10. The impact of the COVID-19 pandemic on migrant women in Milan, Italy: A qualitative study involving key informants working in third sector organizations and public hospitals.

Abstract

Migrants and women were among the groups most adversely affected by the COVID-19 pandemic global disaster. Through an intersectional lens that considers both migrant and female identities, it can be inferred that migrant women were particularly vulnerable to the pandemic. This study aims to investigate the impact of the pandemic on migrant women living in Milan (North-West Italy), by leveraging on the accounts of a pool of key informants (KIs). We performed an in-depth retrospective qualitative study using semi-structured interviews to explore the perspectives of KIs working in third sector organizations and public hospitals. After conducting an extensive mapping of organizations with experience working with migrants providing health, social, and/or administrative services during the pandemic, 28 KIs working in seven organizations were recruited. Interviews were transcribed and inductively analyzed. Overall, the COVID-19 pandemic exacerbated pre-existing vulnerability factors affecting migrant women by triggering several cascading effects. The shutdown of services further marginalized them and hindered their regularization and empowerment path. Migrant women's financial situation was significantly impacted, as in most instances they had informal and gendered jobs. Caregivers and sex workers were the sub-groups most adversely affected by the pandemic. Episodes of gender-based violence appeared to rise, while requests for assistance declined. Accessing the vaccination campaign posed challenges for many migrant women, who at times tended to be skeptical about it. Third sector organizations, despite being confronted with several challenges, proved to be resilient during the pandemic and managed to find alternative methods to support migrant women, often filling institutional gaps. • Caregivers and sex workers were among the hardest hit during the pandemic in Milan. • Many migrant women faced issues accessing vaccination or were skeptical about it. • Gender-based violence rose while support requests declined during COVID-19. • The closure of administrative, health and social services marginalized migrant women. • Third sector organizations were resilient during COVID-19, filling institutional gaps. 13. Medical students' education in disaster medicine: A systematic literature review of existing curricula.

Abstract

Disaster Medicine (DM) is currently underrepresented in medical schools' curricula worldwide, and existing DM courses for medical students are extremely heterogeneous due to the lack of pragmatic and standardized guidelines. Moreover, there is a gap in knowledge regarding the curriculum development methodology used for DM courses. This study aims to identify DM courses for medical students worldwide and to map their curriculum development methodologies by reviewing available literature. The search was conducted on three databases using terms "Disaster medicine" AND "Education". Following the PRISMA approach, twenty-five articles that described the content and implementation of DM curricula were included in the analysis. Nine studies thoroughly described the curriculum development process. Expert opinion and literature review were the methodologies mostly used to develop DM curricula. Only four studies followed a multi-method process made up of four different methodologies, including expert opinion, literature review, survey, and Delphi methodology. Most of the courses adopted a face-to-face approach combining different training modalities, including the use of virtual reality simulations and drills. Overall, this systematic review highlights the need for evidence-based educational curricula in DM and provides recommendations for developing DM courses following a scientific approach. Abstract

Abstract: Steady-state and turn-off switching characteristics of aluminium-implanted 4H-SiC p–i–n diodes designed for high-current density operation, are investigated experimentally and by mean of numerical simulations in the 298–523K temperature range. The diodes present circular structure with a diameter of 350μm and employ an anode region with an aluminium depth profile peaking at 6×1019 cm−3 at the surface. The profile edge and the junction depth are located at 0.2 and 1.35μm, respectively. At room temperature the measured forward current density is close to 370A/cm2 at 5V with an ideality factor always less than 2 before high-current injection or device-series resistance became dominant. The transient analysis reveals a strong potential of this diodes for use in high-speed, high-power applications, especially at high temperature, with a very low turn-off recovery time (<80ns) in the whole range of test conditions. The simulated results match the experimental data, showing that the switching performance is mainly due to the poor minority charge carrier lifetime estimated to be 15ns for these implanted devices. Abstract: In this work the forward J–V characteristics of 4H–SiC p–i–n diodes are analysed by means of a physics based device simulator tuned by comparison to experimental results. The circular devices have a diameter of 350μm. The implanted anode region showed a plateau aluminium concentration of 6×1019 cm−3 located at the surface with a profile edge located at 0.2μm and a profile tail crossing the n-type epilayer doping at 1.35μm. Al atom ionization efficiency was carefully taken into account during the simulations. The final devices showed good rectifying properties and at room temperature a diode current density close to 370A/cm2 could be measured at 5V. The simulation results were in good agreement with the experimental data taken at temperatures up to about 523K in the whole explored current range extending over nine orders of magnitude. Simulations also allowed to estimate the effect of a different p+ doping electrically effective profile on the device current handling capabilities. [Copyright &y& Elsevier]

Abstract: Development of memories based on organic soft materials is a current industrial niche aimed at the upper contacts flash memories. The electrical bistability of such materials makes them ideal candidates for cost-effective, fast-programming switching devices. Hybrid thin films of SiO2 with Rose Bengal (bis-triethylammonium) dopant are here reported for the first time, together with their characterisations. Technology-wise the main advantage of the films is the possibility to cast photolithographically, which is also cost-effective. Abstract: The infrared (IR) absorption dependence on visible light illumination has been measured in doped and undoped hydrogenated amorphous silicon carbide (a-Si1−x C x :H) films grown by plasma enhanced chemical vapour deposition. The measurements were made by a highly sensitive technique which exploits properly designed a-Si1−x C x :H/ZnO test waveguides for lengthening the interaction region between the IR and visible (VIS) radiations in the material. Experimental data show that boron doping strongly enhances the VIS light induced variation of the IR absorption, whereas the increase in carbon content has a quenching effect on the phenomenon. The a-SiC:H films have been also characterized by photoluminescence measurements. The spectra are dominated by a photoluminescence band, ranging between 1.4eV and 1.9eV. This band is enhanced by the increase in carbon content, while is strongly quenched with increasing B-doping level. On the basis of these results, a correlation is found between the measurements of optical absorption, photo-induced absorption and photoluminescence. The type and the density of defects induced in the films by the different growth conditions have been recognized as the origin of the different behaviors observed. [Copyright &y& Elsevier]

A proportional to absolute temperature sensor (PTAT) based on V 2 O 5 /4H-SiC (vanadium pentoxide/4H polytype of silicon carbide) Schottky diodes is presented. The linear dependence on temperature of the voltage difference appearing at the terminals of two constant-current forward-biased diodes has been used for thermal sensing in the wide temperature range from T = 147 K to 400 K which extends down the state-of-the art of more than 80 K. The proposed sensor shows a sensitivity of 307 μV/K, a good reproducibility and a stable linear output also in case of deviation of the two bias currents from the best operating condition. A SPICE model of a complete photovoltaic (PV) system, including a detailed model of photovoltaic cells, a modified cascaded multilevel inverter, and energy storage elements, is presented. The simulation of the system as a whole allows evaluating readily the effects on its performances of the variation of the component parameters, as well as of the external load, the solar irradiation, and the cell temperature. The global conversion efficiency and the total harmonic distortion (THD) of the output waveform are in particular analysed. The role and sizing criteria of a storage element, such as a capacitor, placed at the PV module output are also analysed for various operating conditions. Abstract: Electrically induced phase modulation is characterized for the first time in a waveguide-integrated Fabry–Perot resonating cavity based both on an index- and conductivity- high-contrast amorphous silicon/amorphous silicon carbide (a-Si:H/a-SiC:H) multistack. The device consists of a single mode a-Si:H rib waveguide containing three insulating thin layers of a-SiC:H embedded within the core thickness. The effective refractive index change, Δn eff , at the wavelength of λ=1.55μm is achieved through the application of an electric field across the stack which induces carrier accumulation at all the a-Si:H/a-SiC:H interfaces, resulting in turn in a high interaction between the optical beam and the accumulation layers. This configuration allows to obtain a V π ×L π product of about 5.9Vcm, not far from what observed in high performing electro-optical modulators in crystalline silicon. [Copyright &y& Elsevier]

Abstract: Background: Functional exercises represent an important link between disaster planning and disaster response. Although these exercises are widely performed, no standardized method exists for their evaluation. Study Objectives: To describe a simple and objective method to assess medical performance during functional exercise events. Methods: An evaluation tool comprising three data fields (triage, clinical maneuvers, and radio usage), accompanied by direct anecdotal observational methods, was used to evaluate a large functional mass casualty incident exercise. Results: Seventeen medical responders managed 112 victims of a simulated building explosion. Although 81% of the patients were assigned the appropriate triage codes, evacuation from the site did not follow in priority. Required maneuvers were performed correctly in 85.2% of airway maneuvers and 78.7% of breathing maneuvers, however, significant under-treatment occurred, possibly due to equipment shortages. Extensive use of radio communication was documented. In evaluating this tool, the structured markers were informative, but further information provided by direct observation was invaluable. Conclusion: A three-part tool (triage, medical maneuvers, and radio usage) can provide a method to evaluate functional mass casualty incident exercises, and is easily implemented. For the best results, it should be used in conjunction with direct observation.
