1. TMS provokes target-dependent intracranial rhythms across human cortical and subcortical sites

Abstract

Background: Transcranial magnetic stimulation (TMS) is believed to alter ongoing neural activity and cause circuit-level changes in brain function. While the electrophysiological effects of TMS have been extensively studied with scalp electroencephalography (EEG), this approach generally evaluates low-frequency neural activity at the cortical surface. However, TMS can be safely used in patients with intracranial electrodes (iEEG), allowing for direct assessment of deeper and more localized oscillatory responses across the frequency spectrum. Objective/Hypothesis: Our study used iEEG to understand the effects of TMS on human neural activity in the spectral domain. We asked (1) which brain regions respond to cortically-targeted TMS, and in what frequency bands, (2) whether deeper brain structures exhibit oscillatory responses, and (3) whether the neural responses to TMS reflect evoked versus induced oscillations. Methods: We recruited 17 neurosurgical patients with indwelling electrodes and recorded neural activity while patients underwent repeated trials of single-pulse TMS at either the dorsolateral prefrontal cortex (DLPFC) or parietal cortex. iEEG signals were analyzed using spectral methods to understand the oscillatory responses to TMS. Results: Stimulation to DLPFC drove widespread low-frequency increases (3–8 Hz) in frontolimbic cortices and high-frequency decreases (30–110 Hz) in frontotemporal areas, including the hippocampus. Stimulation to parietal cortex specifically provoked low-frequency responses in the medial temporal lobe. While most low-frequency activity was consistent with phase-locked evoked responses, anterior frontal regions exhibited induced theta oscillations following DLPFC stimulation. Conclusions: By combining TMS with intracranial EEG recordings, our results suggest that TMS is an effective means to perturb oscillatory neural activity in brain-wide networks, including deeper structures not directly accessed by stimulation itself.

Abstract

Laser thermal ablation has become a prominent neurosurgical treatment approach, but in epilepsy patients it cannot currently be safely implemented with intracranial recording electrodes that are used to study interictal or epileptiform activity. There is a pressing need for computational models of laser interstitial thermal therapy (LITT) with and without intracranial electrodes to enhance the efficacy and safety of optical neurotherapies. In this paper, we aimed to build a biophysical bioheat and ray optics model to study the effects of laser heating in the brain, with and without intracranial electrodes in the vicinity of the ablation zone during the LITT procedure. COMSOL Multiphysics finite element method (FEM) solver software was used to create a bioheat thermal model of brain tissue, with and without blood flow incorporation via Penne's model, to model neural tissue response to laser heating. We report that the close placement of intracranial electrodes can increase the maximum temperature of the brain tissue volume as well as impact the necrosis region volume if the electrodes are placed too closely to the laser coupled diffuse fiber tip. The model shows that an electrode displacement of 4 mm could be considered a safe distance of intracranial electrode placement away from the LITT probe treatment area. This work, for the first time, models the impact of intracranially implanted recording electrodes during LITT, which could improve the understanding of the LITT treatment procedure on the brain's neural networks a sufficient safe distance to the implanted intracranial recording electrodes. We recommend modeling safe distances for placing the electrodes with respect to the infrared laser coupled diffuse fiber tip.

Abstract

Pharmacological ascorbate (P-AscH-) combined with standard of care (SOC) radiation and temozolomide is being evaluated in a phase 2 clinical trial (NCT02344355) in the treatment of glioblastoma (GBM). Previously published data demonstrated that paramagnetic iron (Fe3+) catalyzes ascorbate's oxidation to form diamagnetic iron (Fe2+). Because paramagnetic Fe3+ may influence relaxation times observed in MR imaging, quantitative MR imaging of P-AscH--induced changes in redox-active Fe was assessed as a biomarker for therapy response.Gel phantoms containing either Fe3+ or Fe2+ were imaged with T2* and quantitative susceptibility mapping (QSM). Fifteen subjects receiving P-AscH- plus SOC underwent T2* and QSM imaging four weeks into treatment. Subjects were scanned: pre-P-AscH- infusion, post-P-AscH- infusion, and post-radiation (3–4 h between scans). Changes in T2* and QSM relaxation times in tumor and normal tissue were calculated and compared to changes in Fe3+ and Fe2+ gel phantoms. A GBM mouse model was used to study the relationship between the imaging findings and the labile iron pool.Phantoms containing Fe3+ demonstrated detectable changes in T2* and QSM relaxation times relative to Fe2+ phantoms. Compared to pre-P-AscH-, GBM T2* and QSM imaging were significantly changed post-P-AscH- infusion consistent with conversion of Fe3+ to Fe2+. No significant changes in T2* or QSM were observed in normal brain tissue. There was moderate concordance between T2* and QSM changes in both progression free survival and overall survival. The GBM mouse model showed similar results with P-AscH- inducing greater changes in tumor labile iron pools compared to the normal tissue. Conclusions: T2* and QSM MR-imaging responses are consistent with P-AscH- reducing Fe3+ to Fe2+, selectively in GBM tumor volumes and represent a potential biomarker of response. This study is the first application using MR imaging in humans to measure P-AscH--induced changes in redox-active iron.

Abstract

Functional neuroimaging techniques have provided great insight in the field of pain. Utilising these techniques, we have characterised pain-induced responses in the brain and improved our understanding of key pain-related phenomena. Despite the utility of these methods, there remains a need to assess the test retest reliability of pain modulated blood-oxygen-level-dependant (BOLD) MR signal across repeated sessions. This is especially the case for more novel yet increasingly implemented stimulation modalities, such as noxious pressure, and it is acutely important for multi-session studies considering treatment efficacy. In the present investigation, BOLD signal responses were estimated for noxious-pressure stimulation in a group of healthy participants, across two separate sessions. Test retest reliability of functional magnetic resonance imaging (fMRI) data and self-reported visual analogue scale measures were determined by the intra-class correlation coefficient. High levels of reliability were observed in several key brain regions known to underpin the pain experience, including in the thalamus, insula, somatosensory cortices, and inferior frontal regions, alongside “excellent” reliability of self-reported pain measures. These data demonstrate that BOLD-fMRI derived signals are a valuable tool for quantifying noxious responses pertaining to pressure stimulation. We further recommend the implementation of pressure as a stimulation modality in experimental applications.

Abstract

Objectives: Hemostasis and dural defect repair are fundamental aspects of neurosurgical procedures. Some significant bleeding may be caused by dural tears (e.g. bleeding from cerebral dural venous sinuses). Various methods have been previously reported for these purposes, utilizing commercially available surgical materials such as gelatin sponge and fibrin glue. We describe a simple, yet versatile and flexible technique with a critical modification to the previously known combined use of gelatin sponge and fibrin glue. We then present a few illustrative cases and discuss the utility and advantages of the described technique. Patients and methods: The described technique involves separate and sequential application of fibrinogen solution followed by thrombin solution of fibrin glue over gelatin sponge, which enables hemostasis or dural defect repair in a localized, controlled manner. Results: We have utilized this technique effectively for hemostasis and dural defect repair in a variety of neurosurgical procedures for over 10 years. We have found the technique to be highly useful and effective, although the effectiveness of this technique may be difficult to quantify objectively and independently of the other standard surgical maneuvers, given that hemostasis and prevention of cerebrospinal fluid leak is multifactorial. We have not encountered any complications specifically attributable to the use of fibrin glue and/or gelatin sponge for this technique. Conclusion: This technique will add to the neurosurgeons' arsenal for hemostasis and dural defect repair, in addition to the more standard and basic surgical maneuvers. 7. Drug-resistant epilepsy development following stem cell transplant and cyclosporine neurotoxicity induced seizures: Case report in an adult and analysis of reported cases in the literature

Abstract

Introduction: Drug-resistant epilepsy (DRE) occurs in 20–30% of all patients who develop epilepsy and can occur from diverse causes. Cyclosporine-A (CSA) is an immunosuppressive drug utilized to prevent graft-versus-host disease (GvHD) in transplant patients and is known to cause neurotoxicity, including seizures. In some cases, however, patients can develop DRE. Only a limited number of cases have been reported in which DRE has developed after CSA exposure — all in children. Here we present a rare case of an adult developing DRE after post-transplant CSA neurotoxicity. In addition, we provide a comprehensive review and analysis of all reported cases in the literature. Case report: A 29-year-old man with Non-Hodgkin's Lymphoma underwent an allogenic hematopoietic stem cell transplant and experienced a CSA-induced seizure at 7.5 months' post-transplant. The patient was discontinued on CSA and began a low dose tacrolimus regimen. At 33 months' post-transplant, he had seizure recurrence and developed DRE. Imaging revealed right mesial temporal sclerosis (MTS) and video EEG localized ictal activity to the right anterior temporal lobe. He was successfully treated with a right anterior temporal lobectomy and amygdalohippocampectomy. Literature review: Seven peer-reviewed studies described 15 patients who underwent transplantation with post-transplant CSA administration and subsequently developed DRE following an initial CSA-induced seizure. All 15 patients were children suggesting that young age is a risk factor for DRE after CSA-induced seizures. Initial CSA-induced seizures occurred at an average of 1.6 ± 1.1 months after transplant and seizure recurrence 9.2 ± 8.0 months after transplant. All reported CSA routes of administration (n = 6) were intravenous and 7 of 9 (78%) reported CSA blood levels above the therapeutic range. The incidence of MTS (40%) in these 15 patients was significantly higher than the incidence in the general DRE population (24%) and was most effectively treated via epilepsy surgery. Conclusions: The use of cyclosporine for GvHD prophylaxis and treatment following transplantation may cause seizures and be associated with DRE. Although discontinuation and dose decrease of CSA often reverse adverse neurological events, initial CSA-induced seizures may be associated with MTS that and subsequent greater risk of DRE development. Abstract

Information from Magnetic Resonance Imaging (MRI) is useful for diagnosis and treatment management of human neurological patients. MRI monitoring might also prove useful for non-human animals involved in neuroscience research provided that MRI is available and feasible and that there are no MRI contra-indications precluding scanning. However, MRI monitoring is not established in macaques and a resource is urgently needed that could grow with scientific community contributions. Here we show the utility and potential benefits of MRI-based monitoring in a few diverse cases with macaque monkeys. We also establish a PRIMatE MRI Monitoring (PRIME-MRM) resource within the PRIMatE Data Exchange (PRIME-DE) and quantitatively compare the cases to normative information drawn from MRI data from typical macaques in PRIME-DE. In the cases, the monkeys presented with no or mild/moderate clinical signs, were well otherwise and MRI scanning did not present a significant increase in welfare impact. Therefore, they were identified as suitable candidates for clinical investigation, MRI-based monitoring and treatment. For each case, we show MRI quantification of internal controls in relation to treatment steps and comparisons with normative data in typical monkeys drawn from PRIME-DE. We found that MRI assists in precise and early diagnosis of cerebral events and can be useful for visualising, treating and quantifying treatment response. The scientific community could now grow the PRIME-MRM resource with other cases and larger samples to further assess and increase the evidence base on the benefits of MRI monitoring of primates, complementing the animals' clinical monitoring and treatment regime.

Abstract

Recent developments in cognitive neuroscience provide insight into the neurocognitive deficits underlying addiction. At the same time, mindfulness meditation practices have emerged as promising treatments for addictive disorders. This chapter first presents evidence supporting mindfulness-based interventions (MBIs) as effective therapies for addictive disorders. Next, “top-down” and “bottom-up” mechanisms through which MBIs exert their therapeutic effects on addictive behavior are detailed. A neurobiological framework is integrated throughout the text, highlighting parallels between the restorative effects of mindfulness practice on the brain and dysregulated neural functions undergirding addictive behaviors. Finally, MBIs are considered as means of recovery enhancement in the broader context of long-term addiction treatment.

The incidence of MTS (40%) in these 15 patients was significantly higher than the incidence in the general DRE population (24%) and was most effectively treated via epilepsy surgery. Conclusions: The use of cyclosporine for GvHD prophylaxis and treatment following transplantation may cause seizures and be associated with DRE. Although discontinuation and dose decrease of CSA often reverse adverse neurological events, initial CSA-induced seizures may be associated with MTS that and subsequent greater risk of DRE development. Abstract

Background Accurate identification of cranial midline structures is essential for many targeting techniques that use repetitive transcranial magnetic stimulation (rTMS), including the Beam F3 method used for depression treatment. Objective Evaluate whether a novel, laser-sighted device will assist with more accurate identification of the cranial midline relative to standard scalp-based measurement procedures. Methods Three trained TMS technicians performed repeated scalp-based measurements to identify the inion and vertex on five subjects (n = 54 measurements). Measurements were compared to points identified with the midline localizer device and the true midline as defined by MRI midline structures. Results Use of the midline localizer was more accurate for midline identification than technician measurement (p = 0.00025) and the ratio of localizing the midline within 5 mm was higher (78% versus 54%, p = 0.008). Conclusion Use of a laser-sighted midline localizer device can improve the accuracy of scalp measurements associated with target localization for rTMS treatment protocols.

Abstract

Background Diagnosis of chronic low back pain (CLBP) is traditionally predicated on identifying underlying pathological or anatomical causes, with treatment outcomes modest at best. Alternately, it is suggested that identification of underlying pain mechanisms with treatments targeted towards specific pain phenotypes may yield more success. Differentiation between nociceptive and neuropathic components of CLBP is problematic; evidence suggests that clinicians fail to identify a significant neuropathic component in many CLBP patients. The painDETECT questionnaire (PDQ) was specifically developed to identify occult but significant neuropathic components in individuals thought to have predominantly nociceptive pain. Methods Using the PDQ, we classified 50 CLBP patients into two distinct groups; those with predominantly nociceptive pain (Group 1) and those with a significant neuropathic component (Group 2). We characterised these two distinct CLBP sub-groups using a) questionnaire-based behavioural evaluation measuring pain-related function and quality of life, pain intensity and psychological well-being and b) sensory examination, using two-point and tactile threshold discrimination. Objective We sought to determine if differences in the pain phenotype of each CLBP sub-group would be reflected in sensory and behavioural group profiles. Results We report that Group 1 and Group 2 sub-groups demonstrate unique clinical profiles with significant differences in sensory tactile discrimination thresholds and in a wide range of behavioural domains measuring pain intensity, disability and psychological well-being. Conclusion We have demonstrated distinct clinical profiles for CLBP patient sub-groups classified by PDQ. Our results give diagnostic confidence in using the PDQ to characterise two distinct pain phenotypes in a heterogeneous CLBP population., Highlights • Identification of underlying CLBP pain phenotypes is challenging. • We used painDETECT to identify nociceptive and neuropathic CLBP subgroups. • We demonstrate unique sensory and behavioural clinical profiles for each group. • More accurate identification of CLBP pain phenotypes may improve treatment outcomes.

Abstract

Background Sensitive outcome measures are needed to quantify the effects of neuromodulation in mood disorders. Objective This study examined the utility of a novel affective bias (AB) task in identifying transient mood changes induced by amygdala stimulation in a single rare participant. Methods Localized, pulsed electrical stimulation was delivered for 8 min during measures of AB and self-reported mood. Responses were compared with those gathered without stimulation on the same day in the same setting, using paired t -tests. Results Stimulation of the basolateral nucleus of the right amygdala at 50 Hz, 15 V, and 200 μs pulse-width produced a significant positive shift in AB ( t = −2.864, df = 53, P = .006), despite equivocal findings on self-reported mood ( t = −.184, df = 12, P = .857). Conclusion Affective bias may be more sensitive to stimulation-induced fluctuations in mood than subjective report, suggesting utility as an outcome measure in neuromodulation studies.

Abstract

Inhalant misuse is among the most prevalent and pernicious forms of drug use in the United States. Hundreds of low-cost and widely available commercial products containing volatile hydrocarbons are inhaled for their psychoactive effects. Inhalant use is prevalent among adolescents, indigenous populations, persons involved with the juvenile and criminal justice systems, and other marginalized groups. Acute inhalant intoxication can lead to “sudden sniffing death” and is associated with many adverse health and social outcomes. Chronic inhalant use, such as occurs among persons with inhalant use disorder, is associated with significant neurotoxicity and a range of cognitive impairments, psychosocial deficits, and organ pathologies. Limited, though important, progress has been made in understanding the pharmacology and toxicology of inhalant misuse. However, little is known regarding the treatment and prevention of inhalant use disorders.

Abstract

This chapter provides an overview of current invasive recording methodology and experimental paradigms used in the studies of human auditory cortex. Invasive recordings can be obtained from neurosurgical patients undergoing clinical electrophysiologic evaluation for medically refractory epilepsy or brain tumors. This provides a unique research opportunity to study the human auditory cortex with high resolution both in time (milliseconds) and space (millimeters) and to generate valuable information about its organization and function. A historic overview presents the development of the experimental approaches from the pioneering works of Wilder Penfield to modern day. Practical issues regarding research subject population, stimulus presentation, data collection, and analysis are discussed for acute (intraoperative) and chronic experiments. Illustrative examples are provided from experimental paradigms, including studies of spectrotemporal processing, functional connectivity, and functional lesioning in human auditory cortex.

Abstract

During a decade of building the Gulf of Mexico Coastal Ocean Observing System (GCOOS), there has been convincing evidence that an integrated “system of systems” is an effective, efficient system that provides many societal benefits, including economic development opportunities. The Gulf community has come a long way toward establishing a regional ocean observing system, and with input from more than 700 stakeholders, the GCOOS Regional Association has developed a comprehensive blueprint for monitoring in the Gulf. This blueprint, the GCOOS Build-out Plan, can and should serve as a guide for future investments, including those intended to generate meaningful ecosystem indicators that inform the restoration process and ascertain the success of restoration efforts. With an unprecedented opportunity via The Clean Water Act penalty monies, the region is poised to build on extensive multi-institutional, interdisciplinary relationships to expand existing infrastructure into a comprehensive system that can address the myriad issues facing the Gulf.

Abstract

Objective: To assess the relationship between bone marrow (BM) biopsy operator experience and both specimen quality and ancillary testing utilization. Patients and Methods: We evaluated all referred and in-house (IH) BM biopsy specimens obtained over a contiguous 6-week period from April 3, 2017, to May 19, 2017. The BM specimens were assessed for the length of interpretable marrow, and aspirates were assessed for the presence of spicules. Subgroup comparisons included IH BM obtained by a trained team of nurses within our institution, patients clinically referred (CR) to our institution with outside-obtained BM specimens, and outside pathologist-referred (PR) consultation cases. Ancillary study usage was compared between the first 100 cases of each group. Results: A total of 1191 BM specimens were analyzed, including 600 IH, 288 CR, and 303 PR cases with biopsies and/or aspirates. The average interpretable biopsy lengths of IH, CR, and PR cases were 16.0 mm, 10.0 mm, and 7.0 mm, respectively (P
