An analysis of physician services provided to HMO patients using the data from the 1993-1996 National Ambulatory Medical Care Survey is offered. Additional information on models analyzing physician services in a managed care environment is included.

Objectives: To determine cost-effectiveness of three community-based acute HIV infection (AHI) testing algorithms compared to HIV antibody testing alone by focusing on the potential of averting new infections occurring within a one-year time horizon among men who have sex with men (MSM).Methods: Data sources for model parameters included actual cost and prevalence data derived from a community-based AHI screening program in San Diego, and published studies. Main outcome measure was costs per infection averted (IA). The lower end of the cost range of discounted lifetime costs of an HIV infection (i.e. $236,948) was used for defining cost-effectiveness.Results: The most sensitive algorithm for AHI detection, which was based on HIV nucleic acid amplification testing, was estimated to prevent between 5 and 45 transmissions, with simulated costs per infection averted between $965 and $141,256 when compared to HIV antibody testing alone.Conclusion: AHI testing was cost-effective in preventing new HIV infections among at risk MSM in San Diego, and also among other MSM populations with similar HIV prevalence but lower proportions of AHI diagnoses. Vessel buyback programs intended to address overcapacity and excess capitalization in fisheries can lead to dramatically different levels of decapitalization depending on program structure and availability of vessel-specific information. This paper develops a simple theoretical model of self-financing vessel buybacks to examine equilibrium outcomes using sequential versus take-it-or-leave-it auctions, and financing schemes which allocate costs either homogeneously or according to rents captured through the buyback. This model demonstrates that programs which spread costs evenly among remaining vessels limit the possible extent of buybacks, as do programs which buy vessels one at a time in sequence rather than all at once. In addition to the theoretical modeling, a stylized case study inspired by the Inter-American Tropical Tuna Fishery is also provided. This analysis suggests that a wide range of auction structures could roughly half the size of the existing fleet, and starkly demonstrates how information poor settings can entirely derail a buyback. As of the second quarter of 2016 more than 1.1 million solar photovoltaic (PV) homes exist in the US. Capturing the value these PV systems add to home sales is therefore important. Our study enhances the PV-home-valuation literature by analyzing 22,822 home sales, of which 3951 have PV, and which span eight states during 2002–2013. We also, for the first time, compare premiums with contributory value estimates derived from the present value of saved energy costs (income approach) and, separately, the replacement cost of systems at the time of sale (cost approach) to examine market signals. We find home buyers are consistently willing to pay PV home premiums across various states, housing and PV markets, and home types; average premiums equate to approximately $4/W or $15,000 for an average-sized 3.6-kW PV system. We find that a replacement cost net of state and federal incentives is a better proxy for premiums than gross installed costs, and that the income approach is a good signal if it accounts for tiered volumetric retail rates.
