The bootstrap method estimates these limits relying on Monte Carlo methods, which are subject to simulation errors. Therefore this paper develops a computationally efficient enumeration method for exact calculations of the control limits. Time-reversed speech has been known to effectively mask information for speech privacy applications. However, the annoyance and distraction caused by the time-reversed speech-like masking sound is higher than other masking sound. This study investigates the effects of adding artificial reverberation to the time-reversed speech. Subjective listening tests have been conducted to measure the intelligibility of target speech, annoyance and distraction caused by the masking sound. The experimental results suggest that adding artificial reverberation to a speech-like masking sound has a significant effect to reduce the annoyance level while maintaining the masking effectiveness of the original masking sound. A trend was also observed that the addition of artificial reverberation could reduce the level of distraction caused by the masking sound. [ABSTRACT FROM AUTHOR]

Abstract: We analyzed the clinical outcome of stage II to IV malignant thymoma. This study focused on the treatment of 60 cases that combined surgery (biopsy or resection) and radiation therapy (with or without chemotherapy). Univariate and multivariate analyses of prognostic factors predicting survival were carried out. There is a statistically significant relationship between the extent of surgery and the local control (19.4% of relapse after complete resection vs. 41.2% of relapse after partial resection or biopsy, p =0.0001). Mediastinal radiation dose (≥50Gy) had a significant effect in decreasing recurrence (p =0.0001) and distant metastasis (p =0.011). The rates of local recurrence (30%) and distant metastasis (25%) justify recommending a higher dose of mediastinal radiation (≥50Gy) for patients with malignant thymoma. [Copyright &y& Elsevier]

The giant cell tumor is considered to be a locally aggressive benign tumor and has a low tendency toward distant metastasis. They are primarily present in the long bone and are rare in the skull. The osteoclast-like giant cells do not demonstrate mitotic activity and contain large numbers of nuclei. Herein, we described a case of giant cell tumor in the temporal lobe. Heterogenous enhancement is shown on CT imaging in solid areas of tumor. The appearance intermediate to high signal intensity on T1-weighted and very low signal intensity on T2-weighted MR imaging were due to the presence of hemosiderin from prior hemorrhage. Epigenetic modifications play important roles in regulating carcinogenesis, and specific epigenetic modifications have emerged as potential tumor markers. Herein, we summarize several types of epigenetic modifications, explore the role played by epigenetic modifications in gene regulation, and describe the patterns of epigenetic modifications in cancers. Since epigenetic modifications have been reported to regulate the Warburg effect in cancers, the roles of epigenetic modifications in sugar metabolism are discussed. In addition, oxidative stress may play an important role in carcinogenesis, and the role of oxidative stress and epigenetic modification in carcinogenesis is addressed. We also discuss the role of epigenetic modifications as therapeutic targets. Finally, the synergistic effects of the combined treatment of epigenetic regulator and anticancer drugs for cancer therapy are described. [ABSTRACT FROM AUTHOR]

Aims: To determine whether (1) chronic lung disease (CLD) is the prime reason for extremely-low-birth-weight (ELBW) infant readmission during the first 2 years of life, (2) surfactant and other advanced therapies have reduced ELBW infant readmissions, (3) home oxygen therapy (HOT) is efficacious for this group. Study design: The hospital records of these ELBW infants were reviewed retrospectively. Data on age, diagnosis, treatment, and duration of each hospitalization were compiled and analyzed for their association to CLD and to readmission for CLD and other reasons. Subjects: All 60 surviving infants with a birth body weight of less than 1001 g (ELBW) born from January 1993 to February 1998 were followed up to 2 years (mean 20.4±7.4 months) to evaluate their respiratory outcome. Results: Forty-two percent of these infants developed CLD. Upon discharge from the hospital, 28% (7/25) of the patients were given HOT for a median period of 60 days. Of the 47 ELBW infants who were studied the entire 2-year period, 72% were readmitted. Infants with CLD were readmitted more frequently (p=0.045) and had longer hospital stays during the first 2 years of life (p=0.034) than those without CLD. Respiratory illness was the main reason for readmission (55%) of these ELBW infants. The incidence of readmission due to respiratory tract infection was not significantly different in infants with CLD (61%) and infants without respiratory complications (44%) (p=0.159). Conclusions: Infants with CLD (whether receiving HOT or not) showed no higher readmission rate due to respiratory infection, but the HOT group did have higher morbidity. The premature lung itself rather than the presence of CLD, as we would expect, makes ELBW infants more prone to readmission for respiratory illness. [Copyright &y& Elsevier]

Objectives: In current guidelines, early tongue cancer status post partial glossectomy without adverse risk features do not require adjuvant treatment. However, many of these patients developed recurrence with neck metastases soon. The objectives of this study were to investigate the potential risk factors in early tongue cancer that prophylactic management of neck may be considered.Materials and Methods: From January 2010 to September 2015, this retrospective study enrolled 102 patients with T1-2N0 primary oral tongue squamous cell carcinoma according to AJCC 8th edition Cancer Staging System. All patients underwent partial glossectomy with or without selective neck dissection, and did not receive any adjuvant treatment. Patients with any adverse risk features were excluded. We have studied the 4-year cancer-specific survival and neck recurrence rate, and analyzed the relevance between pathologic tumor classification, tumor depth, tumor histologic grade, and measured surgical margin of primary tumor.Results: The median follow up duration was 47 months (range 6-93 months) with the median recurrence interval was 13 months. Histologic grade ≥2 of primary tumor was significantly associated with increased risk of neck recurrence and disease-specific mortality in both univariate and multivariate analysis.Conclusion: Histologic grade ≥2 was an adverse prognostic factor of neck recurrence and was significantly associated with poor cancer-specific survival in T1-2N0 early oral tongue cancer patients. Therefore, prophylactic neck dissection or prophylactic adjuvant radiation therapy to neck may be considered in T1-2N0 early oral tongue cancer with histologic grade ≥2 of primary tumor.
