1. Monoamine oxidases: A missing link between mitochondria and inflammation in chronic diseases ?

Abstract: The role of mitochondria spans from the regulation of the oxidative phosphorylation, cell metabolism and survival/death pathways to a more recently identified function in chronic inflammation. In stress situations, mitochondria release some pro-inflammatory mediators such as ATP, cardiolipin, reactive oxygen species (ROS) or mitochondrial DNA, that are believed to participate in chronic diseases and aging. These mitochondrial Damage-Associated Molecular Patterns (mito-DAMPs) can modulate specific receptors among which TLR9, NLRP3 and cGAS-STING, triggering immune cells activation and sterile inflammation. In order to counter the development of chronic diseases, a better understanding of the underlying mechanisms of low grade inflammation induced by mito-DAMPs is needed. In this context, monoamine oxidases (MAO), the mitochondrial enzymes that degrade catecholamines and serotonin, have recently emerged as potent regulators of chronic inflammation in obesity-related disorders, cardiac diseases, cancer, rheumatoid arthritis and pulmonary diseases. The role of these enzymes in inflammation embraces their action in both immune and non-immune cells, where they regulate monoamines levels and generate toxic ROS and aldehydes, as by-products of enzymatic reaction. Here, we discuss the more recent advances on the role and mechanisms of action of MAOs in chronic inflammatory diseases.

2. Mitochondrial F0F1-ATP synthase governs the induction of mitochondrial fission

Abstract: Summary: Mitochondrial dynamics is a process that balances fusion and fission events, the latter providing a mechanism for segregating dysfunctional mitochondria. Fission is controlled by the mitochondrial membrane potential (ΔΨm), optic atrophy 1 (OPA1) cleavage, and DRP1 recruitment. It is thought that this process is closely linked to the activity of the mitochondrial respiratory chain (MRC). However, we report here that MRC inhibition does not decrease ΔΨm nor increase fission, as evidenced by hyperconnected mitochondria. Conversely, blocking F0F1-ATP synthase activity induces fragmentation. We show that the F0F1-ATP synthase is sensing the inhibition of MRC activity by immediately promoting its reverse mode of action to hydrolyze matrix ATP and restoring ΔΨm, thus preventing fission. While this reverse mode is expected to be inhibited by the ATPase inhibitor ATPIF1, we show that this sensing is independent of this factor. We have unraveled an unexpected role of F0F1-ATP synthase in controlling the induction of fission by sensing and maintaining ΔΨm.

Abstract: Summary: Background: The oncological safety of minimally invasive surgery has been questioned for several abdominal cancers. Concerns also exist regarding the use of minimally invasive distal pancreatectomy (MIDP) in patients with resectable pancreatic cancer as randomised trials are lacking. Methods: In this international randomised non-inferiority trial, we recruited adults with resectable pancreatic cancer from 35 centres in 12 countries. Patients were randomly assigned to either MIDP (laparoscopic or robotic) or open distal pancreatectomy (ODP). Both patients and pathologists were blinded to the assigned approach. Primary endpoint was radical resection (R0, ≥1 mm free margin) in patients who had ultimately undergone resection. Analyses for the primary endpoint were by modified intention-to-treat, excluding patients with missing data on primary endpoint. The pre-defined non-inferiority margin of −7% was compared with the lower limit of the two-sided 90% confidence interval (CI) of absolute difference in the primary endpoint. This trial is registered with the ISRCTN registry (ISRCTN44897265). Findings: Between May 8, 2018 and May 7, 2021, 258 patients were randomly assigned to MIDP (131 patients) or ODP (127 patients). Modified intention-to-treat analysis included 114 patients in the MIDP group and 110 patients in the ODP group. An R0 resection occurred in 83 (73%) patients in the MIDP group and in 76 (69%) patients in the ODP group (difference 3.7%, 90% CI −6.2 to 13.6%; pnon-inferiority = 0.039). Median lymph node yield was comparable (22.0 [16.0–30.0] vs 23.0 [14.0–32.0] nodes, p = 0.86), as was the rate of intraperitoneal recurrence (41% vs 38%, p = 0.45). Median follow-up was 23.5 (interquartile range 17.0–30.0) months. Other postoperative outcomes were comparable, including median time to functional recovery (5 [95% CI 4.5–5.5] vs 5 [95% CI 4.7–5.3] days; p = 0.22) and overall survival (HR 0.99, 95% CI 0.67–1.46, p = 0.94). Serious adverse events were reported in 23 (18%) of 131 patients in the MIDP group vs 28 (22%) of 127 patients in the ODP group. Interpretation: This trial provides evidence on the non-inferiority of MIDP compared to ODP regarding radical resection rates in patients with resectable pancreatic cancer. The present findings support the applicability of minimally invasive surgery in patients with resectable left-sided pancreatic cancer. 6. Impact of PECVD deposition on dielectric charge and passivation for n-GaN/SiOx interfaces

Abstract: Controlling properties of GaN/dielectric interfaces is crucial for determining the characteristics of MOS-HEMT devices and their stability. Interface properties are largely affected by the techniques and specific conditions of dielectric deposition. In this work, a Taguchi design of experiment was applied to study the effect of plasma parameters during deposition of SiOx by PECVD for passivation of n-GaN. SiOx/GaN MIS capacitors were fabricated and characterized by capacitance measurement at a high probing frequency of 1 MHz. The interface states density, hysteresis and flatband voltage were analyzed and modeled in relation with the flow of SiH4, plasma power, chamber pressure and temperature. Excellent fits could be obtained on a single model including linear terms for all studied parameters and quadratic terms for the flow of SiH4 and temperature. We show that it is possible to obtain some control of the flatband voltage while maintaining a good interface quality. Positive flatband voltages are potentially of interest to enable normally-off operation for MOS-HEMTs and this could be obtained mainly by using a high SiH4/N2O ratio. To the contrary, negative flatband voltage values often ensure the most stable operation of MOS-HEMTs and this was achieved with a low SiH4/N2O and high plasma power. MIS capacitors with near-zero flatband voltage were also obtained with low SiH4/N2O ratio and low plasma power. Hysteresis and interface states density in relation with deposition plasma conditions are also analyzed in order to offer the best trade-offs depending on the end applications of MOS-GaN devices. By demonstrating the great impact of plasma conditions during dielectric deposition on electronic properties of MIS devices, we show that the process of gate insulation can be optimized to simultaneously control the density of defects and fixed charge at the interface.

Abstract: The RISMEAU project (RISques liés aux résidus de Médicaments, biocides et antibiorésistance d'origine humaine et vétérinaire sur les ressources en EAU du bassin versant de l'Arve – Risks related to residues of pharmaceuticals and biocides, and antimicrobial resistance of human and veterinary origin on the water resources of the 2083 km2 Arve catchment located in the French Alps) was implanted from 2018 to 2024 on the SIPIBEL observatory. It was devoted to the evaluation of (i) transfers of and processes related to pharmaceutical residues and biocides from both urban sludge and manure spread on fields as fertilisers, and (ii) the environmental impacts of land spreading, in particular the ecotoxicological risks and antimicrobial resistance dissemination. The methodology was based on the physico-chemical, ecotoxicological and antimicrobial resistance (AMR – assessed by molecular biology) characterisation of leachate and soil matrices samples, and focused on organic waste products application at locally representative agronomic rates. This dataset can be reused by other researchers for comparison with their own investigations on this emerging topic under different contexts and conditions, and may contribute to international reviews.The database includes in total 26,217 values, measured on 348 samples of organic waste products, soil, in situ lysimeter leachate, earthworms, plants with 3136 usual physico-chemical values, 15,469 values on pharmaceuticals and biocides concentrations, 6827 bioassay values (ecotoxicity and phytotoxicity) and 785 values of antimicrobial resistance indicators. The 348 samples have been collected from Nov. 2019 to Dec. 2022: 4 samples in 2019, 26 in 2020, 130 in 2021 and 188 in 2022. Regarding AMR, 96 assays coding for most abundant resistance genes in healthy humans, clinically relevant antibiotic resistance genes, biocide resistance genes, heavy metal genes, integrons (class I, II and III), and mobile genetic elements (transposase genes) have been carried out.

8. NPS, PEDs and other emerging drugs on the Clearnet and the Darkweb: Use in sport

Abstract: The aim of the project was to identify new performance enhancing drugs (PEDs) including novel psychoactive substances (NPS) both in the Darkweb as well as in the Clearnet that could be offered to athletes to improve their performance.Searches on the Darkweb revealed that the number of PEDs offered to athletes is limited, and mainly center on well-known anabolic steroids, peptide hormones, stimulants and metabolic modulators. No novel substances were found and the most sophisticated were well known and also offered on the Clearnet.On the Clearnet, we targeted novel psychoactive substances (NPS) reported by the UNODC Early Warning Advisory on NPS during the period of 2018 to July 2023. None of these substances could be found in nutritional supplements targeting the athletic population, nor did they appear in chat rooms exchanging information on performance enhancing stimulants.In conclusion, it appears that PEDs constitute only a minimal part of the Darkweb market and all of the ones offered are well known substances. On the Clearnet, NPS recently released into the market do not make immediate appearance in supplements for the sports community.

Abstract: Summary: Malaria mosquitoes mate in swarms, but how these swarms are formed and maintained remains poorly understood. We characterized three-dimensional spatiotemporal flight kinematics of Anopheles coluzzii males swarming at sunset above a ground marker. The location, shape, and volume of swarms were highly stereotypic, consistent over the complete swarming duration. Swarms have an elliptical cone shape; mean flight kinematics varies spatially within the swarm, but remain rather consistent throughout swarming duration. Using a sensory system-informed model, we show that swarming mosquitoes use visual perception of both the ground marker and sunset horizon to display the swarming behavior. To control their height, swarming individuals maintain an optical angle of the marker ranging from 24° to 55°. Limiting the viewing angle deviation to 4.5% of the maximum value results in the observed elliptical cone swarm shape. We discuss the implications of these finding on malaria mosquito mating success, speciation and for vector control.

The aim was to assess compliance with this guideline during six years after publication.Materials and methods: Nationwide guideline compliance was investigated for three subsequent time periods (2012-2013 vs. 2014-2015 vs. 2016-2017) in patients with pancreatic cancer using five quality indicators in the Netherlands Cancer Registry: 1) discussion in multidisciplinary team meeting (MDT), 2) maximum 3-week interval from final MDT to start of treatment, 3) preoperative biliary drainage when bilirubin >250 mu mol/L, 4) use of adjuvant chemotherapy, and 5) chemotherapy for inoperable disease (non-metastatic and metastatic).Results: In total, 14 491 patients were included of whom 2290 (15.8%) underwent resection and 4561 (31.5%) received chemotherapy. Most quality indicators did not change over time: overall, 88.8% of patients treated with curative intent were discussed in a MDT, 42.7% were treated with curative intent within the 3-week interval, 62.7% with a resectable head tumor and bilirubin >250 mu mol/L underwent preoperative biliary drainage, 57.2% received chemotherapy after resection, and 36.6% with metastatic disease received chemotherapy. Only use of chemotherapy for non-metastatic, non-resected disease improved over time (23.4% vs. 25.6% vs. 29.7%).Conclusion: Nationwide compliance to five quality indicators for the guideline on pancreatic cancer management showed little to no improvement during six years after publication. Besides critical review of the current quality indicators, these outcomes may suggest that a nationwide implementation program is required to increase compliance to guideline recommendations. Abstract: Silicon nitride and boron nitride (Si3N4/h-BN) based ceramic composites were prepared by Spark Plasma Sintering (SPS) using aluminium oxide (Al2O3) and/or yttrium oxide (Y2O3) as sintering additives. The amount of h-BN introduced varied from 0 vol.% to 20 vol.% in order to study its influence on ceramic properties. In the same way, the amount of oxide additives has been controlled and the influence of the additive's nature on the final properties has been underlined. The sintering temperature has been selected in order to obtain equivalent relative densities. The formation of β-SiAlON has been underlined by XRD and TEM analysis. Moreover, due to the fast-sintering process, amorphous secondary phase has been formed. Mechanical properties (hardness, fracture toughness, Young modulus) and thermal diffusivity have been evaluated. It appears that the nature of the oxide additive used plays an important role on the mechanical and thermal behaviour of Si3N4/h-BN composites.

Abstract: Introduction: PRP is gaining increasing interest for pain relief and improvement of joint function in patients with knee osteoarthritis (KOA) but practices and results remain heterogeneous limiting its adoption as standard of care. Current international recommendations are to collect real-life evidence of efficacy with a systematic monitoring of PRP quality and patients' outcomes. We aimed to analyze the response of patients presenting KOA and treated with standardized PRP injection in routine care. We also investigated the potential contributing factors including patient's phenotype and PRP characteristics. Methods: Patients with symptomatic KOA and that failed first-line therapy received a single injection of a qualified PRP prepared using medical devices allowing to recover a high/very high volume of very pure PRP. Visual analogue scale (VAS) and Western Ontario and McMaster Osteoarthritis Index (WOMAC) score were recorded at baseline and during 18 months follow-up. Abstract: Background and purpose: MR-guided radiotherapy adds the precision of magnetic resonance imaging (MRI) to the therapeutic benefits of a linear accelerator. Prior to each therapeutic session, an MRI generates a significant volume of imaging data ripe for analysis. Radiomics stands at the forefront of medical imaging and oncology research, dedicated to mining quantitative imaging attributes to forge predictive models. However, the robustness of these models is often challenged. Materials and methods: To assess the robustness of feature extraction, we conducted reproducibility studies using a 0.35 T MR-linac system, employing both a specialized phantom and patient-derived images, focusing on cases of pancreatic cancer. We extracted shape-based, first-order and textural features from patient-derived images and only first-order and textural features from phantom-derived images. The impact of the delay between simulation and first fraction images was also assessed with an equivalence test. Results: From 107 features evaluated, 58 (54 %) were considered as non-reproducible: 18 were uniformly inconsistent across both phantom and patient images, 9 were specific to phantom-based analysis, and 31 to patient-derived data. Conclusion: Our findings show that a significant proportion of radiomic features extracted from this dual dataset were unreliable. It is essential to discard these non-reproducible elements to refine and enhance radiomic model development, particularly for MR-guided radiotherapy in pancreatic cancer.

14. European community pharmacists practice in tackling influenza

Abstract: Background: In many European countries, flu vaccination coverage rates are below the 75% target. During the COVID-19 pandemic, many pharmacists around Europe were involved as vaccine administrators and demonstrated positive results in improving vaccine uptake. This paper explores the challenges, accomplishments, and best practices of various European pharmacists' associations in administering vaccines and positively contributing to public health. Methods: Eight pharmacists representing various associations from different countries across Europe (Italy, Belgium, Poland, Portugal, France, and Germany) convened to discuss their role as vaccination providers, the advantages, and strategies for improvement, and to identify barriers and gaps in the vaccination administration process, especially focusing on the administration of seasonal flu vaccines. Results: Currently, 15 European countries allow community pharmacists to dispense and administer flu vaccines. Among the ones that attended the meeting, Portugal initiated the flu immunization program at the pharmacy earliest, before the COVID era, but in other countries, the process started only in the last couple of years. Initial hesitancy and reluctance by other HCPs or institutions were overcome as the pilot projects showed positive and cost-effective public health results. Today, pharmacists are considered crucial professional figures to provide immunization services against COVID-19, the flu, and other vaccine-preventable diseases, and pursue important public health goals.Key takeaways to enhance the pharmacist's role in providing immunization services against vaccine-preventable diseases include improving interaction with policymakers and the public, generating real-world evidence highlighting public health benefits, and ensuring ongoing professional education and training for pharmacists. Conclusion: Vaccinating pharmacists are gaining recognition of their role and the benefits derived from their broader involvement in the healthcare system, including immunization programs. Further efforts are needed in each country for an adequate recognition of the profession and a broader utilization of pharmacy services to exploit the benefit of immunization, especially against the flu.

Abstract: Background & Aims: Major bleeding events during orthotopic liver transplantation (OLT) are associated with poor outcomes. The proportion of this risk related to portal hypertension is unclear. Hepatic venous pressure gradient (HVPG) is the gold standard for estimating portal hypertension. The aim of this study was to analyze the ability of HVPG to predict intraoperative major bleeding events during OLT in patients with cirrhosis. Methods: We retrospectively analyzed a prospective database including all patients with cirrhosis who underwent OLT between 2010 and 2020 and had liver and right heart catheterizations as part of their pre-transplant assessment. The primary endpoint was the occurrence of an intraoperative major bleeding event. Results: The 468 included patients had a median HVPG of 17 mmHg [interquartile range, 13-22] and a median MELD on the day of OLT of 16 [11-24]. Intraoperative red blood cell transfusion was required in 72% of the patients (median 2 units transfused), with a median blood loss of 1,000 ml [575-1,500]. Major intraoperative bleeding occurred in 156 patients (33%) and was associated with HVPG, preoperative hemoglobin level, severity of cirrhosis at the time of OLT (MELD score, ascites, encephalopathy), hemostasis impairment (thrombocytopenia, lower fibrinogen levels), and complications of cirrhosis (sepsis, acute-on-chronic liver failure). By multivariable regression analysis with backward elimination, HVPG, preoperative hemoglobin level, MELD score, and tranexamic acid infusion were associated with the primary endpoint. Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

17. Sustainable alternative to irrigated maize monoculture in a maize-dominated cropped area: Lessons learned from a system experiment

Abstract: Maize is the most-produced food crop in the world and is grown in intensive rotations or in monoculture (continuous maize) systems. As maize production has expanded massively across the world, many concerns have emerged about its local environmental and other global impacts. Agronomists have designed innovative cropping systems and assessed them using system experiments to make arable systems more sustainable. However, knowledge is still lacking on the sustainability of innovative cropping systems compared to highly intensive systems such as irrigated maize-based monoculture. Here, we present the assessment results of a nine-year system experiment in Alsace, France, developed to compare an innovative system based on a diversified rotation and innovative management practices (three-year rotation of maize/soybean/winter wheat (plus a cover crop) combined with reduced tillage) with a continuous maize reference system. The results cover a six-year assessment period following an initial three-year design period. Classic criteria, such as profitability, workload, pesticide use, fossil energy consumption and nitrate leaching, were assessed along with other less studied criteria, such as pesticide leaching risk, soil structure, soil chemical quality and soil biological activity. Sustainability – which includes environmental, social and economic dimensions – was assessed with the MASC 2.0 method. Overall sustainability was substantially enhanced in the innovative system (5 out of 7 sustainability classes) in comparison with the low level of the reference system (2 out of 7). This was due to a clear improvement in the environmental performance (from 2 out of 5 to 5 out of 5) while social performance was high in both systems (4 out of 5) and economic performance was low (2 out of 5) due to very low contribution to economic development. Nevertheless, the innovative system had a major drawback: lower profitability, especially when scenarios included high maize prices. Furthermore, herbicide use on maize was higher in the innovative system than in the reference one. Avenues for progress, such as encouraging stakeholder participation at the assessment stage or additional innovations such as multiple cropping, are suggested.

18. Surgical Treatment of Multiple Venous and Arterial Aneurysms Due to Arteriovenous Malformations of the Arm

Abstract: Introduction: Arteriovenous malformations (AVMs) are rare, especially in elderly patients. Occasionally, AVM can produce aneurysmal degenerations, which can lead to bleeding or rupture. The aim of this case report was to describe the surgical treatment of large arterial and venous aneurysms in the arm associated with an AVM. Report: An 83 year old woman of White ethnicity who was a non-smoker presented with a large pulsatile aneurysm at the left elbow with paresis of the first three fingers. The diagnosis was made by duplex ultrasonography (DUS), computed tomography angiography (CTA), and arteriography. Additional tests confirmed aneurysms of the brachial artery and the outflow veins, with the largest more than 7 cm in diameter. A very proximal brachial artery bifurcation and increased venous flow were noted. DUS confirmed the AVM by showing continuous flow in the axillary vein. The decision for surgical resection involved vascular surgeons, radiologists, angiologists, and anaesthetists. Treatment involved opening and excision of multiple venous aneurysms and AVMs. A short segment of the aneurysmal brachial artery was also resected and repaired with end to end anastomosis. The deep brachial artery which supplied AVMs and venous aneurysms was ligated and excision of these lesions was performed. At one year follow up, there were no complications and the revascularisation was patent. Discussion: Arterial and venous aneurysms occurring together with AVMs are rare and not well documented in the medical literature. In this case, surgical intervention, including resection with direct anastomosis of the arterial aneurysm coupled with excision of venous aneurysms and AVM, proved to be effective, as evidenced by stable post-operative outcomes after one year.

Terrestrial configurations detect close objects (i.e., lower vegetation strata) in more detail while airborne configurations detect a more detailed upper strata, with weak backscattered signals from lower strata. Moreover, discrete lidar systems record single or multiple hits from a given pulse at intercepted features in contrast to full-waveform systems, which register the pulse's complete backscattered signal providing complete vertical profiles. In this study, we examine for a boreal and a Mediterranean forest with contrasted conifer canopy densities: (i) the characterization of the vertical distribution and signal occlusion from three laser scanning configurations: full-waveform airborne (ALS(FW)), discrete airborne (ALS(D)), and discrete terrestrial (TLS); (ii) the comparison in the detection of understory vegetation by ALS(FW) and ALS(D) using TLS as reference; and (iii) the use of a methodological procedure based on the Gini index concept to group understory vegetation in density classes from both ALS(FW) and ALS(D) configurations. Our results demonstrate, firstly, that signal occlusion can be quantified by the rate of pulse reduction independently for data from all three laser scanning configurations. The ALS(D) configuration was the most affected by signal occlusion, leading to weak signal returns at the lower strata (z < 4 m) where the rate of pulse reduction was highest as a result of dense canopy covers. Secondly, we demonstrated the capabilities for both airborne laser scanning configurations to detect understory vegetation, albeit significantly more accurately with ALS(FW). Lastly, we demonstrated the use of the Gini index as an indicator to determine understory vegetation density classes, particularly for ALS(FW) data in dense canopy cover. 20. Using deep learning to generate synthetic B-mode musculoskeletal ultrasound images

Abstract: Background and objective\ud Deep learning approaches are common in image processing, but often rely on supervised learning, which requires a large volume of training images, usually accompanied by hand-crafted labels. As labelled data are often not available, it would be desirable to develop methods that allow such data to be compiled automatically. In this study, we used a Generative Adversarial Network (GAN) to generate realistic B-mode musculoskeletal ultrasound images, and tested the suitability of two automated labelling approaches.\ud \ud Methods\ud We used a model including two GANs each trained to transfer an image from one domain to another. The two inputs were a set of 100 longitudinal images of the gastrocnemius medialis muscle, and a set of 100 synthetic segmented masks that featured two aponeuroses and a random number of ‘fascicles’. The model output a set of synthetic ultrasound images and an automated segmentation of each real input image. This automated segmentation process was one of the two approaches we assessed. The second approach involved synthesising ultrasound images and then feeding these images into an ImageJ/Fiji-based automated algorithm, to determine whether it could detect the aponeuroses and muscle fascicles.\ud \ud Results\ud Histogram distributions were similar between real and synthetic images, but synthetic images displayed less variation between samples and a narrower range. Mean entropy values were statistically similar (real: 6.97, synthetic: 7.03; p = 0.218), but the range was much narrower for synthetic images (6.91 – 7.11 versus 6.30 – 7.62). When comparing GAN-derived and manually labelled segmentations, intersection-over-union values- denoting the degree of overlap between aponeurosis labels- varied between 0.0280 – 0.612 (mean ± SD: 0.312 ± 0.159), and pennation angles were higher for the GAN-derived segmentations (25.1° vs. 19.3°; p < 0.001). For the second segmentation approach, the algorithm generally performed equally well on synthetic and real images, yielding pennation angles within the physiological range (13.8–20°).\ud \ud Conclusions\ud We used a GAN to generate realistic B-mode ultrasound images, and extracted muscle architectural parameters from these images automatically. This approach could enable generation of large labelled datasets for image segmentation tasks, and may also be useful for data sharing. Automatic generation and labelling of ultrasound images minimises user input and overcomes several limitations associated with manual analysis.

Abstract: Pain is a frequent and disabling non-motor symptom of Parkinson’s Disease (PD). Yet, no treatment to date can efficiently reduce this pain. This article investigates the brain functional connectivity of PD patients with central pain and the effects of levodopa and oxycodone on this connectivity.Thirty-eight PD patients received either levodopa, oxycodone, or a placebo during an eight-week period. Pain intensity was evaluated using the Visual Analogue Scale and resting-state functional connectivity was measured before and after treatments. PD patients were also separated into two groups: responders and non-responders.At baseline, the intensity of pain was correlated with the connectivity between the anterior insula and the posterior cingulate cortex and between the nucleus accumbens, the brainstem, and the hippocampus. Levodopa and oxycodone had no specific effects on functional connectivity. Responders had a decrease in connectivity between the anterior insula and the posterior cingulate cortex, while non-responders showed an increase in connectivity.The correlation between pain intensity and specific brain connectivity may represent a “hyper-awareness” of pain and a distortion of learning and memory systems in PD patients with central pain, leading to a state of chronic pain. The placebo effect could explain the changes in connectivity that are associated with a potential reduction in pain awareness.

Abstract: Summary: Germinal activity persists throughout life within the ventricular-subventricular zone (V-SVZ) of the postnatal forebrain due to the presence of neural stem cells (NSCs). Accumulating evidence points to a recruitment for these cells following early brain injuries and suggests their amenability to manipulations. We used chronic hypoxia as a rodent model of early brain injury to investigate the reactivation of cortical progenitors at postnatal times. Our results reveal an increased proliferation and production of glutamatergic progenitors within the dorsal V-SVZ. Fate mapping of V-SVZ NSCs demonstrates their contribution to de novo cortical neurogenesis. Transcriptional analysis of glutamatergic progenitors shows parallel changes in methyltransferase 14 (Mettl14) and Wnt/β-catenin signaling. In agreement, manipulations through genetic and pharmacological activation of Mettl14 and the Wnt/β-catenin pathway, respectively, induce neurogenesis and promote newly-formed cell maturation. Finally, labeling of young adult NSCs demonstrates that pharmacological NSC activation has no adverse effects on the reservoir of V-SVZ NSCs and on their germinal activity.

Abstract: Background: We evaluated national compliance to selected quality indicators from the Dutch multidisciplinary evidence-based guideline on pancreatic and periampullary carcinoma and identified areas for improvement. Methods: Compliance to 3 selected quality indicators from the guideline was evaluated before and after implementation of the guideline in 2011: 1) adjuvant chemotherapy after tumor resection for pancreatic carcinoma, 2) discussion of the patient within a multidisciplinary team (MDT) meeting and 3) a maximum 3-week interval between final MDT meeting and start of treatment. Results: In total 5086 patients with pancreatic or periampullary carcinoma were included. 25. Improving field-scale crop actual evapotranspiration monitoring with Sentinel-3, Sentinel-2, and Landsat data fusion

Abstract: One of the primary applications of satellite Land Surface Temperature (LST) observations lies in their utilization for modeling of actual evapotranspiration (ET) in agricultural crops, with the primary goals of monitoring and enhancing irrigation practices and improving crop water use productivity, as stipulated by Sustainable Development Goal (SDG) indicator 6.4.1. Evapotranspiration is a complex and dynamic process, both temporally and spatially, necessitating LST observations with high spatio-temporal resolution. Presently, none of the existing spaceborne thermal sensors can provide quasi-daily field-scale LST observations, prompting the development of methods for data fusion (thermal sharpening) of observations from various shortwave and thermal sensors to meet this spatio-temporal requirement. Previous research has demonstrated the effectiveness of combining shortwave-multispectral Sentinel-2 observations with thermal-infrared Sentinel-3 observations to derive daily, field-scale LST and ET estimates. However, these studies also highlighted limitations in capturing the distinct thermal contrast between cooler LST in irrigated agricultural areas and the hotter, adjacent dry regions. In this study, we aim to address this limitation by incorporating information on thermal spatial variability observed by Landsat satellites into the data fusion process, without being constrained by infrequent or cloudy Landsat thermal observations and while retaining the longwave radiance emission captured by the Sentinel-3 thermal sensor at its native resolution. Two approaches are evaluated, both individually and as a complementary combination, and validated against in situ LST measurements. The best performing approach, which leads to reduction in root mean square error of up to 1.5 K when compared to previous research, is subsequently used to estimate parcel-level actual evapotranspiration. The ET modeling process has also undergone various improvements regarding the gap-filling of input and output data, input datasets and code implementation. The resulting ET is validated using lysimeters and eddy covariance towers in Spain, Lebanon, Tunisia, and Senegal resulting in minimal overall bias (systematic underestimation of less than 0.07 mm/day) and a low root mean square error (down to 0.84 mm/day) when using fully global input datasets. The enhanced LST sharpening methodology is sensor agnostic and should remain relevant for the upcoming thermal missions while the accuracy of the modeled ET fluxes is encouraging for further utilization of observations from Sentinel satellites, and other Copernicus data, for monitoring SDG indicator 6.4.1.

27. Assessing the chronic toxicity of spreading organic amendments on agricultural soil: Tests on earthworms and plants

Abstract: Recycling organic wastes on agricultural soils improves the soil quality, but the environmental and health impact of these organic amendments closely depends on their origins, their bio-physicochemical characteristics and the considered organisms potentially affected. The aim of this study was to assess the potential chronic ecotoxicity of spreading organic amendments on agricultural soils. To do this, we characterized three different organic amendments: sewage sludge from an urban wastewater treatment plant, cow manure and liquid dairy manure. Their chronic ecotoxicity was studied through assays exposing earthworms of the species Eisenia fetida and two plants: Medicago sativa and Sinapis alba. Of the three amendments, the sewage sludge presented the highest concentrations of micropollutants and a considerable fraction of available and biodegradable organic matter. The cow manure and liquid dairy manure had lower chemical contamination and similar characteristics with lower biodegradable fractions of their organic matter. No chronic phytotoxicity was evidenced: on the contrary, particularly with sewage sludge, the germination rate and aerial and root biomass of the two plants increased. Considering earthworms, their biomass increased considerably during the reproduction assays in soil amended with sewage sludge, which contained the more bioavailable organic matter. Nonetheless, the earthworms presented an inhibition close to 78% of the production of juveniles when exposed to sewage sludge exceeding 20 g.kg-1 DW (that means 2 times the agronomic dose). This reprotoxic effect was also observed in the presence of liquid dairy manure, but not with cow manure. At the end of the assays, the glycogen and protein reserves in earthworms exposed to sewage sludge were inferior to that of control earthworms, respectively around 50% and 30%. For the earthworms exposed to liquid dairy manure, protein and lipid reserves increased. In the case of liquid dairy manure, this reprotoxic effect did not appear to be linked to the presence of micropollutants. In conclusion, our results confirm the need to use several ecotoxicity assays at different biological levels and with different biological models to assess the ecotoxic impacts of soil amendments. Indeed, although certain organic wastes present a strong nutritional potential for both plants and earthworms, a not inconsiderable risk was apparent for the reproduction of the latter. An integrated ecotoxicity criterion that takes into account a weighted sum of the different results would guide the utilization of organic amendments while ensuring the good health of agricultural ecosystems.

Abstract: Mechanical properties of two ternary carbides MAX phases (Ti2AlC and Ti3AlC2) were investigated under cyclic bending tests with coupled acoustic emission monitoring (AE). Results show significant differences in terms of observed microstructural changes associated to acoustic emission response. Influence of the proportion of MAX phases and the presence of porosity in the sample were studied. It was found that homogeneous proportions of MAX phases led to an increase of interfaces in the material and thus to lower mechanical properties, with a more acoustically active behaviour. Porosity was found to be a preferential site of initiation and propagation of cracks, leading to samples enduring slower but earlier rupture.

Abstract: Background: This study assessed the potential cost-effectiveness of high (80–100%) vs low (21–35%) fraction of inspired oxygen (FiO2) at preventing surgical site infections (SSIs) after abdominal surgery in Nigeria, India, and South Africa. Methods: Decision-analytic models were constructed using best available evidence sourced from unbundled data of an ongoing pilot trial assessing the effectiveness of high FiO2, published literature, and a cost survey in Nigeria, India, and South Africa. Effectiveness was measured as percentage of SSIs at 30 days after surgery, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: High FiO2 may be cost-effective (cheaper and effective). In Nigeria, the average cost for high FiO2 was $216 compared with $222 for low FiO2 leading to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the average cost for high FiO2 was $184 compared with $195 for low FiO2 leading to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the average cost for high FiO2 was $1164 compared with $1257 for low FiO2 leading to a −$93 (95% CI: −$132 to −$65) difference in costs. The high FiO2 arm had few SSIs, 7.33% compared with 8.38% for low FiO2, leading to a −1.05 (95% CI: −1.14 to −0.90) percentage point reduction in SSIs. Conclusion: High FiO2 could be cost-effective at preventing SSIs in the three countries but further data from large clinical trials are required to confirm this.

Abstract: Despite the increase in the number of blockchain-based Decentralized Autonomous Organizations (DAOs), there is no consensus on what constitutes a DAO. This paper provides an in-depth study of DAOs by analyzing their definitions, characteristics, and emerging developments. Existing definitions in the literature hardly recognize common functionalities and intermingle coded DAOs, DAO deployment platforms, and blockchain DAOs. We developed a comprehensive DAO definition by reviewing the literature and empirically analyzing 1,859 DAOs. The findings show that many DAOs were inactive and that a threshold of 20 tokenholders is a tipping point for DAOs to survive over time and maintain sustained levels of activity. Finally, based on an empirical analysis of 9,845 perceived DAOs, we identified the emerging development of off-chain voting. This emerging development challenges the autonomous nature of DAOs. We recommend further research to investigate the effect of governance structures on their long-term sustainability and viability for both on-chain and off-chain DAOs.

Abstract: Summary: Background: Prostatic artery embolisation (PAE) is a minimally invasive treatment of symptomatic benign prostatic hyperplasia (BPH). Our aim was to compare patient's symptoms improvement after PAE and medical treatment. Methods: A randomised, open-label, superiority trial was set in 10 French hospitals. Patients with bothersome lower urinary tract symptoms (LUTS) defined by International Prostatic Symptom Score (IPSS) > 11 and quality of life (QoL) > 3, and BPH ≥50 ml resistant to alpha-blocker monotherapy were randomly assigned (1:1) to PAE or Combined Therapy ([CT], oral dutasteride 0.5 mg/tamsulosin hydrochloride 0.4 mg per day). Randomisation was stratified by centre, IPSS and prostate volume with a minimisation procedure. The primary outcome was the 9-month IPSS change. Primary and safety analysis were done according to the intention-to-treat (ITT) principle among patients with an evaluable primary outcome. ClinicalTrials.gov Identifier: NCT02869971. Findings: Ninety patients were randomised from September 2016 to February 2020, and 44 and 43 patients assessed for primary endpoint in PAE and CT groups, respectively. The 9-month change of IPSS was −10.0 (95% confidence interval [CI]: −11.8 to −8.3) and −5.7 (95% CI: −7.5 to −3.8) in the PAE and CT groups, respectively. This reduction was significantly greater in the PAE group than in the CT group (−4.4 [95% CI: −6.9 to −1.9], p = 0.0008). The IIEF-15 score change was 8.2 (95% CI: 2.9–13.5) and −2.8 (95% CI: −8.4 to 2.8) in the PAE and CT groups, respectively. No treatment-related AE or hospitalisation was noticed. After 9 months, 5 and 18 patients had invasive prostate re-treatment in the PAE and CT group, respectively. Interpretation: In patients with BPH ≥50 ml and bothersome LUTS resistant to alpha-blocker monotherapy, PAE provides more urinary and sexual symptoms benefit than CT up to 24 months. Abstract: Summary: Septic patients frequently develop skeletal muscle wasting and weakness, resulting in severe clinical consequences and adverse outcomes. Sepsis triggers sustained induction of autophagy, a key cellular degradative pathway, in skeletal muscles. However, the impact of enhanced autophagy on sepsis-induced muscle dysfunction remains unclear. Using an inducible and muscle-specific Atg7 knockout mouse model (Atg7iSkM−KO), we investigated the functional importance of skeletal muscle autophagy in sepsis using the cecal ligation and puncture model. Atg7iSkM−KO mice exhibited a more severe phenotype in response to sepsis, marked by severe muscle wasting, hypoglycemia, higher ketone levels, and a decreased in survival as compared to mice with intact Atg7. Sepsis and Atg7 deletion resulted in the accumulation of mitochondrial dysfunction, although sepsis did not further worsen mitochondrial dysfunction in Atg7iSkM−KO mice. Overall, our study demonstrates that autophagy inactivation in skeletal muscles triggers significant worsening of sepsis-induced muscle and metabolic dysfunctions and negatively impacts survival.

Abstract: Transaminase levels are usually measured as markers of hepatocellular injury following liver resection, but recent evidence was unclear on their clinical value. This study aimed to identify factors that determine peak postoperative transaminase levels and correlated transaminase levels to postoperative complications. All liver resections performed at a single center between 2006 and 2015 were included in the analysis. Multivariate analysis was used to identify factors that determine peak ALT and AST levels and postoperative morbidity and mortality. An ALT and AST cutoff for the prediction of mortality was determined using receiver operating characteristic curves analysis. A total of 539 resections were included. Clavien-Dindo grade III or higher complications, intraoperative transfusion, and operative duration were identified as determinants of peak transaminases. A peak AST cut-off value for predicting mortality was defined at 828 U/L, with an area under the curve of 0.81 (0.73-0.89). The cut-off was an independent predictor of mortality (P < 0.01) along with (intraoperative) transfusion (P < 0.01), fifty-fifty criteria (P < 0.01), and age (P < 0.01). Postoperative transaminase levels are independent predictors of postoperative morbidity and mortality and therefore clinically relevant. Transaminase levels usually peak during the first 24 h after surgery and thus possess early prognostic power in terms of postoperative mortality

Abstract: Minimally invasive (MI) pancreatic surgery appears to be gaining popularity, but its implementation throughout Europe and the opinions regarding its use in pancreatic cancer patients are unknown. A 30-question survey was sent between June and December 2014 to pancreatic surgeons of the European Pancreatic Club, European-African Hepato-Pancreato-Biliary Association and 5 European national pancreatic societies. Incomplete responses were excluded. In total, 237 pancreatic surgeons responded. After excluding 34 incomplete responses, 203 responses from 27 European countries were included. 164 (81%) surgeons were employed at a university hospital, 184 (91%) performed advanced MI surgery and 148 (73%) performed MI distal pancreatectomy. MI pancreatoduodenectomy was performed by 42 (21%) surgeons, whereas 9 (4.4%) surgeons had performed more than 10 procedures. Robot-assisted MI pancreatic surgery was performed by 28 (14%) surgeons. 63 (31%) surgeons expected MI distal pancreatectomy for cancer to be inferior to open distal pancreatectomy concerning oncological outcomes. 151 (74%) surgeons expected to benefit from training in MI distal pancreatectomy and 149 (73%) were willing to participate in a randomized trial on this topic. MI distal pancreatectomy is a common procedure, although its use for cancer is still disputed. MI pancreatoduodenectomy is still an uncommon procedure. Abstract: Summary: Background: Preoperative COVID-19 has been associated with excess postoperative morbi-mortality. Consequently, guidelines were developed that recommended the postponement of surgery for at least 7 weeks after the infection. We hypothesised that vaccination against the SARS-CoV-2 and the large predominance of the Omicron variant attenuated the effect of a preoperative COVID-19 on the occurrence of postoperative respiratory morbidity. Methods: We conducted a prospective cohort study in 41 French centres between 15 March and 30 May 2022 (ClinicalTrials NCT05336110), aimed at comparing the postoperative respiratory morbidity between patients with and without preoperative COVID-19 within 8 weeks prior to surgery. The primary outcome was a composite outcome combining the occurrence of pneumonia, acute respiratory failure, unexpected mechanical ventilation, and pulmonary embolism within the first 30 postoperative days. Secondary outcomes were 30-day mortality, hospital length-of-stay, readmissions, and non-respiratory infections. The sample size was determined to have 90% power to identify a doubling of the primary outcome rate. Adjusted analyses were performed using propensity score modelling and inverse probability weighting. Findings: Of the 4928 patients assessed for the primary outcome, of whom 92.4% were vaccinated against the SARS-CoV-2, 705 had preoperative COVID-19. The primary outcome was reported in 140 (2.8%) patients. An 8-week preoperative COVID-19 was not associated with increased postoperative respiratory morbidity (odds ratio 1.08 [95% CI 0.48–2.13]; p = 0.83). None of the secondary outcomes differed between the two groups. Sensitivity analyses concerning the timing between COVID-19 and surgery, and the clinical presentations of preoperative COVID-19 did not show any association with the primary outcome, except for COVID-19 patients with ongoing symptoms the day of surgery (OR 4.29 [1.02–15.8]; p = 0.04). Interpretation: In our Omicron-predominant, highly immunised population undergoing general surgery, a preoperative COVID-19 was not associated with increased postoperative respiratory morbidity. Abstract: The presented design is a low-cost, compact, and open-source USB-controlled platform for biological tissue and electrode-tissue interface electrical measurements, capable of potentiostatic and galvanostatic electrical impedance spectroscopy up to 10 MHz and cyclic voltammetry with voltage compliance of +−8 V and up to 2.4 mA while ensuring tissue-safety conditions. The data acquisition and generation are based on an Analog Discovery 2 platform (Digilent, USA). We provide accuracy analysis and comparisons with a commercially available calibrated impedance analyzer. Impedance measurements are demonstrated on implanted electrodes for neural stimulation and on an isolated ex-vivo calf brain as an example use case of the presented design.

Abstract: Novel psychoactive substances (NPS) is an umbrella term used to describe a heterogeneous group of compounds that mimic the effects of existing drugs and whose demand and use rapidly emerge, change, or even vanish in the drug market. The novelty of this global phenomenon and its dynamic nature represent major challenges for the scientific community that constantly requires timely evidence-based inputs. Our aim is to review the literature on NPS and compare its temporal evolution according to the topics presented at the International Conference series on NPS over the past decade. Our analysis shows that some new clusters of research recently emerged in comparison to a previous review and that the material presented at the NPS Conferences anticipates the scientific literature by approximately 2.5 years. Such findings not only provide new original insights on the latest NPS trends but also address existing knowledge gaps in the NPS field, while emphasizing the importance of face-to-face thematic events supported by faster publication processes to inform prompt interventions and policy making.

Abstract: Background: In patients with Parkinson’s Disease (PD), two distinct motor subtypes, tremor dominant (TD) and postural instability and gait difficulty (PIGD), can be differentiated using Unified Parkinson’s Disease Rating Scale (UPDRS) sub-scores. This post hoc analysis of pooled data from eight pivotal studies examined the effect of treatment with istradefylline, a selective adenosine A2A receptor antagonist, on these subtypes. Methods: In eight randomized, placebo-controlled phase 2b/3 trials, patients on levodopa with carbidopa/benserazide experiencing motor complications received istradefylline (20 or 40 mg/day) or placebo for 12 or 16 weeks. TD subtype was defined by the UPDRS II/III items kinetic and postural tremor in right/left hand and (resting) tremor in the face, lips, chin, hands, or feet; PIGD items were freezing, walking, posture, gait, and postural instability. The ratio of mean scores from TD:PIGD items determined subtype (TD [TD:PIGD ratio ≥ 1.5], PIGD [TD:PIGD ratio ≤ 1.0], mixed-type [ratio 1–1.5]). Results: In total, 2719 patients were included (PIGD, n = 2165; TD, n = 118; mixed-type, n = 188; not evaluable, n = 248). Among TD subtype patients, the least-squares mean change from baseline versus placebo in UPDRS II/III TD-related total score was significant at 20 mg/day istradefylline (−2.21; 95 % CI, −4.05 to −0.36; p = 0.02). For PIGD subtype patients, there was a significant difference from placebo in UPDRS II/III PIGD-related total score at 40 mg/day istradefylline (−0.25; −0.43 to −0.06; p = 0.01). Conclusions: The data from this analysis of UPDRS-based motor subtypes suggest that istradefylline can improve motor disability in PD patients with motor fluctuations regardless of PD subtype. Future research should characterize the effects of istradefylline on tremor.

Abstract: Background: Age-related hyperkyphosis is multifactorial and involves alterations of vertebral bone, intervertebral discs (IVD) and paraspinal muscles. The relative contribution of these tissues remains unclear. Here, we compared differences in vertebral bone microarchitecture and IVD thickness between prematurely aging mice with spinal hyperkyphosis and wild type littermates. Methods: Thoracolumbar vertebral columns were dissected from homozygous PolgD257A and age-matched wild type littermates. Micro-computed tomography was performed to quantify cortical and trabecular bone parameters at anterior and posterior portions of T8-L4 vertebrae. In addition, vertebral shape, transaxial facet joint orientation and IVD thickness were quantified. Differences in anterior/posterior ratios between genotypes were compared by Student's t-test and association between vertebral bone and IVD parameters was investigated using Pearson correlation analysis. Results: Hyperkyphotic homozygous mice displayed generalized osteopenia that was more pronounced at the posterior compared with anterior portion of thoracolumbar vertebrae. An increase in the anterior/posterior ratio of trabecular bone parameters was revealed at the thoracolumbar junction (T13-L1). PolgD257A displayed diffuse loss of cortical bone thickness, yet anterior/posterior ratios were unchanged. Despite generalized and regional bone loss, vertebral shape was unaffected. PolGD257A mice showed a 10–20 % reduction of IVD thickness at both thoracic and lumbar levels, with only minimal histopathological changes. IVD thickness was negatively correlated with anterior/posterior ratios of trabecular bone parameters, as well as with more coronally oriented facet joints, but negatively correlated with the anterior/posterior ratio of cortical bone thickness. Conclusions: Aging-induced regional changes of vertebral trabecular and cortical bone did not lead to altered vertebral shape in PolgD257A mice but may indirectly cause hyperkyphosis through reduction of IVD thickness. These findings suggest a limited role for aging-induced bone loss in spinal hyperkyphosis and warrants further research on the involvement of paraspinal muscle degeneration.

Abstract: Objectives: We evaluated the contribution of lung lesion quantification on chest CT using a clinical Artificial Intelligence (AI) software in predicting death and intensive care units (ICU) admission for COVID-19 patients. Methods: For 349 patients with positive COVID-19-PCR test that underwent a chest CT scan at admittance or during hospitalization, we applied the AI for lung and lung lesion segmentation to obtain lesion volume (LV), and LV/Total Lung Volume (TLV) ratio. ROC analysis was used to extract the best CT criterion in predicting death and ICU admission. Two prognostic models using multivariate logistic regressions were constructed to predict each outcome and were compared using AUC values. The first model (“Clinical”) was based on patients’ characteristics and clinical symptoms only. The second model (“Clinical+LV/TLV”) included also the best CT criterion. Results: LV/TLV ratio demonstrated best performance for both outcomes; AUC of 67.8% (95% CI: 59.5 - 76.1) and 81.1% (95% CI: 75.7 - 86.5) respectively. Regarding death prediction, AUC values were 76.2% (95% CI: 69.9 - 82.6) and 79.9% (95%IC: 74.4 - 85.5) for the “Clinical” and the “Clinical+LV/TLV” models respectively, showing significant performance increase (+ 3.7%; p-value

Abstract: We assessed long-term associations of Granulocyte-Colony Stimulating Factors (G-CSF) use with patient-reported outcomes (PROs) and hematologic toxicity among chemotherapy-treated, early-stage breast cancer patients in CANTO (NCT01993498).Among 2920 patients longitudinally followed-up until year-4 after diagnosis, 49% used G-CSF. In multivariable-adjusted mixed-models, EORTC QLQ-C30 pain and summary score were not substantially different between groups (overall adjusted mean difference, use vs no-use [95%CI]: +1.27 [-0.33 to +2.87] and −1.01 [-1.98 to −0.04], respectively). PROs were slightly worse at year-4 among patients receiving G-CSF, although differences were of trivial clinical significance. No major differences were observed in leukocyte or platelet count over time.

Abstract: Summary: Background: Many patients report persistent symptoms after COVID-19. Our aim was to determine whether some of these symptoms were more associated with past SARS-CoV-2 infection compared to other conditions. Methods: This prospective survey was nested in CONSTANCES, a randomly selected French population-based cohort, started in 2012. All participants being followed-up by internet completed 2 questionnaires during the first wave of the pandemic focusing on the acute symptoms of their COVID-19-like illness. Serological tests for SARS-CoV-2 were then performed (May-Nov 2020). Between December 2020 and January 2021, participants completed a third questionnaire about symptoms that had lasted more than 2 months. Participants were classified into four groups according to both European Center for Diseases Control (ECDC) criteria for COVID-19 (ECDC+ or ECDC-) and serological SARS-CoV-2 test results (Sero+ or Sero-). To compare the risk of each persistent symptom among the groups, logistic regression models were adjusted for age, sex, educational level, comorbidities, and the number of acute symptoms declared during the first wave of the epidemic. A mediation analysis was performed to estimate the direct effect of the infection on persistent symptoms and its indirect effect via the initial clinical presentation. Findings: The analysis was performed in 25,910 participants. There was a higher risk of persistent dysgeusia/anosmia, dyspnea and asthenia in the ECDC+/Sero+ group than in the ECDC+/Sero- group (OR: 6.83 [4.47–10.42], 1.69 [1.07–2.6] and 1.48 [1.05–2.07], respectively). Abdominal pain, sensory symptoms or sleep disorders were at lower risk in the ECDC+/Sero+ group than in the ECDC+/Sero- group (0.51 [0.24–0.96], 0.40 [0.16–0.85], and 0.69 [0.49–0.95], respectively). The mediation analysis revealed that the association of the serological test results with each symptom was mainly mediated by ECDC symptoms (proportion mediated range 50–107%). Conclusion: A greater risk of persistent dysgeusia/anosmia, dyspnea and asthenia was observed in SARS-CoV-2 infected people. The initial clinical presentation substantially drives the association of positive serological test results with persistent symptoms. Funding: French National Research Agency

Abstract: Summary: Background: The multiplicity, heterogeneity, and dynamic nature of human immunodeficiency virus type-1 (HIV-1) latency mechanisms are reflected in the current lack of functional cure for HIV-1. Accordingly, all classes of latency-reversing agents (LRAs) have been reported to present variable ex vivo potencies. Here, we investigated the molecular mechanisms underlying the potency variability of one LRA: the DNA methylation inhibitor 5-aza-2’-deoxycytidine (5-AzadC). Methods: We employed epigenetic interrogation methods (electrophoretic mobility shift assays, chromatin immunoprecipitation, Infinium array) in complementary HIV-1 infection models (latently-infected T-cell line models, primary CD4+ T-cell models and ex vivo cultures of PBMCs from HIV+ individuals). Extracellular staining of cell surface receptors and intracellular metabolic activity were measured in drug-treated cells. HIV-1 expression in reactivation studies was explored by combining the measures of capsid p24Gag protein, green fluorescence protein signal, intracellular and extracellular viral RNA and viral DNA. Findings: We uncovered specific demethylation CpG signatures induced by 5-AzadC in the HIV-1 promoter. By analyzing the binding modalities to these CpG, we revealed the recruitment of the epigenetic integrator Ubiquitin-like with PHD and RING finger domain 1 (UHRF1) to the HIV-1 promoter. We showed that UHRF1 redundantly binds to the HIV-1 promoter with different binding modalities where DNA methylation was either non-essential, essential or enhancing UHRF1 binding. We further demonstrated the role of UHRF1 in the epigenetic repression of the latent viral promoter by a concerted control of DNA and histone methylations. Interpretation: A better understanding of the molecular mechanisms of HIV-1 latency allows for the development of innovative antiviral strategies. As a proof-of-concept, we showed that pharmacological inhibition of UHRF1 in ex vivo HIV+ patient cell cultures resulted in potent viral reactivation from latency. Together, we identify UHRF1 as a novel actor in HIV-1 epigenetic silencing and highlight that it constitutes a new molecular target for HIV-1 cure strategies. 47. Element enrichment and provenance of the detrital component in Holocene sediments from the western Black Sea

Abstract: Summary: Concentrations of a large set of major and trace elements, and Sr, Nd and Pb isotope ratios were measured in Holocene sediments cored in the western deep Black Sea in order to unravel: (1) the controls of element enrichment, and (2) sources of the detrital component. The transition of the basin from oxic to euxinic resulted in enrichment or depletion in a number of elements in the deep-sea sediments. Authigenic Fe enrichment appears to depend on the amount of Fe mobilized from the sediment through the benthic redox shuttle mechanism and free H2S in the water column (degree of “euxinization”). Manganese enrichment is controlled by diagenetic reactions within the sediment: the dissolution of Mn minerals, Mn2+ diffusion upward and reprecipitation. Barium enrichment is also controlled by diagenetic reactions, sulfate reduction and methanogenesis, that take place above and below the sulfate-methane transition, respectively. The major part of V, Co, Ni, Cu, Zn, Cr, Mo, Cd and Sb is inferred to have co-precipitated with Fe in the euxinic deep waters and to have been incorporated into authigenic Fe-sulfides. Basin reservoir effect additionally influences the Mo enrichment. The U enrichment is interpreted to have a different origin in the two organic-rich stratigraphic units (II and I). It is inferred to be: (i) at the expense of the U inventory of the deepwater pool and a result of inorganic reduction of U at euxinic conditions in the lower Unit II; and (ii) at the expense of the U inventory of the surface water pool and a result of biogenic uptake and transfer to the sediment by the plankton in the upper Unit I. The high field strength elements are closely linked to the detrital component and their depletion in the organic-rich sediments reflects a dilution of the detrital component by the biogenic one. The enrichments of REE, Sn and Th are likely controlled by adsorption on clay minerals. Sr-Nd-Pb isotope compositions of the alumino-silicate component of the studied sediments are relatively uniform. They are most likely controlled by riverine suspended matter supplied mainly in the NW Black Sea (Danube Delta) and transported southward by marine currents, and to a lesser degree by suspended matter from the small rivers draining SE Bulgaria and NW Turkey. Wind-blown dust from the Sahara Desert appears to have a minor contribution to the alumino-silicate component of the sediments. Abstract: The cluster mass test has been widely used for massively univariate tests in M/EEG, fMRI and, recently, pupillometry analysis. It is a powerful method for detecting effects while controlling weakly the family-wise error rate (FWER), although its correct interpretation can only be performed at the cluster level without any point-wise conclusion. It implies that the discoveries of a cluster mass test cannot be precisely localized in time or in space. We propose a new multiple comparisons procedure, the cluster depth tests, that both controls the FWER while allowing an interpretation at the time point level. We show the conditions for a strong control of the FWER, and a simulation study shows that the cluster depth tests achieve large power and guarantee the FWER even in the presence of physiologically plausible effects. By having an interpretation at the time point/voxel level, the cluster depth tests make it possible to take full advantage of the high temporal resolution of EEG recording and give a precise timing of the start and end of the significant effects.

49. A disseminated Mycobacterium marinum infection in a renal transplant HIV-infected patient successfully treated with a bedaquiline-containing antimycobacterial treatment: A case report

Abstract: Background: Disseminated Mycobacterium marinum infections occur rarely, in immunocompromised patients. Treatment with a prolonged multi-drug regimen exposes patients to drug–drug interactions and side effects. Case report: We report a case of disseminated M. marinum infection in a 54-year-old renal transplant, HIV-infected woman. Manifestations of the infection were cutaneous and subcutaneous nodules, mediastinal lymph nodes and left pulmonary infiltrate. Empirical treatment for non-tuberculous mycobacteria was initiated with rifabutin, ethambutol and azithromycin. After identifying M. marinum in sputum, due to unfavourable clinical evolution and severe drug-related adverse events, treatment was changed to doxycycline and rifabutin. Digestive and haematologic side effects motivated a change in antimycobacterial treatment to a combination of moxifloxacin and bedaquiline. Tolerance was satisfactory, and the patient was cured after 12 months of treatment. Conclusion: We report (to the authors’ knowledge) the first case of disseminated M. marinum infection successfully treated with a bedaquiline-containing regimen. Bedaquiline could be an alternative to recommended antimicrobial regimens in cases of non-tuberculous mycobacterial disease, including M. marinum infection.

Abstract: Several postmortem studies have shown iron accumulation in the substantia nigra of Parkinson’s disease patients. Iron concentration can be estimated via MRI-R2∗ mapping. To assess the changes in R2∗ occurring in Parkinson’s disease patients compared to controls, a multicentre transversal study was carried out on a large cohort of Parkinson’s disease patients (n = 163) with matched controls (n = 82). In this study, 44 patients and 11 controls were removed due to motion artefacts, 21 patient and 6 controls to preserve matching. Thus, 98 patients and 65 age and sex-matched healthy subjects were selected with enough image quality.The study was conducted on patients with early to late stage Parkinson's disease. The images were acquired at 3Tesla in 12 clinical centres. R2∗ values were measured in subcortical regions of interest (substantia nigra, red nucleus, striatum, globus pallidus externus and globus pallidus internus) contralateral (dominant side) and ipsilateral (non dominant side) to the most clinically affected hemibody.As the observed inter-subject R2∗ variability was significantly higher than the disease effect, an original strategy (intrasubject subcortical quantitative referencing, ISQR) was developed using the measurement of R2∗ in the red nucleus as an intra-subject reference.R2∗ values significantly increased in Parkinson’s disease patients when compared with controls; in the substantia nigra (SN) in the dominant side (D) and in the non dominant side (ND), respectively (PSN_D and PSN_ND
