Abstract: The organophosphorus (OP) chemical warfare V agent O-ethyl-S-[2(di-isopropylamino)ethyl] methyl phosphonothioate (VX), is a highly toxic compound which mainly penetrates the body via percutaneous pathways. Hence, the following prerequisite: to ascertain compound absorption and percutaneous profile distribution with a view to further assessing the efficacy of topical skin protectants. We first selected the most appropriate receptor fluid to carry out in vitro VX absorption experiments, namely: Hanks''s Balanced Salt Solution (HBSS). After a 24-h topical exposure time lapse, we measured altogether the percentage of applied dose unabsorbed and absorbed, penetration rate, lag time, permeability coefficient (K p), and dose of VXeq present in skin. To such an end, we used full-thickness and split-thickness pig-ear or human abdominal skin membranes. Further, we scrutinised the potential use of two specific molecules as suitable surrogates for VX percutaneous penetration analyses: thus, we compared the present VX toxicokinetic parameters to earlier findings from our research unit, with respect to OP insecticides demethon-S-methyl (DSM) and paraoxon (POX). Within the framework of our study, we wish to highlight the following evidence: (a) pig-ear skin proves a relevant model to predict in vitro human abdominal skin, taking into account a 2-fold higher skin permeability to VXeq; (b) both full or split-thickness skin membranes may be used indiscriminately to gauge penetration rate and absorbed dose; (c) DSM applied on full-thickness pig-ear skin is the most relevant model to mimic the in vitro VX absorption through full-thickness skin model. Abstract: Organophosphorus compounds (OPs), such as pesticides and chemical warfare agents like sarin (GB), soman (GD) and VX, are highly toxic compounds. The OP vapours and their liquid forms are readily absorbed through the skin, therefore, protecting the skin of people who are potentially exposed to these agents is crucial. The development of effective countermeasures relies on a better knowledge of the percutaneous penetration of such molecules. The purpose of this present study is to determine the in vitro percutaneous penetration parameters of two pesticides DSM and DFP, as potential simulants of V and G agents, respectively, using four in vitro systems: full-thickness and split-thickness human abdominal and pig-ear skin membranes mounted on static diffusion cells. Based on the toxicokinetic parameters of the percutaneous penetration of DSM and DFP, we demonstrated that (a) pig-ear skin is a relevant model to predict the in vitro human skin permeability taking into account a 2-fold difference between these two species (b) both full and split-thickness skin membranes could be used indiscriminately, (c) DSM and DFP would be appropriate surrogates for V and G agents to perform skin permeation studies. The multimode Jahn–Teller (JT) and pseudo-Jahn–Teller (PJT) coupling effects in the photoelectron spectrum of methylfluoride are theoretically investigated with the aid of an ab initio quantum dynamical approach. The theoretical findings are compared with the experimental results of Karlsson et al. [Phys. Scripta 16 (1977) 225]. At the vertical configuration, the ground, first and second excited electronic states of the methylfluoride radical cation belong to the 2E, 2A1 and 2E symmetry species. The three doubly degenerate vibrational modes cause a splitting of the degeneracy of the E electronic states of the radical cation and exhibit the (E⊗e)-JT effect. The same vibrational modes may also cause a coupling of the degenerate and the non-degenerate electronic states (PJT effect). In our theoretical approach we devise a model diabatic vibronic Hamiltonian within a quadratic vibronic coupling scheme. The parameters of the Hamiltonian are derived by performing extensive ab initio calculations at the CASSCF-MR-AQCC level of theory. The photoelectron bands are calculated by a quantum dynamical approach based on the Lanczos algorithm. A detailed examination of the various static and dynamic aspects of the problem reveals an interesting interplay of JT and PJT coupling effects in the photoelectron-induced dynamics of CH3F+. The resolved progression in the first photoelectron band corresponding to the ground &Xtilde;2E electronic manifold of CH3F+ is found to be mainly caused by the C–F stretching (a1), C–F bending (e) and CH3 deformation (e) modes. The JT coupling effects are not particularly strong in this electronic manifold. This also holds for the second photoelectron band attributed to the vibronic structure of the Ã2A1 and &Btilde;2E electronic states of CH3F+. However, the extremely large Condon activity of the symmetric vibrational modes in the Ã2A1 electronic state contributes much to the broad and highly diffuse nature of this band which is also observed experimentally. The PJT activity of the anti-symmetric C–F bending (e) vibration in the &Xtilde;2E and &Btilde;2E electronic states is also discussed. [Copyright &y& Elsevier]

The possible stereoselectivity in DNA-photosensitization by carprofen (a NSAID drug) and ofloxacin (a fluoroquinolone agent) was investigated. The different drug stereoisomers or racemic mixtures were UVA-irradiated and the relaxation of the supercoiled circular pBR322 quantified by electrophoresis. Formation of single strand breaks was compared for each group of compounds. Moreover a mechanistic study by means of repair enzymes: T4 endonuclease V (specific of cyclobutane pyrimidine dimers), E. coli endonuclease III (revealing oxidized pyrimidines) and E. coli Formamidopyrimidine-DNA glycosylase (revealing oxidized purines) provided further insights into a possible stereoselectivity of the different reaction pathways in drug photosensitized-DNA damage. Ofloxacin and levofloxacin (its S stereoisomer) were responsible of single strand breaks formation as well as oxidation of pyrimidine and purine bases. No pyrimidine dimers were observed. Racemic, R and S stereoisomers of carprofen were less efficient than ofloxacin in DNA single strand breaks formation and did not induce enzyme-sensitive sites. The photoproducts distribution of drug-photosensitized reactions of 2′-deoxyguanosine and thymidine were established by HPLC as fingerprints for assignment of the DNA-photosensitization mechanism. Both Type I and Type II mechanisms were assigned to nucleoside-photosensitization by ofloxacin and levofloxacin. In the case of carprofen, a weak nucleoside degradation was obtained. The data suggest that levofloxacin, the (S) stereoisomer, might be slightly more efficient than racemic ofloxacin. In the case of carprofen the (S) isomer appears to be somewhat less active than its (R) enantiomer. However, due to the small differences found, the possible stereoselectivity has to be confirmed by future studies. [Copyright &y& Elsevier]

Abstract: Organophosphates are highly toxic compounds, mainly penetrating the organism through the percutaneous route. In this context, having topical skin protectant capable of limitating their percutaneous penetration is of crucial importance for exposed people. Knowledge of the toxicokinetic parameters corresponding to the percutaneous penetration of these compounds is a key step for the development of these protective formulations. The different experimental approaches, in vivo and ex vivo, used to predict percutaneous penetration of these toxics are presented. Objectif de l’étude La radiothérapie stéréotaxique d’ablation est une technique émergente dans la prise en charge des tachycardies ventriculaires réfractaires aux traitements médicamenteux et à l’ablation par cathéter. Nous rapportons ici les résultats des premiers patients traités dans notre centre. Matériel et méthode Les patients inclus souffraient de tachycardies ventriculaires ou d’une tempête rythmique réfractaires aux traitements classiques. Avant chaque traitement, afin de localiser le substrat de la tachycardie ventriculaire, une cartographie tridimensionnelle a été réalisée durant une exploration électrophysiologique endocavitaire. Pour chaque patient, le cardiologue a délimité le substrat de la tachycardie ventriculaire sur la scanographie de radiothérapie grâce aux informations de la cartographie. L’électrode de défibrillation du défibrillateur automatique implantable a servi de fiduciel pour assurer le suivi en temps réel (tracking). Une dose de 25 Gy a été délivrée sur le substrat de la tachycardie ventriculaire grâce à un accélérateur linéaire robotisé Cyberknife® M6. Résultats Depuis septembre 2017, trois patients ont été pris en charge : deux patients électifs et un patient hospitalisé en réanimation en raison d’une tempête rythmique à l’origine de nombreux chocs électriques délivrés par le défibrillateur automatique implantable, et nécessitant une anesthésie générale. Le tracking de l’électrode du défibrillateur automatique implantable a permis de délivrer avec succès la radiothérapie stéréotaxique chez tous les patients. Le volume des substrats de la tachycardie ventriculaire était pour chaque patient de 19 cm3, 21 cm3 et 35 cm3. Après un suivi médian de cinq mois, les patients électifs n’ont pas souffert de récidive rythmique. Le patient en réanimation a été extubé trois jours après la radiothérapie stéréotaxique et sans récidive de tempête rythmique pendant quatre mois, avant de présenter un nouvel épisode à 19 semaines. Cet épisode a été imputé à un nouveau substrat de la tachycardie ventriculaire, distant du premier, confirmé par cartographie tridimensionnelle et traité par ablation par cathéter. Il n’y a eu aucune complication imputable à la radiothérapie stéréotaxique. Conclusion La radiothérapie stéréotaxique apparait comme un traitement prometteur des tachycardies ventriculaires et tempête rythmiques réfractaires aux traitements classiques. Objectif de l’étude Étudier l’efficacité et la tolérance de la radiothérapie stéréotaxique hypofractionnée vertébrale dans une cohorte monocentrique. Matériel et méthode Nous avons réalisé une analyse rétrospective des dossiers des patients traités pour des métastases vertébrales par irradiation stéréotaxique hypofractionnée par CyberKnife® dans notre institution. Le volume tumoral macroscopique (GTV) a été délinéé sur les séquences T1 et T2 de l’IRM dédiée. Les volumes cibles anatomocliniques (CTV) ont été délinéés selon les recommandations internationales du Radiation Therapy Oncology Group (RTOG). Une marge de 1 mm était ajoutée au volume cible anatomoclinique pour créer le volume cible prévisionnel (PTV), ainsi qu’au volume tumoral macroscopique (GTV) pour créer le volume cible prévisionnel avec complément de dose (PTV boost). Le repositionnement en temps réel était assuré par le système XSight Spine du CyberKnife®. La stabilité vertébrale initiale était évaluée par le Spine Instability Neoplastic Score (SINS). Les taux de contrôle local et de survie globale ont été estimés par la méthode de Kaplan–Meier. Résultats Entre octobre 2014 et mars 2018, 70 patients consécutifs avec 106 lésions vertébrales ont été inclus. L’âge médian était de 62 ans (20–87). Les cancers primitifs étaient pulmonaires (34 %), mammaires (27 %) et autres (39 %). Le SINS médian était de 5 (0–13). 18 patients (26 %) avec des lésions instables ont été en situation de stabilité avant (n = 13) ou après la radiothérapie stéréotaxique hypofractionnéee (n = 5) par cimentoplastie ou chirurgie. Les doses médianes prescrites aux PTV et PTV boost étaient respectivement de 30 et 35 Gy en 5 fractions, à l’isodose 80 % ; un complément de dose a été délivré à 85 lésions (80,2 %). Avec un suivi médian de 12,4 mois (extrêmes : 1–43 mois), le taux de survie globale à 12 mois était de 79,9 %. Le taux de contrôle local était de 95,3 %, avec cinq rechutes locales. Aucune toxicité aiguë n’a été rapportée. Quatre fractures vertébrales (3,8 %) sans récidive ont été observées après en médiane 5 mois (extrêmes : 2–36 mois). Il n’y eut aucun cas de myélopathie radio-induite. Conclusion La radiothérapie stéréotaxique vertébrale est bien tolérée et permet un taux de contrôle local très satisfaisant à un an.
