1. Compact two-port antenna with high isolation based on the defected ground for THz communication

Abstract

The two-port antenna is designed for THz communication where isolation is achieved with the defective ground. The radiator of the two-port antenna is inspired by the half-cutting technology which reduces the size and makes it a compact antenna. The T-shaped stub and open-ended slot are used to modify the capacitive and inductive reactance which diminish and discontinued the surface current to improve the port isolation. The T-shaped stub improves the electric length of the ground and enhances the port isolation by up to 3.4 dB and open-ended slot in the ground is used to weaken the mutual current and create the non-uniform current which intensifications the port isolation up to 7.7 dB at 2.2 THz. The dimensions of the design is 100 µm × 105 µm and the edge distance of the antennas is only 0.20λ with maximum isolation is 30 dB at 2.2 THz. The bandwidth of the two-port antenna is 1.74 THz to 2.52 THz that can be applicable in wireless communication applications.

Abstract

The increasing anthropogenic and technogenic activities to compensate the raising population and unending demands of humans ended in severe pollution and detrimental damage to the environment. This environmental pollution due to lethal pollutants, toxic heavy metals and organic wastes has been drastically affecting the ecosystem of the living organisms. These are forced to enter into the food chain as they tend to accumulate in the agricultural soils. In order to eliminate these pollutants from the soils the bioremediation will be an efficient tool and this can be achieved by plant growth promoting rhizobacteria and by green wastes. In this study the plant growth promoting rhizobacteria (PGPR) and green wastes are evaluated for their effectiveness in bioremediating the toxic contaminants. Green wastes are rich sources of naturally occurring polyphenols which are potential eliminating agents of these pollutants, they can perform metal chelation, reduction, antibiotic properties, adsorption, complexation and by supplying the nutrients. However, PGPRs are well known plant life saviors from various biotic and abiotic stresses; they are also the bioremediating agents as they perform heavy metal elimination by various methods. In this study, it is also depicted that the combined application of PGPR and green wastes result in the significant method to bioremediate the contaminated sites. The bioremediation by PGPRs and green wastes is the prominent effective way to terminate the pollutants compared to phytoremediation.

Scrub typhus (caused by Orientia tsutsugamushi) is a neglected and underdiagnosed disease due to its non-specific clinical presentation and challenging diagnostics. We document the first study of direct diagnosis of scrub typhus from the blood sample of a patient by full-length 16S ribosomal RNA gene amplicon analysis using Oxford Nanopore sequencing.

Abstract

Objective: Several studies have demonstrated a shift in the spectrum of infective endocarditis (IE) in the developed world. We aimed to investigate whether demographic and microbiologic characteristics of IE have changed in India. Design: A retrospective analysis of patients with in north India between 2010 and 2020. Methods: The clinical and laboratory profiles of 199 IE admitted to an academic hospital patients who met the modified Duke criteria for definite IE were analysed. Results: The mean age was 34 years, and 84% were males. The main predisposing conditions were injection drug use (IDU) (n = 71, 35.7%), congenital heart disease (n = 46, 21.6%), rheumatic heart disease (n = 25, 12.5%), and prosthetic device (n = 19, 9.5%). 17.1% of patients developed IE without identified predispositions. Among 64.3% culture-positive cases, the most prevalent causative pathogens were Staphylococcus aureus (46.1%), viridans streptococci (7.0%), enterococci (6.0%), coagulase-negative staphylococci (5.5%), gram negative bacilli (5.5%), polymicrobial (5.5%), and Candida (1.0%). The tricuspid (30.3%), mitral (25.6%), and aortic (21.6%) valves were the most common sites of infection, and 60.3% had large vegetations (>10 mm). Systemic embolization occurred in 55.3% of patients at presentation. Cardiac surgery was required for 13.1%. In-hospital mortality was 17.1% and was associated with prosthetic devices (p-value, 0.001), baseline leucocytosis (p-value, 0.036) or acute kidney injury (p-value, 0.001), and a microbial etiology of gram negative bacilli or enterococci (p-value, 0.005). Conclusion: IDU is now the most important predisposition for IE in India, and S. aureus has become the leading cause of native valve endocarditis with or without IDU.

6. Genetic diversity of Orientia tsutsugamushi strains from patients in north India

Abstract

Background: Scrub typhus has emerged as a major cause of acute febrile illness in India in recent years. The causative agent, Orientia tsutsugamushi has more than 20 prototype strains due to a variable 56-kDa outer membrane protein. It is crucial to know the prevailing types in India for the success of diagnostic immunoassays and prospective vaccine candidates. In north India, the principal types circulating are largely unknown. Our tertiary care hospital caters to a large area of north India (around 7 states and one union territory). Therefore, the current study was planned to identify the genotypes of O. tsutsugamushi strains circulating in this wide area of north India. Materials and methods: Adults and children presenting with suspected scrub typhus between July 2013 and December 2013 were included in this study. DNA was extracted from whole blood and a nested PCR was used to amplify a 483-bp region of the 56-kDa antigen gene of O. tsutsugamushi. The PCR products were purified and DNA sequencing was performed and aligned using the CLUSTAL_X2 program. A phylogenetic tree was constructed using neighbour- joining algorithms and analysed using the sequences obtained in this study and those obtained from the GenBank database. Results: A total of 34 samples were positive for PCR. The amplicons were sequenced and analyzed. Karp-like strains predominated in all states studied (64.7%) followed by Gilliam-like (26.47 %) and 8.82% similar to Hualien 1 or S072. We did not find any Kato or Kawasaki-like strain. Karp like strains showed >99% similarity to TH2033, TH2191, TH2208, Xinjiang & Neimeng strains and Gilliam-like strains showed >99% similarity to Clone ISS -11. Conclusion: Orientia tsutsugamushi shows a great diversity in its strains over a large geographical area of north India. 7. Effect of process and formulation variables on the preparation of parenteral paclitaxel-loaded biodegradable polymeric nanoparticles: A co-surfactant study

Abstract

The purpose of this study was to evaluate the effect of process (homogenization speed and evaporation time) and formulation (aqueous/organic phase ratio, surfactant concentration, polymer type and concentration, and drug amount) variables on the preparation of paclitaxel-loaded biodegradable polymeric nanoparticles using modified solvent evaporation technique. Thereafter, a formulation was selected and subjected to evaluation of inclusion of a co-surfactant for further reduction of particle size. Particle size, encapsulation efficiency and in-vitro drug release kinetics were evaluated. It was observed that the inclusion of vitamin E TPGS (0.01%), Poloxamer 188 (0.5%) or Tween 80 (0.25%) reduced the particle size of nanoparticles to 230, 244 or 301 nm from 438 nm, respectively. Encapsulation efficiency increased for both vitamin E TPGS and Poloxamer 188 up to concentration at 0.010% and 0.25%, respectively, while this was not the case for Tween 80. Comparison of drug release kinetics demonstrated that drug release accelerated from paclitaxel-loaded biodegradable nanoparticles prepared with the inclusion of Tween 80 but was delayed for Poloxamer 188 and vitamin E TPGS. Thus, it was concluded that the particle size of the nanoparticles could be reduced further and the paclitaxel release kinetics could easily be adjusted by taking advantage by the inclusion of a co-surfactant.

Abstract

Summary: Bungarus caeruleus (Indian common krait) bite during monsoons is common in Northwest India. This study was undertaken to find the effectiveness of neostigmine and polyvalent antivenom in improving neuromuscular paralysis following bite. All the consecutive patients admitted between June 2007 and December 2008 with common krait bite, identified either from brought snake or circumstantial evidence were studied. Ten vials of polyvalent antivenom and three doses of 2.5 mg neostigmine at 30 min intervals after administration of 0.6 mg of atropine were administered I.V. and patients were assessed for any improvement in neuroparalysis. Seventy-two patients were admitted during the study period. All the patients except two came from rural areas and were brought between June and September. Sixty-two patients were bitten during the day while clearing bricks, cutting grass or walking. The mean time interval between bite and arrival to hospital was 4.5 h. None of the patients showed any improvement following treatment and all patients developed respiratory paralysis, requiring assisted ventilation. Seventy survived and two died. Neostigmine is ineffective in reversing or improving neuroparalytic features in patients with B. caeruleus bite even at higher dose than normally recommended. Abstract

Identification of potential drug compounds and drug–target interactions are fundamental steps in the drug-designing process. However, the conventional process of drug discovery is expensive, time consuming, and insufficient to explore novel interactions and activity predictions. Thus arises the need for in silico chemoinformatics approaches with improved strategies that are useful to elevate predictive performance in drug designing. The objective of this study is to discuss the current approaches and major chemoinformatics tools available to perform data extraction, structure-based virtual screening, designing of new molecule identification using quantitative structure–activity relationship (QSAR), and applications of machine learning (ML) in biological or chemical evaluation. Recently, a large amount of research has been done and considered for applications of chemoinformatics in drug discovery. Reviewing various tools and techniques, the chapter mainly discusses different aspects such as (1) compound databases/libraries, (2) important tools of structure-based and ligand-based virtual screening, including molecular docking and pharmacophore-designing tools, (3) different toolkit and ML algorithms with a strong emphasis on QSAR modeling, and (4) future challenges and directions in the present state. This study would likely provide insights into advanced approaches in utilizing effective strategies and the generation of favorable bioprediction methods in drug designing.

Abstract

Alzheimer’s disease (AD) is a progressive neurodegenerative disorder mostly prevalent among elderly people over 65 years of age. It is the most frequent type of dementia, which badly affects the social and personal skills and behavior of the patients. Patients with AD are increasing day by day creating an effective treatment challenge among health professionals worldwide. New studies are being carried out to incorporate medicinal-based food along with traditional medicines to improve the efficiency of treatment. Caprylic acid is one among various medicinal foods that have special health-promoting benefits and disease control properties. It is a medium chain fatty acid found naturally in palm kernel oil, coconut oil, and some dairy products. In AD, it induces the process of ketosis to provide extra energy to the brain, hence it improves cognitive functions. Research has been carried out to find its effectiveness in the treatment of AD. This chapter focuses on caprylic acid, its properties, and its application in AD.

Abstract

There is a need of a carrier that can deliver drugs to the targeted cells and organs, where it can prolong and localize its effect, interacting with the diseased part of the organ specifically to obtain the maximum therapeutic efficiency. There are various methods aimed at the packaging of drug molecules in nanoparticles that can specifically deliver the drug to the pathological area. These nanoparticles are designed in a way that they target the affected region of the particular organ without interacting with the healthy part. Nanoparticles have the ability to cross the cell membrane easily so that it is targeted to the diseased tissue, which in turn decreases the rate of drug doses that have a more uniform effect, reducing the side effect of drug. However, there are various ways to synthesize nanoparticles for packaging of drug compounds and they require a systematic review to analyze their operational efficiency, safety, up-scaling and costs. Therefore, this chapter undertakes a detailed investigation of various factors that influence nanoparticle-based drug delivery systems.
