Locked metacarpophalangeal joint is an uncommon phenomenon with many possible etiologies. Diagnosis can be difficult because of the many more common pathologies (trigger finger, sagittal band lesion, etc.) that can lead to a clinical picture that may resemble the locked metacarpophalangeal joint. Once the differential diagnoses have been eliminated, the etiology of this blockage must be determined and the surgical procedure must be oriented. The origin of the problem is often difficult to determine, especially since standard imaging tests are often not very informative. Several clinical forms are possible, with blockages in extension or flexion, but which will have a disabling functional impact on the overall function of the hand. Currently, there is no gold standard for the management of this condition. In this study, we performed a review of the literature in order to better understand the different possible etiologies but also to analyze the different diagnostic and therapeutic management. IV. L'objectif de cette étude était d'analyser les modalités de réalisation de ce lambeau ainsi que l'évolution de ses indications afin de définir sa place actuelle en traumatologie. Quarante-quatre cas ont été recensés rétrospectivement de janvier 2000 à décembre 2020 au sein de l'HIA Percy, dont 37 cas de lambeaux libres, principalement réalisés pour des reconstructions de pertes de substance étendues avec exposition osseuse et/ou articulaire. Il était également réalisé dans 10,8 % des cas en sauvetage après échec d'une autre solution de reconstruction. Cette analyse a permis de confirmer l'intérêt du lambeau de grand dorsal dans le cas de pertes de substances délabrantes du membre inférieur, mais également chez le grand brûlé, à des fins de réhabilitation fonctionnelle ou de couverture, afin de permettre une rééducation précoce. Reconstructive surgery's workhorse, the latissimus dorsi flap is increasingly abandoned in favour of fasciocutaneous flaps. The purpose of this study was to analyse the methods used to perform this flap and the evolution of its indications in order to define its current place in traumatology. Forty-four cases were recorded retrospectively from January 2000 to December 2020 at HIA Percy, including 37 cases of free flaps, mainly performed for reconstruction of extensive loss of substance with bone and/or joint exposure. It was also performed in 10.8% of cases for salvage after failure of an alternative reconstruction solution. This analysis confirms the value of the latissimus dorsi flap in cases of significant substance loss in the lower limb, but also in burn patients for functional rehabilitation or to allow early rehabilitation. Previous studies have reported that clinical walk tests could not detect differences between fallers and non-fallers in older adults. With advancements in wearable technology, it may be possible to assess differences in loading parameters in clinical settings using portable data collection methods. Research question: The purpose of this study was to determine if wearable sensors (loadsol®) are reliable for assessing asymmetry of contact time, peak force, loading rate (LR), and impulse in older adults and determine if the insole can detect differences in these parameters between fallers and non-fallers during walking. Fifty-five older adults (74.1 ± 6.1 years) walked at their maximum speed on a flat floor. Force data were collected from insoles (100 Hz) during a 10-m walk test. To assess reliability, an intraclass correlation coefficient [ICC(2,k)] was generated for each asymmetry variable. To determine differences between fallers and non-fallers, analysis of covariance (ANCOVA; covariate: body mass index) was completed for each variable. The ICC of peak force asymmetry (PFA) was 0.942, but other ICCs were less than 0.75. The ANCOVA results indicate that the loadsol® can detect differences in PFA between fallers and non-fallers. The PFA was significantly greater in fallers than in non-fallers. As people age there is a proximal shift of joint moment generation from ankle plantarflexion and knee extension toward hip extension and flexion moments. This age-related redistribution has been documented in the context of propulsive force generation during the push-off phase with less evidence in the context of weight bearing. Additionally, these sagittal plane joint moments have been a primary focus of studies though the hip frontal plane moment also contributes to vertical support but has received less attention. Furthermore, how aging affects the relationships between changes in sagittal and frontal joint moments and changes in vertical support force as a function of walking speed remains unclear How does aging affect the contributions of sagittal and frontal plane joint moments to weight-bearing across different walking speeds? Gait analysis was performed on 24 young and 17 healthy older subjects walked on the treadmill at their preferred and 30 % faster speeds. Stepwise linear regression analysis was performed to determine the joint moments that predict the peak amplitudes of the vertical ground reaction force (VGRF) across different walking speeds. Hip abduction and knee extension moments were the primary contributors to leading limb weight-bearing in young, whereas hip extension moment was the primary contributor in older adults. Ankle plantarflexion moment was the main contributor to trailing limb weight-bearing in young and hip flexion moment was the main contributor in older adults. From preferred to faster walking speed changes in knee extension moment were the primary contributor to changes in the trailing limb weight-bearing in young whereas changes in hip extension moment were the primary contributor in olderadults. Asymmetric weight distribution in sitting has been reported in people after stroke. However, postural strategies used during bilateral symmetric and asymmetric movements performed while seated require more evidence to inform rehabilitation strategies. How do symmetric and asymmetric effort levels exerted during upper limb (UL) pushing movements affect seated postural organization parameters (weight bearing (WB) between hands and hemibody sides, and forward trunk displacement) of stroke compared to healthy individuals? How are these parameters associated? Using an instrumented exerciser, 19 post-stroke individuals were compared to 17 healthy individuals when executing four bilateral UL pushing movements in a seated position: symmetrical pushing at 30 % and 15 % of their maximal force (MF) and asymmetrical pushing with 15 % of their MF for one UL vs. 30 % of the MF for the other UL and vice versa. Anterior and vertical forces of the push, as well as vertical forces under each foot and thigh were compared between groups, sides and conditions. Forward trunk displacement was compared between groups and conditions. Correlations were used to determine the association between trunk displacement, hands and hemibody vertical forces. Increasing pushing effort caused increased WB on thighs and decreased on WB on feet during the 30 % MF symmetric condition compared to the 15 % MF and asymmetric conditions (p < 0.05). Individuals post-stroke showed WB asymmetry and greater forward trunk displacement when compared to healthy persons (p < 0.05). For both groups, hemibody WB and trunk displacement showed moderate association (r > - 0.5) in the asymmetric condition executed with more resistance on the paretic or non-dominant hand. Individuals post-stroke presented a similar WB pattern to that of healthy persons during symmetric and asymmetric bilateral UL movements with greater forward trunk displacement and asymmetry. Foot pressure has an essential impact on the entire musculoskeletal chain. So far, the direct influence of foot pressure onto the spinal posture and vice versa is still unclear. The aim of this study was to determine the relationship between foot pressure and spinal posture in healthy adults under static and dynamic conditions. Systematic research was conducted using PubMed to demonstrate the relationship between foot pressure and spinal posture. The search strategy was based on the PICOS selection criteria. The risk of bias within the studies was assessed using SIGN rankings. Based on the search strategy, 719 articles were identified by screening title and abstract. 11 studies with moderate methodological quality partially showed a relationship between foot pressure and spinal posture (SIGN:2-1- and B-D). Apart from the foot pressure, the included studies analyzed parameters of muscle thickness (sonography), trunk displacement, head position, posterior tilt angle, trunk tilt, pelvic rotation, spinal range of motion, or spinal motion. Strength training and increased muscle thickness in the lumbar spine ensure decreased foot pressure. The head position partly induces a change of foot pressure under static conditions. No correlation was identified between spinal posture and foot pressure during forward and backward walking without manipulation. The static and dynamic measurements only show statistical correlations between foot and back muscles. Foot pressure and spinal posture seem to be interrelated under static and dynamic conditions. Static and dynamic measurements are necessary for body posture analysis, as the conditions do not always show consistent results. Due to the small number of studies and low sample sizes, further investigations are necessary to gain more precise perception of the interplay of the spine and foot pressure. Individuals with stroke commonly demonstrate upper-limb sensorimotor impairments. Upper-limb tasks occur against a background level of postural control and thus require a flexible postural control system to facilitate performance. Anterior precision aiming tasks, for example, benefit from lower medial-lateral (ML) center of pressure (COP) fluctuations (where increased fluctuations erode performance) relative to anterior-posterior (AP) fluctuations (where increased fluctuations do not strongly influence performance). After stroke, individuals may compensate for upper-limb impairments by increasing trunk movement which increases overall COP fluctuations and thus may make it more difficult to modulate COP in a task-sensitive manner. Do upper-limb task demands modulate COP movement patterns after stroke? In this cross-sectional study, adults with chronic stroke (n = 23) and unilateral upper-limb impairments were immersed in a virtual environment displaying an anterior target. Participants aimed to maintain the position of a virtual laser pointer (via handheld controller) in the target with each hand. COP was concurrently recorded. Mixed effects models and correlations were used to detect differences in COP patterns between limbs and movement planes and evaluate associations between task performance and COP patterns, respectively. Participants showed greater COP standard deviation and regularity in the AP compared to the ML direction. The magnitude of difference between AP and ML COP metrics was greater using the nonparetic limb. Task performance was moderately and positively associated with task-sensitive COP patterns (i.e., higher AP:ML ratios of COP metrics) using the paretic upper limb. Participants consistently demonstrated high levels of task performance and task-sensitive COP movement patterns using the nonparetic limb. Impairments in postural control after stroke may be related to the upper limb used. Background: The preferred walk-to-run transition speed (PTS) for healthy adults is approximately 2 m∙s-1, however, PTS is influenced by anthropometric factors. Yet despite known sex differences in anthropometrics, studies have reported no sex differences in PTS.Research Question: Do stature and limb length affect PTS in the same way for both male and female healthy adults?Methods: Thirty-seven (19 female) non-injured adults volunteered for this study. Participants completed a walk-to-run transition protocol, where the treadmill speed was increased from 1.2 m∙s-1 to 2.2 m∙s-1, in increments of 0.1 m∙s-1 every two minutes. An independent t-test compared PTS between sexes. Multiple regression analysis determined the effect of sex and stature and sex and limb length on PTS.Results: Female participants transitioned at a lower PTS than male participants (1.8 (0.2) m∙s-1 versus 1.9 (0.1) m∙s-1; p ≤ 0.026). Sex and stature explained 19% of the variance in PTS, while sex and limb length explained 21% of the variance. Including interactions increased the variance explained by 23% and 2% for sex and stature and sex and limb length, respectively. The significant interaction between sex and stature showed PTS was inversely proportional to stature for male participants but directly proportional for female participants.Significance: These findings suggest that the extent to which stature and limb length influence the preferred transition speed may differ between sexes. Background: Extremity ischemia and necrosis after jellyfish envenomation can be mutilating and cause long-term functional deficits. The best way to manage these presentations is unknown.Objective: The aim of this review was to establish an evidence-based consensus for the management of extremity ischemia after jellyfish envenomation.Methods: A systematic review of cases of extremity ischemia and necrosis after envenomation by marine cnidarians was performed to clarify what is and what is not known about management and outcomes, to draw conclusions about how best to manage these rare presentations, and to establish an evidence-based algorithm.Results: The ischemic sequelae of envenomation typically evolves over a few days. Close medical supervision is necessary to react promptly to the evolving clinical scenario. In the literature, 15 different pharmacologic classes have been used to manage these presentations. Only IV infusions of prostaglandin derivatives and intra-arterial thrombolytics have been found to improve the clinical picture and avoid the need for surgical fasciotomy and debridement in some cases. Anticoagulants, antiplatelet agents, steroids, antibiotics, and nitrates, which are among the most commonly prescribed pharmacologic agents, have not been observed to alter the clinical picture.Conclusions: Surgery for compartment syndrome and necrosis are common sequelae of extremity envenomation by marine cnidarians. Only prompt use of IV prostaglandins or intra-arterial thrombolytics can halt ischemic progression and avoid the need for surgery. Atractylis aristata batt., as an endemic plant from the Asteraceae family, holds a significant position in the Ahaggar region of southern Algeria's traditional medicine. The aerial parts of Atractylis aristata was used to cure inflammation, fever, and stomach disorders. The objective of the present investigation was to ascertain the overall bioactive components and phytochemical components and examine the antioxidant, antidiabetic, anti-inflammatory, acute toxicity, and sedative properties of the crude extract obtained from the aerial portions of Atractylis aristata (AaME). The AaME's antioxidant activity was assessed by the use of pyrogallol autoxidation, (1,1 diphenyl-2-picrylhydrazyl) (DPPH), 2,2′-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid (ABTS), and reducing power (RP) techniques. 1 mg/mL of AaME was used to evaluate the antidiabetic activity by applying the enzyme α -amylase inhibitory power test. At the same time, the bovine serum albumin (BSA) denaturation method was employed to quantify the in vitro anti-inflammatory activity at different concentrations (1.5625, 0.78125, 0.390625, 0.1953125 and 0.09765625 mg/mL). In contrast, following the Organization for Economic Co-operation and Development (OECD) guideline No. 423, which covers acute oral toxicity testing protocols, the limit dosage test was employed to assess in vivo acute toxicity. At the dose of 0.08 mg/mL, the carrageenan-induced paw edema approach was used to assess the anti-inflammatory efficacy in vivo , and the sedative activity was carried out at the dose of 0.08 mg/mL using the measurement of the locomotor method. Different bioactive compounds were identified within AaME using LC-MS/MS and HPLC-UV analysis. The acute toxicity study showed no fatalities or noticeable neurobehavioral consequences at the limit test; this led to their classification in Globally Harmonized System (GHS) category Five, as the OECD guideline No 423 recommended. At a concentration of 0.08 mg/mL (2000 mg/kg), AaME showed apparent inhibition of paw edema and a significant (p = 0.01227) reduction in locomotor activity compared to the control animals. Our findings showed that AaME exhibited considerable antioxidant (IC 50 = 0.040 ± 0.003 mg/mL (DPPH), IC 50 = 0.005 ± 5.77 × 10-5 mg/mL (ABTS), AEAC = 91.15 ± 3.921 mg (RP) and IR% = 23.81 ± 4.276 (Inhibition rate of pyrogallol) and rebuts antidiabetic activities (I% = 57.6241% ± 2.81772). Our findings revealed that the maximum percentage of BSA inhibition (70.84 ± 0.10%) was obtained at 1.562.5 mg/mL. Thus, the AaME phytochemical profile performed using phytochemical screening, HPLC-UV, and LC-MS/MS analysis demonstrated that A. aristata can be a valuable source of chemicals with biological activity for pharmaceutical manufacturers. The phytochemical profiling, determined through HPLC-UV and LC-MS/MS applications, reveals this plant's therapeutic value. The aerial parts of Atractylis aristata contain bioactive molecules such as gallic acid, ascorbic acid, and quercetin, contributing to its significant antioxidant capabilities. Furthermore, identifying alizarin, the active compound responsible for its anti-inflammatory properties, could provide evidence supporting the anti-inflammatory capabilities of this subspecies. Background: Falls are common during pregnancy and present potential for injury to the pregnant individual and the baby.Research Question: Do center of pressure characteristics during single leg stance differ between participants during and after pregnancy and nulligravida participants in the presence and absence of visual input?Methods: Nineteen pregnant participants completed testing during the second trimester, the third trimester, and 4-6 months post-partum. Matched, nulligravida females completed testing once. All participants performed single leg stance on a force platform on each limb for up to 20 s with eyes open and with eyes closed. Center of pressure characteristics were compared between pregnant and nulligravida females using three separate 2 × 2 mixed way ANOVAs, one for each pregnancy time point (second trimester, third trimester, and post-partum) with Bonferroni correction.Results: Pregnant females demonstrated smaller single leg stance time with eyes closed during the third trimester. During the second and third trimester, pregnant participants demonstrated smaller sway and sway velocity across eyes open and eyes closed conditions. During the third trimester and post-partum, pregnant participants demonstrated greater median frequency of the center of pressure data. Pregnant participants also demonstrated smaller sample entropy in the anteroposterior direction during the second and third trimesters and in the mediolateral direction during the second trimester.Significance: The decreased total sway and sway velocity observed during pregnancy may reflect rigidity or a protective strategy during single limb stance. Additionally, center of pressure data were less smooth and more repetitive during pregnancy indicating robust differences in postural control strategies and potentially increased fall risk. Because single limb stance is a component of many activities of daily living, the single limb stance task may have clinical utility for testing or training balance in this population with a goal of decreasing falls. Background: Individuals with lower limb amputation exhibit lower residual limb strength compared to their sound limb. Deficits in residual limb knee flexion and extension strength may impact functional performance during tasks relevant to daily living.Research Question: Does knee flexor and extensor strength in the residual limb impact functional outcome measures, such as walking energetics and performance metrics, in individuals with unilateral transtibial amputation?Methods: Fourteen individuals with traumatic unilateral transtibial amputation were recruited for this observational study. Participants completed metabolic testing at three standardized speeds based on leg length, as well maximum isokinetic knee flexion and extension strength for both residual and sound limbs. Participants also completed a series of functional outcome tests, including a two-minute walk test, timed stair ascent test, and four-square step test. Walking energetics (metabolic cost, heart rate, and rating of perceived exertion) and performance metrics were compared to percent deficit of residual limb to sound limb knee flexion and extension muscle strength. A linear regression assessed significant relationships (p < 0.05).Results: A significant relationship was observed between percent deficit of knee extension strength and heart rate (p = 0.024) at a fast walking speed. Additionally, percent deficit knee flexion strength related to rating of perceived exertion at slow and moderate walking speeds (p = 0.038, p = 0.024). Percent deficit knee extension strength related to two-minute walk time performance (p = 0.035) and percent deficit knee flexion strength related to timed stair ascent time (p = 0.025).Significance: These findings suggest the importance of strength retention of the residual limb knee flexion and extension musculature to improve certain functional outcomes in individuals with unilateral transtibial amputation. Background: Anatomical custom-made prostheses make it possible to reconstruct complicated bone defects following excision of bone tumors, thanks to 3D-printed technology. To date, clinical measures have been used to report clinical-functional outcome and provide evidence for the effectiveness of this new surgical approach. However, there are no studies that quantified the achievable recovery during common activities by using instrumental clinical-functional evaluation in these patients.Research Question: What is the motor performance, functional outcome and quality of life in patients with custom-made 3D-printed pelvic prostheses following bone tumor?Methods: To analyze motor performance, six patients performed motion analysis during five motor activities at follow-up of 32 ± 18 months. Joint angles, ground reaction forces and joint moments of the operated and contralateral limbs were compared. On-off activity of lower-limb muscles were calculated from electromyography and compared to a healthy matched population. To analyze functional outcome and quality of life, differences in measured hip abductor strength between limbs were evaluated, as well as clinical-functional scores (Harris Hip Score, Barthel Index, Musculoskeletal Tumor Society score), and quality of life (SF-36 health survey).Results: We found only slight differences in joint kinematics when comparing operated and contralateral limb. The activity of gluteal muscles was normal, while hamstrings showed out-of-phase activities. Ground reaction forces and hip moments showed asymmetries between limbs, particularly in more demanding motor activities. We found a mean difference in hip abductor strength of 48 ± 82 N between limbs, good clinical-functional scores, and quality of life scores within normative.Significance: Our study showed optimal long-term results in functional recovery, mainly achieved through recovery of the gluteal function, although minor impairments were found, which may be considered for future improvement of this innovative surgery. Background: Due to the wide variations in location, size, local invasiveness, and treatment options, the complications associated with surgery for giant cell tumor of bone have been sporadically reported. For quality assessment, fundamental data based on large-scale surveys of complications under a universal evaluation system is needed. The Dindo-Clavien classification is an evaluation system for complications based on severity and required intervention type and is suitable for the evaluation of surgery in a heterogeneous cohort.Methods: A multi-institutional retrospective survey of 141 patients who underwent surgery for giant cell tumor of bone in the extremity was performed. The incidence and risk factors of complications, type of intervention for complication control, and impact of complications on functional and oncological outcomes were analyzed using the Dindo-Clavien classification.Results: Forty-six cases (32.6%) had one or more complications. Of them, 18 (12.8%), 11 (7.8%), and 17 (12.1%) cases were classified as Dindo-Clavien classification grade I, II, and III complications, respectively. There were no cases with grade IV or V complications. Progression in Campanacci grading (p = 0.04), resection (over curettage, p < 0.0001), reconstruction with prosthesis (p = 0.0007), and prolonged operative duration (p = 0.0002) were significant risk factors for complications. Complications had a significant impact on function (p < 0.0001). Differences in the impact of complication types and tumor location on function were confirmed. Complications had no impact on local recurrence and metastasis development.Conclusion: The Dindo-Clavien classification could provide fundamental information, under a uniform definition and classification system, on postoperative complications in patients with giant cell tumor of bone in terms of incidence, type of intervention for complication control, risk factors, and impact on functional outcome. The data are useful not only for preoperative evaluation for the risk of complications under specific conditions but also for quality assessment of surgery for giant cell tumor of bone. To evaluate the accuracy of the assessment of different neoplasias in the adnexa (ADNEX) model in the differential diagnosis of malignant and benign ovarian tumors, the optimal cutoff value and the accuracy in diagnosing ovarian tumors at different stages, PubMed, Web of Science and Cochrane Library databases were retrieved to search literature with per-patient analysis until publication of the last study in November 2021. STATA 14.1, Meta-Disc 1.4 and Revman software 5.3 were used in the performance of meta-analysis. To explore sources of heterogeneity, a subgroup analysis was conducted for the ADNEX model. The pooled sensitivity, specificity, diagnostic odds ratio, positive likelihood, negative likelihood ratio and area under the summary receiver operating characteristic curve were 0.91 (95% confidence interval [CI]: 0.89-0.93), 0.84 (95% CI: 0.80-0.88), 55.55 (95% CI: 40.47-76.26), 5.71 (95% CI: 4.49-7.26), 0.10 (95% CI: 0.08-0.13) and 0.94 (95% CI: 0.92-0.96) in differentiating benign and malignant ovarian tumors, respectively. The area under the curve in identifying benign, borderline, stage I and stages II-IV were 0.93, 0.73, 0.27 and 0.92. The ability to coordinate actions between the limbs is important for many operationally relevant tasks associated with space exploration. A future milestone in space exploration is sending humans to Mars. Therefore, an experiment was designed to examine the influence of inherent and incidental constraints on the stability characteristics associated with the bimanual control of force in simulated Martian gravity. A head-up tilt (HUT)/head-down tilt (HDT) paradigm was used to simulate gravity on Mars (22.3° HUT). Right limb dominant participants (N = 11) were required to rhythmically coordinate patterns of isometric forces in 1:1 in-phase and 1:2 multifrequency patterns by exerting force with their right and left limbs. Lissajous displays were provided to guide task performance. Participants performed 14 twenty-second practice trials at 90° HUT (Earth). Following a 30-min rest period, participants performed 2 test trials for each coordination pattern in both Earth and Mars conditions. Performance during the test trials were compared. Results indicated very effective temporal performance of the goal coordination tasks in both gravity conditions. However, results indicated differences associated with the production of force between Earth and Mars. In general, participants produced less force in simulated Martian gravity than in the Earth condition. In addition, force production was more harmonic in Martian gravity than Earth gravity for both limbs, indicating that less force distortions (adjustments, hesitations, and/or perturbations) occurred in the Mars condition than in the Earth condition. The force coherence analysis indicated significantly higher coherence in the 1:1 task than in the 1:2 task for all force frequency bands, with the highest level of coherence in the 1–4 Hz frequency band for both gravity conditions. High coherence in the 1–4 Hz frequency band is associated with a common neural drive that activates the two arms simultaneously and is consistent with the requirements of the two tasks. The results also support the notion that neural crosstalk stabilizes the performance of the 1:1 in-phase task. In addition, significantly higher coherence in the 8–12 Hz frequency bands were observed for the Earth condition than the Mars condition. Force coherence in the 8–12 Hz bands is associated with the processing of sensorimotor information, suggesting that participants were better at integrating visual, proprioceptive, and/or tactile feedback in Earth than for the Mars condition. Overall, the results indicate less neural interference in Martian gravity; however, participants appear to be more effective at using the Lissajous displays to guide performance under Earth's gravity. Declining energy and increasing fatigue, common in older age, predict neurodegenerative conditions, but their neural substrates are not known. We examined brain resting state connectivity in relation to declining self-reported energy levels (SEL) and occurrence of fatigue over time. We examined resting-state functional MRI in 272 community dwelling older adults participating in the Health Aging and Body Composition Study (mean age 83 years; 57.4 % female; 40.8 % Black) with measures of fatigue and SEL collected at regular intervals over the prior ten years. Functional connectivity (FC) between cortex and striatum was examined separately for sensorimotor, executive, and limbic functional subregions. Logistic regression tested the association of FC in each network with prior fatigue state (reporting fatigue at least once or never reporting fatigue), and with SEL decline (divided into stable or declining SEL groups) and adjusted for demographic, physical function, mood, cognition, and comorbidities. Higher cortico-striatal FC in the right limbic network was associated with lower odds of reporting fatigue (better) at least once during the study period (adjusted odds ratio [95 % confidence interval], p -value: (0.747 [0.582, 0.955], 0.020), independent of SEL. Higher cortico-striatal FC in the right executive network was associated with higher odds of declining SEL (worse) during the study period (adjusted odds ratio [95 % confidence interval], p -value: (1.31 [1.01, 1.69], 0.041), independent of fatigue. Associations with other networks were not significant. In this cohort of older adults, the cortico-striatal functional connectivity of declining SEL appears distinct from that underlying fatigue. To evaluate 6-month risk stratification capacity of the newly developed TeleHFCovid19-Score for remote management of older patients with heart failure (HF) during the coronavirus disease 2019 pandemic. Monocentric observational prospective study. Older HF outpatients remotely managed during the first pandemic wave. The TeleHFCovid19-Score (0-29) was obtained by an ad hoc developed multiparametric standardized questionnaire administered during telephone visits to older HF patients (and/or caregivers) followed at our HF clinic. Questions were weighed on the basis of clinical judgment and review of current HF literature. According to the score, patients were divided in progressively increasing risk groups: green (0-3), yellow (4-8), and red (≥9). A total of 146 patients composed our study population: at baseline, 112, 21, and 13 were classified as green, yellow, and red, respectively. Mean age was 81±9 years, and women were 40%. Compared to patients of red and yellow groups, those in the green group had a lower use of high-dose loop diuretics (P <.001) or thiazide-like diuretics (P =.027) and had reported less frequently dyspnea at rest or for basic activities, new or worsening extremity edema, or weight increase (all P <.001). At 6 months, compared with red (62.2%) and yellow patients (33.3%), green patients (8.9%) presented a significantly lower rate of the composite outcome of cardiovascular death and/or HF hospitalization (P <.001). Moreover, receiver operating characteristic curve analysis showed a high sensibility and specificity of our score at 6 months (area under the curve = 0.789, 95% CI 0.682-0.896, P <.001) with a score <4.5 (very close to green group cutoff) that identified lower-risk subjects. The TeleHFCovid19-Score was able to correctly identify patients with midterm favorable outcome. Therefore, our questionnaire might be used to identify low-risk chronic HF patients who could be temporarily managed remotely, allowing to devote more efforts to the care of higher-risk patients who need closer and on-site clinical evaluations. Limb contractures are associated with poor outcomes and quality of life in long-term care (LTC) residents. This study examined the rate of developing new joint contracture in the LTC residents and associated risk factors to formulate effective interventions in this critical but understudied area. This is an observational study with data obtained from the Hong Kong Longitudinal Study on LTC Residents between 2005 and 2016. Trained assessors (nurses, social workers, and therapists) used the Minimum Data Set Resident Assessment Instrument (MDS-RAI 2.0) to collect the data of the residents from 9 residential LTC facilities. Limb contractures were defined as a functional limitation in the range of motion involving the upper or lower limbs. Primary outcomes included annual prevalence of joint contractures and factors that were associated with the development of new joint contractures. We analyzed the data for 1914 older residents (674 males, mean age 83.4 years). During the first 5 years since admission, the annual prevalence of upper limb contractures increased from 29.8% to 36.5%, and lower limb contractures increased from 41.5% to 57.4%. Overall, the increment of the prevalence rate of joint contractures per year ranged from 0.7% to 3.2% for the upper limbs and 0.3% to 6.0% per year for the lower limbs. Impaired mobility, presence of neurologic diseases, and older age were the leading independent risk factors for the development of new joint contractures. Joint contractures are highly prevalent among residents admitted to the LTC facilities, and many residents develop new contractures during the first 5 years of their admission. Background: Brush cutter (BC)-related penetrating trauma, similar to other types of power saws, can cause serious injuries, especially to the extremities. Studies of this particular injury are limited.Objective: The objective of this study was to characterize injury patterns and patient demographic characteristics for BC-related injuries.Methods: This was a retrospective, observational study on the victims of BC accidents admitted to the emergency department. Data on demographic characteristics, symptoms, physical examination signs, admission time, and outcomes were collected from the hospital database. The location of injuries and damaged tissues were also analyzed.Results: One hundred and fifty-four male (90.1%) and 17 female (9.9%) patients were included retrospectively. Most injuries occurred in autumn (n = 90 [52.6%]) after the harvesting season. Ninety percent of the victims (n = 153) presented with lower extremity trauma. The most frequently injured body part was the anteromedial portion of the left foot (n = 61 [55.9%]). Seven (4.1%) cases had complete digits or limb amputations and 21 (12.3%) cases had incomplete amputation. Twenty-nine (17%) cases had a coexisting vascular injury, 61 (35.7%) had fractured bones, and 129 (75.4%) had tendon damage.Conclusions: BC-related accidents are often preventable but can result in severe disabilities or death. The purpose of this study was to investigate the extent of electromyographic responses associated with manual high-velocity, low-amplitude (HVLA) spinal manipulation systematically applied to the upper and lower cervical and upper thoracic spines in a cohort with mild neck disability. The study was a descriptive observational investigation, with all participants receiving the same interventions. Nineteen participants with mild neck disability received 6 manual HVLA manipulations to the cervical and upper thoracic spine. Bipolar surface electromyography electrode pairs were used to measure responses of 16 neck, back, and limb outlet muscles bilaterally. The number of electromyographic responses was then calculated. Electromyographic responses associated with cervical and thoracic manipulation occurred in a median of 4 of the 16 (range: 1-14) recorded muscles. Cervical spinal manipulation was associated with the highest rates of electromyographic responses in neck muscles, whereas responses in back muscles were highest after upper thoracic manipulation. Cervical spinal manipulation was associated with the highest rate of electromyographic responses in muscles of the cervical spine (sternocleidomastoid and splenius cervicis), whereas responses in back muscles (upper and middle trapezius, latissimus dorsi, and longissimus thoracis) were highest after upper thoracic manipulations. Background Context: Despite established guidelines, long-term management of surgically-treated low back pain (LBP) and lower extremity pain (LEP) remains heterogeneous. Understanding care heterogeneity could inform future approaches for standardization of practices.Purpose: To describe treatment heterogeneity in surgically-managed LBP and LEP.Study Design/setting: Retrospective study of a nationwide commercial database spanning inpatient and outpatient encounters for enrollees of eligible employer-supplied healthcare plans (2007-2016).Patient Sample: A population-based sample of opioid-naïve adult patients with newly-diagnosed LBP or LEP were identified. Inclusion required at least 12-months of pre-diagnosis and post-diagnosis continuous follow-up.Exposure: Included treatments/evaluations include conservative management (chiropractic manipulative therapy, physical therapy, epidural steroid injections), imaging (x-ray, MRI, CT), pharmaceuticals (opioids, benzodiazepines), and spine surgery (decompression, fusion).Outcome Measures: Primary outcomes-of-interest were 12-month net healthcare expenditures (inpatient and outpatient) and 12-month opioid usage.Methods: Analyses include interrogation of care sequence heterogeneity and temporal trends in sequence-initiating services. Comparisons were conducted in the framework of sequence-specific treatment sequences, which reflect the personalized order of healthcare services pursued by each patient. Outlier sequences characterized by high opioid use and costs were identified from frequently observed surgical treatment sequences using Mahalanobis distance.Results: A total of 2,496,908 opioid-naïve adult patients with newly-diagnosed LBP or LEP were included (29,519 surgical). In the matched setting, increased care sequence heterogeneity was observed in surgical patients (0.51 vs. 0.12 previously-unused interventions/studies pursued per month). Early opioid and MRI use has decreased between 2008 and 2015 but is matched by increases in early benzodiazepine and x-ray use. Outlier sequences, characterized by increased opioid use and costs, were found in 5.8% of surgical patients. Use of imaging prior to conservative management was common in patients pursuing outlier sequences compared to non-outlier sequences (96.5% vs. 63.8%, p<.001). Non-outlier sequences were more frequently characterized by early conservative interventions (31.9% vs. 7.4%, p<.001).Conclusions: Surgically-managed LBP and LEP care sequences demonstrate high heterogeneity despite established practice guidelines. Outlier sequences associated with high opioid usage and costs can be identified and are characterized by increased early imaging and decreased early conservative management. Ultrasound (US) is known to stimulate endogenous shear-dependent pathways, and can lower microvascular resistance through mediators that are conducted downstream from US exposure. We hypothesized that endovascular US, already in use for thrombolysis in humans, can improve tissue perfusion in the setting of acute limb ischemia through downstream-conducted effects. Models of severe peripheral arterial disease were developed in mice and in rhesus macaques. An endovascular US catheter (2.3 MHz, 0.5-1.1 MPa) was used to expose the limb adductor in mice for 10 min or the femoral artery distal to stenosis in macaques for 15 min. Quantitative contrast-enhanced ultrasound perfusion imaging was performed to assess flow augmentation in the adductor muscle of mice and the calf muscle of macaques. Microvascular blood flow in the ischemic limb relative to the contralateral control limb was reduced to 22 ± 8% in mice and 36 ± 20% in macaques. US produced immediate 2.3- and 3-fold increases (p < 0.05) in the murine and macaque ischemic limbs, respectively. In macaques, perfusion in the ischemic limb was increased to a normal level. We conclude that non-cavitating US produced by endovascular catheters that are used to enhance thrombolysis in humans can reduce vascular resistance and increase limb perfusion in the setting of acute ischemia. Lymphoedema after cancer treatment is a chronic and disabling complication that presents a significant health care burden during survivorship with limited treatment options. Vascularised lymph node transfer (VLNT) can reconstruct lymphatic flow to reduce limb volumes, but limited higher-order evidence exists to support its effectiveness. The aim of the study was to systematically review and meta-analyse the effectiveness of VLNT in reducing upper limb (UL) or lower limb (LL) volume and cellulitis episodes in patients with cancer treatment–related lymphoedema (CTRL). PubMed, Medline (Ovid) and Embase databases were searched between January 1974 and December 2019. Full-length articles where VLNT was the sole therapeutic procedure for CTRL, reporting volumetric limb, frequency of infection episodes and/or lymphoedema-specific quality-of-life data, were included in a random-effects meta-analysis of circumferential reduction rate (CRR). Methodological quality was assessed using STROBE/CONSORT, and a novel, lymphoedema-specific scoring tool was used to assess lymphoedema-specific methodological reporting. Sensitivity analyses on the site of VLNT harvest and recipient location were performed. Thirty-one studies (581 patients) were eligible for inclusion. VLNT led to significant limb volume reductions in UL (above elbow pooled CRRs [CRR P ] = 42.7% [95% confidence interval (CI): 36.5–48.8]; below elbow CRR P = 34.1% [95% CI: 33.0–35.1]) and LL (above knee CRR P = 46.8% [95% CI: 43.2–50.4]; below knee CRR P = 54.6% [95% CI: 39.0–70.2]) CTRL. VLNT flaps from extra-abdominal donor sites were associated with greater volume reductions (CRR P = 49.5% [95% CI: 46.5–52.5]) than those from intra-abdominal donor sites (CRR P = 39.6% [95% CI: 37.2–42.0]) and synchronous autologous breast reconstruction/VLNT flaps (CRR P = 32.7% [95% CI: 11.1–54.4]) (p < 0.05). VLNT was also found to reduce the mean number of cellulitis episodes by 2.1 episodes per year (95% CI: −2.7– −1.4) and increased lymphoedema-specific quality-of-life scores (mean difference in Lymphoedema-Specific Quality of Life (LYMQOL) "overall domain" = +4.26). VLNT is effective in reducing excess limb volume and cellulitis episodes in both UL and LL lymphoedema after cancer treatment. Background: CT angiography (CTA) is increasingly used in the evaluation of arterial injury in extremity trauma. While it may provide additional objective data, it comes with inherent risks and expense. The purpose of this study was to compare CTA to physical exam in the evaluation of arterial injury in extremity trauma.Methods: We performed a retrospective review of patients who underwent CTA for evaluation of upper or lower extremity trauma at a Level 1 trauma center over a 10 month period. Patients were classified based on initial vascular exam (normal, soft signs, hard signs), and arterial injury on CTA was classified as major (named arteries) or minor (un-named arteries). We evaluated rates of vascular intervention in each group and compared the sensitivity, specificity, negative predictive value (NPV), and positive predictive value (PPV) for physical exam and CTA in identifying arterial injury requiring intervention.Results: A total of 135 CTA studies were included. On initial vascular exam, 71% of patients had a normal exam, 22% had soft signs, and 6% had hard signs. The NPVs for arterial injury requiring intervention of a normal physical exam and negative CTA were both 100%. The PPVs for arterial injury requiring intervention of major injury on CTA and hard signs on physical exam were 35% and 50%, respectively.Conclusion: A normal physical exam can likely rule out the need for vascular intervention and eliminate the need for CTA. Background: Children with cerebral palsy indicate poor continuous gait inter-limb coordination compared to typically developing children. Limited research exists in the understanding of the coordinative relationship between the arms and legs of these children and if the phasing relationships between limbs can be improved.Research Question: Which motor control factors impact coordination in children with cerebral palsy and can coordination improve with intervention?Methods: This literature review provides a comprehensive overview of the current knowledge of continuous coordination in a pediatric, pathologic population with an emphasis on inter-limb coordination. Peer-reviewed research articles related to inter-limb coordination, with a focus on gait, were reviewed to identify relevant research and any gaps in the literature which could inform future study design. Consideration of the most appropriate analysis for evaluation of such movement is also presented.Results: The coordinative difficulties experienced by children with cerebral palsy may originate from the deficits in motor control, muscle tone, and weakness caused by damage to the central nervous system, which is vital in motor control of inter-limb coordination. Continuous inter-limb coordination in children with cerebral palsy may be improved with enhanced function of the upper extremities through intensive motor-learning based rehabilitation or botulinum toxin injection of the more-affected arm. Importantly, analysis of limb movements should include continuous measures of relative phase, as it provides a more detailed description of coordination compared to discrete measures.Significance: Improved upper extremity function may produce a positive change in the phasing relationship between the arms and legs. This type of coordination should be analyzed using relative phase analysis, as this type of analysis can provide more information compared to traditional spatiotemporal parameters. However, more research is required to fully understand the connection between improved upper extremity function and its positive impact on gait coordination in children with cerebral palsy. This study characterized the center of pressure planar displacement by palindromic strings. The objective is to test if the center of pressure pathway of able-bodied girls and those with a moderate and severe scoliosis displayed similar palindromic tendencies. The center of pressure excursions of 21 able-bodied girls were compared to 14 girls with a moderate scoliosis and 14 girls with severe one. Each girl was asked to stand upright on a force platform for 64 s. A crisscross grid of nine areas was centered around the mean center of pressure position (G) to define three other zones to use the MATLAB built-in nucleotide sequence analysis function. These were the antero-posterior extremities A, the coronal extremities C and the tilted or the four corners of the crisscross grid, T. The center of pressure positions were associated to any of the 4 zones using the GATC acronym. For all groups center of pressure pattern in decreasing order was A, G, T and C. Able-bodied girls favored the A zones. Girls with moderate scoliosis displaced their center of pressure mostly in the A zones with shifts in the T sections (P ≤ 0.001). Girls with severe scoliosis, additionally displaced their center of pressure in the C zones (P ≤ 0.001). An ankle modality characterized able-bodied girl's standing balance. Girls with a moderate scoliosis privilege the palindromic zones in the antero-posterior extremities with excursions in the corners of the base of support, girls with severe scoliosis further relied on the medio-lateral zones, suggesting a wobbling standing balance. • Center of pressure displacements were measured for 21 able-bodied and 28 scoliotic girls. • MATLAB nucleotide analysis function allowed the center of pressure palindromic sequencing. • The palindromic strings revealed that scoliotic subjects adopted a wobbling standing balance. • Standing balance control follows organized pathways in different zones of the base of support. Nyctanthes arbor-tristis L. is a mythical plant used in traditional Indian medicinal systems for the treatment of inflammation, rheumatoid arthritis, and pain-related responses. However, its bioactive compounds and underlying mechanism of action have not been fully elucidated. This investigation aimed to study the anti-inflammatory and anti-nociceptive effects of the bioactive extract of N. arbor-tristis (NATE), both in vitro and in vivo, elucidate the possible mechanism of action, and determine its chemicals. We studied the anti-inflammatory and anti-nociceptive activities of NATE on lipopolysaccharide-stimulated RAW264.7 macrophages, paw-ear edema, and acetic acid-induced pain in rats and analysed its chemical components using Liquid Chromatography Electrospray Ionization Tandem Mass Spectrometric (LC-ESI-MS). NATE efficiently reduced the production of various inflammatory mediators and factors, such as free radicals, lipid peroxidation, nitrous oxide (NO), reactive oxygen species (ROS), inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), tumor necrosis factor-alpha (TNFα), interleukin-6 (IL-6), interleukin-1beta (IL-1β), and IL-10, as well as their corresponding mRNA expression in LPS-induced RAW264.7 cells (p < 0.001). Furthermore, NATE inhibited the activation of a key signaling pathway, nuclear factor-kappa B (NF-kB), as it caused a decrease in the degradation of inhibitor of kB alpha (IkBa). Administration of NATE significantly inhibited carrageenan-induced paw edema (p < 0.001), TPA-induced ear edema, and the production of inflammatory factors (p < 0.01). NATE revealed anti-nociceptive impacts in acetic acid-induced writhing and tail immersion experiments (p < 0.001) as well as no toxicity signs. A total of six compounds, namely iridoid glycoside (6,7-di-O-benzonylnyctanthoside), arborsides A, arborsides C, betulinic acid, kaempferol 3-O-glucoside, and kaempferol 3-O-rutinoside, were characterized through the examination of their mass spectra in correlation with those documented in a database of mass spectra. The present study furnishes scientific corroboration of the inhibitory potency of N. arbor-tristis as a promising herbal treatment for inflammation and pain responses without toxicity, offering a scientific basis for future drug development strategies aimed at ameliorating inflammatory diseases. Background: Individuals with incomplete spinal cord injury (iSCI) demonstrate greater postural sway and increased dependency on vision to maintain balance compared to able-bodied individuals. Research on standing balance after iSCI has focused on the joint contribution of the lower limbs; however, inter-limb synchrony in quiet standing is a sensitive measure of individual limb contributions to standing balance control in other neurological populations. It is unknown if and how reduced inter-limb synchrony contributes to the poor standing balance of individuals with iSCI.Research Question: How does an iSCI affect inter-limb synchrony and weight-bearing symmetry in standing?Methods: Eighteen individuals with non-progressive motor iSCI and 15 age- and sex-matched able-bodied individuals (M-AB) were included in the study. Participants stood in a standardized position on two adjacent force plates in eyes open and closed conditions for 70 s per condition. Net centre-of-pressure (COP) root mean square (RMS), net COP velocity, COP inter-limb synchrony (i.e. cross-correlation between left and right COP), and weight-bearing asymmetry (i.e. vertical force from each limb over total vertical force) were calculated. Muscle strength of the lower limbs was assessed with manual muscle testing.Results: Individuals with iSCI demonstrated reduced inter-limb synchrony when standing with eyes open and eyes closed, but did not differ to M-AB with respect to weight-bearing asymmetry. They also produced greater net COP RMS and velocity when compared to M-AB. Muscle strength of the two lower limbs demonstrated an overall asymmetry in individuals with iSCI.Significance: Individuals with iSCI demonstrated impaired balance control as evidenced by reduced inter-limb synchrony and greater COP RMS and velocity compared to M-AB individuals. This increased understanding of how balance control is impaired following iSCI may inform balance assessment and intervention for this population. Introduction: Concern exists regarding the pulmonary effects of using tourniquets for secondary extremity fractures in patients also undergoing intramedullary nail (IMN) fixation of femoral or tibial shaft fractures. Our hypothesis was that tourniquet use would be associated with increased ventilator days.Methods: At a Level I trauma center, we conducted a retrospective review of 1966 patients with 2018 fractures (1070 femoral shaft and 948 tibial shaft) treated with IMN from December 2006 to September 2014. Medical record review and bivariate and multiple variable regression analyses were conducted, and the main outcome measurement was number of ventilator days.Results: No statistically significant negative association was found between use of a tourniquet and number of ventilator days in the femoral or tibial fracture group. Use of tourniquets in the upper extremities showed a statistically significant decrease in amount of ventilator days in the femoral group (-2.2 days, p = 0.003) but no association in the tibial group (1.1 days, p = 0.36). Use of tourniquets concurrently in both upper and lower extremities of both femoral and tibial groups also had a protective effect (-6.8 days, p < 0.001 and -2.3 days, p = 0.009, respectively). Stratified and sensitivity analyses (to account for effects of mortality and missing data) showed consistently similar results.Conclusion: Tourniquet use for secondary extremity fractures, in patients also undergoing IMN fixation for femoral or tibial shaft fractures, was not associated with an increased number of ventilator days. Background: Contracture of the gastrocnemius has been associated with different foot and ankle pathologies. The present study's aim is to evaluate the effect of the proximal medial gastrocnemius release (PMGR) in triceps surae strength.Methods: Prospective study with 14 patients (12 women; mean age 52 years). Inclusion criteria were patients undergoing PMGR due to forefoot and/or hindfoot injury with medial gastrocnemius contracture that has not improved with physical therapy. Isometric and isokinetic force evaluation tests with an isokinetic dynamometer (Con-Trex) were performed preoperatively, at 6 and 12 months postoperative, of both limbs.Results: After isokinetic assessment, the statistically significant difference in the preoperative isometric strength of the triceps between the two ankles was confirmed, being stronger the asymptomatic limb. An improvement in the triceps strength in isokinetics was observed at 60°/s at 6 months after surgery (p=0.008), that was maintained after one year (p=0.05). No differences were observed at 120°/sec speed.Conclusion: Patients with gastrocnemius contracture present a decrease in isometric force with respect to the asymptomatic limb preoperatively. Background: At a clinical level, the intensity of dynamic balance tasks incorporating cross-coordination movements (CCM) is typically progressed by changing the stability of the support surface on which the movement is undertaken. However, biomechanical changes in CCMs performed on stable and unstable surfaces have not yet been quantified.Research Question: Do movement patterns, muscle activity, coordination strategies, knee joint loading and center of mass (CoM) movement differ during a CCM performed on stable and unstable surfaces?Methods: Motion analysis was used to monitor limb kinematics and surface electromyography to analyze supporting leg muscle activity in sixteen healthy athletes during a single-limb support task involving a cyclic CCM on a stable and unstable surface. Angle-angle plots were used to explore coordination strategies in sagittal movement of the hip and shoulder, while differences in kinematics and muscle activity between stable and unstable conditions were evaluated using dependent t-tests (α-level = 0.05).Results: CCMs on an unstable surface were performed at a slower speed (p < .05), with a more flexed posture of the support knee (p < .05) and ankle (p < .05) and resulted in reduced hip and shoulder movement of the swing limbs (p < .05). Instability increased activation of selected muscles of the ankle and knee (p < .05), resulted in a two-fold increase in the peak knee adduction moment (p < .05), and was accompanied by greater CoM movement (p < .05). Three coordination patterns of the swing limbs observed when performing CCM on a stable surface, which were mostly preserved on the unstable surface.Significance: Despite adopting several stabilization strategies, CCM undertaken on an unstable surface still evoked greater excursion of the center of mass and, as such, presented a greater challenge to sensorimotor control. Background: Recent decades have seen increased interest in how anxiety-and associated changes in conscious movement processing (CMP)-can influence the control of balance and gait, particularly in older adults. However, the most prevalent scale used to measure CMP during gait (the Movement-Specific Reinvestment Scale (MSRS)) is generic (i.e., non-gait-specific) and potentially lacks sensitivity in this context.Methods: In a preliminary study, we first sought to evaluate if MSRS scores associated with the number of CMP-related thoughts self-reported by older adults while walking. The next aim was to develop and validate a new questionnaire (the Gait-Specific Attentional Profile, G-SAP) capable of measuring gait-specific CMP, in addition to other attentional processes purported to influence gait. This scale was validated using responses from 117 (exploratory) and 107 (confirmatory factor analysis) older adults, resulting in an 11-item scale with four sub-scales: CMP, anxiety, fall-related ruminations, and processing inefficiencies. Finally, in a separate cohort of 53 older adults, we evaluated associations between scores from both the G-SAP CMP subscale and the MSRS, and gait outcomes measured using a GAITRite walkway in addition to participants' fall-history.Results: MSRS scores were not associated with self-reported thoughts categorised as representing CMP. In regression analyses that controlled for functional balance, unlike the MSRS, the G-SAP subscale of CMP significantly predicted several gait characteristics including velocity (p = .033), step length (p = .032), and double-limb support (p = .015).Significance: The G-SAP provides gait-specific measures of four psychological factors implicated in mediating the control of balance and gait. In particular, unlike the MSRS, the G-SAP subscale of CMP appears sensitive to relevant attentional processes known to influence gait performance. We suggest that the G-SAP offers an opportunity for the research community to further develop understanding of psychological factors impacting gait performance across a range of applied clinical contexts. Background: The variability of center of pressure (COP) is a measure of stability commonly examined during quiet standing. While more recent research has examined the variability of COP during walking, an adequate comparison between shod and unshod walking conditions has yet to be made.Research Question: What is the influence of athletic footwear on the variability of COP displacement during walking?Methods: In this intervention study, twenty healthy women (age 18-30 years) completed 2, 10-min walking trials, 1 shod and 1 unshod, during which ground reaction forces (GRF) and COP movement were collected by an instrumental treadmill. COP displacement was examined in the medial-lateral (ML) and anterior-posterior (AP) directions after being divided into quadrants based on the peaks and trough of each steps associated vertical GRF. A single MANOVA was used to determine effects of footwear and limb for each quadrant with the probability of a Type I error set at 0.05.Results: Significant differences in variability were seen between footwear conditions in all quadrants in the AP direction and quadrants one and four in the ML direction. These results may be due to the structure of footwear, including midsole cushioning, altering the dynamics of the foot during walking.Significance: The results of this study suggest that on average, athletic footwear reduces the variability of COP displacement in ML and AP directions. While the morphology of the adhesive system of geckos has been investigated predominantly in a single species, the tokay Gekko gecko (Linnaeus, 1758), compared to that, there is still the need to examine those traits in a broad diversity of gecko species. Here we focus on the Asian representatives of eublepharid geckos, as a closely related group within Gekkota, and study the anatomy of micro-ornamentation as well as the morphology of bone structures of toes and tails in three species of Eublepharidae. Surface microstructures are described by using SEM microscopy. Osseous structures present in limbs and in the tail are compared by using 3D analysis based on micro-CT data. The presence of epidermal outgrowths associated with friction-enhancement in arboreal representatives of Eublepharidae and the lack of setae in ground-dwelling species is discussed. A clear allocation of the term 'seta' is provided. Apart from a comparative consideration of ecomorphological specialisations in limbs, adaptations to the lateral flexibility of the prehensile tail found in Aeluroscalabotes felinus (Günther, 1864) are illustrated. Correlations between the life style of the species studied and the morphology of their osseous structures in limbs and tail are shown and their significance for locomotion is outlined. Most previous studies have examined the associations between carotenoids and anthropometrics with cross-sectional designs. Few studies have investigated the associations between serum carotenoids and fat mass and fat distribution (general vs central type). This study aimed to explore the associations of serum carotenoids with body fat and fat distribution in Chinese adults. Cross-sectional and longitudinal analyses of a prospective, community-based cohort were performed. There were 4,048 participants aged 40 to 75 years recruited in the Guangzhou Nutrition and Health Study from 2008 to 2013. Serum carotenoids were assessed at baseline. Anthropometrics, fat mass (FM), and percentage FM (%FM) over the total body, trunk, limbs, and android and gynoid regions were obtained by dual-energy x-ray absorptiometry for 3,002 participants between 2011 and 2013 and for 2,537 participants after 3.1 years. Cross-sectional and longitudinal analyses were performed to compare the mean differences in adiposity indices among the quartiles of carotenoids. Covariance analyses showed significant inverse associations between serum total carotenoid levels and adiposity indices cross-sectionally (all P trends<0.05). The percentage mean differences in quartile 4 (vs 1) in FM and %FM were much more pronounced in the trunk (–15.4% and –7.74%) and android area (–16.6% and –8.59%) than those in the limbs (–8.31% and –4.51%) and gynoid area (–7.76% and –2.71%) (all P <0.001). Longitudinal results revealed that higher total carotenoids were associated with significantly lower 3-year increases in body mass index (calculated as kg/m2); waist circumference; waist-to-hip ratio; body FM in the limbs and android and gynoid area; and %FM in total body, trunk, and limbs (all P trends<0.05). Regarding individual carotenoids, β-carotene tended to have the most notable beneficial associations with the majority of fat indices, especially for cross-sectional analyses. Evaluate multisegmental postural sway after upper- vs lower-extremity manipulation. Participants were healthy volunteers (aged 21-40 years). Upper- or lower-extremity manipulations were delivered in a randomized crossover design. Postural assessments were made pre-post manipulation, in floor and rocker board conditions. Analysis included traditional balance measures of pathlength and range and sample entropy (SampEn) to examine the temporal structure of sway of the head, trunk, and surface. No statistical changes in pathlength or sway range on the ground surface condition were observed. No increases in the amount of sway occurred in any condition. Chiropractic manipulation of either upper or lower extremities led to reductions in traditional measures of postural control on the rocker board. In the anteroposterior direction (sagittal plane), lower-extremity manipulation led to increased trunk SampEn while on the ground, and conversely a decreased SampEn while on the rocker board. In the mediolateral rocker board condition (frontal plane), manipulation elicited a change in SampEn that differed according to site of manipulation; upper-extremity manipulation increased SampEn, whereas lower-extremity manipulation reduced SampEn. Both upper- and lower-extremity manipulation influenced several measures of postural sway on both the ground and the rocker board. Lower-extremity manipulation improved the organization of sway at the trunk (anteroposterior direction) and the board (mediolateral direction). Given the reduction and reorganization of sway metrics seen in this study, we propose extending this line of research to the elderly who are at greatest risk of increased sway and falls. Background: Charcot-Marie-Tooth disease type 4J (CMT4J) originates from mutations in the FIG4 gene and leads to distal muscle weakness. Two null alleles of FIG4 cause Yunis Varón syndrome with severe central nervous system involvement, cleidocranial dysmorphism, absent thumbs and halluces and early death.Objectives: To analyse the phenotypic spectrum of FIG4-related disease and explore effects of residual FIG4 protein.Methods: Phenotyping of five new patients with FIG4-related disease. Western Blot analyses of FIG4 from patient fibroblasts.Results: Next generation sequencing revealed compound heterozygous variants in FIG4 in five patients. All five patients presented with peripheral neuropathy, various degree of dysmorphism and a central nervous system involvement comprising Parkinsonism in 3/5 patients, cerebellar ataxia (1/5), spasticity of lower limbs (1/5), epilepsy (1/5) and/or cognitive deficits (2/5). Onset varied between the first and the seventh decade. There was no residual FIG4 protein detectable in fibroblasts of the four analysed patients.Conclusion: This study extends the phenotypic spectrum of FIG4-related disease to Parkinsonism as a feature and demonstrates new phenotypes on a continuum between CMT4J and Yunis Varón syndrome. Recent neurophysiological studies discovered the sparse rotational patterns in the dynamics of neural population during motor control. In this work, we show that a computational model guided by the dynamical system theory of motor coding can successfully generate the similar network behaviors as found in the electrophysiological studies. The RNN-based model learns the arm reaching control policy from self-generated movements. Essential biomechanical and neural properties including multiphasic neural response and the sparse rotation naturally emerge after training for the movement control tasks. The temporal dynamics in the trained network is analyzed to illustrate how the sparse rotational patterns correlate to the generalization capability of the control policy. We find that the trial-and-error motor learning, which naturally brings in the generalization capability, lead to the existence of low-dimensional manifold in the population dynamics of the motor network. • A protocol was established to investigate tissue response to prosthesis loading. • The protocol was evaluated on individuals without amputation, who displayed distinct responses. • Ischaemic and inflammatory precursors were observed under low loads in all limbs, including one example residual limb. • Results show potential measurements for early detection of skin and sub-dermal damage risk during prosthesis use. In the early stages of rehabilitation after primary amputation, residual limb soft tissues have not been mechanically conditioned to support load and are vulnerable to damage from prosthetic use. There is limited quantitative knowledge of skin and soft tissue response to prosthetic loading. An in-vivo protocol was developed to establish suitable measures to assess tissue tolerance during loading representative of early prosthesis use. Ten participants without amputation one participant with trans-tibial amputation were recruited, and pressure applied to their calf in increments from 20 to 60 mmHg. Measurements were recorded at relevant skin sites including interface pressures, transcutaneous oxygen (T C PO 2) and carbon dioxide (T C PCO 2) tensions and inflammatory biomarkers. At the maximum cuff pressure, mean interface pressures were between 66 and 74 mmHg, associated with decreased T C PO 2 values. On the release of pressure, the ischaemic response was reversed. Significant upregulation (p < 0.05) in inflammatory biomarker IL-1α and its antagonist IL-1RA were observed at all sites immediately following loading. The protocol was successful in applying representative prosthetic loads to lower limb tissues and monitoring the physiological response, both in terms of tissue ischemia and skin inflammation. Results indicated that the measurement approaches were sensitive to changes in interface conditions, offering a promising approach to monitor tissue status for people with amputation. Background: Long-term cardiovascular and limb outcomes after revascularization for peripheral artery disease and, in particular, prognosis after post-procedure major adverse limb events (MALE) are not well-studied.Objectives: This study sought to describe outcomes after peripheral revascularization and assess relationships between post-procedure MALE hospitalization and subsequent events.Methods: Patients undergoing peripheral artery revascularization between January 1, 2009, and September 30, 2015, in the Premier Healthcare Database were examined for the co-primary outcomes of interest, composite myocardial infarction (MI) or stroke and composite major amputation or peripheral revascularization. Multivariable adjusted Cox proportional hazards models with post-procedure MALE hospitalization included as a time-dependent covariate were developed to estimate hazard ratios for outcomes.Results: Among 393,017 revascularized patients followed for a median of 2.7 years (interquartile range: 1.3 to 4.4 years), the cumulative incidence of MI or stroke was 9.8% and that of major amputation or peripheral revascularization was 41.9%. A total of 50,750 patients (12.9%) had at least 1 post-procedure MALE hospitalization. In time-dependent covariate adjusted models, post-procedure MALE hospitalization was associated with greater risk of subsequent MI or stroke (hazard ratio: 1.34; 95% confidence interval: 1.28 to 1.40) and major amputation or peripheral revascularization (hazard ratio: 8.13; 95% confidence interval: 7.96 to 8.29). After peripheral revascularization with or without post-procedure MALE hospitalization, risk of limb events increased rapidly post-procedure and more slowly after the first year, whereas cardiac risk increased steadily during follow-up.Conclusions: Revascularized peripheral artery disease patients face earlier limb and later cardiovascular ischemic risk that is heightened among patients with post-procedure MALE hospitalization. Early intervention is effective for developing motor ability and preventing contractures and deformities in patients with cerebral palsy (CP). Gait training using the newly developed Hybrid Assistive Limb (HAL) shows promise as an intervention to prevent deterioration in walking ability and deformities in pediatric CP patients. The purpose of this pilot study was to examine the safety and immediate effects on walking ability after gait training using the HAL in pediatric CP patients. Nineteen patients (six females, 13 males; mean age 8.5 years; mean height 120.5 cm; mean weight 23.2 kg) were enrolled. The Gross Motor Functional Classification Scale level was I in two patients, II in two, III in eight, and IV in seven. The HAL was used for a single session of gait training. The primary outcome was safety of the HAL for use in pediatric CP patients. The secondary outcome was the immediate effect after gait training with HAL, evaluated by passive range of motion (ROM) and gait parameters, including gait speed (m/s), step length (cm), and cadence (step/min). All 19 patients were able to carry out the gait training without any severe adverse events. Significant improvements were observed for mean internal/external rotation and abduction angles of the hip joint, and ankle dorsiflexion angles (n = 19). Significant improvements were observed for mean gait speed and step length based on expansion of the hip flexion-extension range (n = 11). Introduction: Our aim was to observe the efficacy of the induced membrane technique in the treatment of extremity osteomyelitis and to analyse the causes of infection recurrence and its risk factors.Methods: We retrospectively analysed 424 cases of extremity osteomyelitis treated with the induced membrane technique in our department between May 2013 and June 2017. Infection recurrence time, recurrence sites and other relevant information were collected, summarized, and analysed.Results: A total of 424 patients were considered as "cured" of osteomyelitis after the first stage and the induced membrane technique was performed to rebuild the bone defects. After a mean follow-up of 31.6 (16-63) months, 52 patients had recurrence of infection, including 42 tibias and 10 femurs. The recurrence rate was 12.26%. Symptoms were relieved in 16 patients after intravenous antibiotic treatment. In the remaining 36 cases (8.49%), the infection was uncontrolled by intravenous antibiotics and surgical debridement was performed. The recurrence rate of infection of the tibia (16.22%) was higher than that of the femur (8.70%). The recurrence rate of post-traumatic osteomyelitis (14.66%) was significantly higher than that of hematogenous osteomyelitis (2.41%). Patients in whom Pseudomonas aeruginosa was isolated at the first stage had a recurrence rate of 28% (7/25), which was higher than that with the other isolated bacteria. Logistic regression analysis showed that repeated operations (≥3), post-traumatic osteomyelitis, and internal fixation at the first stage were risk factors for recurrence of infection, with odds ratios (ORs) of 2.30, 5.53 and 5.28 respectively.Conclusions: The induced membrane technique is an effective method in the treatment of extremity osteomyelitis, although infection recurs in some cases. Repeated operations, post-traumatic osteomyelitis, and internal fixation at the first stage were risk factors for recurrence of infection. A laryngotracheoesophageal cleft (LC) is a rare congenital anomaly of the upper aerodigestive tract resulting from the absence of fusion of the posterior cricoid lamina, which affects an abnormal communication between the larynx, trachea and esophagus. The genetic etiology of LC remains elusive. The involvement of genetic factors in the development of LC is suggested by reports of familial occurrence, and the increased prevalence of component features among first-degree relatives of affected individuals and murine knockout models. No consistent pattern of inheritance has been found in nonsyndromic patients, except for cases associated with described syndromes. Once the syndrome related to the laryngeal cleft is considered, an active search for the cleft must be initiated. The genetic evaluation of patients with LCs should be guided by the type and location of the malformation, specific medical history and a detailed physical examination. The primary objective of this study was to examine the degree of inter-limb asymmetry in impact force magnitudes and rates during the first and second landings of a drop vertical jump in adolescent athletes with a history of anterior cruciate ligament (ACL) reconstruction. We also compared the degree of asymmetry exhibited by the athletes who had undergone ACL reconstruction to a group of uninjured athletes. This study included 14 athletes who had undergone ACL reconstruction and 28 uninjured athletes, matched for age, sex, and sport. All athletes completed a double-leg drop vertical jump task. Peak vertical ground reaction forces (vGRFs) and loading rates were examined for both limbs during the first and second landings of the drop vertical jump. For the athletes who had undergone ACL reconstruction, peak vGRFs were 11.9% greater for the uninvolved limb vs. the involved limb during the first landing; however, peak vGRFs were only 2.4% greater for the uninvolved limb (vs. the involved limb) during the second landing. The athletes who had undergone ACL reconstruction exhibited greater asymmetry in peak vGRFs for the first landing compared to the uninjured athletes; however, there was no difference between the groups (ACL reconstruction, uninjured) for the second landing. The digestive physiology of maras (Dolichotis patagonum) has not been investigated in detail. Maras have a particular limb anatomy facilitating a unique cursoriality among rodents. This may also have led to additional adaptations such as a reduced volume of the gastrointestinal tract. We performed macroanatomical measurements of, and determined mean particle size along, the digestive tract of 10 semi-free-ranging animals (7.04 ± 1.05 kg). Additionally, we measured CH 4 emission in five captive animals (7.67 ± 0.98 kg) fed a diet of pelleted lucerne, and measured food intake, digestibility, and digesta mean retention time (MRT) of a solute and three particle markers (fed at <2, 10 and 20 mm particle size). The digestive tract contents represented 11.1 ± 1.4% of body mass, similar to other mammals and rodents, and there was slight indication of selective small particle retention in the caecum. Secondary peaks in marker elimination patterns suggested the possibility of caecotrophy. The MRTs were 15.4 h for the solute and 13.6 h, 13.3 h and 13.3 h for the three particle markers, respectively. At a dry matter intake of 61 ± 12 g kg body mass-0.75 d−1, the maras digested organic matter and neutral detergent fibre to 48 ± 8% and 34 ± 10%, respectively, which is in the lower range of results from horses fed on a diet with a similar fibre content. The respiratory quotient (CO 2 /O 2) was 0.93 ± 0.03, the resting metabolic rate 346 ± 35 kJ kg body mass-0.75 d−1, and CH 4 emissions averaged at 3.85 ± 0.47 L d−1 and 14.5 ± 5.2 L per kg dry matter intake; this at a CH 4 /CO 2 ratio of 0.042 ± 0.004. Thus, the methane yield was of a magnitude expected for a hypothetical ruminant of this body mass. The results are consistent with the general understanding of hystricomorph rodent digestive physiology, including caecotrophy, but do not indicate a reduction of digestive capacity to support cursoriality. These results, and those obtained from other hystricomorph rodents, suggest that CH 4 production may be more prominent in rodents than previously thought. We describe the digestive anatomy and physiology of mara, a small caecum fermenter, including adaptations for coprophagy, a metabolism that is at the mammalian average, and a methane production of a magnitude expected of similar-sized ruminants. High-fat diet-induced obesity can lead to hepatic insulin resistance (IR) and alter glucose metabolism. The decreased protein expression involved in the PI3K-Akt pathway may enhance hepatic glycogenolysis and gluconeogenesis. Obesity-associated glucose dysregulation and IR are risk factors for the development of peripheral arterial disease. Glutamine (Gln) has immunomodulatory properties and was found to attenuate IR and hyperglycemia in diabetic condition. Thus, in this study we hypothesized that Gln administration modulates hepatic glucose metabolism and improve IR via PI3K-Akt pathway in obese mice with limb ischemia. Mice were divided into a high-fat group (HC), and a high-fat Gln group (HG). Mice in the HC group were fed the high-fat diet for 8 weeks, while the HG group was initially fed the high-fat diet for 4 weeks followed by a high-fat diet with Gln for an additional 4 weeks. Part of the mice in the HC and HG groups were subjected to a limb ischemic operation and were euthanized after the operation. Liver tissues and blood samples were collected for analysis. The results showed that high-fat diet-induced obesity resulted in increased plasma glucose and insulin levels. Also, impairment of hepatic insulin signaling by downregulating PI3K-Akt pathway-associated protein expression was observed. Administration of Gln increased protein expression associated with PI3K-Akt signaling pathway, while reducing G6PC and FOXO1 expression in the hepatocytes that may promote glycogen synthesis and inhibit gluconeogenesis. Propulsive deficits following stroke have been attributed to reduced plantarflexion moments and a reduced trailing limb angle. We sought to determine the validity of the trailing limb angle as a surrogate measure of the anterior ground reaction force, as well as to determine the anatomical landmarks for the trailing limb angle that best approximate the orientation of the ground reaction force. Forty-four participants with chronic stroke walked on a treadmill. At peak propulsion, we computed the trailing limb angle, the anterior orientation of the ground reaction force (the gold standard), and the hip extension angle for correlational analyses. Further, we compared the absolute error of the various trailing limb angle computations to determine which landmarks provided the most accurate representation of the anterior angle of the ground reaction force. For the paretic and non-paretic limbs, the anterior angle of the ground reaction force was related to all measures of trailing limb angle as well as the peak propulsive force. The hip extension angle, however, was not related to the angle of the ground reaction force. Only the choice of distal landmarks significantly influenced the error between trailing limb angle and the anterior angle of the ground reaction force. These data suggest that measuring the sagittal plane orientation of the entire limb serves as a suitable surrogate for measuring the anterior angle of the ground reaction force. Background: Increased femoral anteversion (IFA) is defined as forwardly rotated femoral head relative to the transcondylar knee axis which may have a potential to reduce the functional quality of adolescents. Therefore, the aim of our study was to investigate the effects of IFA on lower-extremity function, falling frequency, and fatigue onset in neurologically intact children.Research Question: Does increased femoral anteversion influence lower extremity function, falling frequency and fatigue on set in healthy children?Methods: Sixty-five participants with increased femoral anteversion (IFA) and thirty-two healthy peers as control were included into the study. For the function, the lower extremity function form (LEFF) which is adapted from Lower Extremity Function Test used. Falling frequency and fatigue onset time were assessed by a Likert-type scale. In addition, the activities which cause frequently fall for the participants were questioned.Results: Lower extremity function was found deteriorated (p= 0.02) and falling frequency was higher (p = 0.00) in IFA than in controls. Fatigue onset time was not different between groups, although lower extremity function was strongly correlated with fatigue onset (rho = -0.537, p < 0.001). IFA children fall four times more during running (60%), three times more during fast walking (21.42%) than their healthy peers (14.28%, 7.14% respectively).Significance: IFA leads functional problems, especially in the form of high falling frequencies. According to the LEFF score, the most difficult functional parameters for these children were walking long distances, becoming tired, walking more than a mile, and standing on one spot. Also, shorter fatigue onset time may worsen the lower-extremity function secondarily. Because of the higher frequency of falling and functional problems, children with IFA may be more defenseless to injuries, especially in high-motor-skill activities such as running and soccer. Abstract Electromyography (EMG) signal is one of the widely used biological signals for human motor intention prediction, which is an essential element in human-robot collaboration systems. Studies on motor intention prediction from EMG signal have been concentrated on classification and regression models, and there are numerous review and survey papers on classification models. However, to the best of our knowledge, there is no review paper on regression models or continuous motion prediction from EMG signal. Therefore, in this paper, we provide a comprehensive review of EMG-based motor intention prediction of continuous human upper limb motion. This review will cover the models and approaches used in continuous motion estimation, the kinematic motion parameters estimated from EMG signal, and the performance metrics utilized for system validation. From the review, we will provide some insights into future research directions on these subjects. We first review the overall structure and components of EMG-based human-robot collaboration systems. We then discuss the state of arts in continuous motion prediction of the human upper limb. Finally, we conclude the paper with a discussion of the current challenges and future research directions.
