Patients with inflammatory bowel diseases (IBD) are exposed to drug-related nephrotoxicity and kidney-related extra-intestinal manifestations (EIMs). Patients should be monitored but guidance is lacking in current international recommendations. The objective of the Kidney Function Monitoring in Inflammatory Bowel Disease (MONITORED) initiative was to achieve an expert consensus about monitoring kidney function in IBD. A literature review was first conducted. Then, an expert consensus meeting, involving 28 attendees representing French-speaking gastroenterologists and nephrologists, was held as part of an academic initiative on May 28, 2021. An anonymous Delphi process was used to discuss and vote on statements. Agreement was defined as at least 75% of participants voting for any one statement. Experts reached consensus on 11 criteria for referral to the nephrologist. Concerning kidney function monitoring, participants unanimously validated the use of serum creatinine with estimation of the glomerular filtration rate via the MDRD or CKD-EPI equations. A blood ionogram and a urine sample with measurement of a protein-to-creatinine ratio were also broadly agreed validated. Experts recommended performing this monitoring at IBD diagnosis, prior introducing a new treatment, and annually for EIMs screening and evaluation of treatment tolerance. An evaluation 3 months after starting mesalamine and then every 6 months was felt necessary, while for biologics an annually monitoring was deemed sufficient. The MONITORED consensus proposed guidelines on how to monitor kidney function in IBD. Chronic abdominal pain occurs frequently in pediatric patients with inflammatory bowel disease (IBD) in remission. To assess the prevalence and factors associated with Functional Abdominal Pain Disorders among IBD children in remission (IBD-FAPD). Patients with IBD for > 1 year, in clinical remission for ≥ 3 months were recruited from a National IBD network. IBD-FAPDs were assessed using the Rome III questionnaire criteria. Patient- or parent- reported outcomes were assessed. Among 102 included patients, 57 (56%) were boys, mean age (DS) was 15.0 (± 2.0) years and 75 (74%) had Crohn's disease. Twenty-two patients (22%) had at least one Functional Gastrointestinal Disorder among which 17 had at least one IBD-FAPD. Past severity of disease or treatments received and level of remission were not significantly associated with IBD-FAPD. Patients with IBD-FAPD reported more fatigue (peds-FACIT-F: 35.9 ± 9.8 vs. 43.0 ± 6.9, p = 0.01) and a lower HR-QoL (IMPACT III: 76.5 ± 9.6 vs. 81.6 ± 9.2, p = 0.04) than patients without FAPD, and their parents had higher levels of State and Trait anxiety than the other parents. This paper focuses on the horse pinworm, Oxyuris equi , in archaeology during the Holocene period, and presents an overview of past published occurrences, early mentions in texts, and new data from our paleoparasitology research. This original compilation shows that the most ancient record of the horse pinworm dates to ca. 2500 years before present (ybp) in Central Asia and to ca. 2020 ybp in Western Europe. It also shows that the parasite is not detected on the American continent until contemporary periods. The role of European migrations from 1492 (Christopher Columbus) is discussed to explain the transfer of the horse pinworm from the Old World to the Americas. The absence of any record of this parasite before ca. 2500 ybp in Eurasia could be explained by parasite ecology, unfavorable sampling and scarcity of horse archeological remains. Because parasite data reveal essential information about the behavior and history of their hosts, it is possible to use them as tracers of host evolution. A table built from the analysis of the data contained in the book by Ashford and Crewe "The Parasites of Homo sapiens" allows counting and cross comparing the parasites according to the main descriptors used by the authors: Taxonomic groups , for each group number of parasites species identified in humans; Status , numbers of reported human cases and their dispersion; Geographic distribution , parasite specific richness recorded in biogeographic regions; Habitat , parasite location in or on the human body; Transmission , contamination pathways to man; Hosts , non-human hosts, which have a role in the maintenance of a parasite; Host-specificity status , relative role of man or other hosts in the maintenance of parasite populations. A strong positive correlation is observed between the number of parasites species recorded in humans and the global parasite species richness for each taxonomic group. About 74% of the parasites recorded in humans are rare, sporadic or nowhere common; 10% only are common or abundant worldwide. The Palearctic exhibits the highest parasite species diversity; the Oriental, Nearctic, Neotropical and Aethiopian regions have roughly similar richness values; the Australian Region is the poorest. Earliest domesticated animals, such as dog, cat, cattle or pig, share more parasite species with Humans than tardily domesticated as horse, rabbit or camel. More than one third of our parasites have elected our alimentary canal as a home and about two third are using the digestive tract path for contamination. Abstract: A new Oxyurid genus and species are described in a rodent recently discovered in Lao PDR: Laonastes aenigmamus which happens to be the single survivor of the Diatomyidae, a family considered to be extinct since the Miocene. The morphological characters of the new parasite species allow classifying it within the Syphaciinae Railliet, 1916, a subfamily whose members are exclusively parasites of Lagomorpha and Rodents. Male Syphaciinae have developed several types of ventral cuticular ornamentation used to firmly grip the female during mating. The ornamental characters observed in the new species include a finger like appendix, which, until now, has not been described in the subfamily. The originality of this apparatus justifies the creation of a new genus and a new species for the pinworm parasite of Laonastes. Using morphological characters, the new species is analyzed phylogenetically to describe its affinities with representatives of the main groups distinguished within the Syphaciinae. The phylogenetic study produces a cladogram similar to the phylogeny recently proposed for the hosts of the subfamily and in agreement with a close association of the Diatomyidae with the Ctenodactylidae. Such a phenomenon of cophylogeny is interpreted as the result of the existence of a strict specificity between the Syphaciinae and their respective hosts, due to the very close adaptation of their life cycle with the behaviors of their hosts. In Lagomorpha and Rodents, caecotrophy and grooming activities allow a direct transmission of the parasite eggs and favor successive self-infestations, increasing the chances for the parasite to maintain itself in the same host species but decreasing the probability of host switching. The resulting high host specificity allowed the Syphaciinae to out-compete other pinworms and maintain themselves in their specific host over millions of years. [Copyright &y& Elsevier]

Background & Aims: Celiac disease may be associated with autoimmune diseases. The aims of the present study were to determine in celiac patients which factors modulate the risk of autoimmune disease and to evaluate the effect of the gluten-free diet. Methods: The occurrence of autoimmune disease and compliance to gluten-free diet were specified retrospectively in 924 celiac patients recruited from 27 French pediatric and adult gastroenterology centers. Results: One or several autoimmune diseases had developed in 178 patients. The cumulative risk of autoimmune disease was 8.1% ± 1% at age 15, and 15.7% ± 1.5% at age 30. Factors associated with an increased risk were family history of autoimmunity (hazard ratio, 2.36; 95% confidence interval, 1.71–3.31) and diagnosis of celiac disease before 36 years of age (hazard ratio, 2.65; 95% confidence interval, 1.79–3.85). After diagnosis of celiac disease, 55 of 788 patients developed an autoimmune disease. The cumulative risk of subsequent autoimmune disease was lower in patients compliant to a gluten-free diet versus noncompliant patients (at 10 years, 6% ± 2% vs 15.6% ± 5.9%, respectively; P = .02). The incidence of autoimmune diseases was 5.4 per 1000 patient-years during adherence to a gluten-free diet versus 11.3 per 1000 patient-years during nonadherence to the diet (P = .002). Results were similar in both the pediatric and the adult populations. Conclusions: Celiac patients most at risk for autoimmune disease are those diagnosed early in life and having a family history of autoimmunity. The gluten-free diet has a protective effect. Abstract: The Enterobiinae includes 47 species of pinworms parasite of primates. A previous cladistic analysis of this subfamily supported its monophyly and its subdivision into three genera. Based on morphological characters, this cladistic analysis excluded characters describing the shape of the genital papillae of male pinworms, because the corresponding patterns could not be described using discrete characters. In this study, the shape of the genital papillae of the males of 35 within the 47 species is analyzed using geometric morphometric approaches. The aims of this study are to investigate: (i) the relationships between the phylogeny and the shape patterns of the caudal bursa, (ii) the shape differences between and within monophyletic groups, and (iii) the functional implications of the shape patterns observed within the subfamily. Results demonstrate that different patterns of evolution of the caudal bursa, each one characterized by a particular spatial distribution of the phasmids and genital papillae may be recognized, which are consistent with the classification of the Enterobiinae into three groups. On the whole, these patterns may be related to particular mating behavior of the pinworms. When incongruence is observed between shape patterns distribution and species distribution into monophyletic groups, they are found to correspond to homoplasic events. This suggests that convergent selective pressures are involved in the evolution of the shape of the genital papillae. This analysis also confirms that morphometric shape patterns cannot be interpreted unequivocally without the support of a pre-existing phylogenetic framework. [Copyright &y& Elsevier]

Nod2 (Card15) belongs to the family of the recently described Nod molecules, which also includes the closely related protein Nod1 (Card4). Nod proteins have been initially described as intracellular activators of the caspase and NF-κB signaling pathways. Recent progress has enabled research to demonstrate genetically that NOD2 (CARD15) is involved in the predisposition to Crohn''s disease and Blau syndrome. In addition, biochemical evidence has unraveled the role of Nod1 (Card4) and Nod2 (Card15) as intracellular sensors of bacterial peptidoglycan. Together, studies on Nod2 (Card15) provide a conceptual link between inflammatory disorders, such as Crohn''s disease and Blau syndrome, and bacterial sensing. [Copyright &y& Elsevier]

Autophagy-related 16 like-1 (ATG16L-1), immunity-related GTPase-M (IRGM), and nucleotide-binding oligomerization domain-containing 2 (NOD2) regulate autophagy, and variants in these genes have been associated with predisposition to Crohn''s disease (CD). However, little is known about the role of autophagy in CD. Intestinal biopsies from untreated pediatric patients with CD, celiac disease, or ulcerative colitis were analyzed by immunohistochemistry and electron microscopy. We observed that autophagy was specifically activated in Paneth cells from patients with CD, independently of mucosal inflammation or disease-associated variants of ATG16L1 or IRGM. In these cells, activation of autophagy was associated with a significant decrease in number of secretory granules and features of crinophagy. These observations might account for the disorganization of secretory granules previously reported in Paneth cells from patients with CD. [Copyright &y& Elsevier]

Background & Aims: Patients with inflammatory bowel disease (IBD) who have been exposed to thiopurines might have an increased risk of skin cancer. We assessed this risk among patients in France. Methods: We performed a prospective observational cohort study of 19,486 patients with IBD, enrolled from May 2004 to June 2005, who were followed up until December 31, 2007. The incidence of nonmelanoma skin cancer (NMSC) in the general population, used for reference, was determined from the French Network of Cancer Registries. Results: Before the age of 50 years, the crude incidence rates of NMSC among patients currently receiving or who previously received thiopurines were 0.66/1000 and 0.38/1000 patient-years, respectively; these values were 2.59/1000 and 1.96/1000 patient-years for the age group of 50 to 65 years and 4.04/1000 and 5.70/1000 patient-years for patients older than 65 years. Among patients who had never received thiopurines, the incidence of NMSC was zero before the age of 50 years, 0.60/1000 for the ages of 50 to 65 years, and 0.84/1000 for those older than 65 years. A multivariate Cox regression model stratified by propensity score quintiles showed that ongoing thiopurine treatment (hazard ratio [HR], 5.9; 95% confidence interval [CI], 2.1–16.4; P = .0006) and past thiopurine exposure (HR, 3.9; 95% CI, 1.3–12.1; P = .02) were risk factors for NMSC. They also identified age per 1-year increase as a risk factor for NMSC (HR, 1.08; 95% CI, 1.05–1.11; P < .0001). Conclusions: Ongoing and past exposure to thiopurines significantly increases the risk of NMSC in patients with IBD, even before the age of 50 years. In this study, we characterize the diversity and estimated infection levels of gastrointestinal parasites circulating in two galago species, Galago demidoff and G. thomasi in two sites situated in the Southeastern forests of Gabon. Our study reveals that eleven parasites including nine helminthes ( Ascaris spp., Ankylostoma spp., Dicrocoelium spp., Gongylonema spp., Oesophagostomum spp., Lemuricola spp. , Strongyloides spp. Trichostrongylus spp. and Trichuris spp.) and two protozoans ( Balantidium spp. and Entamoeba spp.) may infect Galago spp. with high infection rates. The results show that: a very similar parasite spectrum is found in both host species; all the taxa identified were previously observed in other Primate species and/or Man. They also show that age, gender and forest type may influence infection rates and/or parasite diversity found in a particular host and/or geographic area. [ABSTRACT FROM AUTHOR]

Abstract: Mycobacterium avium subsp. paratuberculosis (Map) causes a chronic enteric disease in ruminants, called paratuberculosis or Johne’s disease. The current model proposes that after ingestion by the host, Map crosses the intestinal barrier via internalization by the M cells. Experimental observations suggest, however, that Map may also transcytose the intestinal wall via the enterocytes, but the mechanisms involved in this process remain poorly understood. Cytoadherence assays performed on epithelial cells with Map revealed that the addition of laminin to the cell culture increases adhesion. A Map protein was isolated by heparin–Sepharose chromatography and identified as a laminin-binding protein like. The gene encoding this protein named Lbp/Hlp was identified in the Map genome sequence at locus MAP3024 (annotated Hup B). The deduced Map Lbp/Hlp amino acid sequence reveals 80% identity with that reported for other mycobacteria. The C-terminal domain involved in adhesion is mainly composed of arginine and lysine residues modified by methylation. In vitro tests demonstrated that recombinant Lbp/Hlp binds laminin, heparin, collagen and epithelial cells. Interestingly, we found that this adhesin corresponds to the antigen described as the target of pANCA and serum antibodies of patients with Crohn’s disease. [Copyright &y& Elsevier]

Abstract: Remote sensing, referring to the remote study of objects, was originally developed for Earth observation, through the use of sensors on board planes or satellites. Improvements in the use and accessibility of multi-temporal satellite-derived environmental data have, for 30 years, contributed to a growing use in epidemiology. Despite the potential of remote-sensed images and processing techniques for a better knowledge of disease dynamics, an exhaustive analysis of the bibliography shows a generalized use of pre-processed spatial data and low-cost images, resulting in a limited adaptability when addressing biological questions. NOD2 plays an important role in the innate immunity of the intestinal tract. By sensing the muramyl dipeptide (MDP), a bacterial wall component, NOD2 triggers the NF-κB signaling pathway and promotes the release of proinflammatory cytokines such as interleukin-8. Mutations in Nod2 (1007FS, R702W, G908R) impinge on NOD2 functions and are associated with the pathogenesis of Crohn disease, a chronic inflammatory bowel disease. Although NOD2 is usually described as a cytosolic receptor for MDP, the protein is also localized at the plasma membrane, and the 1007FS mutation delocalizes NOD2 to the cytoplasm (Barnich, N., Aguirre, J. E., Reinecker, H. C., Xavier, R., and Podoisky, D. K. (2005) J. Cell Biol. 170, 21-26; McDonald, C., Chen, F. F., Ollendorff, V., Ogura, Y., Marchetto, S., Lecine, P., Borg, J. P., and Nunez, G. (2005)1. Biol. Chem. 280, 40301-40309). In this study, we demonstrate that membrane-bound versions of NOD2 and Crohn disease-associated mutants R702W and G908R are capable of responding to MDP and activating the NF-κB pathway from this location. In contrast, the 1007FS mutant remains unable to respond to MDP from the plasma membrane. We also show that NOD2 promotes the membrane recruitment of RICK, a serine-threonine kinase involved in NF-κB activation downstream of NOD2. Furthermore, the artificial attachment of RICK at the plasma membrane provokes a constitutive and strong activation of the NF-κB pathway and secretion of interleukin-8 showing that optimal RICK activity depends upon its subcellular localization. Finally, we show that endogenous RICK localizes at the plasma membrane in the THP1 cell line. Thus, our data suggest that NOD2 is responsible for the membrane recruitment of RICK to induce a regulated NF-κB signaling and production of proinflammatory cytokines. [ABSTRACT FROM AUTHOR]

Captive chimpanzees living in confined environments like sanctuaries or primatology centers are frequently affected by gastrointestinal parasites. Some of these are likely to be transmitted to humans and may seriously affect public health. However little information is currently available on the gastrointestinal parasites of primates living in such environments. Here, we characterize the diversity and prevalence of gastrointestinal parasites in two populations of captive chimpanzees living in south-eastern Gabon. Our study reveals that at least nine parasite species infect the chimpanzees with high prevalence, including several helminths (Ascaris spp. , Enterobius spp. , Strongyloides spp. , Trichuris spp. , Hymenolepis spp. , Mammomonogamus spp), three protozoa (Balantioides spp. , Entamoeba spp. and Troglodytella spp) and several unidentified parasites. All the parasite taxa we identified had previously been identified in other primates, including humans. Congenital Sodium Diarrhea (CSD) due to SLC9A3 mutation is a rare cause of neonatal diarrhea explained by dysfunction of the Na+/H+ antiporter 3 in intestine. To date only 10 patients have been described. We report a male patient with typical antenatal symptoms (polyhydramnios and intestinal dilation) and neonatal diarrhea with fecal sodium and bicarbonates loss. Next generation sequencing revealed a missense homozygous mutation in exon 6 of the SLC9A3 gene (NM_004174.3:c.1039G > A, NP_004165.2:p.Glu347Lys). Oral electrolytes supplements (Sodium and Bicarbonates) allowed a normal growth to the child currently aged twenty months. CSD symptomatology usually begins during third trimester of pregnancy. Antenatal signs are polyhydramnios and diffuse intestinal dilation. Main differential diagnoses are intestinal obstruction and Congenital Chloride Diarrhea. Diarrhea begins from the first days of life and its severity is variable. Based on the report and on the literature we suggest that non syndromic CSD can be detected during third trimester of pregnancy. With adequate electrolytes supplementation good evolution is possible.
