11. Predictive potential of serum and cerebrospinal fluid biomarkers for disease activity in treated multiple sclerosis patients.

Abstract: • Elevated levels of the kappa free light chains index (FKLCi) have emerged as a predictor for therapeutic failure (TF), defined as the occurrence of a second relapse, two or more T2 lesions, or PIRA. • An FKLCi exceeding 130 was associated with TF in both univariate and multivariate analyses, adjusted for sex, age, presence of acute inflammatory activity, EDSS at the time of sampling, administered line of treatment, and time from CSF analysis to treatment initiation. • Initiating second-line disease-modifying therapies in patients with elevated FKLCi levels would be advisable, as they have shown to be a robust protective factor against TF. • Age emerged as a weak protective factor against TF. • In contrast to findings from other studies, oligoclonal IgM Bands, Chitinase-3 Like-1, and serum neurofilament (analyzed as either raw values or age-adjusted Z-scores) were not associated with worse disease outcomes. Background: Our objective was to explore various biomarkers for predicting suboptimal responses to disease-modifying treatments (DMTs) in patients with MS (pwMS). Methods: We conducted a longitudinal, bicentric study with pwMS stratified based on their DMTs responses. Treatment failure (TF) was defined as the onset of a second relapse, presence of two or more T2 new lesions, or disability progression independent of relapse during the follow-up period. We evaluated intrathecal synthesis (ITS) of IgG and IgM using OCB, linear indices, and Reibergrams. Free kappa light chains ITS was assessed using the linear index (FKLCi). NfL and GFAP in serum and CSF, and CHI3L1 in CSF were quantified. Quantitative variables were dichotomized based on the third quartile. Predictive efficacy was assessed through bivariate and multivariate analyses, adjusting for age, sex, EDSS, acute inflammatory activity (AI) -defined as the onset of a relapse or gadolinium-enhancing lesions within a 90-day window of lumbar puncture-, treatment modality, study center, and time from disease onset to treatment initiation. In case of collinearity, multiple models were generated or confounding variables were excluded if collinearity existed between them and the biomarker. The same methodology was used to investigate the predictive potential of various combinations of two biomarkers, based on whether any of them tested positive or exceeded the third quartile. Results: A total of 137 pwMS were included. FKLCi showed no differences based on AI, no correlation with EDSS and was significantly higher in pwMS with TF (p = 0.008). FKLCi>130 was associated with TF in bivariate analysis (Log-Rank p = 0.004). Due to collinearity between age and EDSS, two different models were generated with each of them and the rest of the confounding variables, in which FKLCi>130 showed a Hazard Ratio (HR) of 2.69 (CI: 1.35–5.4) and 2.67 (CI: 1.32–5.4), respectively. The combination of either FKLC or sNfL exceeding the third quartile was also significant in bivariate (Log-Rank p = 0.04) and multivariate (HR=3.1 (CI: 1.5–6.5)) analyses. However, when analyzed independently, sNfL did not show significance, and FKLCi mirrored the pattern obtained in the previous model (HR: 3.04; CI: 1.51–6.1). Treatment with highefficacy DMTs emerged as a protective factor in all models. Discussion: Our analysis and the fact that FKLCi is independent of EDSS and AI suggest that it might be a valuable parameter for discriminating aggressive phenotypes. Abstract: This paper presents a new non-boundary-fitted-grid numerical technique for solving partial differential equations (PDEs) in multi-hole domains. A multiply-connected domain is converted into a simply-connected domain of rectangular or non-rectangular shape that is discretised using a Cartesian grid. Compact radial basis function (RBF) stencils, which are constructed through integration rather than the conventional differentiation, are used to discretise the field variables. The imposition of inner boundary conditions is conducted by means of body forces that are derived from satisfying the governing equations and prescribed boundary conditions in small subregions. Salient features of the proposed method include: (i) simple pre-processing (Cartesian grid), (ii) high rates of convergence of the solution accuracy with respect to grid refinement achieved with compact integrated-RBF stencils, where both nodal function and derivative values are included in the approximations, (iii) the system matrix kept unchanged for the case of moving holes, and (iv) no interpolation between Lagrange and Euler meshes required. Several linear and nonlinear problems, including rotating-cylinder flows and buoyancy-driven flows in eccentric and concentric annuli, are simulated to verify the proposed technique. • This paper presents a new non-boundary-fitted-grid method. • The field variables are approximated using compact integrated-RBF stencils. • Body forces are derived from satisfying the PDEs and boundary conditions in subregions. • Interpolation between the two Euler and Lagrange systems is not required. • Multiply-connected domains with moving holes are considered in verification. 13. Predicting depression in old age: Combining life course data with machine learning.

Abstract: With ageing populations, understanding life course factors that raise the risk of depression in old age may help anticipate needs and reduce healthcare costs in the long run. We estimate the risk of depression in old age by combining adult life course trajectories and childhood conditions in supervised machine learning algorithms. Using data from the Survey of Health, Ageing and Retirement in Europe (SHARE), we implement and compare the performance of six alternative machine learning algorithms. We analyse the performance of the algorithms using different life-course data configurations. While we obtain similar predictive abilities between algorithms, we achieve the highest predictive performance when employing semi-structured representations of life courses using sequence data. We use the Shapley Additive Explanations method to extract the most decisive predictive patterns. Age, health, childhood conditions, and low education predict most depression risk later in life, but we identify new predictive patterns in indicators of life course instability and low utilization of dental care services. • We predict old age depression risk by combining retrospective life course data and machine learning models. • Models are estimated on SHARElife biographical data. • The best-performing models are those that include high-dimensional, semi-structured data. • Depression in women is more foreseeable than in men. • Our method uncovers under-studied predictors such as emotional-status changes and low lifetime utilization of dental care services. 14. Improving the efficiency of free kappa light chains as diagnostic biomarker of Multiple Sclerosis by using a novel algorithm.

Abstract: • The combined use of certain algorithms involving index of free kappa light chains along with oligoclonal IgG bands and other calculations can increase sensitivity, negative predictive value, and accuracy at the expense of a reduction in positive predictive value and specificity offered by the sole use of these parameters. • Through the use of these algorithms, the consumption of oligoclonal bands can be minimized without losing the ability to predict intrathecal synthesis. • An algorithm named KRO, based on determining the index of free kappa light chains initially, followed by the reiber formula with oligoclonal bands reserved for borderline index of free kappa light chains values resulting negative in the reiber formula, was the one that achieved higher sensitivity and accuracy in detecting intrathecal IgG synthesis. This came at the expense of losing some sensitivity and a slight decrease in positive predictive value. • By using a combination of free kappa light chains and oligoclonal bands, it is possible to compensate for the inherent limitations of each individual technique. • According to the KRO algorithm, the usage of oligoclonal bands can be reduced by 91%. Furthermore, they can be employed when seeking information about IgG clonality or determining the intrathecally synthesized immunoglobulin isotype. Intrathecal immunoglobulin synthesis (ITS) plays a crucial role in the diagnosis of multiple sclerosis (MS). Traditionally, the gold standard method for detecting ITS has been through the analysis of oligoclonal bands (OCB). However, the paradigm has shifted with the introduction of the free kappa light chains (FKLC) method. In fact, a recent consensus recommends evaluating FKLC index (FKLCi) as the primary approach and reserving oligoclonal bands with borderline results. The objective of our study is to investigate the diagnostic efficiency of combining FKLC with other methods to predict ITS while minimizing the reliance on OCB. A total of 192 patients were included in the study, consisting of 145 individuals diagnosed with multiple sclerosis (pwMS) and 46 with other neurological diseases (controls). Among the MS cases, 100 patients were assigned to the Training Cohort (TC), while an external Validation Cohort (VC) comprised of 45 MS patients was established. Diagnostic efficiency was assessed for FKLCi, OCB, Link index, and the Reiber formula for IgG and FKLC. Optimal cutoff values for Link index and FKLCi were also determined. The last procedure was developed for diverse algorithms using the parameters mentioned above, which included the optimal cutoffs previously obtained. The calculations were conducted independently for both the TC and the VC, as well as for a composite cohort formed by combining data from all patients (OC) One algorithm, named KRO, was developed based on the determination of FKLCi and Reiber Formula as the primary diagnostic parameters. For cases where the FKLCi result was mildly increased, OCB was utilized as a supplementary test. The KRO algorithm demonstrated superior diagnostic accuracy in the OC (89%), resulting in a reduction of OCB consumption by 91%. The KRO algorithm demonstrated superior sensitivity and accuracy although lower specificity and NPV compared to the use of FKLCi and OCB alone. The present research aligns with the new consensus recommendations regarding the diagnostic approach. Our findings indicate that employing a combined marker approach via KRO could prove to be a proficient screening tool for multiple sclerosis. This approach also holds the potential to address inherent limitations associated with each individual marker. 15. Is inertial flywheel resistance training superior to gravity-dependent resistance training in improving muscle strength? A systematic review with meta-analyses.

Abstract: Objective: The primary aim of this systematic review was to determine if inertial flywheel resistance training is superior to gravity-dependent resistance training in improving muscle strength. The secondary aim was to determine whether inertial flywheel resistance training is superior to gravity-dependent resistance training in improving other muscular adaptations.Design: A systematic review with meta-analyses of randomised and non-randomised controlled trials.Methods: We searched MEDLINE, Scopus, SPORTDiscus, Web of Science and Cochrane Central Register of Controlled Trials with no publication date restrictions until November 2016. We performed meta-analyses on randomised and non-randomised controlled trials to determine the standardized mean difference between the effects of inertial flywheel and gravity-dependent resistance training on muscle strength. A total of 76 and 71 participants were included in the primary and secondary analyses, respectively.Results: After systematic review, we included three randomised and four non-randomised controlled trials. In the primary analysis for the primary outcome muscle strength, the pooled results from randomised controlled trials showed no difference (SMD=-0.05; 95%CI -0.51 to 0.40; p=0.82; I2=0%). In the secondary analyses of the primary outcome, the pooled results from non-randomised controlled trials showed no difference (SMD=0.02; 95%CI -0.45 to 0.49; p=0.93; I2=0%; and SMD=0.03; 95%CI -0.43 to 0.50; p=0.88; I2=0%). Meta-analysis on secondary outcomes could not be performed.Conclusion: Based on the available data, inertial flywheel resistance training was not superior to gravity-dependent resistance training in enhancing muscle strength. Abstract: Soft biological tissues demonstrate strong time-dependent and strain-rate mechanical behavior, arising from their intrinsic visco-elasticity and fluid–solid interactions. The time-dependent mechanical properties of soft tissues influence their physiological functions and are related to several pathological processes. Poro-elastic modeling represents a promising approach because it allows the integration of multiscale/multiphysics data to probe biologically relevant phenomena at a smaller scale and embeds the relevant mechanisms at the larger scale. The implementation of multiphase flow poro-elastic models however is a complex undertaking, requiring extensive knowledge. The open-source software FEniCSx Project provides a novel tool for the automated solution of partial differential equations by the finite element method. This paper aims to provide the required tools to model the mixed formulation of poro-elasticity, from the theory to the implementation, within FEniCSx. Several benchmark cases are studied. A column under confined compression conditions is compared to the Terzaghi analytical solution, using the L2-norm. An implementation of poro-hyper-elasticity is proposed. A bi-compartment column is compared to previously published results (Cast3m implementation). For all cases, accurate results are obtained in terms of a normalized Root Mean Square Error (RMSE). Furthermore, the FEniCSx computation is found three times faster than the legacy FEniCS one. 17. Anisotropic mechanical characterization of human skin by in vivo multi-axial ring suction test.

Abstract: Human skin is a soft tissue behaving as an anisotropic material. The anisotropy emerges from the alignment of collagen fibers in the dermis, which causes the skin to exhibit greater stiffness in a certain direction, known as Langer's line. The importance of determining this anisotropy axis lies in assisting surgeons in making incisions that do not produce undesirable scars. In this paper, we introduce an open-source numerical framework, MARSAC (Multi-Axial Ring Suction for Anisotropy Characterization: https://github.com/aflahelouneg/MARSAC), adapted to a commercial device CutiScan CS 100® that applies a suction load on an annular section, causing a multi-axial stretch in the central zone, where in-plane displacements are captured by a camera. The presented framework receives inputs from a video file and converts them into displacement fields through Digital Image Correlation (DIC) technique. From the latter and based on an analytical model, the method assesses the anisotropic material parameters of human skin: Langer's line ϕ , and the elastic moduli E 1 and E 2 along the principal axes, providing that the Poisson's ratio is fixed. The pipeline was applied to a public data repository, https://search-data.ubfc.fr/femto/FR-18008901306731-2021-08-25%5fIn-vivo-skin-anisotropy-dataset-for-a-young-man.html , containing 30 test series performed on a forearm of a Caucasian subject. As a result, the identified parameter averages, ϕ ˆ = 40. 9 ± 8. 2 ∘ and the anisotropy ratio E 1 ˆ / E 2 ˆ = 3. 14 ± 1. 60 , were in accordance with the literature. The intra-subject analysis showed a reliable assessment of ϕ and E 2. Abstract: Computational fluid dynamics (CFD) represents an attractive tool for estimating wind pressures and wind loads on high-rise buildings. The CFD analyses can be conducted either by low-fidelity simulations (RANS) or by high-fidelity ones (LES). The low-fidelity model can efficiently estimate wind pressures over a large range of wind directions, but it generally lacks accuracy. On the other hand, the high-fidelity model generally exhibits satisfactory accuracy, yet, the high computational cost can limit the number of approaching wind angles that can be considered. In order to take advantage of the main benefits of these two CFD approaches, a multi-fidelity machine learning framework is investigated that aims to ensure the simulation accuracy while maintaining the computational efficiency. The study shows that the accurate prediction of distributions of mean and rms pressure over a high-rise building for the entire wind rose can be obtained by utilizing only 3 LES-related wind directions. The artificial neural network is shown to perform best among considered machine learning models. Moreover, hyperparameter optimization significantly improves the model predictions, increasing the R 2 value in the case of rms pressure by 60%. A segmented multipart mesh (bones, cartilage and tissue) was constructed and a three-step FE-based DVC procedure with heterogeneous elastic regularization was implemented. With the proposed framework, the large displacement field of a hard/soft breast sample was recovered with low registration residuals and small error between the measured and manually determined deformations of phase interfaces. Abstract: Background: Biologic drug survival in psoriasis reflects long-term performance in real-life settings. Previous studies have yielded inconsistent results.Objectives: We sought to analyze long-term biologic survival and its associated variables in a large, real-life cohort of patients with moderate to severe chronic plaque psoriasis.Methods: This was an observational retrospective study. Data were extracted from clinical records of 427 patients treated with biologic agents over a 4-year period. Drug survival was analyzed using the Kaplan-Meier method and the influence of several covariates was assessed using Cox regression.Results: We analyzed 703 treatment courses. Overall median drug survival was 31.0 months. Cumulative probability of drug survival was lower in obese patients (23.0 months, 95% confidence interval 17.4-28.6) than in patients with body mass index less than 30 (37.3 months, 95% confidence interval 29.4-45.1, P = .001), and it was significantly higher for ustekinumab than for any other biologic agent (log rank test P < .001). Multivariate analysis showed that obesity, etanercept treatment, and strict adherence to approved doses were associated with an increased probability of drug withdrawal, whereas ustekinumab treatment, and PASI75 and PASI90 responses at week 16 prolonged drug survival.Limitations: Data were collected retrospectively.Conclusions: These findings can facilitate the daily treatment of psoriatic patients and promote long-term effectiveness of biologic therapies. Abstract: Background Allergy to pollen from short ragweed (Ambrosia artemisiifolia) is a serious and expanding health problem in the United States and in Europe. Objective We sought to investigate the presence of undescribed allergens in ragweed pollen. Methods Ragweed pollen proteins were submitted to high-resolution gel electrophoresis and tested for IgE reactivity by using sera from 92 American or European donors with ragweed allergy. Pollen transcriptome sequencing, mass spectrometry (MS), and recombinant DNA technologies were applied to characterize new IgE-binding proteins. Results High-resolution IgE immunoblotting experiments revealed that 50 (54%) of 92 patients with ragweed allergy were sensitized to a 37-kDa allergen distinct from Amb a 1. The full-length cDNA sequence for this molecule was obtained by means of PCR cloning after MS sequencing of the protein combined with ragweed pollen RNA sequencing. The purified allergen, termed Amb a 11, was fully characterized by MS and confirmed to react with IgEs from 66% of patients. This molecule is a 262-amino-acid thiol protease of the papain family expressed as a combination of isoforms and glycoforms after proteolytic removal of N- and C-terminal propeptides from a proform. Three-dimensional modeling revealed a high structural homology with known cysteine proteases, including the mite Der p 1 allergen. The protease activity of Amb a 11, as well as its capacity to activate basophils from patients with ragweed allergy, were confirmed. The production of a nonglycosylated recombinant form of Amb a 11 in Escherichia coli established that glycosylation is not required for IgE binding. Conclusion We identified the cysteine protease Amb a 11 as a new major allergen from ragweed pollen. Given the similar physicochemical properties shared by the 2 major allergens, we hypothesize that part of the allergenic activity previously ascribed to Amb a 1 is rather borne by Amb a 11. Root biomass production varied widely for all these macrophytes in control conditions (0.08 μM) according to the sampling site. Root biomass production of T. latifolia and I. pseudacorus in the 2.5–25 μM Cu range depended on the sampling location but not on the Cu dose in the growth medium. For P. australis, J. effusus, S. lacustris, and P. arundinacea, an intra-specific variability of RP depending on both the sampling location and the Cu-dose was evidenced. This intra-specific variability of RP depending on the sampling location and of Cu-tolerance for these last four species suggests that Cu constitutive tolerance for all rooted macrophytes is not a species-wide trait but it exhibits variability for some species. 26. Expression profile of hypothalamic neuropeptides in chicken lines selected for high or low residual feed intake.

Abstract: Abstract: The R+ and R− chicken lines have been divergently selected for high (R+) or low (R−) residual feed intake. For the same body weight and egg production, the R+ chickens consume 40% more food than their counterparts R− lines. In the present study we sought to determine the hypothalamic expression profile of feeding-related neuropeptides in these lines maintained under fed or food-deprived conditions. In the fed condition, the suppressor of cytokine signaling 3 (SOCS3) was 17-fold lower (P <0.05) and the ghrelin receptor was 7-fold higher (P <0.05) in R+ compared to R− chicken lines. The hypothalamic expression of the other studied genes remained unchanged between the two lines. In the fasted state, orexigenic neuropeptide Y and agouti-related peptide were more responsive, with higher significant levels in the R+ compared to R− chickens, while no significant differences were seen for the anorexigenic neuropeptides pro-opiomelanocortin and corticotropin releasing hormone. Interestingly, C-reactive protein, adiponectin receptor 1 and ghrelin receptor gene expression were significantly higher (12-, 2- and 3-folds, respectively), however ghrelin and melanocortin 5 receptor mRNA levels were lower (4- and 2-folds, P =0.05 and P =0.03, respectively) in R+ compared to R− animals. We identified several key feeding-related genes that are differently expressed in the hypothalamus of R+ and R− chickens and that might explain the difference in feed intake observed between the two lines. Abstract: This study investigates the column leaching of a soil contaminated mainly with Cr and Ni by using two chelants: citric acid (biodegradable) and EDTA (non-biodegradable) followed with water rinse. The chelants lead to Cr and Ni leaching, in addition to major elements (Ca, Fe, Mg, Al, Mn and Zn) showing the dissolution of soil mineral constituents. EDTA leaches more major elements and Ni than citric acid related to the respective stability of metal–chelant complexes; citric acid leaches more Cr than EDTA, certainly because of a substitution reaction with Cr(VI). In the case of alternating chelant/water applications, leaching occurs during the chelant applications, but also during water applications. In the case of chelant/water applications followed by continuous water application, both Cr and Ni leach over time. 29. Complications after ERCP in liver transplant recipients.

Abstract: Background: Complications of the biliary tract after liver transplantation are successfully managed with ERCP; however, the incidence and risk factors for post-ERCP complications remain unknown. Objective: To examine the incidence, risk factors, and short-term outcome of post-ERCP complications in liver transplant (LT) recipients. Design: Retrospective evaluation of all ERCPs performed in LT recipients at our institution during a 7-year, 4-month period. Setting: Tertiary referral center. Patients: A total of 243 ERCPs performed in 121 LT recipients with duct-to-duct anastomosis. Main Outcome Measurements: Incidence of post-ERCP complications. Predictive factors were determined by univariate and multivariate analyses. Results: Overall complications occurred in 22 procedures (9%) (13 mild, 9 moderate): pancreatitis in 9 patients (3.7%), cholangitis in 8 patients (3.3%), postsphincterotomy bleeding in 4 patients (1.6%), and subcapsular hematoma in 1 patient (0.4%). The mean hospitalization for post-ERCP complications was 4.8 days (range 2-11 days). Logistic regression identified mammalian target of rapamycin inhibitors (odds ratio [OR], 4.65; 95% CI, 1.01-21.81; P = .049), serum creatinine level greater than 2 mg/dL (OR, 4.17; 95% CI, 1.07-16.26; P = .04), biliary sphincterotomy (OR, 3.03; 95% CI, 1.07-8.53; P = .037), and more than 2 pancreatic duct contrast injections (OR, 2.95; 95% CI, 1.10-7.91; P = .032) as independent risk factors for post-ERCP complications, whereas steroid therapy (OR, 0.23; 95% CI, 0.08-0.63; P = .004) was an independent protective factor. Limitations: Single-center retrospective study. Conclusions: The rate of complications after ERCP in LT recipients seems to be similar to that of non-LT recipients. Complications in this analysis were more common in LT recipients receiving mammalian target of rapamycin inhibitors and those with renal failure, biliary sphincterotomy, and more than 2 pancreatic duct injections, whereas they were less common in those patients on steroid therapy. Aim: To prospectively investigate the usefulness and safety of EUS-guided cytology brushing (EUS BR) in the cellular diagnosis of pancreatic cysts. Methods: Cysts >15mm were sampled with a 19G needle. The fluid was aspirated and processed for cytology. The brush was introduced to scrub the cystic wall and processed as standard brushings. Antibiotic prophylaxis was administered. Complications were assessed in the first 24h and 7 days after the procedure. Results: 30 patients were included. In 8 patients the technique failed for technical reasons. EUS BR provided with a cellular diagnosis in 20/22 cases (91%). The EUS BR was superior to the aspirated fluid for detecting diagnostic cells (73% vs. 36%, p =0.08) and mucinous cells (50% vs. 18%, p =0.016). In the 8 patients operated on, the specimen was consistent with EUS BR diagnosis. Three patients (10%) had complications, one of them a subacute retroperitoneal haemorrhage in a patient on anticoagulation therapy who died for complications 1 month later. Conclusions: EUS BR increases cellular diagnosis of pancreatic cystic lesions as compared with fluid analysis, mainly in mucinous lesions. Abstract: A Mn-rich slag sample coming from a pyrometallurgical plant recycling alkaline batteries was submitted to environmental conditions during one year. After crushing, the slag was buried in topsoil and the leachate was periodically collected in order to monitor the leached amounts of metallic elements (ME). Results evidenced a low release of the slag constituents (Mn: < 0.01%). The SEM/EDS investigations did not show alteration features supposing a weak dissolution of the primary phases (tephroite, bustamite and leucite). The modification of the pH induced by the slag enhanced the mobility of soil components and of the organic matter, particularly in the vicinity of the soil/slag contact zone. This suggested that the slag indirectly influenced the ME mobility through the organic matter and colloidal transports. 33. The beneficial effects of argon plasma coagulation in the management of different types of gastric vascular ectasia lesions in patients admitted for GI hemorrhage.

Abstract: Background: Despite different forms of treatment, few studies have been performed on the outcome and prognosis of patients admitted to the hospital because of gastric vascular ectasia (GVE) and upper-GI bleeding (UGIB). There is also little knowledge on the efficacy of argon plasma coagulation (APC) in different subgroups of GVE lesions. Objective: This study was designed to evaluate the efficacy of APC in patients admitted to the hospital with UGIB because of GVE. Design: Prospective evaluation of consecutive cases of UGIB because of GVE. Setting: Tertiary and university-affiliated hospital. Patients and Interventions: Twenty-nine patients were included and divided into 3 subgroups: focal vascular ectasia lesions (FVE) (n = 10), portal hypertensive gastropathy (PHG) (n = 11), and gastric antral vascular ectasia (GAVE) (n = 8). Patients were followed at 3 months and every 6 months thereafter during a mean of 23.1 months (range 18-37 months). All patients received intensive APC treatment that was repeated, depending on the endoscopic appearance or clinical evaluation. Results: The overall success of APC treatment was 86%, with only one recurrence of UGIB during the follow-up period. The number of APC sessions was 1.2, 2.2, and 2.3, in each subgroup (not significant), with a total number of sessions of 1.9 ± 1.3. Treatment success was 90% in the FVE group, 81% in the PHG group, and 87.5% in the GAVE group (NS). The rise in hematocrit from baseline values in the overall group and in each subgroup was significant (P > .01). Limitations: A single-center study and small sample. Conclusions: Endoscopic thermal ablation with APC is effective in managing UGIB and in reducing transfusion requirements in patients admitted for GI hemorrhage because of different endoscopic types of GVE. Abstract: Abstract: EDTA and citric acid were tested to solubilize metals and enhance their uptake by Datura innoxia, chosen because of its ability to accumulate and tolerate metals. Two application modes were used on an industrial soil contaminated mainly by Cr and Ni. The results showed that citric acid was the most effective at increasing the uptake of Cr and EDTA for Ni. These results are consistent with the effectiveness of both chelants in solubilizing metals from the soil. The translocation factor (TF) of Ni was 1.6- and 6.7-fold higher than the control, respectively, for one and two applications of 1mmolkg−1 EDTA. After two applications of 5 and 10mmolkg−1 citric acid, the TF of Cr increased 2- and 3.5-fold relative to the control. Whatever the concentration, the application of EDTA modified the plant physiology significantly. For citric acid this was only observed with the highest dose (10mmolkg−1). [Copyright &y& Elsevier]

Abstract: Background: Early upper GI hemorrhage (UGH) is a potential complication after laparoscopic Roux-en-Y gastric bypass (RYGBP), and early reoperative intervention is the most accepted treatment. Experience with endoscopic treatment is limited. Objective: Our purpose was to describe the role of endoscopy and injection therapy in the management of early UGH after laparoscopic RYGBP. Design: Case series study. Setting: Endoscopy Unit, Hospital Clínic, University of Barcelona, Barcelona, Spain. Patients: We describe the endoscopic treatment of 6 patients with early UGH within 24 hours after a RYGBP. Instrumentation: Upper endoscopy was performed in all 6 cases. The origin of the bleeding was identified at the staple line in all cases, and epinephrine alone or combined with polidocanol was successfully injected in 5 of 6 patients. Results: Endoscopic therapy arrested active bleeding without any complications in all cases without the need for further surgery or endoscopic treatments. Limitation: Our experience is limited to 6 cases. Conclusion: Early postoperative UGH after RYGBP may be adequately controlled with endoscopic treatment and may obviate the need for surgery. Abstract: Abstract: The mobilization of chromium and nickel from an industrial soil was investigated using two biodegradable chelants (citric acid and histidine), compared with a persistent one (ethylenediaminetetraacetic acid). Successive metal mobilizations were carried out in batch experiments. The main reactions involved were estimated by modeling the system with MINEQL+. For a single mobilization, citric acid was the most effective for Cr mobilization and EDTA for Ni. Their effectiveness could be explained by their ability to solubilize the mineral matrix and by the competition for the surfaces sites to desorb Cr(VI). Before and after the mobilizations, the distribution of metals was determined by a sequential extraction procedure. Only slight modifications were observed due to the low percentage of solubilized metal. A concentration of 0.05molL−1 (citric acid and EDTA) allows a good compromise between metal mobilization and preservation of the soil mineral integrity. [Copyright &y& Elsevier]

Abstract: Abstract: Laboratory experiments were conducted to investigate the potential use of the aquatic lichen Dermatocarpon luridum as bioindicator of copper pollution. Lichen thalli were exposed to 0.00, 0.25, 0.50, 0.75 and 1.00mM copper in synthetic freshwater to solve the problems of metal bioavailability. The mineral composition of this media was prepared so that it corresponded to the ion composition of natural waters in D. luridum ecosystems. Sequential elution procedures using NiCl2 or Na2–EDTA (20mM) were used to determine the distribution of metals at different cellular sites. The copper concentration extracted from thalli was correlated with pollution intensity, the greater correlation being with the Na2–EDTA extractant. The malondialdehyde concentration in thalli can be used as indicator of copper pollution; however, similar membrane degradation was observed for 0.25 and 0.50mM copper and for 0.75 and 1.00mM copper. Abstract: Abstract: The efficacy of a new rhamnolipid biosurfactants mixture to enhance the removal of pyrene from a soil artificially contaminated was investigated. The molar solubilization ratio (MSR) and the partition coefficient between the micelles and water (log K m) were found to be 7.5×10−3 and 5.7, respectively. From soil column studies, the pyrene removal increased linearly with the concentration of the injected biosurfactants solution above the effective critical micellar concentration (0.4gL−1). Flushing with a 5.0gL−1 biosurfactants solution increased the pyrene concentration in the effluent by 178 times. At high biosurfactants'' concentrations (2.5 and 5.0gL−1), the cumulative pyrene recovery reached 70%. This pyrene remobilization takes place independently of the soil organic carbon solubilization. Abstract: The aim of this work is to characterize the mechanical parameters governing the in-plane behavior of human skin and, in particular, of a keloid-scar. We consider 2D hyperelastic bi-material model of a keloid and the surrounding healthy skin. The problem of finding the optimal model parameters that minimize the misfit between the model observations and the in vivo experimental measurements is solved using our in-house developed inverse solver that is based on the FEniCS finite element computational platform. The paper focuses on the model parameter sensitivity quantification with respect to the experimental measurements, such as the displacement field and reaction force measurements. The developed tools quantify the significance of different measurements on different model parameters and, in turn, give insight into a given model's ability to capture experimental measurements. Finally, an a priori estimate for the model parameter sensitivity is proposed that is independent of the actual measurements and that is defined in the whole computational domain. Abstract: Introduction In our previous work we demonstrated the importance of altered extracellular matrix (ECM) in disease processes and how treatment with 670 nm LLLT can restore cell behavior. Objective Of this study focuses on the signaling properties of altered ECM in preeclampsia, its contribution to preeclampsia, and the possibility of a novel therapeutic modality restoring the trophoblast phenotype with LLLT. Methods To study signaling pathways we decellularized placentas from healthy and preeclampsic subjects with 1%SDS followed by triton X and thorough wash steps. First trimester human trophoblasts (HTG8) were cultured on these two different matrices. This allows us to study trophoblast behavior in their proper microenvironment and follow the trophoblast phenotype changes. Immunohistochemistry and western blot analysis were performed with a focus on PLGF, ERRg, FoxP3, and HLA-G expression comparing the different matrices. Readouts are fluorescence intensity and western blot density in arbitrary units (AU). Furthermore, 670 nm light treatment was applied to HTG8s at an energy level of 4 J/ cm[b] once and cellular changes were studied 24hours later. Results Our results show that the expression of PLGF in HTG8s was decreased from 102AU to 68 AU comparing culture on control matrix to preeclampsic matrix. ERRg expression was significantly reduced in HTG8s from 80.33AU to 18.6AU. FoxP3 expression was decreased from109.38AU to 82.1AU. HLA-G expression was significantly decreased from 105.94 to 49.48AU. LLLT restored the phenotype of HTG8s cultured on preeclampsic matrix to the phenotype of HTG8s cultured on control placenta. Statistics were performed using the Student's t-test. Discussion Our data demonstrate for the first time that the ECM of the placenta and therefore the microenvironment has great impact on trophoblast signaling. Preeclampsic ECM alters the trophoblast phenotype and LLLT restored the phenotype to the phenotype found in trophoblasts cultured on control ECM. [ABSTRACT FROM AUTHOR]

Abstract: Introduction Le pollen d’ambroisie ( Ambrosia artemisiifolia ) est une cause majeure d’allergies respiratoires sévères en Amérique du Nord et en Europe. Récemment, nous avons identifié Amb a 11, un nouvel allergène majeur du pollen d’ambroisie de la famille des protéases à cystéines. L’objectif de la présente étude est la production et la caractérisation d’Amb a 11 mature produit sous une forme recombinante. Méthodes La protéine Amb a 11 recombinante est obtenue par renaturation in vitro du zymogène à partir de corps d’inclusions d’ Escherichia coli , puis maturation à pH acide. Des expériences de mutagenèse ont permis d’étudier le processus de maturation. Une caractérisation de la protéine mature a été réalisée par spectrométrie de masse et dichroïsme circulaire et l’activité protéasique a été mesurée par un test fluorimétrique. La réactivité IgE a été analysée avec des patients allergiques à l’ambroisie. Résultats Le zymogène Amb a 11 renaturé peut-être maturé à pH acide par un mécanisme autocatalytique. L’analyse par spectrométrie de masse confirme l’élimination des deux propeptides N- et C-terminaux au cours de ce processus. Le spectre de dichroïsme circulaire indique l’acquisition d’une structure secondaire similaire à celle d’Amb a 11 naturel. De même que l’allergène naturel, Amb a 11 recombinant possède une forte activité cystéine protéase et est reconnu par les IgE sériques de 60 % des patients allergiques au pollen d’ambroisie. Enfin, aucune réactivité croisée n’est observée avec la protéase à cystéine allergénique Der p 1 de Dermatophagoides pteronyssinus . Conclusion Nous avons produit la protéine Amb a 11 recombinante, nouvel allergène majeur du pollen d’ambroisie, qui possède des caractéristiques structurales et fonctionnelles similaires à la molécule naturelle. 47. Orofacial granulomatosis: clinical study of 20 patients.

Abstract: Objectives: The objective of this study was to analyze the clinical features of a series of patients with orofacial granulomatosis (OFG). Study Design: Twenty patients diagnosed with OFG at Bellvitge Hospital (Barcelona, Spain) from 1985 to 2010 were included in the study. Results: All of our patients (9 men and 11 women, median age 48.1 years) presented with labial swelling. Six patients presented with recurrent orofacial swelling, 12 with permanent swelling, and 2 with progressive swelling. Fissured tongue was observed in 9 cases, and 2 patients presented with recurrent episodes of peripheral facial paralysis. The median follow-up time was 65.1 months, ranging from 4 to 300 months. None of our patients developed sarcoidosis or Crohn disease. Conclusions: In the south of Europe, OFG does not appear to be as frequently associated with Crohn disease as in northern Europe. Although several treatments may achieve transient control of the orofacial swelling, there is no curative treatment for OFG and some patients may develop embarrassing lesions. [ABSTRACT FROM AUTHOR]
