There is no consensus regarding the terminology, definition, classification, diagnostic criteria, and algorithm, or reporting standards for the disease of infective native aortic aneurysm (INAA), previously known as mycotic aneurysm. The aim of this study was to establish this by performing a consensus study. The Delphi methodology was used. Thirty-seven international experts were invited via mail to participate. Four two week Delphi rounds were performed, using an online questionnaire, initially with 22 statements and nine reporting items. The panellists rated the statements on a five point Likert scale. Comments on statements were analysed, statements revised, and results presented in iterative rounds. Consensus was defined as ≥ 75% of the panel selecting "strongly agree" or "agree" on the Likert scale, and consensus on the final assessment was defined as Cronbach's alpha coefficient >.80. All 38 panellists completed all four rounds, resulting in 100% participation and agreement that this study was necessary, and the term INAA was agreed to be optimal. Three more statements were added based on the results and comments of the panel, resulting in a final 25 statements and nine reporting items. All 25 statements reached an agreement of ≥ 87%, and all nine reporting items reached an agreement of 100%. The Cronbach's alpha increased for each consecutive round (round 1 =.84, round 2 =.87, round 3 =.90, and round 4 =.92). Thus, consensus was reached for all statements and reporting items. This Delphi study established the first consensus document on INAA regarding terminology, definition, classification, diagnostic criteria, and algorithm, as well as reporting standards. The results of this study create essential conditions for scientific research on this disease. The presented consensus will need future amendments in accordance with newly acquired knowledge. Abstract Peripheral Arterial Occlusive Disease (PAOD) is an aging disease that affects the quality of life of many people by its intermittent claudication and critical limb ischemia presentations. Traditional treatment and management of PAOD are asking patients to make a life change and medication with antiplatelet, statins and cilostazol, which decrease the possibility of clot formation. Our strategy has employed a magnetic Fe 3 O 4 -PLGA polymersome to carry the cilostazol into the ischemic area by magnetic attraction following remote-control drug release through low-energy ultrasound exposure. In the animal studies, the cilostazol-loaded Fe 3 O 4 -PLGA polymersomes were injected and accumulated at ischemic leg through magnetic attraction. Then, using a clinical-use ultrasound machine the leg was irradiated to forward cilostazol release from the accumulated polymersomes. Dramatically, we found an observable result of bloody flux recovery in the leg after 7 days compared to the non-treated leg that showed no evidence of the blood recovery. Valvular heart diseases in pulmonary valves may exhibit different degrees of aortic stenosis or congenital defects. Valve repair or replacement surgery is one of the important procedures commonly performed to relieve valvular dysfunction and improve the significant regurgitation. Hence, it is necessary to assess pulmonary valve insufficiency for pre-surgery decision-making and post-surgery outcome evaluation. This study proposes an assistive technology to quantify regurgitation using the regurgitation fraction (RF) and heart pump efficiency (HPE). In signal preprocessing stage, the detrending and zero-crossing processes are used to remove the unwanted flow fluctuations and identify the end-systolic and end-diastolic periods per each cardiac cycle. The fractional-order integrations are employed to calculate the stroke volume (SV) and regurgitation volume (RV). Then, the regurgitation flow can be quantified that indicates the high correlation with HPE. For a mimicking pulmonary circulation loop system, the proposed screening model can be validated to assess the valve stent efficacy. Experimental results also indicate that pulmonary valve replacement, such as handmade trileaflet valves, can improve severe pulmonary regurgitations. Background: Open surgical treatment for an infected aortic aneurysm has a high rate of surgical morbidity and mortality and does not guarantee eradication of the infected nidus. The use of endovascular aortic repair (EVAR) might simplify the procedure and provide a good alternative for this critical condition, but this remains to be proved. This study assessed the efficacy and outcome of EVAR with an adjunctive antibiotic treatment strategy. Methods: We focused on the experiences and results of the management of infected aortic aneurysms with positive blood cultures. We drew the blood for culture study, immediately prescribed broad-spectrum antibiotics, performed EVAR procedures, and followed this with sensitive antibiotics and adjunctive procedures. Results: Twelve consecutive patients (mean age, 70 years) were included in this EVAR strategy. Three patients had thoracic, two thoracoabdominal, and the remaining seven had infected abdominal aneurysms. Ten Salmonella, one Staphylococcus, and one Streptococcus spp were identified. There was no hospital death. Three patients underwent computed tomography (CT)-guided drainage, and one underwent open laparotomy debridement. Mean follow-up was 24 months. One late death occurred but was unrelated to reinfection. All patients seemed well, with no evidence of EVAR graft infection at a mean follow-up of 23.6 months. Conclusions: This small multi-institutional study summarizing the experiences of patients with an infected aortic aneurysm managed by EVAR and an aggressive antibiotic strategy revealed that this EVAR strategy might be a suitable approach to treating this disease. These favorable results may be typical for Salmonella infection, which was present in most of the patients. Persistent sciatic artery (PSA) is a rare congenital malformation. In the early embryonic stage, the sciatic artery is the major blood supply for the lower limb bulb and is later replaced by the iliofemoral artery as the limb develops. Its failure to regress, sometimes associated with femoral arterial hypoplasia, and therefore becoming the dominant inflow to the lower extremity is called PSA. This anomaly is often associated with a higher rate of aneurysm formation or thromboembolic complications causing lower extremity ischemia. Here, we describe a 79-year-old male patient who presented with acute left lower extremity ischemia. He was treated initially with conventional embolectomy through inguinal and popliteal incisions. The bilateral PSA with thrombosed aneurysms was not identified at first on computed tomographic angiography. It was later diagnosed intraoperatively due to the discontinuity of the superficial femoral artery and popliteal artery found with embolectomy catheter, and was managed successfully with ePTFE graft bypass. Background: Surgical treatment for mycotic aortic aneurysms is not optimal. Even with a large excision, extensive debridement, in situ or extra-anatomical reconstruction, and with or without lifelong antibiotic treatment, mycotic aneurysms still carry very high mortality and morbidity. The use of endovascular aneurysm repair (EVAR) for mycotic aortic aneurysms simplifies the procedure and provides a good alternative for this critical condition. However, the question remains: if EVAR is placed in an infected bed, what is the outcome of the infection? Does it heal, become aggravated, or even cause a disastrous aortic rupture? In this study, we tried to clarify the risk factors for such an adverse response. Methods: A literature review was undertaken by using MEDLINE. All relevant reports on endoluminal management of mycotic aortic aneurysms were included. Logistic regressions were applied to identify predictors of persistent infection. Results: A total of 48 cases from 22 reports were included. The life-table analysis showed that the 30-day survival rate was 89.6% ± 4.4%, and the 2-year survival rate was 82.2% ± 5.8%. By univariate analysis, age 65 years or older, rupture of the aneurysm (including those with aortoenteric fistula and aortobronchial fistula), and fever at the time of operation were identified as significant predictors of persistent infection, and preoperative use of antibiotics for longer than 1 week and an adjunct procedure combined with EVAR were identified as significant protective factors for persistent infection. However, by multivariate logistic regression analysis, the only significant independent predictors identified were rupture of aneurysm and fever. Conclusions: EVAR seems a possible alternative method for treating mycotic aortic aneurysms. Identification of the risk factors for persistent infection may help to decrease surgical morbidity and mortality. EVAR could be used as a temporary measure; however, a definite surgical treatment should be considered for patients present with aneurysm rupture or fever. [Copyright &y& Elsevier]

Objectives: The aim of the current study was to evaluate the effect of recipient age on the regenerative response to implantation with young skeletal myoblasts (SKMCs) after a coronary artery ligation. Background: In contrast with previous findings in animals, the initial clinical trials of cell transplantation after a myocardial infarction have reported only limited improvements in ventricular function. The restricted regenerative capacity of cells isolated from older patients is certainly a factor; however, the present study investigated the impact of another potentially significant factor: recipient age. Methods: We compared the myogeneic capacities of SKMCs isolated from young rats (3 months old) and older rats (24 months old). Highly myogenic SKMCs derived from young rats (or culture media, in control rats) were then transplanted into the infarcted myocardium of young and older recipients at 1 week after coronary ligation. Results: In vitro, proliferation and myotube formation were significantly greater in SKMCs derived from young rats than from older rats. In vivo, young and older recipients of SKMCs exhibited increases in cell density, vascular density, and collagen preservation relative to age-matched control animals. However, cell therapy produced significantly greater functional improvements in young recipients than in older, along with relative increases in stem cell factor, cell density, cell survival, and angiogenesis. Conclusions: Functional improvement after the post–myocardial infarction implantation of young SKMCs was limited in older recipients, likely due to reductions in their cardiac and systemic responses to cell transplantation. Abstract: Background:: Anticoagulation is an essential part ofthe management of numerous cardiovascular conditions. Although warfarin sodium has been in clinical use for >40 years, only 1 proprietary warfarin preparation was in use for long-term anticoagulation in Taiwan until 2001, when the Taiwan Food and Drug Administration approved a new generic formulation. Objective:: This study evaluated the efficacy and tolerability of switching from an established dose of the branded warfarin sodium product (in mg/wk) to the same dose of the generic product. Methods:: This was a randomized, observer-blinded,crossover study in Taiwanese patients who received a mechanical heart valve to replace an original valve or a previously implanted mechanical valve from March 2003 to August 2004 and had received branded warfarm therapy for ≥2 months postoperatively, with a mean baseline international normalized ratio (INR) in the range from 1.7 to 3.0 in the 8- to 12-week run-in period. Patients were randomized to 1 of 2 treatment sequences, each involving three 28-day periods: the generic product for 1 period followed by the branded product for 2 periods, or the branded product for 1 period followed by the generic product for 2 periods. Prothrombin time and INR were measured, and anticoagulant activity was compared. At each visit, treatment-emergent adverse events were recorded and evaluated by the blinded observer, an internal medicine physician who was not involved in patients'' care. Results:: Thirty-five patients were enrolled (25 men, 10 women; mean age, 52.4 years [range, 34.5–74.4 years]). All but 2 of the patients had received a replacement for a natural heart valve. Thirty-four patients (16 who received the generic product initially, 18 who received the branded product initially) completed the study without a dose change and were ≥75% compliant with therapy. There was no difference in pooled mean (SE) INR between the 2 products (2.28 [0.06] and 2.27 [0.06], respectively). The 90% CI for the difference was 96.4 to 104.9, well within the range for bioequivalence (80–120). There were no differences in the adverse-event profiles of the 2 formulations. Conclusions:: No therapeutic inequivalence wasdemonstrated in this small, single-blind study, suggesting that the generic and branded warfarin products studied were equally effective in maintaining anticoagulation in patients with mechanical prosthetic heart valves. Patients suffering from end-stage renal disease usually receive dialysis therapy. The arteriovenous (AV) shunt is a vital vascular access for achieving sufficient blood flow in the vein during hemodialysis and can be either an arteriovenous fistula (AVF) or an arteriovenous graft (AVG). However, the lumens of dialysis accesses are frequently narrowed by thrombosis, resulting in stenosis at the venous anastomosis site or progression of inflow stenosis at the arterial anastomosis site. A narrowed vascular wall will produce abnormal physical stress and cause turbulent flow and high blood pressure. Hence, murmur sounds will occur around the stenotic site. The auscultation method is a noninvasive technique to detect these sounds as phonoangiograph (PAG) signals. The empirical mode decomposition (EMD) method is used to decompose intrinsic fast and slow oscillation components from PAG signals. In this study, the slow oscillation component containing key spectral energy distributions (< 100, 100–300 Hz, and > 300 Hz) will be used to identify the normal and abnormal conditions for further stenosis level assessment. After extracting key frequency-based features using EMD, 1D convolutional and pooling processes, feature patterns can be extracted from spectral patterns, which can distinguish distinct feature patterns and reduce the dimensions of feature patterns and the number of feature datasets for training the classifier. Next, a convolutional neural network-based classifier is used to assess the stenosis levels at a near venous anastomosis site. Its model can solve nonlinear mapping applications and nonlinear separable classifications, including the normal condition, AVG stenosis, and AVF stenosis. The experimental tests with cross-validation will indicate that the proposed method provides a promising result in clinical trials, including mean recall (%), mean precision (%), mean accuracy (%), and a mean F1 score of 95.35 %, 89.49 %, 86.82 %, and 0.9232, respectively, by offering an automatic procedure for AV shunt stenosis assessment without the need for manual feature extraction and classification. Surgical repair of a left ventricular pseudoaneurysm is challenging. Percutaneous closure with an AMPLATZER Septal Occluder (AGA Medical Corporation, Golden Valley, MN) has evolved to be an effective alternative to conventional surgery with less complication. We report a case of severe hemolytic hyperbilirubinemia and acute renal failure after percutaneous left ventricular pseudoaneurysm endoprothesis repair, which required conventional surgical repair through a left thoracotomy to treat this complication. The rationale and results of such an off-the-label usage of AMPLATZER Septal Occluder were reexamined. Residual leakages were not rare findings in the reviewed literature. The bi-atrial approach “in-situ” technique of vertical confluence to the left atrial repair avoids the risks of anastomosis site kinking and twisting for infracardiac-type total anomalous pulmonary venous connection reconstruction, but how to obtain the adequate left atrial orifice size is an important issue. Herein, we describe our vertical confluence vein flap with the modified T-shaped left atrium incision technique in managing those patients with infracardiac type total anomalous primary venous reconstruction. Anatomical limitations have been identified as barriers to the more widespread application of thoracic endovascular aortic repair. Here, we report a case in which a novel technique was used as a solution for an extremely dilated aortic landing zone. An elderly gentleman in profound shock was diagnosed with a ruptured distal aortic arch aneurysm. Open repair was considered, but owing to the presence of multiple comorbidities and an extremely dilated aorta in the landing zone (45 mm) which made thoracic endovascular aortic repair infeasible, it appeared to entail a high risk for perioperative morbidity and mortality. Therefore, to save the patient''s life, we designed a novel double-barreled cannon thoracic endovascular aortic repair method, and further developed a formula for choosing appropriate stent graft sizes. A 1.9-kg premature boy with transposition of the great arteries, ventricular septal defect, and patent ductus arteriosus received a Jatene procedure at 16 days of age. His coronary artery pattern was type A. His arteries were harvested and translocated to appropriate holes in the sinus portion of his neoaorta. Partial obstruction due to torsion of the translocated right coronary artery was suspected, because the right ventricle turned pink in color to blue and bradycardia developed when cardiopulmonary bypass support was weaned. This was relieved by clockwise rotation of the heart, and the patient recovered well. Follow-up echocardiography 6 months later revealed good biventricular function. [Copyright &y& Elsevier]

Objective The handmade expanded polytetrafluoroethylene (ePTFE) trileaflet-valved conduit could potentially be used as a substitute pulmonary valve replacement material, especially in children. The current study investigated (1) the function of the ePTFE trileaflet-valved conduits in an ex vivo experimental system and (2) the short-term performance of the conduit in a porcine model to verify its clinical applicability. Methods The competency of the ePTFE trileaflet-valved conduits was estimated through ex vivo (using a pulmonary mock circulation loop) and in vivo (in a porcine model with a damaged pulmonary valve) experiments. Explants were examined by gross morphology and histopathologic examination. Results In the ex vivo experiment, the ePTFE trileaflet-valved conduits were determined to effectively increase mean pulmonary pressure from 10.2 to 14.4 mm Hg compared with defective silicon-valved conduits. In addition, the regurgitation fraction value of ePTFE trileaflet-valved conduits was 15.9% to 18.1%, which was significantly better than the defective valve conduits (regurgitation fraction = 73.5%-85.7%). In the in vivo experiment, the valved conduits were confirmed to be with good valve position maintenance, and the valve and leaflets showed no signs of thickening or peeling after a short-term implantation period. There were also no significant signs of inflammation reaction on histopathologic examination. Conclusions The ePTFE trileaflet-valved conduits for pulmonary valve reconstruction showed acceptable performance and outcomes in the ex vivo and in vivo experiments. We describe a new method for aortic anastomosis in the repair of acute aortic dissection. The anastomosis sites are prepared and sutured in an interrupted horizontal mattress manner with telescoping technique. By this kind of method, the interrupted sutures decrease damage to the fragile aortic wall and the antegrade blood flow promotes anastomotic sealing. Sano''s modification of the right ventricle to main pulmonary artery shunt improves the surgical results of the Norwood procedure for hypoplastic left heart syndrome. Usually the transected distal stump of the main pulmonary artery is reconstructed by direct suturing of a polytetrafluoroethylene tube or suturing on a pericardial bifurcation patch. However, the resulting pulmonary artery geometry significantly affects the outcome of the staged procedure. Herein, we report our surgical strategy to improve the pulmonary artery geometry by using a commercially available Venaflo II graft (Bard, Tempe, AZ) as the right ventricle to main pulmonary artery conduit, to avoid the potential for pulmonary artery stenosis after the Norwood procedure. Although cerebral hyperperfusion syndrome is rarely encountered, this condition can be serious after procedures that increase cerebral blood flow; most reports are related to carotid revascularization. Sharp cerebral hemodynamic changes, along with impaired cerebrovascular autoregulation, are the main mechanisms contributing to this syndrome. We report a patient who underwent surgical correction for congenital supravalvular aortic stenosis and presented with postoperative hypertension, headache, impairment of consciousness, and cerebral edema 2 days after operation. He recovered well when the blood pressure declined and the brain edema subsided. Cerebral hyperperfusion syndrome was considered, and this uncommon neurologic complication of cardiac operations is discussed. [ABSTRACT FROM AUTHOR]

We report a rare case that manifested as severe aortic regurgitation (AR) resulting from spontaneous aortic valve (AV) rupture caused by valvular myxomatous transformation in a middle-aged woman. Before operation, 1 ruptured hole on the left coronary cusp (LLC) was clearly visualized with transesophageal echocardiography (TEE). There were no clinical findings of vegetations or sinus Valsalva aneurysms. The patient was treated successfully with mechanical AV replacement. Prompt imaging evaluation is mandatory in this rare critical situation for correct diagnosis and appropriate management. [Copyright &y& Elsevier]

Abstract: Ruptured abdominal aortic aneurysm is a true emergency for emergency physicians and surgeons. Achieving effective proximal control may ameliorate further hemodynamic deterioration and buy time for patients awaiting further repair. An 82-year-old man was referred to our hospital with shock resulting from a ruptured abdominal aortic aneurysm. At the moment of impending cardiac arrest, aortic occlusion was achieved with a transfemoral endovascular balloon, without fluoroscopic guidance. The octogenarian then underwent a prosthetic graft reconstruction and recovered well. In this report, the safeguards and pitfalls of aortic occlusion using an endovascular balloon are discussed. Prospective electrocardiography (ECG)-gated cardiac computed tomography angiography (CTA) is widely used for pediatric patients with congenital heart disease (CHD) due to the lower radiation dose compared with the ECG-gated technique. However, functional parameters acquired using ECG-gated cardiac CT to predict pulmonary hypertension (PH) in children with CHD have not yet been reported. This study aimed to investigate the potential of diastolic and systolic right ventricular diameters (RVD) on prospective ECG-gated cardiac CTA to predict PH in children with CHD. A total of 44 children with CHD were divided into two groups: CHD with PH (n = 22) and CHD without PH (n = 22). The association between ECG-gated CTA parameters and PH was evaluated by logistic regression. The receiver operating characteristic curve (ROC) was used to find the best cut-off point for the parameters measured by Youden's index. Patients with higher RVD-BSA [aOR (95% CI) diastolic: 2.76 (1.23–6.23); systolic: 6.15 (1.72–22.06)] had higher risk of PH after adjusting for age and patent ductus arteriosus. The area under the curve (AUC) of D-RVD-BSA was 0.907 and the AUC of S-RVD-BSA was 0.917. Logistic regression showed that patients with D-RVD-BSA over 6.86 or S-RVD-BSA over 5.87 had significantly higher risk of PH after adjustments (aOR = 23.52, 95% CI = 2.89–191.03; aOR = 31.14, 95% CI = 2.75–352.85). In conclusion, in children with CHD, measurements of diastolic or systolic BSA-modified RVDs on prospective ECG-gated CTA are non-invasive markers of PH. BSA-modified D-RVD of 6.86 or BSA-modified S-RVD of 5.87 may be used to identify PH in children with CHD. • In children with PH, an association is found between ECG-CTA-gated image parameters and PH. • Diastolic-RVD & systolic-RVD are associated with pulmonary arterial systolic pressure. • Patients with D-RVD-BSA over 6.86 had significantly higher risk of PH. • Patients with S-RVD-BSA over 5.87 had significantly higher risk of PH. • The AUCs of diastolic-RVD and systolic-RVD were 0.907 and 0.917, respectively. Abstract: Though promazine and chlorpromazine elicited cutaneous anesthesia, no study of spinal anesthesia with chlorpromazine and promazine has been reported. This study was to examine whether chlorpromazine and promazine produce spinal anesthesia. Using a rat model via intrathecal injection, we tested spinal blockades of motor function and nociception by promazine, chlorpromazine or bupivacaine, and so were dose–response studies and durations. We demonstrated that chlorpromazine and promazine elicited dose-dependent spinal blockades in motor function and nociception. On the 50% effective dose (ED50) basis, the rank of potency of these drugs was bupivacaine>promazine>chlorpromazine (P <0.05 for the differences). On an equipotent basis (25% effective dose [ED25], ED50, and ED75), the block duration caused by chlorpromazine or promazine was longer than that caused by the long-lasting local anesthetic bupivacaine (P <0.01 for the differences). Chlorpromazine and promazine, as well as bupivacaine, showed longer duration of sensory block than that of motor block. Our data reported that intrathecal promazine and chlorpromazine with a more sensory-selective action over motor blockade had less potent and longer-lasting spinal blockades when compared with bupivacaine.
