Metastatic gastrointestinal cancer is not an uncommon situation, especially for pancreatic, gastric, and colorectal cancers. In this setting, few data are available on the impact of the treatment of the primary tumour. Oligometastatic disease is associated with longer survival in comparison with more advanced disease. Metastasis-directed therapy, such as stereotactic body radiotherapy, seems related to better outcomes, but the level of evidence is low. Gastrointestinal cancers are one of the most frequent cancers and a leading cause of cancer deaths worldwide. We provide an overview of the most important practice-changing trials that were either published or presented at the international scientific meetings in 2021–2023. Highlights included reports on three phase III trials (CONCORDE/PRODIGE 26, ARTDECO, and a study by Xu et al.) that evaluated dose escalation in the definitive setting for locally advanced oesophageal cancers, as well as two phase III trials that evaluated the role of chemotherapy (neo-AEGIS) and targeted therapy (NRG/RTOG 1010) in the neoadjuvant setting for adenocarcinoma oesophageal cancers or gastroesophageal junction cancer. CheckMate 577 evaluated nivolumab in patients who had residual pathological disease after neoadjuvant chemoradiation followed by complete resection. The use of radiation therapy for borderline and locally advanced pancreatic cancer is also discussed (SMART and CONKO-007 trials). Stereotactic body radiation therapy followed by sorafenib was compared to sorafenib alone in patients with hepatocellular carcinoma in the NRG/RTOG 1112 study. New options in the management of rectal cancer are emerging such as total neoadjuvant treatment (PRODIGE 23, RAPIDO, PROSPECT), organ preservation (OPRA, OPERA), and the role of immunotherapy in patients with DNA mismatch-repair deficient/microsatellite instability. Despite recent advances, the prognosis of pancreatic adenocarcinomas remains poor, even for patients with resectable tumors. For these latter, new approaches based on neoadjuvant treatment have been developed. Two components are used: chemotherapy and radiation therapy (RT). Indeed, pre-operative RT has many advantages in terms of efficacy and tolerance. It increases notably the chances of subsequent complete tumor resection. Several prospective trials are currently ongoing to clarify its place in the therapeutic arsenal. We present the update of the recommendations of the French society for radiation oncology on external radiotherapy and brachytherapy of anal canal carcinoma. We present the update of the recommendations of the French society of oncological radiotherapy on radiotherapy of pancreatic tumors. Currently, the use of radiation therapy for patients with pancreatic cancer is subject to discussion. In the adjuvant setting, the standard treatment is six months of chemotherapy with 5-fluorouracile, irinotecan and oxaliplatin. Chemoradiation may improve the survival of patients with incompletely resected tumours (R1). This remains to be confirmed by a prospective trial. Neoadjuvant chemoradiation is a promising treatment especially for patients with borderline resectable tumours. For patients with locally advanced tumours, there is no standard. An induction chemotherapy followed by chemoradiation for non progressive patients reduces the rate of local relapse. Whereas in the first trials of chemoradiation large fields were used, the treated volumes have been reduced to improve tolerance. Tumour movements induced by breathing should be taken in account. Intensity modulated radiation therapy allows a reduction of doses to the organs at risk. Whereas widely used, this technique has poor evidence-based recommendation. Chemoradiotherapy with concurrent cisplatin has been the standard treatment for locally advanced head and neck squamous cell carcinoma (HNSCC) for over 20 years. Recently, immunotherapy, a new therapeutic class, has emerged for patients with recurrent or metastatic HNSCC and has significantly extended their survival. Will it bring the same benefit to patients with localized tumors? There is a strong rationale for combining radiation therapy and checkpoint inhibitors for HNSCC. Indeed, radiation therapy can have both immunostimulatory and immunomodulatory effects. This is what explains the famous abscopal effect. The aim of this article is to review unrecognized toxicities resulting from radiation therapy of digestive neoplasms. Due to their precocious occurrence, acute toxicities are well-known by radiation oncologist, and their treatment well-established. Thus, acute toxicities will not be described in this review. We will focus on incidence, diagnosis, and management of late and uncommon toxicities occurring in the digestive tract and digestive organs. Prevention, by respecting healthy tissues constraints, is the main tool to reduce incidence of those rare complications. Nonetheless, once installed, late toxicities remain a major burden in terms of quality of life and can even be life threatening. Longtemps, la chimioradiothérapie adjuvante est restée incontournable dans la prise en charge thérapeutique des adénocarcinomes gastriques et pancréatiques. Pour ces tumeurs, l'exérèse chirurgicale, seul espoir d'offrir au patient une survie prolongée, n'est possible que dans 20 % des cas. La survie médiane des patients opérés n'est que de 12 à 20 mois en raison de la fréquence des récidives locorégionales et/ou métastatiques. Pour les cancers de l'estomac, la chimioradiothérapie adjuvante est justifiée par les résultats de l'essai de phase III de l'Intergroup 0116, publiés par MacDonald et al. Le gain de survie l'était au prix d'une toxicité non négligeable. Ce traitement a été supplanté au début des années 2000 par la chimiothérapie périopératoire. Actuellement, des essais de chimioradiothérapie néoadjuvante sont en cours dans le but d'améliorer l'observance et la tolérance des traitements. For many years, adjuvant chemoradiotherapy remained essential in the therapeutic management of gastric and pancreatic adenocarcinomas. For these tumours, surgical excision, the only hope of offering the patient prolonged survival, is only possible in 20% of cases. The median survival of operated patients is only 12 to 20 months due to the frequency of locoregional and/or metastatic recurrences. For stomach cancers, adjuvant chemoradiotherapy is justified by the results of the phase III trial Intergroup 0116 published by MacDonald et al. Résumé Lors du diagnostic de cancer du pancréas, environ 15 % des patients sont atteints d’une tumeur résécable, 50 % d’une tumeur métastatique et 25 % d’une tumeur localement évoluée (non métastatique mais non résécable du fait d’un envahissement vasculaire) ou à la limite de la résécabilité. Malgré les progrès techniques réalisés dans le domaine de la radiothérapie et l’amélioration de l’efficacité de la chimiothérapie, le pronostic reste défavorable. Ces dernières années, la place de la radiothérapie dans la prise en charge des cancers du pancréas a été très discutée. Résumé Le traitement des carcinomes épidermoïdes des voies aérodigestives localement évolués repose sur la chimioradiothérapie concomitante. Un traitement séquentiel associant chimiothérapie d’induction par association de docétaxel, cisplatine et 5-fluoro-uracile (TPF), suivie d’une (chimio)radiothérapie, est utilisé fréquemment dans le cadre des stratégies de préservation laryngée. En dehors de cette situation particulière, le bénéfice en termes de survie d’une chimiothérapie d’induction a été beaucoup discuté ces dernières années. Dans cinq essais randomisés récents, une chimioradiothérapie a été comparée à une chimiothérapie d’induction par TPF, suivie d’une chimioradiothérapie. Parmi ces cinq essais, quatre concluent que ces traitements sont similaires. Un seul essai a montré un bénéfice de la chimiothérapie d’induction mais sa méthodologie est très discutable. Après la chimiothérapie par TPF, la chimioradiothérapie est moins bien tolérée. En cas d’envahissement ganglionnaire important (stade N2b-c-N3), la chimiothérapie d’induction permet de diminuer la survenue de métastases à distance. Le statut selon l’ Human papilloma virus (HPV) ne doit pas influencer la décision thérapeutique. The treatment of locally advanced head and neck squamous cell carcinoma is based on concomitant chemoradiotherapy. A sequential treatment combining induction chemotherapy with docetaxel, cisplatin and 5-fluorouracil (TPF), followed by (chemo)radiotherapy is frequently used as part of laryngeal preservation strategies. Apart from this particular situation, the benefit in terms of survival of induction chemotherapy has been much discussed in recent years. In five recent randomized trials, chemoradiotherapy was compared with TPF induction chemotherapy followed by chemoradiotherapy. Of these five trials, four concluded that these treatments were similar. A single trial reports a benefit for induction chemotherapy but its methodology is highly debatable. After TPF chemotherapy, chemoradiotherapy is less well tolerated. In patients with significant lymph node invasion (N2b-c-N3), induction chemotherapy reduces the occurrence of distant metastasis. Résumé Le carcinome épidermoïde du canal anal est une maladie rare et souvent curable dont le traitement standard repose sur la radiothérapie en association avec une chimiothérapie à base de 5-fluoro-uracile et de mitomycine-C. La surveillance après ce traitement reste cependant mal codifiée. L’objectif de cet article est de proposer une mise au point sur les différentes modalités de surveillance après traitement à la recherche de rechute ou d’effets secondaires. Si cette surveillance est essentiellement clinique, nous évoquerons les différentes perspectives de surveillance morphologique (échoendoscopie endorectale, imagerie par résonance magnétique pelvienne, tomodensitométrie et tomographie par émission de positons) ainsi que biologique ( squamous cell carcinoma antigen et histologie) et leur apport diagnostique et pronostique. Anal canal carcinoma is a rare and curable disease for which the standard of care is radiation therapy with concurrent 5-fluoro-uracil and mitomycine-based chemotherapy. Post-treatment follow-up however is rather poorly defined. This article offers a review of the various post-treatment surveillance options both for early diagnosis of relapse and care for late treatment effects. Radiation therapy (with associated chemotherapy) is the standard treatment for nasopharyngeal carcinoma. Conformal intensity-modulated radiation therapy is a new and particularly interesting technique for these tumors, due to their complex volumes close to many critical organs. Better dosimetric results and improved protection of adjacent healthy tissue have been shown compared with conventional 2D or 3D radiation therapy, with significantly reduced side-effects, notably xerostomia. Résumé Le traitement de référence des carcinomes épidermoïdes du canal anal localement évolués (stades II et III) est la chimioradiothérapie exclusive. Celle-ci délivre une dose de 45 Gy à raison de cinq fractions de 1,8 Gy par semaine dans un volume pelvien, puis un complément de dose de 14 à 20 Gy dans la tumeur. La chimiothérapie concomitante la plus souvent utilisée comprend une association de 5-fluoro-uracile et de mitomycine-C délivrée lors de la première et cinquième semaine d’irradiation. Une couverture optimale des volumes cibles est parfois difficile à obtenir avec une irradiation conformationnelle (jonctions des faisceaux, combinaison d’électrons et photons parfois complexes dans les aires ganglionnaires). La toxicité cutanéomuqueuse et digestive induite par le traitement peut nécessiter un intervalle libre avant la réalisation du complément d’irradiation. Celui-ci peut avoir un effet délétère sur les résultats s’il est prolongé (plus de 38 jours). Les différentes techniques de radiothérapie conformationnelle avec modulation d’intensité (statique segmentée, dynamique, arcthérapie volumique modulée et tomothérapie hélicoïdale) ont leur intérêt dans cette localisation. Elles permettent d’obtenir une stérilisation tumorale et ganglionnaire avec une baisse des doses intermédiaires et élevées aux organes à risque (intestin grêle, périnée/organes génitaux, vessie, moelle osseuse) entraînant une réduction de la toxicité aiguë de grade 3 ou plus ainsi qu’une meilleure conservation fonctionnelle du sphincter anorectal. Une description de la réalisation de l’arcthérapie volumique modulée est présentée dans cet article. Les avantages et inconvénients de cette technique par rapport aux autres y sont discutés. The standard treatment of locally advanced (stage II and III) squamous cell carcinoma of the anal canal consists of concurrent chemoradiotherapy (two cycles of 5-fluoro-uracil, mitomycin C, on a 28-day cycle), with a dose of 45 Gy in 1.8 Gy per fraction in the prophylactic planning target volume and additional 14 to 20 Gy in the boost planning target volume (5 days per week) with a possibility of 15 days gap period between the two sequences. While conformal irradiation may only yield suboptimal tumor coverage using complex photon/electron field junctions (especially on nodal areas), intensity modulated radiation therapy techniques (segmented static, dynamic, volumetric modulated arc therapy and helical tomotherapy) allow better tumour coverage while sparing organs at risk from intermediate/high doses (small intestine, perineum/genitalia, bladder, pelvic bone, etc.). Such dosimetric advantages result in fewer severe acute toxicities and better potential to avoid a prolonged treatment break that increases risk of local failure. These techniques also allow a reduction in late gastrointestinal and skin toxicities of grade 3 or above, as well as better functional conservation of anorectal sphincter. At the time of diagnosis, around 20% of patients with pancreatic cancer present at a resectable stage, 50% have metastatic disease and 30% have locally advanced tumour, non-metastatic but unresectable because of superior mesenteric artery or coeliac encasement. Despite advances in chemoradiotherapy and improved systemic chemotherapeutic agents, patients with locally advanced pancreatic cancer suffer from high rates of distant metastatic failure and from local progression, with a median survival time ranging from 5 to 11 months. In the past 30 years, modest improvements in median survival have been attained for these patients treated by chemoradiotherapy or chemotherapy protocols. Background Pancreatic carcinoma is one of the leading causes of cancer-related mortality. At the time of diagnosis, 30% of patients present with a locally advanced pancreatic carcinoma (LAPC). As circulating tumor cells (CTCs) count may be a surrogate of the cancer metastatic abilities, CTC detection rates and prognostic value were studied in a prospective cohort of LAPC patients. Patients and methods An LAP07 international multicenter randomized study assesses in patients whose LAPC is controlled after 4 months of chemotherapy whether chemoradiotherapy could increase survival versus continuation of chemotherapy. A subgroup of patients included in the LAP07 trial was screened for CTCs (CellSearch®) before the start of the chemotherapy and after 2 months of treatment. Patient characteristics and survival were obtained prospectively and were correlated with CTC detection. Results Seventy-nine patients were included. One or more CTCs/7.5 ml were detected in 5% of patients before treatment and in 9% of patients after 2 months of treatment (overall detection rate: 11% of patients). CTC positivity was associated with poor tumor differentiation (P = 0.04), and with shorter overall survival (OS) in multivariable analysis (RR = 2.5, P = 0.01), together with anemia. Abstract: Pancreatic carcinoma is the fifth leading cause of cancer-related mortality. The 5-year overall survival is less than 5 %. This very poor prognosis can be explained both by late diagnosis and by treatment resistance, including resistance to radiation therapy. A better understanding of the pancreatic tumorigenesis and knowledge of the most frequent mutations in pancreatic adenocarcinoma (KRAS, p16, TP53, Smad4) open new perspectives for the development of more effective treatments. Abstract: Radiation damages to the kidney represent uncommonly reported toxicity, not because kidneys are radioresistant, but because clinical symptomatology is not specific and may not manifest for years after treatment. Nevertheless, late effects can result in considerable morbidity and mortality, in considering moderate dose of irradiation. Data on tolerance of the kidney to radiation therapy are reviewed here, early and late radiation-induced effects are described and dose recommendations are suggested for clinical practice. Abstract: The future development of safe CO2 storage on land needs a particular effort on monitoring methods. In the specific geological context of the sedimentary Paris Basin (France), the Géocarbone-Monitoring project carried out from 2006 to 2008 the evaluation and testing of different monitoring methods. The targeted reservoirs are either depleted reservoirs in the carbonate Dogger formation or saline aquifers in the silico-clastic formations of the Triassic. To map the migration of the CO2 plume, the project evaluated geophysical methods as active seismic, electrical resistivity and gravity. Results show that the geological context implies further research to be conducted to overcome problems related either to data acquisition in the case of gravity or to the geological context in the case of seismic and electrical resistivity. The InSAR remote sensing method was not able to detect ground deformation over seasonal gas storage, mainly because of the dense vegetation cover. Methods for soil gas measurement at the surface were successfully tested on natural analogs, revealing the complexity of mechanisms involved in the origin of the CO2 flux, its temporal and spatial variations, as well as the need for continuous surface measurements in order to control seasonal variations. Abstract: FDG-(18F) PET can now be usually included in the treatment strategy of esophageal cancers for the pretreatment staging in operable tumours or for the diagnosis of recurrence. PET is also a good tool in conformal radiation therapy for improving the target coverage to treat the metabolic target volume or the biological target volume. Furthermore, PET seems to be interesting for evaluation of tumour response and could modify the treatment strategy after neoadjuvant chemotherapy or concurrent chemotherapy and radiation therapy. New radiotracers could allow advances in biological and molecular tumour delineation and contribute to change in treatment strategy based on functional and biological imaging. Abstract: During the 5 past national courses organised by the French society of radiation oncology (SFRO), three different types of survey were performed to analyse demography, motivations and quality of training of the young specialists. During the 5 past years, 50 radiation oncologists were training for the whole country (about 15 per year were graduated). A recent increase the number of young specialists is observed with a total number of 50 in 2000 to 75 in 2005. Nevertheless, the number of young specialists is dramatically insufficient and exposes for the future to an important demographic crisis. Analysis of motivations of choice for radiation oncology confirms the influence of a practical stage of oncology during the second cycle of the medical studies for 60% of the young specialists. Analysis of practical and theoretical training was performed according to the point of view and living experiences of the students. On the other hand, informations from teachers were less complete. Some needs are emphased as: 1) the quality of the follow during the training (importance of the recent implementation of a logbook); 2) importance of theoretical and practical training at the radiotherapy department; 3) help and incentive for research and scientific publication. Grâce aux succès des inhibiteurs de checkpoint , l'immunothérapie occupe désormais une place majeure dans la prise en charge d'un grand nombre de tumeurs solides, tandis que le nombre des indications continue de croître et que de nouvelles combinaisons pourraient, dans un avenir proche, modifier encore les standards de traitement. Cependant, les taux de réponse des immunothérapies en monothérapie sont modestes et leur utilisation s'envisage de plus en plus au sein de combinaisons avec d'autres traitements anticancéreux (chimiothérapies, chirurgie, radiothérapie ou certaines thérapies ciblées). Les combinaisons avec la radiothérapie semblent particulièrement attractives car il existe un fort rationnel expérimental qui lie une partie de l'efficacité des rayonnements ionisants à une stimulation induite sur le système immunitaire (réponse innée et adaptative). De nombreux essais de phase précoce et un certain nombre de grands essais randomisés de combinaison ont donné des résultats d'efficacité et de tolérance, tandis que des essais importants sont encore en cours et apporteront des réponses dans un avenir proche. Cette revue rapide rappelle le rationnel biologique expérimental de l'immuno-radiothérapie et présente certains axes fondamentaux en cours d'exploration, puis présente les résultats cliniques d'efficacité et de tolérance disponibles à ce jour, ceux qui sont attendus prochainement, et enfin esquisse un tableau des perspectives dans ce domaine en évolution rapide. Thanks to the success of checkpoint inhibitors, immunotherapy now plays a major role in the management of a large number of solid tumors, while the number of indications continues to grow and new combinations could, in the near future, further modify treatment standards. However, the response rates of immunotherapies as monotherapy are modest and their use is increasingly considered in combination with other cancer treatments (chemotherapy, surgery, radiotherapy or certain targeted therapies). Combinations with radiotherapy seem particularly attractive because there is a strong experimental rationale linking part of the efficacy of ionizing radiation to an induced stimulation of both of the innate and adaptive response. Many early phases and a number of large randomized combination trials have published efficacy and safety results, while important trials are still ongoing and will provide answers in the near future. This short review recalls the experimental biological rationale for immuno-radiotherapy and highlights some of the fundamental directions being explored, then presents the clinical efficacy and safety results available to date, those expected in the near future, and finally outlines the outlook in this rapidly evolving field. La prise en charge des cancers rectaux dépend des caractéristiques de la tumeur (localisation par rapport au sphincter, stades T et N, atteinte sphinctérienne) et du patient. La chirurgie radicale (exérèse totale du mésorectum), précédée d'une radiothérapie courte ou d'une chimioradiothérapie classique, encadrée de chimiothérapie (d'induction ou de consolidation [traitement néoadjuvant total]), reste le pilier du traitement des cancers du rectum localement évolués (T3cd, T4 et atteinte ganglionnaire). Néanmoins, dans la prise en charge des stades précoces, cette chirurgie radicale pourrait être évitée au profit d'un traitement conservateur associant radiothérapie (externe, de contact, curiethérapie) plus ou moins associée à de la chimiothérapie (concomitante, d'induction ou de consolidation) ou être limitée, pour les patients en situation de réponse, à l'exérèse de la cicatrice, dans une stratégie de préservation d'organe. La préservation d'organe pourrait également être proposée, de façon opportuniste, aux patients en réponse complète clinique après le traitement d'induction, quel que soit les caractéristiques initiales de leur cancer. Cette stratégie, appelée Watch and Wait , est basée sur l'évaluation clinique, endoscopique et radiologique et sur la surveillance régulière. Des études sont en cours pour augmenter les taux de réponse soit en intensifiant la chimiothérapie, soit en intensifiant la radiothérapie en augmentant la dose du boost par contact-thérapie ou par curiethérapie. Radical surgery (total mesorectal excision) following short-course radiotherapy (RT) or standard chemo-radiotherapy, associated with induction or consolidation chemotherapy (total neoadjuvant treatment), remains the cornerstone of locally advanced rectal cancer (T3cd, T4 and/or N+) treatment. Nevertheless, for early stages, this radical resection could be avoided in favor of conservative approaches combining RT (external, contact, brachytherapy) with or without chemotherapy (concurrent, induction or consolidative), or even be limited, for good responders, to a local excision with view of organ-preservation strategies. This conservative approach could also be offered selectively to patients with complete clinical response after the induction sequence, irrespective of initial tumor characteristics. The Watch and Wait strategy relies on clinical, endoscopic and radiological evaluations, as well as sustained surveillance. Objectif de l’étude Le phénotype MSS/MSI (MSS : absence d’instabilité des microsatellites ; MSI : instabilité des microsatellites) a un impact pronostique dans les cancers colorectaux de stade II–III avec un moindre risque métastatique pour le statut MSI. Son impact prédictif sur la réponse à la chimiothérapie et à l’immunothérapie est établi. Celui sur la réponse à la radiothérapie est inconnu. Cette étude avait pour objectif d’évaluer l’impact du statut MSI sur la réponse à la radiothérapie néoadjuvante des cancers du rectum localement évolués. Matériel et méthode Nous avons analysé rétrospectivement les dossiers de 17 patients avec le statut MSI et 338 patients avec celui MSS, atteints de cancer de stade T3–T4 et/ou avec atteinte ganglionnaire, pris en charge par chimioradiothérapie néoadjuvante de 45 à 50 Gy en cinq semaines avec une chimiothérapie à base de fluoropyrimidines ou 25 Gy en cinq fractions. Ces patients ont été appariés sur l’âge et le stade tumoral à l’aide d’un score de propension. Le critère de jugement principal était la réduction du stade T et/ou N sur la pièce opératoire après traitement néoadjuvant. Résultats Le suivi médian était de 36,7 mois. Il y avait parmi les patients avec le statut MSI, deux tumeurs ypN+ (12 %), huit ypT1-2N0 (47 %) et sept ypT3-4N0 (41 %) contre respectivement 106 (31 %), 130 (38 %) et 102 (30 %) avec le statut MSS. Une réduction du stade tumoral a été observé chez 14 patients avec le statut MSI (93,3 %) contre 200 avec le statut MSS (59,2 %) ( odds ratio [OR] : 0,10 ; intervalle de confiance à 95 % [IC 95 %] : 0,01–0,80, p = 0,029). Le taux de rechute locale n’était pas significativement différent entre les deux populations, une patiente avec le statut MSI (7,7 %) contre 24 avec celui MSS (7,2 %) (OR = 0,93 ; IC 95 % : 0,12–7,43, p = 0,94), de même que la survie globale ( hazard ratio [HR] : 0,39 ; IC 95 % : 0,05–2,87, p = 0,35). Conclusion Le dysfonctionnement somatique du système de réparation des mésappariements de l’ADN semble entraîner une meilleure réponse à la radiothérapie néoadjuvante des cancers du rectum localement évolués. En 2021, la Commission d'Éthique de la Société française de Radiothérapie Oncologique (SFRO) s'est saisie de la thématique délicate de la pratique des soins à visée palliative en oncologie radiothérapique. L'oncologie radiothérapie joue un rôle central dans la prise en charge des patients atteints de cancer en phase palliative. Mais derrière la dénomination large de radiothérapie palliative, on retrouve en réalité un grand nombre de situations relevant d'enjeux éthiques très divers. Le rôle de l'oncologue radiothérapeute sera de prendre en compte de multiples facteurs mobilisés à l'occasion d'un processus décisionnel complexe. Si la question de l'indication thérapeutique, et du choix technique permettant de la mettre en œuvre reste centrale, la réflexion ne saurait se limiter à ces seuls aspects décisionnel et technique. Il est aussi question de pouvoir créer les conditions d'un soin attentif à la singularité des personnes et de construire au-delà de la technicité, une authentique relation de soin. C'est au travers de ce travail éthique quotidien, en lien étroit avec les personnes malades, et dans des conditions indispensables de pluri disciplinarité et de pluri professionnalité, que se déploie pleinement notre rôle fondamental de soignant. In 2021, the Ethics Commission of the SFRO has chosen the issue of the practice of palliative care in radiotherapy oncology. Radiation oncology plays a central role in the care of patients with cancer in palliative phase. But behind the broad name of palliative radiotherapy, we actually find a large variety of situations involving diverse ethical issues. Radiation oncologists have the delicate task to take into account multiple factors throughout a complex decision-making process. While the question of the therapeutic indication and the technical choice allowing it to be implemented remains central, reflection cannot be limited to these decision-making and technical aspects alone. It is also a question of being able to create the conditions for a singularity focused care and to build an authentic care relationship, beyond technicity. It is through this daily ethical work, in close collaboration with patients, and under essential conditions of multidisciplinarity and multiprofessionalism, that our fundamental role as caregiver can be deployed. Oligometastatic cancers designate cancers in which the number of metastases is less than five, corresponding to a particular biological entity whose prognosis is situated between a localized and metastatic disease. The liver is one of the main sites of metastases. When patients are not suitable for surgery, stereotactic body radiotherapy provides high local control rate, although these data come mainly from retrospective studies, with no phase III study results. The need for a high therapeutic dose (biologically effective dose greater than 100 Gy) while respecting the constraints on the organs at risk, and the management of respiratory movements require expertise and sufficient technical prerequisites. The emergence of new techniques such as MRI-guided radiotherapy could further increase the effectiveness of stereotactic radiotherapy of liver metastases, and thus improve the prognosis of these oligometastatic cancers. Objectifs L’essai de phase II randomisé multicentrique CONCORDE compare chez des patients atteints d’un cancer de l’œsophage localement évolué des chimioradiothérapies de 50 et 66 Gy. Le contrôle de qualité de la radiothérapie dans cet essai inclut un cas test ou dummy run dont la validation est obligatoire pour chaque centre investigateur. Patients et méthodes Les plans de radiothérapie des dummy run des centres participants ont été analysés et comparés au plan de référence validé par les experts : délinéation des volumes cibles et des organes à risque, couverture du volume cible prévisionnel, respect des contraintes de dose délivrée aux organes à risquer (poumons, cœur, moelle, foie reins). Les centres ont été classés en trois catégories : respect du protocole, déviation mineure ou déviation majeure. Les centres avec une déviation majeure ne sont pas autorisés à inclure des patients. Résultats Trente-deux centres de radiothérapie ont été évalués. Un seul (3 %) a dû refaire la délinéation du volume tumoral macroscopique (adénopathie non incluse) et trois (9 %) du volume cible prévisionnel. Deux plans (6 %) présentaient un « sous-dosage » significatif du volume cible prévisionnel de 66 Gy (dose minimale de moins de 90 %). Deux centres (6 %) ont présenté une déviation majeure sur la dose délivrée aux poumons et quatre sur le cœur (12 %). Le dummy run a été validé sans correction dans 65 % des centres. Plus de 80 % des dummy runs validés présentaient des déviations mineures. Seize centres (50 %) ont utilisé une technique d’irradiation conformationnelle tridimensionnelle et 16 (50 %) une technique avec modulation d’intensité (fixe ou rotationnelle). Conclusions L’analyse des volumes et des paramètres dosimétriques des dummy run a montré que la plupart des centres ont adhéré au protocole. Le taux élevé de déviations mineures témoigne de la difficulté à couvrir de manière homogène des volumes cibles souvent étendus tout en respectant la dose délivrée aux organes à risque. L'obtention du consentement aux soins impose à l'oncologue radiothérapeute la délivrance d'une information loyale et l'assurance de sa bonne compréhension par le patient. La preuve d'une telle démarche incombe au praticien. La Société française de radiothérapie oncologique (SFRO) ne recommande pas la signature par le patient d'un formulaire de consentement mais, recommande à l'oncologue radiothérapeute d'être en mesure de fournir tous les éléments démontrant la réalité d'un circuit d'information complet. Obtaining consent to care requires the radiation oncologist to provide loyal information and to ensure that the patient understands it. Proof of such an approach rests with the practitioner. A 77-year-old transgender man (assigned female sex at birth, gender identity male, i.e. female-to-male) was referred for a palpable mass of the right chest wall. Biopsies revealed invasive lobular breast carcinoma. After discussion by a multidisciplinary tumour board meeting, the patient was treated with total mastectomy, adjuvant hypofractionated radiation therapy, and hormone therapy. At 1.5-year follow-up, there was no sign of recurrence or long-term radiation side effects. To our knowledge, this is the first reported case of adjuvant hypofractionated radiation therapy in a transgender patient with breast cancer. [ABSTRACT FROM AUTHOR]

La dernière année de l'internat d'oncologie-radiothérapie en France est devenue une phase d'autonomisation, dite de « docteur junior », permettant aux internes la validation d'actes réservés auparavant uniquement aux médecins séniors. Cette étude s'est focalisée sur les responsabilités données à la première promotion de docteur junior et à leur opinion sur ce nouveau de statut. Une enquête transversale a été réalisée par les associations françaises d'internes et de jeunes médecins en oncologie, l'Association pour l'enseignement et la recherche des internes en oncologie (Aerio) et la Société française des jeunes radiothérapeutes oncologues (SFjRO). Un questionnaire a été adressé aux référents de promotion de chaque ville pour transmission aux docteurs juniors de l'année 2021–2022 du 1er septembre au 30 novembre 2022. Les questions concernaient la formation, les modalités de cette année et les impressions. Les réponses ont été analysées anonymement via le logiciel R. 4.3.1. Pour l'oncologie–radiothérapie, 33 réponses ont été obtenues, de 21 villes. Pour l'essentiel des docteurs juniors, trois ou quatre localisations (51 %) ont été réalisées avec en moyenne cinq nouveaux patients par semaine. Les contours ont été revus soit systématiquement (51 %), soit uniquement au début (32 %). La dosimétrie a été rapportée comme jamais contresignée chez 19 %. Quatre-vingt pour cent des docteurs juniors ont déclaré avoir été seul(e) oncologue radiothérapeute lors de réunion de concertation pluridisciplinaire. Les deux axes principaux d'amélioration étaient la formation théorique (45 %) et sur les cadres légaux/contrats (42 %). L'analyse des résultats portant sur cette première promotion montre une globale concordance avec les recommandations du Collège national des enseignants en cancérologie (Cnec). Les retours des docteurs juniors sont positifs. La supervision des activités de curiethérapie et de dosimétries, la présence seule en réunion de concertation pluridisciplinaire restent des points de vigilance. The last year of the radiotherapy oncology internship in France has become a phase of empowerment, called "junior doctor", allowing interns to validate acts previously reserved only for senior doctors. This study focused on the responsibilities given to the first promotion of junior doctors in France and their feelings on this new status. A cross-sectional survey was carried out by the French associations of interns and young doctors in oncology, Aerio and SFjRO. A questionnaire was sent to the class referents of each city for transmission to the junior doctors of the year 2021–2022 from September 1st to November 30th, 2022. The questions concerned training, the modalities of this year and the feelings. Responses were analyzed anonymously using R. 4.3.1 software. For radiation oncology, 33 responses were obtained from 21 cities. For most junior doctors, three to four localizations (51%) were performed with an average of five new patients per week. The contours were reviewed either systematically (51%) or only at the beginning (32%). Dosimetry was reported as never countersigned in 19%; 80% of junior doctors described having been the only radiation oncologist during multidisciplinary staff meetings. The two main areas of improvement were theoretical training (45%) and legal frameworks/contracts (42%). These results relating to the first class of junior doctors showed an overall agreement with the recommendations of the Cnec. Feedback from interns was positive. Résumé La place de la radiothérapie chez les patients atteints de cancer du pancréas opérable ou localement évolué est actuellement controversée. En situation adjuvante, le traitement standard est une chimiothérapie par gemcitabine et capécitabine pendant six mois. En association avec une chimiothérapie concomitante, la radiothérapie postopératoire permettrait d’améliorer la probabilité de survie des patients après une résection tumorale incomplète (R1). Ceci reste à démontrer dans un essai prospectif. La chimioradiothérapie néoadjuvante est une approche prometteuse de plus en plus utilisée pour les tumeurs à la limite de la résécabilité (borderline). Pour les tumeurs localement évoluées, il n’existe pas de standard thérapeutique. Une chimiothérapie première suivie chez les patients en situation de non progression d’une chimioradiothérapie permet de diminuer le taux de rechute locale. Alors que dans les premiers essais de radiothérapie pancréatique étaient utilisés de grands faisceaux d’irradiation, les volumes traités ont été réduits afin d’améliorer la tolérance. Les mouvements de la tumeur liés à la respiration doivent être pris en compte. La radiothérapie conformationnelle avec modulation d’intensité permet de diminuer les doses reçues par les organes à risque. Bien que de plus en plus utilisée, elle n’est pas encore validée dans cette indication. Currently, the use of radiation therapy for patients with pancreatic cancer is subject to discussion. In adjuvant setting, the standard treatment is 6 months of chemotherapy with gemcitabine and capecitabine. Chemoradiation (CRT) may improve the survival of patients with incompletely resected tumors (R1). This should be confirmed by a prospective trial. Neoadjuvant CRT is a promising treatment especially for patients with borderline resectable tumors. For patients with locally advanced tumors, there is no a standard. An induction chemotherapy followed by CRT for non-progressive patients reduces the rate of local relapse. Whereas in the first trials of CRT large fields were used, the treated volumes have been reduced to improve tolerance. Tumor movements induced by breathing should be taken in account. Intensity modulated radiation therapy allows a reduction of doses to the organs at risk. Whereas widely used, this technique is not recommended. [ABSTRACT FROM AUTHOR]
