variabilidad entre cardiólogos 6,3-93%, p = 0,0001). Si esta era ≥ 7,5, se optimizaba en el 46,3% y si era ≥ 9 en el 63,2%. Conclusiones: La coexistencia de fibrilación auricular y diabetes mellitus define una población de ele- vadísimo riesgo cardiovascular.
