This study aims to describe childhood SS (cSS) features to help guide clinicians in their consideration of and workup for cSS. We retrospectively reviewed medical records of patients with cSS referred to three Italian pediatric rheumatology centers from 2015 to 2019 and we conducted a literature review of cSS. Statistical analysis was performed to detect associations between clinical/laboratory features. We reviewed 12 cases (9 female) followed in 3 Italian centers and 240 cases (191 female) in the published literature reporting individual information. The median age at disease onset was 10 years for both cohorts. The most frequently reported clinical SS-specific feature was parotitis in both cohorts (67% each). Extraglandular manifestations were very common and joint involvement was the most frequent. In the cluster analysis, we identified a significant association between parotitis and younger patients (< 11 years). We verified the presence of the main SS features (exocrine gland inflammation, exocrine gland dysfunction, and presence of autoantibodies) in the Italian cohort and the literature review-based cohort: 92% and 80% of the cohorts, respectively, had at least 2/3 main characteristics. We described cSS features with relative frequencies and we found that parotid involvement was related to cSS in younger patients. The majority of patients showed various combinations of exocrine gland inflammation, exocrine gland dysfunction, and presence of autoantibodies giving a theoretical basis for future research to pave the way for the development of cSS specific diagnostic criteria. Technical advancements in the digital era have eased up the time required to complete complex tasks by a decisive rate. These advancements introduce several threats. Hence, there is a compelling need to provide security for systems against these threats. Identity and data thefts are the major threats which require immediate, effective and responsive solutions. Face recognition systems have evolved over the last few years, providing a suitable solution for authenticating an individual. However, the system running on facial recognition can be interfered by disguise. This paper discusses the approaches used in detecting a disguise and analysing the captured face among the other non-disguised faces in the dataset. The parameters used for detection and analysis are the keypoints of the face. These keypoints may be blocked, hidden or disoriented due to the presence of props on the face. The proposed approach aims to avoid this interference by creating an estimate of the face based on fewer available keypoints. The estimated face so created, is then matched with the available set of faces to determine the probabilistic chance of the matched person's presence even though a disguise had been detected. Thus the solution provides a clear distinction if the image obtained proves to be of an impostor or an intruder with obfuscation with a said probability. The physical demands on evacuators were investigated when using different types of sled-type stair descent devices designed for the emergency evacuation of high rise buildings. Twelve firefighters used six sled-type stair descent devices during simulated evacuations. The devices were evaluated under two staircase width conditions (1.12, and 1.32 m). Dependent measures included electromyographic (EMG) data, heart rates, Borg Scale ratings, and descent velocities. All stair descent speeds were below those reported during pedestrian egress trials. With the exception of the inflatable device, the devices operated by two evacuators had higher descent speeds than those operated by a single evacuator. High friction materials under the sleds facilitated control and reduced the muscle demands on stairs but increased physical demands on the landings. Usability assessments found devices with shorter overall lengths had fewer wall contacts on the landing, and handles integrated in the straps were preferred by the evacuators. [ABSTRACT FROM AUTHOR]

The physical demands on firefighting personnel were investigated when using different types of track-type stair descent devices designed for the emergency evacuation of high rise buildings as a function of staircase width and evacuation urgency. Twelve firefighters used five track-type stair descent devices during simulated urgent and non-urgent evacuations. The devices were evaluated under two staircase width conditions (1.12, and 1.32 m), and three devices were also evaluated under a narrower staircase condition (0.91 m). Dependent measures included electromyographic (EMG) data, spine motion, heart rates, Borg Scale ratings, task durations and descent velocities. Stair descent speeds favored the devices that had shorter fore/aft dimensions when moving through the landing. EMG results indicated that there were tradeoffs due to design features, particularly on the landings where the physical demands tended to be greater. Abstract: Twisting and lateral bending motions in repetitive lifting tasks are associated with occupational low back injuries and can be challenging to reduce with engineering controls. This study tested the hypothesis that twisting and lateral bending can be reduced by changing the transfer distance. Eighteen males, with no material handling experience lifted 10.9 kg boxes from 0.9 m above the floor and placed the boxes at a destination located 0.50, 0.75, 1.00, 1.25, 1.50, or 1.75 m away and at heights of 0.5 m, 0.9 m, and 1.3 m above the floor. Overall, twisting and forward bending decreased with increased transfer distance when placing the box. Conversely, the lateral bending when lifting and placing the box increased with increasing transfer distance. In short, having a transfer distance between 1 and 1.25 m when performing palletizing tasks to different heights may optimally balance spine kinematics, back injury risk, and productivity measures. Abstract: INTRODUCTION: Haemangioma of ovary is a rare tumour. PRESENTATION OF CASE: We report an ovarian haemangioma which presented as an acute abdomen due to an adnexal mass. DISCUSSION: We report an ovarian haemangioma which presented as an acute abdomen due to an adnexal mass. Haemangiomas have been reported in other organs but ovarian haemangioma is a rare tumor, usually asymptomatic and presenting as an incidental finding. Large lesions tend to present clinically with pain. A few cases have been described in the literature. CONCLUSION: Considering their rare occurrence such tumors are a diagnostic challenge. Abstract: Mezzanine surfaces can be made from concrete, bar grate, or composite materials. Anecdotal data indicate that mezzanines in distribution centers made from composite materials, due to their increased compliance, may be a more comfortable working surface. Prior research suggested that a measure of tibial shock, peak tibial acceleration, could potentially discriminate the biomechanical differences between these surfaces. The objective of this study was to quantify differences in tibial accelerations as 27 people walked on mezzanines constructed from concrete and a wood composite material. Accelerometers were attached bilaterally to the shins of volunteers, and data were collected as they walked 30.5 m on each surface at their normal walking speed, a faster-than-normal walking speed, and a slower-than-normal walking speed. Peak acceleration values obtained from the leg with the highest values were compared. On average, the peak acceleration values were 5% higher on the concrete mezzanine as compared with the wood composite mezzanine (p = .036). These findings suggest that individuals working on mezzanines in distribution centers constructed from composite surfaces would potentially experience less discomfort associated with long exposure periods on these surfaces. We sought to assess the accuracy of a novel parameter proportional to the rod shear stress (RSS) in identifying patients at risk of rod fracture (RF) after surgery for correction of adult spinal deformity. We performed a retrospective medical record review of patients aged ≥18 years treated for adult spinal deformity between 2004 and 2014 with ≥24 months of follow-up. The primary outcome was RFs identified radiographically. Patient weight (w), number of instrumented levels (N), and minimum rod diameter (d) were recorded and used to calculate the RSS parameter (R S S = N w d 2 ). Receiver operating characteristic curves were produced and the area under the curve (AUC ± 95% confidence interval [CI]) was calculated to compare this parameter's discriminative accuracy to that of its constituent variables. The sensitivity, specificity, and likelihood ratios (LRs) were calculated. A total of 28 RF-positive and 154 RF-negative patients were included. The average age was 59.2 ± 9.6 years, and 93.4% were women. The RSS parameter produced the greatest AUC (0.73 ± 0.11). At an RSS cutoff of 30.1, it achieved a sensitivity of 71.4% and specificity of 71.4% (LR, 2.5; 95% CI, 1.8–3.5). The number of instrumented levels produced the next-greatest AUC (0.65 ± 0.12), with a sensitivity of 78.6% and specificity of 50.0% at a cutoff of 15 (LR, 1.6; 95% CI, 1.2–2.0). The RSS is calculated using easily obtainable information and shows potential as a tool for predicting patient-specific risk of RF after spinal fusion. The number of instrumented levels also correlates strongly with the occurrence of RFs and is not significantly less accurate than the RSS. A larger sample size and prospective validation would be useful in determining with greater confidence which parameter is superior for predicting RFs after spinal fusion. [ABSTRACT FROM AUTHOR]

Abstract: The physical demands on firefighting personnel were investigated when using different types of hand-carried stair descent devices designed for the emergency evacuation of high rise buildings as a function of staircase width and evacuation urgency. Twelve firefighters used three hand-carried stair descent devices during simulated urgent and non-urgent evacuations. The devices were evaluated under three staircase width conditions (0.91, 1.12, and 1.32 m). For comparison, an urgent manual carry was also performed on the 1.12 m wide stairs. Dependent measures included electromyographic (EMG) data, heart rates, Borg Scale ratings, task durations and descent velocities. Results indicated that the stair chair with extended front handles, which allows the front person to descend the stairs facing forward, reduced the time integrated back muscle EMG by half and showed a descent velocity that was 1.8 times faster than the other stair descent devices in the study. Follicular dendritic cell (FDC) sarcomas are rare mesenchymal tumours, which are fatal in 20% of the patients and usually occur in secondary lymphoid organs or extranodal localizations. Due to the rareness of these tumours, only few studies have been conducted on molecular level. In the present study, we performed microRNA (miRNA) profiling of 31 FDC sarcomas and identified two subgroups, one with high miRNA expression and the other group with low miRNA expression levels. The first group showed a strong similarity to fibroblasts and myopericytomas, whereas the second group was more closely related to FDCs from Castleman's disease. Both groups showed important differences compared with myeloid-derived dendritic cells, confirming mesenchymal origin of FDCs and their derived sarcomas. The two FDC sarcoma groups did not differ on morphological grounds, mitotic activity or BRAF mutation status. However, patients of group I presented a tendency to a shorter overall survival and more frequent podoplanin expression by immunohistochemistry. The importance of these newly recognized FDC sarcoma subgroups in terms of clinical behaviour and therapeutic implications should be assessed in a larger cohort in future studies. Background: Although matrix metalloproteinase (MMP) activity increases, endothelial function decreases after myocardial infarction (MI). The antibiotic doxycycline inhibits MMP activity in vitro. The role of doxycycline-mediated MMP inhibition in endothelial function is unclear.Hypothesis: Doxycycline ameliorates endothelial dysfunction, in part, by inhibiting MMP activity.Methods: We subjected Sprague-Dawley male rats to MI by ligating the left anterior descending arteries. We subjected another group of rats to sham surgery. We administered doxycycline in drinking water (0.67 mg/ml) to both groups 2 days before surgery: the sham group underwent sham surgery and received doxycycline therapy, and the MI group underwent MI and received doxycycline therapy (n = 6 in each group). After 4 weeks, we anesthetized rats and prepared left ventricular rings from infarcted-ischemic (I), non-infarcted near-infarcted (NI), and sham surgery hearts with and without doxycycline treatment.Results: The MMP-2 activity increased significantly in I and NI hearts, and we observed a selective increase in MMP-9 activity only in I hearts, when compared with other groups (p < 0.05), measured by zymography. Cardiac inhibitor of metalloproteinase decreased only in I hearts (p < 0.05 vs other groups), measured by Western analysis, and doxycycline treatment reversed this decrease. Contractile response of rings to acetylcholine was attenuated in the I group, suggesting nitric oxide–mediated dysfunction, and was reversed by doxycycline. The response to nitroprusside was attenuated in I hearts and ameliorated by doxycycline, suggesting cardiomyocyte dysfunction. Bradykinin induced relaxation in rings from sham surgery hearts and from NI hearts, but induced paradoxic contraction in rings from I hearts. Treatment with doxycycline reversed the paradoxic contraction.Conclusion: Results suggest a protective action of doxycycline in the ischemic heart, possibly because of additional pharmacologic actions such as metalloproteinase inhibition.
