Abstract: Objective: To profile training habits and injuries in football players participating in a national Masters tournament. Methods: A cross-sectional retrospective study design was used to survey male football players attending the 2008 New Zealand Masters Games. Information regarding player demographics, football injuries, football related training, and risk factors for injury were collected. Results: 199 Players were recruited, with a median age of 44 yrs (range 35–73) and a median football playing history of 15 yrs (range 0–66). Irrespective of age, 112 (84%) players included a warm-up and 104 (78%) included a stretching regime in their regular training programme. In the 12 months prior to the tournament, 128 football related injuries were reported by 93 players (64 injuries/100 players or 46 injured players/100 players). The most frequently injured region was the lower limb; specifically the lower leg (n =23), ankle (n =18), hamstring (n =17), knee (n =15), and Achilles tendon (n =15). Conclusion: This study provides a preliminary insight into the training habits and injury profiles of Masters football players. Despite all players including some form of injury prevention strategy in their training, a significant number of players experienced an injury in the 12 months prior to the tournament. The aims of this study were to describe the spectrum of recessively inherited POLG -related disorders, to report new POLG mutations and to discuss genotype-phenotype correlations in order to propose a strategy for diagnosis. Twenty eight patients diagnosed with two POLG mutations at 12 tertiary European centers of adult neurology were studied. Exhaustive phenotypic data, brain MRI, muscle analysis, mitochondrial DNA and POLG analysis findings were collected. Five distinct phenotypes were observed: Sensory Ataxic Neuropathy, Dysarthria and Ophthalmoparesis (SANDO), autosomal recessive Progressive External Ophthalmoplegia (arPEO), Spino Cerebellar Ataxia with Epilepsy (SCAE), Mitochondrial Neuro Gastro Intestinal Encephalopathy (MNGIE)-like phenotype and Sensory Ataxic Neuropathy with Ophthalmoparesis but without dysarthria which we propose to name SANO. An increasing gradient of functional severity was appreciated from PEO with the best prognosis, to SANO, SANDO and finally SCAE respectively. Four new missense mutations were found. Regarding genotype/phenotype correlations, P587L mutation was associated with SANO rather than with SANDO (p < 0.005) and W748S mutation was associated with SANDO or SCAE (with more severe disease progression), rather than with SANO or PEO (p < 0.004). Distinguishing between various phenotypes can have important diagnosis and prognosis implications. POLG mutations should be priority searched for in cases of SANDO or SANO. Mitochondrial respiratory chain and mitochondrial DNA studies should be considered in the case of negative POLG analysis or other phenotypes. [ABSTRACT FROM AUTHOR]

Introduction L’anticorps anti-HMGCR est associé à une forme sévère de myopathie nécrosante auto-immune (MNAI). Les MNAI anti-HMGCR sont très souvent réfractaires aux traitements immunosuppresseurs et sont associées à un fort taux de rechutes justifiant ainsi l’utilisation prolongée d’immunoglobulines intraveineuses (IgIV). Le rôle pathogène des anticorps anti-HMGCR a récemment été démontré. Notre objectif était d’évaluer rétrospectivement l’efficacité d’un anticorps monoclonal anti-CD20 dans une cohorte de patients avec MNAI anti-HMGCR. Patients et méthodes Neuf patients avec MNAI anti-HMGCR ayant été traités par rituximab (RTX) ont été identifiés dans deux centres hospitaliers universitaires. Un recueil rétrospectif des données cliniques, biologiques, sérologiques et histologiques, ainsi que l’évolution des patients au dernier suivi a été réalisé. L’amélioration de l’atteinte musculaire a été définie par : une amélioration du testing manuel et/ou d’une diminution du taux de créatine phosphokinase (CPK) et/ou d’une amélioration des hypersignaux T2/STIR musculaires en IRM. Résultats L’âge moyen des patients à l’introduction du RTX était de 43 ans et 7 patients (77 %) étaient des femmes. Tous les patients présentaient un déficit des ceintures et trois d’entre-eux avaient une présentation pseudo-dystrophique évoluant en moyenne depuis 16,7 ans. Deux patients avaient été préalablement exposés aux statines. Le taux de CPK moyen à la présentation inaugurale était de 8577 IU/L [5600–13 537], et de 1602 IU/L [63–3427] à l’introduction du RTX. En moyenne, les patients avaient reçu 3 agents immunosuppresseurs avant le RTX. Trois patients étaient préalablement dépendants des immunoglobulines polyvalentes (IgIV) et quatre recevaient des IgIV à l’introduction du RTX. Tous les patients, sauf un, ont reçu une perfusion de RTX 1 g à j0 et j15, puis tous les 6 mois selon le jugement du clinicien. Un patient a reçu protocole de RTX 375 mg/m 2 /surface corporelle pendant 4 semaines consécutives. Trois des neuf patients réfractaires aux traitements immunosuppresseurs, ont présenté une amélioration de l’atteinte musculaire après avoir été traités par RTX. Le RTX a permis l’arrêt des IgIV et la diminution des corticoïdes à faibles doses (≤ 11 mg/jour) chez deux patients. La mesure quantitative des taux d’anti-HMGCR (HMGCR-ALBIA) a démontré une diminution marquée des titres sous RTX chez un de ces trois patients répondeurs. Chez les patients non répondeurs, 5 patients se sont avérés IgIV-dépendants et demeurent toujours sous IgIV au long cours. Conclusion Le RTX semble apporter un bénéfice dans un tiers des cas de MNAI anti-HMGCR réfractaires. Une étude prospective est nécessaire pour confirmer cette observation. La grande majorité des patients était particulièrement sensibles aux IgIV, et la moitié d’entre eux était dépendants aux IgIV au long cours. [ABSTRACT FROM AUTHOR]
