Abstract

Degenerative dementias such as Alzheimer's disease and frontotemporal dementia result in distinct alterations in emotional processing, emotional experiences, and mood. The neuropathology of these dementias extends to structures involved in emotional processing, including the basolateral limbic network (orbitofrontal cortex, anterior temporal lobe, amygdala, and thalamus), the insula, and ventromedial frontal lobe. Depression is the most common emotion and mood disorder affecting patients with Alzheimer's disease. The onset of depression can be a prodromal sign of this dementia. Anxiety can also be present early in the course of Alzheimer's disease and especially among patients with early-onset forms of the disease. In contrast, patients with behavioral variant frontotemporal dementia demonstrate hypoemotionality, deficits in the recognition of emotion, and decreased psychophysiological reactivity to emotional stimuli. They typically have a disproportionate impairment in emotional and cognitive empathy. One other unique feature of behavioral variant frontotemporal dementia is the frequent occurrence of bipolar disorder. The management strategies for these alterations of emotion and mood in degenerative dementias primarily involve the judicious use of the psychiatric armamentarium of medications.

Abstract

Apathy is an important brain-behavior disorder and a part of many neuropsychiatric behaviors and neurodegenerative disorders. Although originally defined as a lack of emotion, apathy is more precisely a disturbance of the motivation processes of the brain. Advances in neuroscience have helped identify the underlying molecular and neuroanatomical basis of this behavioral disorder. These advances indicate that apathy involves motivation–action–reward pathways involving prefrontal cortical regions such as the anterior cingulate and the ventromedial prefrontal cortex, the basal ganglia, particularly ventral structures, and the dopaminergic system. A frontal “corticobasal loop” evaluates motivation and reward, values the stimuli, makes an action choice, and recruits other regions for action. Studies have only begun to link apathy to genetic changes and mutations, and future studies can further clarify the underlying neural and genetic mechanisms of apathy.

Abstract

Disturbances in language and communication are prominent symptoms in dementia. Many dementing illnesses lead to aphasia, others primarily impair verbal fluency or result in motor speech disturbances such as dysarthria. This chapter discusses language and communication disturbances that occur in non-Alzheimer's dementias, including related abnormalities in cognition, aspects of neuropathology, and neuroimaging. The language deficits in vascular dementia (VaD), frontotemporal dementia (FTD) (e.g., Pick's disease), Huntington's disease (HD), progressive supranuclear palsy (PSP), Creutzfeldt-Jakob disease (CJD), and primary progessive aphasia (PPA) are included. Implications for future research are also noted.

Abstract

Publisher Summary This chapter describes the cortical auditory disorders correlated with the auditory cortex. A variety of higher auditory disorders result from lesions of the cortex. These include pure word deafness, amusia, auditory agnosia, and cortical deafness. Unilateral lesions may destroy a part of the left core auditory cortex or its radiations and callosal fibers from the opposite auditory region. Much more common are cases with bilateral lesions that destroy a part of the left core auditory cortex or its radiations and the right core auditory cortex. Amusia usually occurs in patients with bilateral lesions but when unilateral, the lesions are predominantly in the right core auditory cortex. The recognition of rhythm and other specific aspects of music are heavily lateralized to the right hemisphere. Melody recognition is not as lateralized, and positron emission tomography scanning shows that in those who are musically skilled, musical analysis involves both hemispheres. Patients with generalized auditory agnosia have damage to both core and belt areas of both hemispheres. These are more extensive lesions than those in patients with selective auditory agnosia.
