Background The efficacy of neoadjuvant chemoradiotherapy (NACRT) for resectable and borderline resectable pancreatic cancer is important for predicting outcomes after radical surgery, but few clinical indicators predict outcome before resection. This study examined the utility of FDG-PET in predicting the efficacy of NACRT and outcome after radical surgery. Methods Eighty-three pancreatic cancer patients who underwent FDG-PET before and after NACRT and had positive standard uptake values (SUVs) before NACRT were enrolled in this study. Peri-operative clinical factors, including FDG-PET findings, were examined to predict the efficacy of NACRT and outcome after surgery. Results Evans grade I, IIA, IIB, III, and IV was determined in 11, 31, 27, 11, and 3 patients, respectively. The maximum SUVs after NACRT (post SUV-max) and tumor size were significantly decreased compared to pretreatment values (p < 0.001 and p = 0.007, respectively). The post SUV-max and regression index were significantly related to grade III/IV (p = 0.04 and p < 0.001, respectively), but only the regression index predicted NACRT efficacy (p = 0.002). The AUC of the regression index for the detection of grade III/IV was 0.822, and 13 of 14 grade III/IV patients were picked up using 50% as the threshold (p < 0.001). Patients with a regression index >50% had a significantly better prognosis after radical resection than patients with <50% (p = 0.032). Regression index as well as pathological lymph node status and resectability status were independent prognostic factors in multivariate analysis (exp 2.086, p = 0.043). The extent of sympathetic reinnervation of transplanted hearts varies among recipients. The purpose of this study is to investigate the relation between the functional response of the left ventricular (LV) myocardium to adrenergic stimulation and the findings of [123I]metaiodobenzylguanidine (MIBG) scintigraphy in cardiac transplant recipients. MIBG scintigraphy was performed in 20 heart transplant recipients (mean of 7.6 years after transplantation) and in 20 age-matched individuals with hemodynamically stable (NYHA functional class I) dilated cardiomyopathy (DCM). Heart rate and blood pressure as well as the ECG during exercise- or dobutamine-induced stress tests were measured in all patients. All patients also underwent endomyocardial biopsy, and some of the biopsy specimens were subjected to mRNA analysis. LV isovolumic contraction (dP/dt max) and relaxation (tau) were measured simultaneously during stress tests in six transplant recipients and five patients with DCM. MIBG uptake was virtually undetectable in transplant recipients, with the exception of one individual who showed evidence of reinnervation in the anteroseptal region. The heart-to-mediastinum ratio was significantly smaller in transplant recipients than in patients with DCM (1.32 ± 0.6. vs. 2.22 ± 0.3, p < 0.0001). The change in dP/dt max at peak heart rate during stress tests did not differ significantly between the two groups (83 ± 26 vs. 93 ± 20%, p = 0.335), whereas that in tau was smaller in transplant recipients than in DCM patients (31 ± 8 vs. 42 ± 9%, p < 0.05). RT-PCR analysis revealed that the amount of β-adrenergic receptor kinase-1 mRNA in the ventricular myocardium tended to be smaller in recipients who showed an attenuated response in stress tests than in those with a good response LV contractility during adrenergic stimulation by dobutamine or exercise was preserved, whereas LV relaxation was attenuated, in transplant recipients. The discrepancy between the responses of LV myocardial contractility and relaxation to adrenergic stimulation and MIBG scintigraphic findings may provide insight into the reinnervation of transplanted hearts. Introduction Oxidative stress has been implicated in various disease states and ischemia/reperfusion injury is a direct consequence of oxidative stress in lung transplantation. Because the success rate of organ transplantation in which ischemia/reperfusion is inevitable is highly influenced by oxidative stress, development of strategies to control oxidative stress would be beneficial. Here we identified natural compounds to reduce oxidative stresses in isolated mouse lungs. Methods We screened compounds associated with antioxidative stress in 200 plant extracts by monitoring the activities of nuclear factor erythroid 2-related factor 2 (NRF2). Compounds found to ameliorate antioxidative stress were enriched and mice were administered the extract orally every day for 1 week. Then, the lungs were isolated and cultured in the culture medium at 37°C. Lung damage was monitored by lactate dehydrogenase (LDH) released in the culture medium. Arterial (left ventricle) blood gas levels were also monitored after hilar clamping. Results We found that Callicarpa longissima extract was rich in NRF2 activators. The responsible compounds were carnosic acid and its oxidative product, carnosol. Carnosol induced heme-oxygenase 1 (HO-1) expression, which is downstream of NRF2, more efficiently than carnosic acid. Conclusions Lungs from mice treated with C longissima extract were less damaged than those from control mice and accompanied by HO-1 induction. Objectives: To determine the capability of reduced-dose chest CT with three-dimensional (3D) automatic exposure control (AEC) on quantitative assessment of emphysematous change in smoker' lung parenchyma, compared to standard chest CT. Methods: Twenty consecutive smoker patients (mean age 62.8 years) underwent CT examinations using a standard protocol (150 mAs) and a protocol with 3D-AEC. In this study, the targeted standard deviations number was set to 160. For quantitative assessment of emphysematous change in lung parenchyma in each subject using the standard protocol, a percentage of voxels less than -950 HU in the lung (%LAA(-950)) was calculated. The 3D-AEC protocol's %LAA was computed from of voxel percentages under selected threshold CT value. The differences of radiation doses between these two protocols were evaluated, and %LAAs(-950) was compared with the 3D-AEC protocol %LAAs. Results: Mean dose length products were 780.2 ± 145.5 mGy cm (standard protocol), and 192.0 ± 95.9 (3D-AEC protocol). There was significant difference between them (paired Student's t test, p<0.00001). Meanwhile, only setting -960 HU yielded no significant difference (paired Student's t test, p=0.32) between %LAAs(-950) and 3D-AEC protocol %LAAs. In adopting the feasible threshold CT values of the 3D-AEC protocol, the 3D-AEC protocol %LAAs were significantly correlated with %LAAs(-950) (r = 0.98, p<0.001) and limits of agreement from Bland-Altman analysis was 0.52 ± 4.3%. Conclusions: Changing threshold CT values demonstrated that reduced-dose chest CT with 3D-AEC can substitute for the standard protocol in assessments of emphysematous change in smoker' lung parenchyma. Purpose: The purpose of this study was to directly and prospectively compare the capability of dynamic O(2)-enhanced MRI and quantitatively assessed thin-section MDCT to assess smokers' COPD in a large prospective cohort. Materials and Methods: The GOLD criteria for smokers were used to classify 187 smokers into four clinical stage groups as follows: smokers without COPD (n=56) and with mild (n=54), moderate (n=52) and severe or very severe COPD (n=24). All smokers underwent dynamic O(2)-enhanced MRI, MDCT and pulmonary function tests. Mean relative enhancement ratio and mean wash-in time on MRI and CT-based functional lung volume (CT-based FLV) as well as the ratio of airway wall area to total airway area on MDCT were computationally calculated. Then, all indexes were significantly correlated with functional parameters. To determine the efficacy of all indexes for clinical stage classification, the indexes for the four clinical groups were statistically compared by using Tukey's honestly significant difference multiple comparison test. Results: All indexes had significant correlations with functional parameters (p<0.0001). All indexes except CT-based FLV in all groups had significant differences each other (p<0.05). Integrated positron emission tomography/computed tomography (PET/CT) with 2-[18F]fluoro-2-deoxy-d-glucose (FDG) is a useful technique to acquire both glucose metabolic and anatomic imaging data using a single device in a single diagnostic session and has opened a new field in clinical oncologic imaging. FDG-PET/CT has been used successfully for the staging, optimization of treatment, re-staging, therapy monitoring, and prognostic prediction of uterine cervical cancer and endometrial cancer as well as various malignant tumours. The present review discusses the current role of FDG-PET/CT in the management of uterine cancer, discussing its usefulness and limitations in the imaging of these patients. [ABSTRACT FROM AUTHOR]

Aim: To assess the characteristics of [18F]-fluoro-2-deoxy-d-glucose (FDG) uptake in cases of ovarian metastasis using positron-emission tomography/computed tomography (PET/CT). Materials and methods: Twelve patients with 16 ovarian metastases arising from colon cancer (n =6), breast cancer (n =4), gastric cancer (n =3), and pancreatic cancer (n =3) who underwent FDG-PET/CT examination were included in this study. The effect of lesion size and morphological pattern (predominantly solid or cystic) on FDG uptake was evaluated using the quantitative standardized uptake value (SUV). Results: The mean maximum SUV for the 16 lesions was 4.6±2.4 (range 1.8∼9.9). The Pearson correlation coefficient test showed no significant correlation between maximum SUV and lesion size (r =0.21, p =0.42). The maximum SUV of solid (n =5) and cystic (n =11) lesions was 5.5±2.7 and 4.3±2.2, respectively, and the difference was not significant (p =0.43). Breast cancer showed the highest maximum SUV (6.4±3.6), followed by colon cancer (5.3±1.4), gastric cancer (3.3±0.5), and pancreatic cancer (2.2±0.6). Aim: To evaluate the diagnostic accuracy of the combination of computed tomography (CT) during arterial portography (CTAP) and double-phase CT hepatic arteriography (CTHA) with multidetector-row CT (MDCT) for the evaluation of hepatocellular carcinomas (HCCs) in patients with cirrhosis. Materials and methods: The combination of CTAP and double-phase CTHA was performed on 46 patients with 54 nodular HCCs. Three readers reviewed the images obtained with CTAP alone, first-phase CTHA alone, double-phase CTHA, and the combination of CTAP and double-phase CTHA. The review of the images was conducted on a segment-by-segment basis, with 368 hepatic segments, including 50 segments with 54 HCCs, reviewed for detection of HCCs with the aid of a five-point confidence scale. Diagnostic accuracy was evaluated by comparing the receiver-operating characteristic (ROC) analysis results. Results: The sensitivity for detecting HCCs was significantly higher with either double-phase CTHA or the combination of CTAP and double-phase CTHA than with first-phase CTHA alone (90 and 93 versus 85%, respectively, p <0.01). The specificity for detecting HCCs was significantly higher with the combination of CTAP and double-phase CTHA than with CTAP alone (97 and 94%, respectively, p <0.01). The positive predictive values for detecting HCCs were significantly higher with double-phase CTHA than with first-phase CTHA alone (86 and 82%, respectively, p <0.05). The area under the ROC curve (Az) values were significantly higher with the combination of CTAP and double-phase CTHA (0.983) than with first-phase CTHA alone (0.959; p <0.05). Abstract: Renal ischemia-reperfusion (I/R) injury during renal transplantation is a significant cause of renal dysfunction. The pathological role of free radicals in this process is a major concern. We investigated the effect of a free radical scavenger, edaravone (MCI-186), in renal I/R injury. Male Lewis rats (270 to 320 g) were used for the model. The right kidney was harvested and left renal artery and vein were clamped as laparotomy. The kidney was reperfused after 90 minutes of ischemia. Edaravone (10 mg/kg) was delivered intravenously before ischemia and after reperfusion to prevent the neutrophil activation. In the nontreatment I/R group, no rat survived beyond 4 days. However, in the edaravone I/R treatment group, one among five rats survived more than 7 days. These results suggested that treatment with edaravone ameliorated renal I/R injury, and that the agent has the potential to ameliorate preservation injury in renal transplantation. Purpose: To evaluate two different iodine concentrations of contrast material for detecting hypervascular hepatocellular carcinomas (HCCs) in cirrhotic liver by multi-detector row helical CT (MDCT) when a fixed contrast material volume and injection rate is used. Materials and Methods: Institutional Review Board approval was obtained, and informed consent was obtained from all patients. In this prospective study, 105 patients were randomly assigned a group A (an iodine concentration of 300 mg I/mL), and a group B (an iodine concentration of 370 mg I/mL). In both groups the volume of contrast material was 100 mL and the injection rate was 4 mL/s. Fifty-two patients had 122 hypervascular HCCs. The diagnosis of HCCs was established histopathologically (n=24) and by imaging findings (n=98). Three readers independently analyzed four image sets: an arterial phase (AP), a portal phase (PP), an equilibrium phase (EP), and combined all three phase images set. Sensitivity, specificity, and diagnostic accuracy were calculated by receiver operating characteristic (ROC) analysis. Results: The mean sensitivity for detecting hypervascular HCCs of the AP set, EP set, and combination set in group B (0.94, 0.81, and 0.93) was significantly higher than in group A (0.84, 0.69, and 0.80). Area under the ROC curve of the AP set and the combination set in group B (0.974 and 0.981) was significantly higher than in group A (0.939 and 0.958). Conclusion: At the same contrast material volume and injection rate, higher iodine concentration of contrast material was effective for detecting hypervascular HCCs by MDCT. [ABSTRACT FROM AUTHOR]
