1. Comparative estimates of crude coverage of the Mexican immunization program: Findings from a national survey

Abstract: The purpose of the study is to provide estimates for immunization coverage, considering single-dose and schemes (three or five vaccines), by comparing self-report method to immunization cards, while also assessing the timeliness of immunization in Mexico, with reference to Mexican Immunization Program guidelines.Data on immunization was obtained from the Mexican Immunization Survey conducted in 2017 that aimed to assess crude (card-based) coverage at the regional level. Timely immunization was defined with reference to National Immunization Program guidelines, and immunization coverage was defined as a three or five vaccine scheme, based on previous national reports of immunization coverage. Immunization coverage estimates account for sample weights from the complex survey design. We used weighted immunization coverage estimates to assess the extent to which immunization cards and self-reporting concurred.It was found that most Mexican children are not receiving their full vaccine schedule in a timely manner. Concerning children under twelve months of age, the coverage targets for National Immunization of 95 % was not reached for either vaccine, and only 2.94 % (95 % CI 0.92–9.01) who had been receiving a three-vaccine scheme were considered as fully immunized in a timely manner. In contrast, coverage increased to 33.94 % (95 % CI 26.99–41.66), when untimely immunizations were taken into account, and the 95 % target was reached for five vaccines. Likewise, there is little correlation between self-report and immunization cards but rates show more concurrence, when only considering the proportion of true positives.In conclusion it was find that children at a local level are vaccinated in an incomplete and untimely manner. In order to improve immunization systems, a nominal registry of administered doses is thus of paramount importance. There is a need to address underlying health inequalities, as well as the factors associated with these, resulting in improved chances of a disease-free childhood and healthy life.

2. Solar photovoltaic Maximum Power Point Tracking controller optimization using Grey Wolf Optimizer: A performance comparison between bio-inspired and traditional algorithms

Abstract: [EN] Solar photovoltaic systems are widely used; however, their performance is bound to weather conditions, depending on irradiation, temperature, and the effect of shadows. Maximum Power Point Tracking techniques have been developed to solve this issue. Standard methods use mainly-two algorithms: Perturb and Observe and Incremental Conductance. However, such algorithms perform differently when the Solar photovoltaic system works under sudden solar irradiation changes, temperature, and load changes. This research proposes an opti-mized Maximum Power Point Tracking controller based on the Grey Wolf Optimization algorithm using the MATLAB/Simulink software as an alternative to the traditional techniques. Global efficiency and Root Mean Square Error evaluate the controller's performance. The response time is analyzed using the Grey Wolf Optimizer algorithm, Wolf Optimizer Algorithm, Simulated Annealing, and Particle Swarm Optimization. These four metaheuristic algorithms are compared to the Perturb and Observe, and Incremental Conductance algorithms. The models are analyzed for the transient state and full-day operation scenarios for constant and variable ir-radiations, temperatures, and loads. The comparative results show that the Maximum Power Point Tracking controller optimized by the Grey Wolf Optimizer algorithm has superior performance, giving an average 6% output power higher than the other controllers under the test scenarios evaluated. The efficiency of the proposed model was, on average, 3% higher than the Incremental Conductance and Perturb & Observe controllers. For the MPPT controller tunning stage, the Grey Wolf Optimizer Algorithm had the best performance with an RMSE of 255.3549 with a compute time of 27.3 min; the worst performing was the Particle Swarm Optimization with an RMSE of 332.4075 and 27.8 min computation time. The proposed GWO optimized MPPT controller had the faster settling time for each irradiation level compared, with an average of 0.175 s. Also, results showed an improvement of the system response throughout the Maximum Power Point Tracking controller optimized by the Grey Wolf Optimizer algorithm since a lower curling effect is obtained at power converter outputs., This research has been funded by the PURPOSED project (ref: PID2021-128822OB-I00), financed by the Spanish State Investigation Agency and by of the Catedra de Transicion Energetica Urbana -a chair hosted at the Universitat Politècnica de València and funded by Ajuntament de València-Las Naves and Fundacio València Clima i Energia.

3. Simulations and experimental study to compare the behavior of a genset running on gasoline or syngas for small scale power generation

Abstract: [EN] Syngas produced from small-scale biomass gasification plants (tens of kW) can generate power from biomass resources. However, due to the syngas gensets demand is marginal, compared to gasoline gensets, it is not easy to find a genset specially designed to run on syngas. In this paper, an experimental study of a genset running on two kinds of fuel has been carried out. The genset used is a 10 kW ICE initially designed to run on gasoline, in parallel, simulations based on CHEMKIN-PRO software were made. The study focuses on the comparison of the ICE operation independently working on gasoline and syngas. Modifications were required in the genset to adapt the intake system to run on syngas. The syngas is obtained from a gasifier. According to the test, it is possible to convert a gasoline engine to a 100% syngas engine through straightforward modifications in the inlet manifold but reducing the efficiency and power. The electrical power running the genset on gasoline was 6.7 kW, whereas, with syngas, it is 4.8 kW (31.4% of reduction). The efficiency running the ICE on gasoline was 13.7% at its rated power, whereas for the genset fuelled on syngas, it was 10.5% (23.4% reduction).

As a result, this kind of system is being proposed as a cost-effective option for power generation in the built environment. However, some authors consider that better regulatory support is still needed to deploy the PV potential in cities. In this research, four illustrative cases show that the latter could be the case in a Mediterranean city with high annual solar radiation, such as Valencia. Unlike most previous research based on statistics, this research uses real data and real market equipment information to develop the analysis. The Software HOMER is used to simulate the performance of the study cases; the findings show how the change of the regulation improves the viability of these urban renewable energy systems. However, although all four cases are technically sound, further changes in the regulation are found necessary for all of them to be economically viable. With a net metering system not yet allowed in Spain, the payback of the investments would be reduced by one-third. This study shows that the orientation, angle of roofs, shadowing, and others greatly affect the surface availability and maximum installable power. Hence, this study discusses real data and accurate simulations of the cost-effectiveness of photovoltaic systems in the built environment. Then, the influence of regulation on the economic viability of urban photovoltaic systems is considered. Finally, recommendations for regulatory development are proposed to meet the expectations for the deployment of PV generation potential.

The results show one of the main limitations to reducing the LCOE is the impossibility of selling energy when revenues are greater than purchases (Current regulation in Spain). Without such limitation, the LCOE would be reduced (cases 1 and 3 reduces LCOE from 17.5 to 15.2 c€/kWh and from 18.5 to 17.3 c€/kWh, respectively). With a net metering system not yet permitted in Spain, the payback of the investments would be reduced by one-third. Finally, recommendations for regulatory development are proposed to meet the expectations for the deployment of PV generation potential.

Abstract: [EN] Off grid electrification is key to achieving universal electricity access. Despite the potential of biomass gasifiers as a clean technology to ensure reliable systems, they are not as well looked upon as their advantages suggest. In particular, the software HOMER, which researchers and technicians use the most to simulate and design Microgrids, does not include biomass gasification among its simulation technologies. Although some authors did simulate biomass gasifiers in HOMER, the necessary parameters and procedures to calculate and include them in the tool remain unclear, non-replicable, and leave open research questions as regards modelling. Using the inbuilt Biogas power plant modules in HOMER, this paper presents a set of steps to include the technical and economic parameters to simulate fuelling an electric generator through the syngas produced in a downdraft biomass gasification plant. Two case studies of isolated rural communities in Honduras and Zambia show the viability of the procedure. These case studies also confirm the technical and economic viability of islanded biomass-photovoltaic hybrid renewable energy microgrids. In both cases, the energy demand supplied and distributed by the microgrid had a Levelized Cost of Energy lower than the alternative of extending the electric grid to the communities. Abstract: [EN] Cities are expected to be protagonists of the energy transition and, among other challenges, the decarbonation of the residential consumption could greatly benefit from photovoltaic generation in the built environment. To assess its potential, the present study analyses the possibility to cover the electric demand of the city of Valencia with rooftop-installed PV panels. Specific types of buildings has been selected to study the potential of each one of them with respect to their demand and with a specific production/consumption model. The total potential energy production for the city of Valencia is estimated to be enough to satisfy the demand of domestic electricity. The economic investment scenario has been analysed (best- and worst-case scenarios), and the corresponding environmental benefits have been studied. The results of the present article are encouraging in the context of a global energy transition, necessary in the present times to keep human consumption within the planetary limits.

9. A cascade hybrid PSO feed-forward neural network model of a biomass gasification plant for covering the energy demand in an AC microgrid

Abstract: [EN] Agriculture and forestry crop residues represent more than half of the world's residual biomass; these residues turn into synthesis gas (syngas) and are used for power generation. Including Syngas Gensets into hybrid renewable microgrids for electricity generation is an interesting alternative, especially for rural communities where forest and agricultural waste are abundant. However, energy demand is not constant throughout the day. The variations in the energy demand provoke changes in both gasification plant efficiency and biomass consumption. This paper presents an Artificial Neural Network (ANN) based model hybridized with a Particle Swarm Optimization (PSO) algorithm for a Biomass Gasification Plant (BGP) that allows estimating the amount of biomass needed to produce the required syngas to meet the energy demand. The proposed model is compared with two traditional models of ANNs: Feed Forward Back Propagation (FF-BP) and Cascade Forward Propagation (CF-P). ANNs are trained in MATLAB software using a set of historical real data from a BGP located in the Distributed Energy Resources Laboratory of the Universitat Polite`cnica de Vale`ncia in Spain. The model performance is validated using the Mean Squared Error (MSE) and linear regression analysis. The results show that the proposed model performs 23.2% better in terms of MSE than de other models. The tunning parameters of the optimal PSO algorithm for this application were found. Finally, the model was validated to predict the necessary biomass and syngas to cover the energy demand

Abstract: [EN] A residential load management system equips smart meters (SMs) to measure load utilization at residencies. SM reports electricity usage based on electronic appliances. In this paper, a smart residential load management system is designed by three-fold along with the provisioning of consumer security. Load management encompasses load categorizing by Hopfield neural network, fuzzy-logic based bill payments identification and load state prediction using Markov chain. Residential load is based on three classes: active load, affordable load and inactive load. The estimation of residential load ensures to manage the load at residency either to turn off the load or intimate excess consumption of electricity. The registered consumers of a specific residency are enabled to receive load status by Internet of Things (IoT) devices. SMs at residencies periodically compute the electricity manipulation and those readings are encrypted using hybrid blowfish and elliptic curve cryptography algorithm with considering the behavior information from IoT device and partial key storage assists security, even if the device is theft. This smart residential security assisted load management (SRS-LM) system is developed in network simulator 3 and the results showed improvements in terms of power usage, load power, peak load reduction, total power consumption, power cost and computation time

11. Multicriteria power generation planning and experimental verification of hybrid renewable energy systems for fast electric vehicle charging stations

Abstract: [EN] The installation of fast electric vehicle charging stations (EVCS) will be essential to promote the acceptance by the users of electric vehicles (EVs). However, if EVCS are exclusively supplied by the grid, negative impacts on its stability together with possible CO2 emission increases could be produced. Introduction of hybrid renewable energy systems (HRES) for EVCS can cope with both drawbacks by reducing the load on the grid and generating clean electricity. This paper develops a weighted multicriteria methodology to design the most suitable configuration of HRES for EVCS. This method determines the local renewable resources and the EVCS electricity demand. Then, taking into account environmental, economic and technical aspects, it deduces the most adequate HRES power generation planning for EVCS. Besides, an experimental stage to validate the design deduced from the multicriteria methodology is included. Therefore, the final power generation planning for the HRES in EVCS is supported not only by a complete numerical evaluation, but also by an experimental verification of the demand being fully covered. Methodology application to Valencia (Spain) proves that an off-grid HRES with solar PV, wind and batteries support would be the most suitable configuration for the system. Abstract: [EN] A Model Predictive Control (MPC) strategy based on the Evolutionary Algorithms (EA) is proposed for the optimal dispatch of renewable generation units and demand response in a grid-tied hybrid system. The generating system is based on the experimental setup installed in a Distributed Energy Resources Laboratory (LabDER), which includes an AC micro-grid with small scale PV/Wind/Biomass systems. Energy storage is by lead-acid batteries and an H2 system (electrolyzer, H2 cylinders and Fuel Cell). The energy demand is residential in nature, consisting of a base load plus others that can be disconnected or moved to other times of the day within a demand response program. Based on the experimental data from each of the LabDER renewable generation and storage systems, a micro-grid operating model was developed in MATLAB(C) to simulate energy flows and their interaction with the grid. The proposed optimization algorithm seeks the minimum hourly cost of the energy consumed by the demand and the maximum use of renewable resources, using the minimum computational resources. The simulation results of the experimental micro-grid are given with seasonal data and the benefits of using the algorithm are pointed out.

13. Low-cost web-based Supervisory Control and Data Acquisition system for a microgrid testbed: A case study in design and implementation for academic and research applications

Abstract: [EN] This paper presents the design and implementation of a low-cost Supervisory Control and Data Acquisition system based on a Web interface to be applied to a Hybrid Renewable Energy System (HRES) microgrid. This development will provide a reliable and low-cost control and data acquisition systems for the Renewable Energy Laboratory a Universitat Politecnica de Valencia (LabDER-UPV) in Spain, oriented to the research on microgrid stability and energy generation. The developed low-cost SCADA operates on a microgrid that incorporates a photovoltaic array, a wind turbine, a biomass gasification plant and a battery bank as an energy storage system. Sensors and power meters for electrical parameters, such as voltage, current, frequency, power factor, power generation, and energy consumption, were processed digitally and integrated into Arduino-based devices. A master device on a Raspberry-PI board was set up to send all this information to a local database (DB), and a MySQL Web-DB linked to a Web SCADA interface, programmed in HTML5. The communications protocols include TCP/IP, I2C, SPI, and Serial communication; Arduino-based slave devices communicate with the master Raspberry-PI using NRF24L01 wireless radio frequency transceivers. Finally, a comparison between a standard SCADA against the developed Web-based SCADA system is carried out. The results of the operative tests and the cost comparison of the own-designed developed Web-SCADA system prove its reliability and low-cost, on average an 86% cheaper than a standard brandmark solution, for controlling, monitoring and data logging information, as well as for local and remote operation system when applied to the HRES microgrid testbed.

Abstract: We present the first wide-range measurement of the charged-particle pseudorapidity density distribution, for different centralities (the 0-5%, 5-10%, 10-20%, and 20-30% most central events) in Pb-Pb collisions at $\sqrt{s_{\rm NN}} = 2.76$ TeV at the LHC. The measurement is performed using the full coverage of the ALICE detectors, $-5.0 < ��< 5.5$, and employing a special analysis technique based on collisions arising from LHC "satellite" bunches. We present the pseudorapidity density as a function of the number of participating nucleons as well as an extrapolation to the total number of produced charged particles ($N_{\rm ch} = 17165 \pm 772$ for the 0-5% most central collisions). From the measured ${\rm d}N_{\rm ch}/{\rm d}��$ distribution we derive the rapidity density distribution, ${\rm d}N_{\rm ch}/{\rm d}y$, under simple assumptions. The rapidity density distribution is found to be significantly wider than the predictions of the Landau model. We assess the validity of longitudinal scaling by comparing to lower energy results from RHIC. Abstract: [EN] Renewable energies are a central element in the search for energy sustainability, so they are becoming a substantial component of the energy scenario of every country, both as systems connected to the grid or in stand-alone applications. Feasibility of these renewable energy systems could be necessary not only in their application in isolated areas, but also in systems connected to the grid, in this last case when their contribution reaches a substantial fraction of the total electricity demand. To overcome this reliability problem, hybrid renewable systems could become essential and activities to optimize their design should be addressed, both in the simulation and in the experimental areas. In this paper, a laboratory to simulate and verify the reliability of hybrid renewable systems is presented and its application to the feasibility analysis of multicomponent systems including photovoltaic panels, wind generator and biomass gasification plant, plus energy storage in a battery bank, are described.

Abstract: The elliptic, $v_2$, triangular, $v_3$, and quadrangular, $v_4$, azimuthal anisotropic flow coefficients are measured for unidentified charged particles, pions and (anti-)protons in Pb-Pb collisions at $\sqrt{s_{\rm NN}} = 2.76$ TeV with the ALICE detector at the Large Hadron Collider. Results obtained with the event plane and four-particle cumulant methods are reported for the pseudo-rapidity range $|��|8$ GeV/$c$. The small $p_{\rm T}$ dependence of the difference between elliptic flow results obtained from the event plane and four-particle cumulant methods suggests a common origin of flow fluctuations up to $p_{\rm T}=8$ GeV/$c$. Abstract: Renewable systems are especially suited for electricity generation in remote areas but, where high reliability of the electricity supply is required, a hybrid system including electricity storage could be the only solution to avoid the lack of feasibility in single renewable systems. In this paper a hybrid system composed by photovoltaic panels, a biomass gasifier and a battery bank is presented. The system was designed and constructed at the Distributed Energy Resources Laboratory (LabDER) in order to supply electricity to a laboratory complex in the Institut Superieur des Techniques Appliquees, Democratic Republic of Congo (ISTA-DRC). System behavior was also tested in LabDER prior being installed and operated in the final place. Design characteristics and commissioning results are presented together with the experimental verification of the capability to fulfill the demand with very low fluctuations in the generated electricity supply. Abstract: The aim of this research is to contribute into the diffusion of biomass power systems by analyzing and testing the throat sizing influence on the operation of a gasification plant coupled with an internal combustion engine. In order to do this, the assessment of the proper operation range for some of the driving process parameters has been carried out. The analysis has been focused on such parameters as pressure drop of the fixed bed reactor, the inlet air flow, the syngas production, electrical power production and efficiency, looking for improving the performance and guaranteeing the proper system operation. Two different campaigns of tests have been carried out for figuring out the best design on the reactor. Based on this analysis, the most convenient throat diameter has been determined (in this case, around 10 cm), producing an increment in the production of syngas of about 31%. This modification has demonstrated also an increment of the electrical power produced by the gasification plant of about 40%, which means an increment in the motor generator efficiency of 35%.

Abstract: Acute kidney injury (AKI) diagnosis relies on plasma creatinine concentration (Cr pl ), a relatively insensitive, surrogate biomarker of glomerular filtration rate that increases only after significant damage befalls. However, damage in different renal structures may occur without increments in Cr pl , a condition known as subclinical AKI. Thus, detection of alterations in other aspects of renal function different from glomerular filtration rate must be included in an integral diagnosis of AKI. With this aim, we adapted to and validated in rats (for preclinical research) the furosemide stress test (FST), a tubular function test hitherto performed only in humans. We also tested its sensitivity in detecting subclinical tubular alterations. In particular, we predisposed rats to AKI with 3 mg/kg cisplatin and subsequently subjected them to a triggering insult (ie, 50 mg/kg/d gentamicin for 6 days) that had no effect on nonpredisposed animals but caused an overt AKI in predisposed rats. The FST was performed immediately before adding the triggering insult. Predisposed animals showed a reduced response to the FST (namely, reduced furosemide-induced diuresis and K + excretion), whereas nonpredisposed animals showed no alteration, compared to the controls. Computational modeling of epithelial transport of solutes and water along the nephrons applied to experimental data suggested that proximal tubule transport was only minimally reduced, the sodium-chloride symporter was upregulated by 50%, and the renal outer medullary potassium channel was downregulated by 85% in predisposed animals. 20. Using Social Indicators to Describe Neighborhood-Level Disparities in Adolescent Health in Baltimore City Circa 2017.

Abstract: Purpose: The purpose of this study was to use social indicators to compare adolescent health disparities across neighborhoods in Baltimore, Maryland, circa 2017. Neighborhoods heavily influence adolescent health outcomes. Baltimore remains a hypersegregated city along racial boundaries with a recently growing population of Latino immigrant youth. This segregation may promulgate adolescent health disparities, yet the magnitude of needs and how they may differ among Baltimore's minoritized adolescents remain unknown., Methods: The most predominantly white, black, and Latino neighborhoods in Baltimore were analyzed across six indicators relevant to adolescent health: teen birth rate, high school achievement, poverty, health insurance, youth mortality rate, and lead paint violation rate. The indicators were used to create a composite adolescent deprivation index. Measures of absolute and relative disparity were then calculated between white, black, and Latino neighborhood clusters., Results: Both black and Latino neighborhoods had similar adolescent deprivation relative to white neighborhoods. Latino neighborhoods had the highest teen birth rate and children without health insurance. Black neighborhoods had the lowest educational achievement and the highest poverty, youth mortality, and lead paint violation rate., Conclusions: The overall magnitude of social deprivation is similar across communities of color in Baltimore. However, black adolescents tend to live in neighborhoods with greater physical deprivation and youth mortality that limits within-group bonding capacity, whereas Latino adolescents tend to live in neighborhoods with limited health and social resources that prevent between-group bridging capacity. Abstract: The clinical utility of aminoglycoside antibiotics is partly limited by their nephrotoxicity. Co-administration of a variety of candidate nephroprotectants has been tested at the preclinical level. According to a recent meta-analytic study, antioxidants are the only family of compounds with enough preclinical documentation to draw solid conclusions on their class nephroprotective capacity in animal models. In this study a systematic analysis of the relation between the level of antioxidation and the level of nephroprotection was performed. A regression model is presented which crosses the y-axis (i.e. the axis representing the level of nephroprotection) very nearly the zero value, meaning that maximal prevention of the oxidative stress induced by aminoglycosides results in almost maximal nephroprotection. This indicates that oxidative stress plays a central role in the hierarchy of pathophysiological mechanisms underlying aminoglycoside nephrotoxicity. In addition, this model may potentially serve: i) as a standard to evaluate the role of the antioxidant effect of candidate nephroprotectants; ii) to reveal additional, antioxidant-independent effects among those compounds providing more nephroprotection than that expected from its antioxidant activity; and thus iii) to discriminate and focus most effective nephroprotectants on clinical usage., (Copyright © 2017 Elsevier B.V. 22. Inappropriate Fentanyl Prescribing Among Nursing Home Residents in the United States.

Abstract: Objective: We quantified transdermal fentanyl prescribing in elderly nursing home residents without prior opioid use or persistent pain, and the association of individual and facility traits with opioid-naïve prescribing., Design: Cross-sectional study., Setting: Linked Minimum Data Set (MDS) assessments; Online Survey, Certification and Reporting (OSCAR) records; and Medicare Part D claims., Participants: From a cross-section of all long-stay US nursing home residents in 2008 with an MDS assessment and Medicare Part D enrollment, we identified individuals (≥65 years old) who initiated transdermal fentanyl, excluding those with Alzheimer disease, severe cognitive impairment, cancer, or receipt of hospice care., Measurements: We used Medicare Part D to select beneficiaries initiating transdermal fentanyl in 2008 and determined whether they were "opioid-naïve," defined as no opioid dispensing during the previous 60 days. We obtained resident and facility characteristics from MDS and OSCAR records and defined persistent pain as moderate-to-severe, daily pain on consecutive MDS assessments at least 90 days apart. We estimated associations of patient and facility attributes and opioid-naïve fentanyl initiation using multilevel mixed effects logistic regression modeling., Results: Among 17,052 residents initiating transdermal fentanyl, 6190 (36.3%) were opioid-naïve and 15,659 (91.8%) did not have persistent pain. In the regression analysis with adjustments, residents who were older (ages ≥95 odds ratio [OR] 1.69, 95% confidence interval [CI] 1.46-1.95) or more cognitively impaired (moderate-to-severe cognitive impairment, OR 1.99, 95% CI 1.73-2.29) were more likely to initiate transdermal fentanyl without prior opioid use., Conclusion: Most nursing home residents initiating transdermal fentanyl did not have persistent pain and many were opioid-naïve. Abstract: Bone loss and increased fractures are common complications in chronic kidney disease. Because Wnt pathway activation is essential for normal bone mineralization, we assessed whether Wnt inhibition contributes to high-phosphorus-induced mineralization defects in uremic rats. By week 20 after 7/8 nephrectomy, rats fed a high-phosphorus diet had the expected high serum creatinine, phosphorus, parathyroid hormone, and fibroblast growth factor 23 (FGF23) levels and low serum calcium. There was a 15% reduction in tibial mineral density and a doubling of bone cortical porosity compared to uremic rats fed a normal-phosphorus diet. The decreases in tibial mineral density were preceded by time-dependent increments in gene expression of bone formation (Osteocalcin and Runx2) and resorption (Cathepsin K) markers, which paralleled elevations in gene expression of the Wnt inhibitors Sfrp1 and Dkk1 in bone. Similar elevations of Wnt inhibitors plus an increased phospho-β-catenin/β-catenin ratio occurred upon exposure of the osteoblast cell line UMR106-01 either to uremic serum or to the combination of parathyroid hormone, FGF23, and soluble Klotho, at levels present in uremic serum. Strikingly, while osteoblast exposure to parathyroid hormone suppressed the expression of Wnt inhibitors, FGF23 directly inhibited the osteoblastic Wnt pathway through a soluble Klotho/MAPK-mediated process that required Dkk1 induction. Thus, the induction of Dkk1 by FGF23/soluble Klotho in osteoblasts inactivates Wnt/β-catenin signaling. This provides a novel autocrine/paracrine mechanism for the adverse impact of high FGF23 levels on bone in chronic kidney disease., (Copyright © 2016 International Society of Nephrology. Abstract: Tubulointerstitial fibrosis is characterized by an accumulation of extracellular matrix in the renal interstitium, myofibroblast activation, cell infiltration, and tubular cell apoptosis, leading to chronic renal failure. Activin receptor-like kinase 1 (ALK1) is a transforming growth factor-β1 type I receptor with a pivotal role in endothelial proliferation and migration, but its role in the development of renal fibrosis is unknown. To assess this we used the unilateral ureteral obstruction model of tubulointerstitial fibrosis in ALK1 haploinsufficient (ALK1(+/-)) and wild-type mice. After 15 days, there was an increase in extracellular matrix protein expression in the obstructed kidneys from both ALK1(+/+) and ALK1(+/-) mice, but obstructed kidneys from ALK1(+/-) mice showed significantly higher expression of type I collagen than those from wild-type mice. Ureteral obstruction increased kidney myofibroblasts markers (α-smooth muscle actin and S100A4), without differences between mouse genotypes. ALK1 expression was increased after ureteral obstruction, and this increased expression was located in myofibroblasts. Moreover, cultured renal fibroblasts from ALK1(+/-) mice expressed more collagen type I and fibronectin than fibroblasts derived from wild-type mice. Thus, ALK1 modulates obstruction-induced renal fibrosis by increased extracellular matrix synthesis in myofibroblasts, but without differences in myofibroblast number.

Abstract: Tubulointerstitial fibrosis is characterized by the presence of myofibroblasts that contribute to extracellular matrix accumulation. These cells may originate from resident fibroblasts, bone-marrow-derived cells, or renal epithelial cells converting to a mesenchymal phenotype. Ras GTPases are activated during renal fibrosis and play crucial roles in regulating both cell proliferation and TGF-beta-induced epithelial-mesenchymal transition. Here we set out to assess the contribution of Ras to experimental renal fibrosis using the well-established model of unilateral ureteral obstruction. Fifteen days after obstruction, both fibroblast proliferation and inducers of epithelial-mesenchymal transition were lower in obstructed kidneys of H-ras knockout mice and in fibroblast cell lines derived from these mice. Interestingly, fibronectin, collagen I accumulation, overall interstitial fibrosis, and the myofibroblast population were also lower in the knockout than in the wild-type mice. As expected, we found lower levels of activated Akt in the kidneys and cultured fibroblasts of the knockout. Whether Ras inhibition will turn out to prevent progression of renal fibrosis will require more direct studies.

Abstract: Reactive oxygen species (ROS) mediate MC contraction, proliferation and apoptosis induced by gentamicin (G) in vitro and in vivo. Sustained increases in cytosolic free calcium, increased iNOS expression and elevated nitric oxide (NO) production are associated with MC apoptosis in vitro. As NO strongly activated c-Jun N-terminal kinase (JNK) and increased AP1 expression, and these two factors are involved in MC proliferation in vitro, we have measured Jun-AP1 expression in rat glomeruli from G-treated rats, and the effect of G on Jun-AP1 expression and JNK activity in cultured MC. Moreover, we studied the expression of inducible (iNOS) and constitutive (cNOS) NO synthases in rat glomeruli. Glomeruli were obtained from rats treated with G (100 mg/kg body weight/day) along 6 days, and MC primary cultures were evaluated after 24, 48 and 72 h incubation with 10(-5) M G. G induced an increase in the expression of iNOS, cNOS and Jun-AP1 in rat glomeruli and in MC cultures. Moreover, G activated JNK; JNK activation was reduced by co-incubation with the calcium channel blocker verapamil and with the ROS scavengers superoxide dismutase and catalase. These results strongly suggest a role for reactive oxygen/nitrogen species produced by increased NOS activity in G-induced MC activation. These reactive oxygen molecules and increased intracellular free calcium may mediate the increase in Jun-AP1 expression and JNK activation induced by G treatment in MC.

Abstract: Background: Gentamicin (G)-induced acute renal failure is characterized by an impairment of glomerular function without apparent changes in glomerular structure. However, G stimulates reactive oxygen species (ROS)-mediated mesangial cell proliferation in vitro. We studied whether G promotes mesangial cell apoptosis in vitro, and if apoptosis and proliferation in parallel may occur in glomerular cells in vivo after a renal damage induced by G treatment., Methods: For in vivo studies, rats were treated with G (100 mg/kg body weight/day) for 6 days, and functional and histologic studies were performed. For in vitro studies, mesangial cell proliferation and apoptosis were evaluated after 24, 48, and 72 hours of 10(-5) mol/L G incubation., Results: After G injections, the number of nuclei per glomerulus did not change, whereas proliferating and apoptotic cell numbers increased. G increases DNA synthesis and cell number in cultured mesangial cells, and increases markedly the apoptotic cell number. ROS scavengers superoxide dismutase and catalase reduce G-induced mesangial cell apoptosis, whereas the incubation with the ROS donor system xanthine plus xanthine oxidase increases apoptosis to levels similar to G. G-induced cellular proliferation and apoptosis either in vitro or in vivo is associated to an early increase in the pro-apoptotic protein Bax and a delayed increase in the survival protein Bcl-2., Conclusion: G simultaneously induces proliferation and apoptosis of mesangial cells in vitro and glomerular mesangial cells in vivo. ROS may mediate G-induced mesangial apoptosis in vitro. The equilibrium proliferation/apoptosis may maintain mesangial cell number within normal limits after a G-induced glomerular insult.

Abstract: Background: Reactive oxygen species (ROS) have been shown to be involved in the reduction of glomerular filtration rate observed after gentamicin (Genta) treatment in vivo, a phenomenon directly related with mesangial cell (MC) contraction. Our previous study reported that Genta induces concentration-dependent MC contraction and proliferation in vitro., Methods: To study the possible mediation of ROS in the effect of Genta, ROS production was measured in primary cultures of rat MC stimulated with Genta (10-5 mol/L). In addition, the MC response to Genta in the presence of the ROS scavengers superoxide dismutase (SOD) and catalase (CAT) was studied. MC activation and O2- production were studied in the presence of an inhibitor of the NADP(H) oxidase, diphenylene iodinium (DPI), and in the presence of L-NAME, an inhibitor of nitric oxide synthases (NOS). Finally, the effects of Genta on SOD activity and mRNA expression were examined., Results: Genta (10-5 mol/L) induced an increase in O2- production and SOD activity that was neither accompanied by an elevation in cytosolic Cu/Zn-SOD mRNA expression nor by H2O2 accumulation. Genta induced MC contraction and proliferation that were inhibited by SOD plus CAT. Both the extracellular and intracellular ROS donor systems, xantine+xantine oxidase (X+XO) and dimethoxinaphtoquinone (DMNQ), respectively, also stimulated MC contraction and proliferation. Genta-induced MC activation and O2- production were inhibited by DPI. Genta-induced O2- production was inhibited by L-NAME. Furthermore, Genta did not induce detectable changes in membrane fluidity and lipid peroxidation., Conclusions: These results strongly suggest that an oxidative-mediated pathway exists in Genta-induced MC activation. A portion of the production of O2- may be due to NADP(H) oxidase and NOS activation. The amount of ROS produced, rather than having a toxic effect, might play a role as a mediator of Genta-induced MC activation
