1. Concordance between pharmacotherapeutic complexity calculated and perceived by HIV+ patients with antiretroviral treatment

Abstract

Objective: To determine the difference between the pharmacotherapeutic complexity index by Medication Regimen Complexity Index and it’s perceived by patients through a visual analogue scale in patients HIV+ with antiretroviral treatment. Method: Prospective, observational study of patients HIV+ > 18 years of age with stable antiretroviral treatment in the last three months, followed tp by external consultations of pharmaceutical care between October´17 and February ´18. The main variable of the study was the concordance between the median of the score obtained in the pharmacotherapeutic complexity perceived by the patients using the visual analog scale whose range of values oscillates between 0-10, categorized in low complexity (0-1) and high complexity (2-10), and the median of the score obtained for the theoretical pharmacotherapeutic complexity using the Medication Regimen Complexity Index tool whose ranges of values oscillate between 1 and infinity, categorized in low complexity (0-11) and high complexity > 11. The overall complexity was calculated: antiretroviral treatment and concomitant treatment. Results: We included 236 patients in the study. There was a discrete concordance between the pharmacotherapeutic complexity perceived by the patients and that calculated according to the Medication Regimen Complexity Index tool (Cohen’s Kappa index 0.203). The median of the Medica tion Regimen Complexity index of the total medication was 6 (interquartile range: 4-10) versus the median of the Complexity Index measured by visual analog scale of 2 (interquartile range: 0-4). Conclusions: Patients perceive a pharmacotherapeutic complexity lower than that calculated. Therefore, we must include the two scales in pharmaceutical care for a better understanding of the patient’s perception.

Resumen: Objetivo: Evaluar el grado en que los sucesos adversos (SA) ligados a la asistencia sanitaria alcanzan al paciente y su severidad. Analizar los factores contribuyentes a la aparición de SA, la relación con el daño provocado y el grado de evitabilidad. Diseño: Estudio descriptivo retrospectivo. Emplazamiento: Servicio de Atención Primaria de Porriño desde enero de 2014 a abril de 2016. Participantes y/o contexto: Se incluyeron notificaciones de SA en el Sistema de Notificación y Aprendizaje para Seguridad del Paciente (SiNASP). Método: Variables de medida: incidente adverso (IA) si no alcanzó al paciente o no produjo daño, evento adverso (EA) si llegó al paciente con daño. 3. Evaluation of HIV+ patients experience with pharmaceutical care based on AMO-methodology

Abstract

Objective: To evaluate the experience of HIV+ patients with pharmaceutical care based on the ability-motivation-opportunity methodology. Method: Cross-sectional, unicentric study. The participants included were HIV+ patients attended during November-2016 and to whom the IEXPAC questionnaire was conducted. This validated questionnaire allows to know the experience of chronic patients about the health care they receive. Results: 91 patients were included. The overall mean score of the IEXPAC questionnaire was: 9.7 ± 0,3. The items that obtained higher scores were: respect lifestyle (9.9 ± 0.5); coordination to offer good care (9.9 ± 0.5); helping to continue treatment (9.9 ± 0.4); making sure that I take medication (9.9 ± 0.4); worried about well-being (9.9 ± 0.4). The points with the possibility of improvements included: worried about me when I get home after being in hospital (8.7 ± 1.2). Conclusions: The evaluation of the pharmaceutical care process based on the ability-motivation-opportunity model achieve a high level in each of the identified points of interaction.

Abstract

Road transportation constitutes a key sector in developed countries, as an essential catalyst for economic and social activities. Nevertheless, it is relevant to emphasize the negative impacts of this activity identified in Economics as negative externalities. At the European Union, road transportation is the main cause of the air pollution impact on the population. Thus, this study explores the factors that influence the willingness to pay (WTP) on behalf of the citizens to reduce air pollution generated by road transport. In doing so, we propose two fundamental theoretical frameworks to explain individual behavior towards the environment actions: the Theory of Planned Behavior (TPB) and the Value-Belief-Norm (VBN) models. A questionnaire survey with 1,612 residents was used to collect data in 65 localities located in the Spanish Pyrenees and performing a statistical analysis with the resulting data relied on application of Structural Equation Models (SEM). Moreover, the survey results highlight the importance of psychological aspects as predictors of proenvironmental behaviors. Our empirical results provide a novel contribution about how governments and educational policies can enhance the positive attitude towards environmental actions, unifying the struggle in favor of environmental protection from early childhood. Abstract

Knowledge is a basic factor of competitiveness with a firm’s exploration and exploitation capabilities acting as the main antecedents of innovation. However, a firm has two main options to obtain new knowledge: internal generation and external acquisition. This paper analyzes how tourism firms located in tourist districts develop ambidexterity through the combination of co-exploration and co-exploitation. Specifically, we study how the features that characterize a tourist district, such as the level of firm and institutional agglomeration, affect the development of co-exploration and co-exploitation capabilities, taking into account the mediation effect of social capital. The population under study includes all the Spanish hotels located in Spanish coastal towns, making a total sample of 210 establishments. The results confirm that agglomeration has a positive impact on the ambidexterity of Spanish hotels. Moreover, the results show that agglomeration causes an increase of social capital in hotels, and that social capital has a positive impact on ambidexterity. We find that social capital partially mediates the effect of agglomeration on ambidexterity. Some implications for managers and policymakers are presented.

Abstract

REIPI-SEIMC COVID-19 group and COVID@HULP group., [Background] The clinical presentation of COVID-19 in patients admitted to hospital is heterogeneous. We aimed to determine whether clinical phenotypes of patients with COVID-19 can be derived from clinical data, to assess the reproducibility of these phenotypes and correlation with prognosis, and to derive and validate a simplified probabilistic model for phenotype assignment. Phenotype identification was not primarily intended as a predictive tool for mortality., [Methods] In this study, we used data from two cohorts: the COVID-19@Spain cohort, a retrospective cohort including 4035 consecutive adult patients admitted to 127 hospitals in Spain with COVID-19 between Feb 2 and March 17, 2020, and the COVID-19@HULP cohort, including 2226 consecutive adult patients admitted to a teaching hospital in Madrid between Feb 25 and April 19, 2020. The COVID-19@Spain cohort was divided into a derivation cohort, comprising 2667 randomly selected patients, and an internal validation cohort, comprising the remaining 1368 patients. The COVID-19@HULP cohort was used as an external validation cohort. A probabilistic model for phenotype assignment was derived in the derivation cohort using multinomial logistic regression and validated in the internal validation cohort. The model was also applied to the external validation cohort. 30-day mortality and other prognostic variables were assessed in the derived phenotypes and in the phenotypes assigned by the probabilistic model., [Findings] Three distinct phenotypes were derived in the derivation cohort (n=2667)—phenotype A (516 [19%] patients), phenotype B (1955 [73%]) and phenotype C (196 [7%])—and reproduced in the internal validation cohort (n=1368)—phenotype A (233 [17%] patients), phenotype B (1019 [74%]), and phenotype C (116 [8%]). Patients with phenotype A were younger, were less frequently male, had mild viral symptoms, and had normal inflammatory parameters. Patients with phenotype B included more patients with obesity, lymphocytopenia, and moderately elevated inflammatory parameters. Patients with phenotype C included older patients with more comorbidities and even higher inflammatory parameters than phenotype B. We developed a simplified probabilistic model (validated in the internal validation cohort) for phenotype assignment, including 16 variables. In the derivation cohort, 30-day mortality rates were 2·5% (95% CI 1·4–4·3) for patients with phenotype A, 30·5% (28·5–32·6) for patients with phenotype B, and 60·7% (53·7–67·2) for patients with phenotype C (log-rank test p, [Interpretation] Patients admitted to hospital with COVID-19 can be classified into three phenotypes that correlate with mortality. Resumen: Objetivo: Evaluar el grado en que los sucesos adversos (SA) ligados a la asistencia sanitaria alcanzan al paciente y su severidad. Analizar los factores contribuyentes a la aparición de SA, la relación con el daño provocado y el grado de evitabilidad. Diseño: Estudio descriptivo retrospectivo. Emplazamiento: Servicio de Atención Primaria de Porriño desde enero de 2014 a abril de 2016. Participantes y/o contexto: Se incluyeron notificaciones de SA en el Sistema de Notificación y Aprendizaje para Seguridad del Paciente (SiNASP). Abstract

In this research two concepts of current relevance in the management literature are related, such as Corporate Social Responsibility (CSR) and green human resource management (GHRM). So far, research in both fields has been developed in parallel; However, we consider that it is necessary to deepen the connection between both variables and their impact on the environmental and financial outcomes of firms. This paper aims to analyze the relationship between CSR and firm performance, considering on the one hand the direct relationship, and on the other hand, the possible mediation in said relationship of some variables such as GHRM and environmental outcomes. A variance-based structural equation modeling (Partial Least Squares) was applied to a sample of Spanish hotel firms. The results obtained show the existence of a direct and positive relationship between CSR and performance. In addition, we have found an indirect effect on the aforementioned relationship through the mediation of GHRM and environmental outcomes. The results of this study make several contributions to the CSR and GHRM literature, as well as practical contributions for managers.

Abstract

This paper analyzes the role played by internal knowledge generation and external knowledge acquisition in the impact of agglomeration on the profitability of Spanish hotels. The results are of interest not only because they demonstrate that knowledge management strategies have a partial mediation effect in the relationship between agglomeration and profitability, but also because they find a substitution effect between internal knowledge generation and external knowledge acquisition. The analysis uses a sample of 2003 Spanish hotels and helps us to understand the real reasons behind the district effect.

Abstract

The purpose of this study consists in analyzing the existing literature on international business from all the research papers published in Journal of International Business Studies (JIBS) during the period of time comprised between 2000 and 2015. The authors apply bibliometric methods to that end –in particular, document citation and co-citation analyses (DCAs)– so that they can try and identify the main research lines within the aforementioned scientific field or domain; in other words, the ‘intellectual structure’ of the discipline examined. Social networks analysis was also used to carry out the representation or visualization of such structure or knowledge base. The highly interdisciplinary nature of the research undertaken, since the study field still keeps its characteristic diversity and interdisciplinarity or the need to integrate a variety of theoretical frameworks or approaches into the development of a widely-accepted theory about the field under study become evident. Amongst these theories would be the new institutional theory, the strategic theory or perspective or others such as the transaction cost theory or the agency theory, applied to the context of internationalization and to the field of international business, on whose integration the future development of this field may largely depend.

Abstract

Resumen: Introducción: Estudios recientes demuestran que muchos niños prematuros sin secuelas neurológicas aparentes presentan dificultades en diferentes áreas, como la coordinación o el equilibrio, durante la etapa escolar. El Movement Assessment Battery for Children-2 (MABC-2) constituye una herramienta validada para la valoración de la coordinación, mientras que la estabilometría sería la prueba gold standard para el equilibrio. Pacientes y método: Estudio de casos y controles realizado en niños prematuros de 7-10 años y controles sanos nacidos a término de la misma edad. En ambos grupos se aplicó la franja de edad número 2 del MABC-2 y se analizó el equilibrio estático mediante estabilometría. Resultados: Se incluyeron 89 sujetos: 30 prematuros de peso al nacimiento ≤ 1.500 g, 29 prematuros de peso > 1.500 g y 30 controles. Los prematuros obtuvieron peores puntuaciones totales de forma global en destreza manual y equilibrio en el MABC-2, independientemente del peso al nacimiento. La menor edad gestacional supuso la obtención de peores puntuaciones en destreza manual y scores totales en el MABC-2. El equilibrio fue similar mediante la estabilometría, independientemente de la prematuridad. Conclusiones: Algo más de un 10% de prematuros y controles podría tener trastornos de la coordinación o estar en riesgo de desarrollarlos empleando el MABC-2. A pesar de que la coordinación visomotriz fue similar, los prematuros podrían tener mayores dificultades en destreza manual, mientras que en ausencia de comorbilidad neurológica, el equilibrio postural parece ser equiparable al de los controles sanos de su misma edad. Abstract: Introduction: Recent studies show that many preterm children without apparent neurological sequelae present some difficulties in different areas, such as coordination or balance during their school years. The Movement Assessment Battery for Children-2 (MABC-2) has demonstrated to be a useful tool to validate the coordination, while the stabilometric platform was the reference standard test for validating the balance. Patients and method: Case-control study carried out on preterm children from 7 to 10 years old and healthy term infant controls of the same ages. The same age band of MABC-2 was applied and the static balance by the stabilometric platform was analysed. Results: A total of 89 subjects were included, 30 preterm children ≤ 1,500 g birthweight, 29 preterm children > 1,500 g birthweight, and 30 controls. Preterm children obtained the lowest scores on an overall basis in hand dexterity and balance tests in MABC-2, regardless of their birthweight. Lower gestational age was associated with poorer outcomes in hand dexterity and total scores in MABC-2. Balance results were similar using the stabilometric platform, regardless of prematurity. Conclusions: A little more than 10% preterm and term children could have coordination disorders or be at risk of developing them using the MABC-2. Despite the visual-motor coordination being similar, preterm children could face greater difficulties in hand dexterity while, in the absence of neurological comorbidity, preterm and term children balance could be comparable.

Abstract

The present paper brings together two lines of research that the literature has more often than not studied separately: the generation of externalities in industrial districts; and the international strategy based on location advantages. The application of industrial district approach principles serves to identify the tourist holiday districts situated along the Spanish coastline. This is followed by an analysis of the extent to which the degree of firm agglomeration specific to each district, as well as the presence of institutions generating specific technical knowledge, influence the globalization of the tourism sector, both regarding internationalized Spanish hotel chains and foreign hotels installed in Spain.

Abstract

The purpose of this study is to analyze the existing literature on hospitality management from all the research papers published in The International Journal of Hospitality Management (IJHM) between 2008 and 2014. The authors apply bibliometric methods – in particular, author citation and co-citation analyses (ACA) – to identify the main research lines within this scientific field; in other words, its ‘intellectual structure’. Social network analysis (SNA) is also used to perform a visualization of this structure. The results of the analysis allow us to define the different research lines or fronts which shape the intellectual structure of research on hospitality management.

Abstract

a b s t r a c t The importance of human resource qualification for the tourism industry in general and for hotel firms in particular has been a recurrent theme for several years now. The present paper deals with training policy in hotels and has as its aim to know the extent to which their strategic orientation influences: (1) the investment that firms make in training; (2) the preparation of formal plans; and (3) the type of training developed. It is also our intention to check whether this strategy-training link has an impact on hotel performance. With that aim in mind, an attempt will be made to see whether the effect of training on business results is direct or materializes through a number of intermediate variables such as client satisfaction or changes in the behavior of employees (indirect effect). A comprehensive analysis of the literature was conducted to identify the main research and establishing the hypotheses to be tested. Our paper used a sample of Spanish companies with the partial least squares (PLS) method being applied for the analysis. The results show that the development of a training policy oriented to human capital development in the case of hotels following a differentiation strategy leads to improved business results both directly and indirectly, while simultaneously influencing investment as well as formality in training

Abstract

Few studies have been found that to assess the factors that explain higher levels of family burden in adults with intellectual disability (ID) and intellectual disability and mental disorders (ID-MD). The aims of this study were to assess family burden in people with ID and ID-MD and to determine which sociodemographic, clinical and functional disability variables account for family burden. The sample is composed of pairs of 203 participants with disability and their caregivers, of which 33.5% are caregivers of people with ID and 66.5% of ID-MD. Assessments were performed using scales of clinical and functional disability as the following instruments: Weschler Adult Intelligence Scale-III (WAIS-III), Inventory for Client and Agency Planning (ICAP), Psychiatric Assessment Schedule for Adults with Development Disability (PAS-ADD checklist), Disability Assessment Schedule of the World Health Organization (WHO-DAS-II) and family burden (Subjective and Objective Family Burden Inventory - SOFBI/ECFOS-II). Abstract

Coastal tourism is deep into a dichotomy between its potential to provide a wide range of social and natural benefits, and its traditional link to mass models, provoking a profound debate around the generation of negative externalities. This study examines the role of innovation and strategic corporate social responsibility orientation in the relationship between CSR and corporate performance to determine the mechanisms that underlie this main relationship. To this end, a sample of 202 hotels on the Spanish coast is analysed by applying the Partial Least Square Methodology. Useful results are derived from the empirical testing of the theoretical model, such as the existence of a positive direct effect between CSR and hotel performance. Likewise, we shed light on the mediating role of innovation and the moderating effect of a strategic CSR orientation in the main relationship, providing valid solutions for both academics and managers of coastal hotels, in terms of adopting CSR practices and promoting its integration into the core business of the hotel, and further developing innovations that will boost the benefits derived from their responsible practices, as well as their strengths to overcome the barriers traditionally faced when carrying out CSR strategies.

Abstract

We present a case of cecal bascule in a 60-year-old woman with abdominal pain and vomiting. Imaging tests revealed a cecal bascule causing mechanic obstruction of the stomach. Besides a small bowel dilatation was not seen, the distended cecum was extrinsically obstructing the antrum and therefore, the gastric outlet. Cecal bascule is a form of cecal volvulus without the axial twisted component. The cecum folds anterior or anteromedially on itself and, because of a valve mechanism, becomes distended. The pressure increases and it begins to compromise arterial blood flow so life-threatening complications, such as bowel ischemia and infarction can occur. Prompt diagnosis is therefore crucial and plain radiographs and computed tomography are the preferred imaging techniques for this purpose.
