For the suitability assessment of ceramics or refractory materials for tools, crucibles or handling aids in metallurgy and metals processing, e.g., for thixoforming or rheocasting, besides properties such as wetting behaviour, wear resistance, thermal conductivity and thermal shock resistance, mainly the corrosion behaviour against the processed alloy influences the service life of a tool material. Abstract: Headaches are one of the most common medical complaints. The differentiation of benign primary headaches from the small number of patients with secondary headaches may be challenging, but failure to recognize a serious headache can be fatal. We report the case of a 49-year-old renal transplant patient, who was admitted to hospital because of intractable right-sided headaches. Cerebral imaging was unremarkable. Not until 2 days later did the patient develop a rash of grouped vesicles located in the right dermatome C3. Consecutively cerebrospinal fluid tested positive for Varizella zoster virus (VZV), indicating VZV meningitis. Therapy was started with intravenous acyclovir with rapid improvement. Here in we have described an atypical case of VZV reactivation in a renal transplant patient, who initially presented with headaches without any skin manifestation. Because of their compromised immune system, transplant patients have a high risk for visceral involvement of VZV infections, which are a life-threatening emergency. Therefore, vaccination of seronegative patients should be part of the pretransplant workup. Accurate and fast diagnosis of infection is essential to immediately start antiviral therapy. Abstract: Background: Cutaneous vasculitis is a rare symptom after renal transplantation. With a broad spectrum of differential diagnosis, the new appearance of a skin rash in transplanted patients can be challenging. Case Report: We present the case of a 69-year-old man with palpable purpura, skin ulcerations, and diffuse arthralgias. He had a history of cadaveric renal transplantation owing to biopsy-proven isolated immunoglobulin (Ig)A nephropathy and never suffered from any skin manifestation before. Skin biopsy confirmed Henoch–Schoenlein purpura (HSP), which developed under maintenance immunsuppressive therapy with tacrolimus and mycophenolate mofetil. Renal biopsy showed recurrent IgA nephropathy with positive mesangial and capillary IgA staining. Discussion: This is the first case to describe a new manifestation of HSP following renal transplantation owing to isolated IgA nephropathy. Here, we summarize the differential diagnosis of cutaneous vasculitis following renal transplantation. Moreover we give a short review of the recurrence of IgA nephropathy and HSP after renal transplantation followed by possible strategies for prevention and therapy of recurrent disease. L'administration orale de bromélaïne, trypsine et rutine (Oral Enzyme Combination – OEC) est utilisée en Allemagne depuis de nombreuses années pour traiter les arthroses, mais son mécanisme d'action n'est pas entièrement connu. Dans cette étude clinique, nous avons étudié les effets de OEC sur l'inflammation, le métabolisme du cartilage et les symptômes des personnes souffrant d'arthrose fémoro-tibiale. Il s'agit d'une étude croisée exploratoire multicentrique, randomisée, en double aveugle, contrôlée par placebo (PBO) impliquant des patients (âge ≥ 40 ans, IMC ≤ 35 kg/m2) souffrant d'arthrose symptomatique du genou (grade radiologique de Kellgren & Lawrence II–III, douleur au repos ou à la marche ≥ 40 sur une échelle visuelle analogique [EVA]. Les critères d'évaluation exploratoires étaient les changements dans le temps des marqueurs (1) de l'inflammation systémique, (2) de la réponse immunitaire et (3) du métabolisme du cartilage. Le statut algo-fonctionnel était évalué en utilisant le Knee Injury and Osteoarthritis Outcome Score (KOOS). Les traitements étaient administrés pendant deux périodes de 8 semaines. Après la première période de traitement et une fenêtre thérapeutique « wash-out » de 1 mois, les traitements étaient inversés. Les données recueillies après chaque période de traitement ont été analysées à l'aide d'un modèle linéaire mixte généralisé. Les moyennes géométriques ont été utilisées pour comparer les effets inter-groupes avec un intervalle de confiance à 95 %. Au total, 56 patients ont été sélectionnés, dont 45 ont été randomisés (n = 24 femmes, âge moyen/IMC 63 ans/27,4 kg/m2, douleur VAS au repos/à la marche 38,3/56,9, score global KOOS 48,7) et 43 participants ont terminé les deux périodes d'études. OEC augmentait significativement le taux sérique d'IL-10 alors que le placebo n'avait pas d'effet. Au terme des deux périodes de traitement, les taux d'IL-10 étaient plus élevé dans le groupe OEC que dans le groupe placebo (p < 0,001). De même, les taux d'alpha-2-macroglobuline étaient augmenté de manière significative avec OEC (p = 0,038). Le biomarqueur urinaire du cartilage CTX-II était significativement diminué avec OEC (p = 0,038) alors qu'il augmentait légèrement dans le groupe placebo. Les augmentations des scores KOOS « Symptômes » (p = 0,026) et « Douleur » étaient plus importantes dans le OEC que dans le groupe placebo (p = 0,046). Les effets indésirables étaient comparables entre les groupes, et aucun changement cliniquement pertinent dans la biologie clinique et les paramètres hémorhéologiques n'a été observé dans le groupe OEC. Il s'agit de la première étude clinique explorant rigoureusement les mécanismes d'action d'OEC dans l'arthrose. OEC augmente de manière significative l'IL-10, une cytokine anti-inflammatoire, et diminue CTX-II, un marqueur de la dégradation du cartilage. Associés à la réduction des symptômes, ces résultats suggèrent qu'OEC pourrait être un médicament antirhumatismal modificateur de la maladie (DMARD). [ABSTRACT FROM AUTHOR]

Pharmacoresistance is a major burden in the treatment of epilepsy. The search for an effective and well-tolerated antiepileptic drug (AED) is overall achieved by trial and error. This process often turns out to be challenging and irksome for both, the patient and the clinician. Genetic biomarkers can help in the choice of AEDs in order to individualize the treatment. In the framework of the EpiPGX consortium, a European research project on epilepsy pharmacogenomics, 5000 patients were recruited and meticulously phenotyped. Patient samples were supplied by 10 specialized epilepsy centers. More than 500 phenotype items were assessed. Detailed definitions for pharmacogenomics phenotypes such as multiresistance or late response – i.e. response to a second or later given AED, see below – were created. The consistency of the supplied phenotypes across the participating centers was guaranteed by repeated phenotyping workshops and two rounds of validation exercises on a set of 10 anonymized medical records. Mean inter-rater agreement was 82% for AED outcome classification. True misclassifications accounted for only 5–9% of all cases. We identified and phenotyped patients with focal epilepsies that were late responders to specific AEDs. Late response was defined as 12 months of seizure freedom after failure of at least one well-tolerated AED. We identified 338 late responders to levetiracetam, lamotrigine, or lacosamide. Those are currently being analyzed in the setting of a genome wide association study (GWAS) against a much larger group of non-reponders to the same drugs and to a group of normal controls. For idiopathic generalized epilepsies, we are comparing responders and non-responders to lamotrigine and valproate (VPA). For VPA we identified 487 responders and 164 non-responders; for LTG 158 responders and 217 non-responders. Furthermore we identified 53 subjects that were responders only to the combination of VPA and LTG. The results of the GWAS analysis are pending and are expected in February 2015.
