Abstract

Guidelines recommend cisplatin over carboplatin for treatment of advanced urothelial carcinoma since 2008. This recommendation is based on (a meta-analysis of) two small RCTs, one with a questionable censoring approach. Secondary analysis of individual patient data from these RCTs did not demonstrate overall survival benefit from cisplatin over carboplatin. Considering lower toxicity and larger population eligibility for carboplatin, guideline recommendations should be reconsidered. Introduction: For many years EAU guidelines have recommended the use of cisplatin-based regimens over carboplatin for treatment of advanced urothelial cell carcinoma (UCC) in eligible patients. The claim of an overall survival (OS) benefit is based on (a meta-analysis of) 2 RCTs totalling 190 patients, of which one study has methodological flaws. These studies warrant secondary analysis to substantiate the evidence for an OS benefit of cisplatin-versus carboplatin-based regimens. Patients and Methods: Individual patient data (IPD) were reconstructed from the 2 RCTs, assessing OS in both treatment arms. IPD of both studies were then jointly reanalysed to assess an OS estimate with Kaplan-Meier methods, with, and without an alternative censor ing scenar io to assess the impact of the original biased censoring approach. Kaplan-Meier curves were compared by calculating restricted mean survival time (RMST) differences. Results: In each study individually, and in both studies combined, the survival benefit of cisplatin versus carboplatin was less than 1 month and not significant in a follow-up window of 12 months. This was also the case when an alternative censoring scenario was applied. Conclusion: Careful scrutiny of the data on which guidelines base the recommendation of cisplatin-based chemotherapy for the treatment of advanced UCC does not uphold the finding that cisplatin leads to an OS benefit when compared to carboplatin. Abstract

Body composition has been associated with disease outcome in several cancer types. Results for localized and metastatic renal cell cancer (RCC) are limited and inconsistent. Our aim was to examine the association between body composition and survival in RCC. We conducted a population-based historical cohort study including patients diagnosed with RCC from 2008 to 2012. Diagnostic Computed Tomography images at the third lumbar vertebra (L3) were assessed for skeletal muscle index (SMI), skeletal muscle density (SMD), visceral adipose tissue index (VATI) and subcutaneous adipose tissue index (SATI). Clinical data was retrieved from medical records. Multivariable Cox regressions with restricted cubic splines were used to determine hazard ratios (HRs) and 95% confidence intervals (95%CIs) for 10-unit increases in body composition features with overall survival (OS) and recurrence-free survival (RFS). We included 719 stage I-III (of whom 254 (35.3%) died and 148 (21.9%) experienced recurrence) and 320 stage IV RCC patients (of whom 298 (93.1%) died). Median follow-up was 6.35 years (interquartile range; 1.41–8.23). For stage I-III, higher SMD was associated with better OS (men: HR 0.86; 95% CI 0.68–1.08; women: HR 0.69; 95% CI 0.50–0.95). Lower compared to median VATI was associated with worse OS for both men (HR 1.38; 95%CI 1.05–1.83 for VATI = 25) and women (HR 1.67; 95%CI 1.01–2.78 for VATI = 20). For stage IV, higher SMD and higher VATI were associated with better OS among men (HR 0.74; 95% CI 0.59–0.94 and HR 0.93; 95% CI 0.88–0.99, respectively). Results for women were similar but non-significant. No statistically significant associations were found for SMI or SATI. Higher SMD and higher VATI were marginally associated with better survival in RCC patients and might be useful for better prognostication. Abstract

The American Academy of Dermatology (AAD) has been sponsoring free skin cancer screening clinics in the United States since 1985. In 1989 and 1990, free skin cancer screening clinics were established in the Netherlands according to the guidelines established by the AAD. This article describes the experience of these clinics. A total of 2,564 people were screened for skin cancer in 7 different clinics located in Oss and Arnhem. One dermatologist could examine between 150 and 200 people in one day. Ten percent of the people who were screened were identified as candidates for having cancerous or precancerous conditions, and four percent were suspected of having skin cancer. All of the patients who were suspected of having skin cancer were referred to their family physicians for further testing. Ninety percent of the patients who were told to seek further medical attention actually did so, but 10 percent did not. More than half (57 percent) the referred patients who sought further medical attention were diagnosed as having skin cancer. The overall incidence of skin cancer among the 2,564 participants in the screening clinics was 2.1 percent. The total cost of running the clinics, including follow-up visits to doctors' offices, was $6,000. An effective skin cancer screening clinic should include public education, an organized screening session, and mandatory follow-up visits for those suspected of having cancer. Abstract: Iran is rapidly becoming an “ageing society” with a related increase in cancer incidence including breast cancer. This paper evaluates the trend in breast cancer incidence from the past to the present, in order to predict the future burden in Iran and to quantify the effect of changes in known risk factors on incidence over time. Currently, breast cancer incidence in Iran is low with approximately 5000 new cases annually. Under conservative assumptions, the number of new cases of breast cancer in 2030 will be more than 15000. In addition to demographic factors, changes in the prevalence of established risk factors such as reproductive factors and obesity are likely to result in changes in breast cancer patients over time. Extrapolating the increasing prevalence of obesity to the future, we expect that this specific factor will strongly contribute to the increased breast cancer incidence in the future unless preventive measures counteract this effect. Abstract: Objectives: In muscle invasive bladder cancer (MIBC), careful clinical staging is essential for patient counseling and decision-making. Bimanual palpation (BP) is an integral part and guideline advice of clinical staging. Until now, however, the value of BP has never been studied. With this study, we aim to determine the accuracy of clinical staging through BP. Methods: Detailed clinical data were collected from a population-based series of 1,409 patients with MIBC, diagnosed between 1989 and 2005, in the region of the Comprehensive Cancer Centre East in The Netherlands. Selected were all patients who underwent BP (n = 738). Preoperative tumor-stage (cT-stage) determined through BP was compared with post-cystectomy pT-stage. Contingency tables were made to determine the correlation between cT-stage and pT-stage. Results: In 18 of 142 patients in whom BP showed an organ-confined tumor, the tumor was unresectable (pT4) at the time of surgery. Four out of 9 patients who had a suspected T4 tumor on BP but who underwent cystectomy anyway appeared to have operable tumors at cystectomy. In 87 patients (57.6%), accurate staging through BP was observed. In 17 patients (11.3%), clinical overstaging was found, and in 47 patients, (31.1%) clinical understaging. Conclusions: Frequently, pT-stage after cystectomy does not correlate with preoperative cT-stage based on BP. Discrepancy was observed in 42% of the patients: in 11%, clinical overstaging and in 31%, clinical understaging. Based on these data, some caution is suggested when interpreting the outcome of BP. Prospective data is needed for a more formal evaluation of the staging accuracy of BP. [Copyright &y& Elsevier]

Abstract: Aim: E-cadherin plays a role in carcinogenesis. For two genetic polymorphisms in the gene (CDH1) promoter, a reduced transcription has been reported: a C/A single nucleotide polymorphism (SNP) and a G/GA SNP at −160 bp and −347 bp, respectively, upstream of the transcriptional start site. We studied the association between both polymorphisms and the risk of bladder cancer. Methods: One hundred and ninety-seven patients with bladder cancer and 344 population controls were genotyped and haplotyped for both SNPs. Results: A borderline significantly increased risk for bladder cancer was found for A allele carriers (OR 1.36; 95% CI: 0.96–1.94). We did not find any association between the –347 G/GA SNP and bladder cancer. Haplotype analyses did not yield much stronger associations with bladder cancer than the –160 C/A genotype analyses. Conclusion: This study supports earlier suggestions that the –160 C/A SNP in the CDH1 promoter is a risk factor for bladder cancer. [Copyright &y& Elsevier]

Abstract: Objectives: Frequently, statistically significant prognostic factors are reported in published studies with suggestions that disease management should be modified. However, the clinical relevance of such factors is rarely quantified. We evaluated the accuracy of predicting the need for invasive treatment among patients with benign prostatic hyperplasia treated conservatively with alpha1-blockers. Methods: Information on eight prognostic factors was collected from 280 patients treated with alpha1-blockers. Using the proportional hazards regression coefficients, a risk score for retreatment was calculated for each patient. The analyses were repeated on 1000 groups of 280 patients sampled from the original case series. The results from these “bootstrap analyses” were compared with the original results. Results: Three statistically significant predictors of retreatment were identified. The 20% of patients with the greatest risk score had an 18-month risk of retreatment of only 20% (this should ideally approach 100%). Analyses of less than one half of all the bootstrap samples resulted in the same three significant prognostic factors. The 20% of patients with the greatest risk score in each of the 1000 samples experienced a highly variable risk of retreatment of 0% to 42%. Conclusions: Strongly significant predictors for retreatment suggest the need for a change in disease management, but 4 of the 5 high-risk patients would be overtreated with a modified policy. The subclassification of patients with a relatively low risk and high risk of retreatment appeared far from accurate. Internal validation procedures may warn against the invalid translation of statistical significance into clinical relevance. [Copyright &y& Elsevier]

Because it is not possible to predict which patient will have fertility problems after treatment, cryopreservation should be offered to every testicular cancer patient. Abstract

Although level I evidence is lacking that radical cystectomy (RC) is superior to bladder-preserving therapy (BPT), RC is still advocated as the recommended treatment in patients with nonmetastatic muscle-invasive bladder cancer (MIBC). This study sought to compare the survival of patients with MIBC treated with BPT versus those treated with RC. All patients with nonmetastatic MIBC diagnoses were identified via the population-based Netherlands Cancer Registry. Only patients treated with BPT or RC were included. The primary endpoint was 2-year disease-free survival (DFS), defined as time from start of treatment until locoregional recurrence, distant metastasis, or death. The secondary endpoint was overall survival (OS). Inverse propensity treatment weighting (IPTW) was used based on propensity scores to adjust for baseline differences between treatment groups. Survival was analyzed with Kaplan-Meier and Cox proportional hazards models. A total of 1432 patients were included, of whom 1101 underwent RC and 331, BPT. Median follow-up was 39 months (range, 27-51 months). The IPTW-adjusted 2-year DFS was 61.5% (95% CI, 53.5%-69.6%) with BPT and 55.3% (95% CI, 51.6%-59.1%) with RC, with an adjusted hazard ratio of 0.84 (95% CI, 0.69-1.05). The adjusted 2-year OS for patients treated with BPT versus RC was 74.0% (95% CI, 67.0%-80.9%) versus 66.0% (95% CI, 62.7%-68.8%), respectively, with an adjusted hazard ratio of 0.80 (95% CI, 0.64-0.98). There was no statistically significant difference between the 2-year DFS of patients treated with BPT and RC.
