Abstract: Neutron-diffraction and magnetic measurements have been used in order to study the structure and magnetic changes of La0.7Sr0.3Mn1− x Ti x O3 (x=0, 0.10, 0.20, and 0.30) perovskites. Magnetic measurements show that our samples exhibit a ferromagnetic–paramagnetic transition with decreasing temperature. Effects of Ti doping on the magnetic and nuclear structures are studied here by neutron-powder-diffraction for La0.7Sr0.3Mn1− x Ti x O3 (x=0.10, 0.20, and 0.30). Our analysis indicates that the three representative samples, with x=0.10, 0.20, and 0.30, have a hexagonal system of R3¯c space group, and that the magnetic moments order ferromagnetically into the (101) crystallographic planes, i.e. the magnetic moments of the Mn ion are in the ac plane, with no component along the b-axis. There is no crystalline structure changes when the Ti doping level increases. [Copyright &y& Elsevier]

Structure, magnetic, and transport properties of La0.7Sr0.3 Mn1−x TixO3 with x=0, 0.1, 0.2, and 0.3 are studied. The system exhibits a rhombohedrally distorted perovskite structure. The structure refinement by Rietveld analysis of the X-ray powder diffraction data shows that the average distance (Mn, Ti)–O increases. The Curie temperature decreases when the titanium content increases. The Ti substitution also destroys the metallic state and the resistivity increases many orders of magnitude when the percentage of Ti increases. [Copyright &y& Elsevier]

Endothelial dysfunction is probably one of the mechanisms of long COVID-19 symptoms. Sulodexide has pleiotropic properties within the vascular endothelium that can prove beneficial in the long COVID-19 symptoms. We aimed to evaluate the effect of sulodexide when used in patients with endothelial dysfunction and long COVID-19 symptoms. We conducted a prospective multicenter longitudinal case-control study. Endothelial function was evaluated with DTM "E4-Diagnose" Polymath based on the Endothelium Quality Index (EQI). A group of patients with endothelial dysfunction (EQI < 2.0) received sulodexide. All the patients were followed-up 21 days after inclusion. Primary outcomes were defined as endothelial function amelioration (delta EQI) and long COVID-19 symptoms evolution during the follow-up. A total of 410 patients were included in this study. Patients were included at an average time of 1.89 ± 1.2 month after COVID-19 infection. At inclusion, 210 (51.2%) patients had an EQI < 2. The median age was 49 ± 13.8 (18–80) years. Among the patients with endothelial dysfunction, only 79 patients received sulodexide. Patients in sulodexide group had lower EQI than the non-medical intervention group (0.94 ± 0.6 vs. 1.52 ± 0.4; P < 10−3). They were more diabetic, hypertensive, had more coronary artery disease and received more long-term medications (aspirin, Bblockers and statins) than the others (P = 0.01, 0.002, 0.01, 0.009, 0.001 and 0.01, respectively). At the 21-days follow-up, patients in sulodexide group presented lower long COVID symptoms especially chest pain, palpitations, fatigue and neuro-cognitive difficulties associated to a significant amelioration of endothelial function (delta EQI 1.26 ± 1.07 vs. 0.22 ± 0.7; P < 10−3). Sulodexide in patients with long COVID-19 may be a good intervention to ameliorate chest pain, palpitations, fatigue and neuro-cognitive difficulties associated to endothelial dysfunction. The COVID-19 disease is a multisystem disease due to in part to the vascular endothelium injury. Lasting effects and long-term sequalae could persist after the infection and may be due to persistent endothelial dysfunction. Our study focused on the study of endothelial function measurement by digital thermal monitoring (DTM) of endothelial quality index with E4 diagnosis Polymath in a large cohort of long COVID-19 patients to determine whether long COVID-19 symptoms are due to endothelial dysfunction. This is a prospective multicenter longitudinal observational cohort study. Endothelial function was evaluated with "E4-Diagnose" Polymath Tunisia based on the Endothelium Quality Index (EQI). A complete echocardiographic evaluation analysis was performed. Primary outcomes were defined as the occurrence of long COVID-19 symptoms in patients with endothelial dysfunction measured by EQI. A total of 798 patients were included in this study. Patients were included at an average time of 68.93 ± 43.1 days. The mean EQI was 2.02 ± 0.99 [0–5]. A total of 397 (49.7%) patients had poor or very poor EQI and 211 (26.4%) patients had very poor EQI. The median age was 49.94 ± 14.2 (18–80) years. A total of 618 patients (77.4%) had long COVID-19 symptoms. Patients with long COVID-19 symptoms had a reduced EQI (1.99 ± 0.97 vs. 2.09 ± 1.05, P = 0.24). Among long COVID-19 symptoms, fatigue was the most common symptom reported in 42.2%. Fatigue and chest pain were significantly associated to the endothelial dysfunction (P = 0.04 and 0.001 respectively). Patients with chest pain had significantly lower EQI (1.74 ± 1.0 vs. 2.09 ± 0.9, P ≤ 10−3) and LVGLS (−16.35 ± 3.0 vs. −17.16 ± 2.5, P = 0.04). Long COVID-19 symptoms specifically chest pain and fatigue are due to persistent poor endothelial quality index. These findings allow a better care of patients with long COVID-19 symptoms. The occurrence of tension pneumopericardium in patient in spontaneous ventilation after blunt trauma is rare. The diagnosis is difficult and it may remain unrecognized. The authors reported the case of a 50 year-old trauma patient with a tension pneumothorax associated with a pneumopericardium. The patient was not mechanically ventilated at any time. Abstract: The transport properties of the samples La0.7Sr0.3Mn1− x Ti x O3 were investigated using admittance spectroscopy. In contrary to free compound, when titanium is introduced, the conductance becomes frequency and temperature dependent. From DC conductance study, electronic conduction is found to be dominated by thermally activated hopping and depending strongly on Ti content. Activation energy is deduced from the variation of conductance with temperature. This energy decreases from 168 to 143meV when x increases from 0.05 to 0.20. The AC conductance spectrum is found to obey Jonscher universal power law. Finally, at ambient temperature, DC conductance decreases with the value of x and proves a clear effect of titanium in transport properties of the material. This decrease of conductance is explained by a reduction of Mn3+–O–Mn4+ network responsible for electrical conduction in perovskite. [Copyright &y& Elsevier]

Abstract: Background: Intrathecal morphine (IT) is commonly used for postoperative analgesia after caesarean section. The addition of intrathecal (IT) magnesium to spinal bupivacaine-fentanyl anaesthesia increases the duration of spinal analgesia for labour without additional side effects. In this prospective, randomized, double blind, controlled study, we evaluated whether adding intrathecal magnesium could prolong spinal morphine analgesia after caesarean section. Parturient and methods: After ethics committee approval and obtaining written consent, one hundred and five (ASA I or II) adult patients undergoing caesarean section were recruited. They were randomly allocated to one of three groups: (1) group Morphine (M): 10mg of isobaric bupivacaine 0.5% (2ml)+100μg morphine (1ml)+10μg fentanyl (0.1ml)+1ml of isotonic saline solution, (2) group Magnesium (Mg): 10mg of isobaric bupivacaine 0.5% (2ml)+100mg of magnesium sulphate 10% (1ml)+10μg fentanyl (0.1ml)+1ml of isotonic saline solution, (3) group Morphine+Magnesium (MMg): 10mg of isobaric bupivacaine 0.5% (2ml)+100mg of magnesium sulphate 10% (1ml)+100μg morphine (1ml)+10μg fentanyl (0.1ml). We recorded the following: time to the first analgesic request, pain scores with the visual analogic scale at rest and in movement at h0, h1, h2, h4 and then every 4h for the first 36 postoperative hours, the occurrence of adverse events and patients’ satisfaction. Results: Time of the first analgesic request was 28±8h in group MMg versus 19±6h in group M and 7±6h in group Mg (p <0.01). Pain scores were statistically lower in group MMg (9±7 and 17±9mm respectively) compared to group M (16±9 and 28±11mm respectively) and Mg (21±9 and 37±13mm respectively) (p <0.01). There was no difference in adverse events among the three groups. Patients satisfaction was better in group MMg (p <0.01). Conclusion: In patients undergoing caesarean section under spinal anaesthesia, the addition of IT magnesium sulphate (100mg) to morphine 100μg improved the quality and the duration of postoperative analgesia without increasing the incidence of adverse effects. Abstract: Transport measurements have been performed on La0.7Sr0.3Mn1−x Cr x O3 using impedance spectroscopy technique, in order to study its electrical properties. The results indicate that the electrical properties of the material are strongly dependent on temperature and frequency. Evidences of temperature dependent electrical relaxation phenomena in the material have also been observed. Impedance spectrum analysis shows that the material can be described as a grain and grain boundary medium and permits to estimate the grain boundary contribution. Hopping mechanism dominates at small concentration. Electronic conduction is found to be dominated by thermally activated hopping of small polarons at high temperature and variable range hopping at low temperature. When the concentration of chromium increases, electrical behaviour of the material changes from semi-insulating to metallic behaviour. A regime of percolation appears and a metallic conduction becomes dominant. [Copyright &y& Elsevier]

Abstract: Titanium was added to La0.7Sr0.3MnO3 (LSM) to study its effect on electrical conductivity. Samples were characterised by impedance measurements. It was found that conductivity in the reference material without titanium is temperature independent. On the contrary, when Ti is introduced different conduction mechanisms are observed. Electronic conduction is found to be dominated by thermally activated hopping of small polarons at high temperature and variable-range hopping at low temperature. Impedance spectrum analysis shows that the material can be described as a grain and grain boundary medium. The activation energy deduced from analysis of the conductance spectrum matches very well with the value estimated from the relaxation time; then, it is thought that relaxation process and electrical conductivity are attributed to the same defect. Abstract: The electrical conductance of 20% Ti-doped La0.7Sr0.3MnO3 (LSMO) was measured using admittance spectroscopy over a wide temperature and frequency ranges. The impedance plane plot shows semicircle arcs at different temperatures and an electrical equivalent circuit has been proposed to explain the impedance results. Activation energy inferred from conductance spectrum matches very well with the value estimated from relaxation time indicating that relaxation process and conductivity have the same origin. The electrical conductance of La0.7Sr0.3Mn0.8Ti0.2O3 is found to be dependent on temperature and frequency. Also, the electronic conduction appears to be dominated by thermally activated hopping of small polaron (SPH) at high temperatures and by variable range hopping (VRH) at low temperatures. Abstract: Objective: Opioids are routinely omitted at the induction of general anesthesia for Caesarean delivery because of the risks of respiratory neonatal depression. The short-acting opioid remifentanil may afford advantages at the induction and surgical stimulation, without subsequent neonatal depression. Patients and methods: In this double-blinded study, 40 at term women undergoing elective Caesarean section and requiring general anaesthesia were allocated randomly to receive either remifentanil (0,5 μg/kg) at the induction of anaesthesia (G1, n=20) or placebo (G2, n=20). Induction of anaesthesia was performed with propofol 2 mg/kg and succinylcholine 1 mg/kg. Anaesthesia was maintained with nitrous oxide in oxygen (50/50%, v/v), propofol (100 μg/kg/min), remifentanil (0.2 μg/kg/min) and atracurium. Neonates were assessed by using Apgar scores, possible respiratory depression, with or without ventilation in the mask or intubation and umbilical cord blood gas (artery: UA and vein: UV). Values are expressed as mean values ±SD. Pearson''s Chi squared and t-test were used for statistical analysis P <0.05 was considered significant. Results: Maternal systolic pressure, mean pressure and heart rate were significantly higher in G1 at induction. Apgar scores, heart and respiratory rate were similar between groups. Seven episodes of respiratory depressions were noted (3 in G1, 4 in G2). Five neonates required only brief assisted ventilation by face-mask (2 in G1, 3 in G2). Conclusion: Remifentanil (0.5 μg/kg) at the induction of anaesthesia in elective Caesarean section under general anaesthesia can be used without subsequent neonatal depression. However, we believe that further research is necessary to extrapolate these results to a pregnancy carrying an acutely distressed foetus. [Copyright &y& Elsevier]

Résumé: Le carcinome à petites cellules de l’ovaire de type hypercalcémiant est rare. Nous rapportons le cas d’une patiente âgée de 34ans, qui consultait pour des douleurs pelviennes. L’imagerie a mis en évidence une volumineuse masse de l’ovaire droit. La patiente a eu une annexectomie droite puis une hystérectomie totale, annexectomie gauche, curage iliaque et lombo-aortique et biopsie du Douglas. L’examen anatomopathologique a conclu à un carcinome à petites cellules de l’ovaire de type hypercalcémiant stade IA de la classification internationale de gynécologie-obstétrique (Figo) de 2009. La patiente a reçu par la suite six cycles de chimiothérapie adjuvante puis une radiothérapie pelvienne. Le carcinome à petites cellules de l’ovaire est une pathologie très rare. Son traitement doit associer une chirurgie optimale, une chimiothérapie et une radiothérapie. Son pronostic reste défavorable malgré un traitement agressif et multimodal. [Copyright &y& Elsevier]
