Primary cutaneous CD4 + small/medium T-cell lymphoproliferative disorder (PCSMLPD) is a benign behaving condition, typically manifesting as solitary head or neck papules, frequently creating cosmetic concerns. Optimal management of this rare disease is unclear. Herein, patterns of care and treatment outcomes are described, with particular focus on low-dose RT. Eligibility required biopsy-proven PCSMLPD on central review, diagnosed between 2007–2022. Patterns of care, treatment responses and relapse patterns were assessed. Freedom-from-progression (FFP) was compared between RT and surgery. 41 patients were eligible. First-line treatments were: RT, 19 (46.3 %); surgery, 17 (41.5 %) (3 received adjuvant RT); watchful waiting, 5 (12.2 %). Median follow-up was 37.7 months. Overall, 24 patients received RT (19 definitive first-line, 3 adjuvant, 2 second-line). 10 (42 %) received 4 Gy in 2 fractions (with no acute toxicities); 14 (58 %) received 20–40 Gy. Complete response rate was 100 %. No post-RT relapses observed. After first-line surgery alone (n = 14, 3 with positive margins), 4 (28.5 %) experienced relapse (2 local, 2 distant). Watchful-waiting (n = 5) led to partial resolution post-biopsy in 4 patients; no complete resolution seen. 3-year FFP for RT alone was 100 % vs 61 % for surgery alone (p = 0.12). RT is a successful, non-invasive option for PCSMLPD: 100 % achieved complete response, with no relapses, and FFP appearing numerically superior to surgery in this cohort. In this first series of low-dose RT for PCSMLPD, 4 Gy in 2 fractions appears an excellent treatment option, offering durable disease control, no acute toxicities and convenient treatment time of only 2 days. Objectives: The purpose of the study was to examine the loss and grief experiences of patients waiting for and living with new hearts. Background: There is much scholarship on loss and grief. Less attention has been paid to these issues in clinical transplantation, and even less on the patient experience. Methods: Part of a qualitative inquiry oriented to the work of Merleau-Ponty, a secondary analysis was carried out on audiovisual data from interviews with thirty participants. Results: Patients experience loss and three forms of grief. Pre-transplant patients waiting for transplant experience loss and anticipatory grief related to their own death and the future death of their donor. Transplanted patients experience long-lasting complicated grief with respect to the donor and disenfranchised grief which may not be sanctioned. Conclusions: Loss as well as anticipatory, complicated and disenfranchised grief may have been inadvertently disregarded or downplayed. More research and attention is needed. [ABSTRACT FROM AUTHOR]

Engaging 3rd-5th graders for nutrition education in both in-person and remote settings Social Ecological Model Youth grades 3-5 from limited-resource families who are eligible for SNAP-Ed or EFNEP. The Tennessee Nutrition and Consumer Education Program (UT SNAP-Ed: TNCEP) and Expanded Food and Nutrition Education Program (EFNEP) offers nutrition education classes to youth, which are often delivered in schools during class time. Because of the COVID-19 pandemic, many elementary schools began delivering education online resulting in barriers for TNCEP and EFNEP educators to reach youth. During listening sessions, county educators identified the need for youth videos to complement existing nutrition curricula in conjunction with a remote class or to be shown separately by teachers to reinforce nutrition messages taught by nutrition educators. Topic needs identified included increasing fruit and vegetable intake, reducing sugar intake, increasing physical activity and proper handwashing. TNCEP and ENFEP contracted a production company to create four videos. The state team worked with the company to create videos to be educational, engaging and appropriate for youth. Online surveys were conducted with county educators to determine usage, reach, appropriateness and acceptability by youth. The survey yielded 22 responses. Educators reported disseminating videos through remote classrooms, in-person classrooms, providing to teachers to show in class and sharing via social media. Of those who responded, 60% indicated the videos were very useful and 71% indicated the videos were very well received by youth. Educators reported reaching over 8,000 students with videos utilizing the methods above. Educators stated videos were best received by younger students 4th grade or below. Videos can complement traditional nutrition education classes, especially in remote learning settings. Additional videos may be needed to target 5th graders. This presentation will focus on lessons learned and best practices for creating effective nutrition education videos for youth. Background: A key element missing in disease-management programs for heart failure (HF) is participation of the community pharmacist. The purpose of this study is to determine if a simple and efficient clinical tool will allow community pharmacists to identify patients at risk for worsening HF. Design: The One Minute Clinic for Heart Failure (TOM-CHF) was developed as a simple six-item symptom screening tool to be used during routine patient/customer interactions. Setting: Ten community pharmacies located in the upper Midwest. Patients: Self-identified HF patients. Results: 121 unique patients were evaluated over a 12-month period. The application of this clinical tool took between 1 and 5 minutes in over 80% of the interactions. Seventy-five patients (62%) had one or more signs or symptoms of worsening HF. The most common symptoms detected included edema (39%) and increased shortness of breath (17%). Self-reported weight gain of more than 5 pounds was seen in 19% of patients. Conclusion: The TOM-C HF tool was used to identify patients in a time-efficient manner in the community pharmacy setting who appear to be developing worsening HF. Inclusion of the community pharmacists as an early screen for HF decompensation may be an important link in disease-management programs to help reduce hospital readmission rates. This metasynthesis of empirical and nonempirical literature analyzed 39 journal articles and book chapters that examined issues related to the preparation of information literate teachers in the U.S., Australia, Canada, New Zealand, Taiwan, and the U.K. All 39 items were published after March 1998 (when the Association of College and Research Libraries reported that teacher education programs had made no real progress in modifying course requirements and performance expectations to address information literacy concerns). The literature indicates that teacher education programs have made significant progress in addressing information literacy concerns during these last 10 years; however, much work remains to be done. Increased opportunities for collaboration between educators and librarians at the preschool–Grade 12 (P-12) and postsecondary levels are needed to prepare information literate teachers who can effectively teach information literacy skills and research strategies to their P-12 students. Abstract: Griffon vultures (Genus Gyps) have large areas of bare skin on their body, and by changing their posture they can vary the extent to which these are covered by feathers. We used a mathematical model to estimate the amount of bare skin exposed in the postures adopted in cold and hot conditions. Measurements of heat flow through different parts of museum skins, which differed in their feather density, were then used together with the estimates of proportions of body surface covered by each feather density type, to determine rates of heat loss from the whole body. Postural change can cause the proportion of body surface composed of bare skin areas to change from 32% to 7%, and in cold conditions these changes are sufficient to account for a 52% saving in heat loss from the body. We suggest that the bare skin areas in griffon vultures may play an important role in thermoregulation. [Copyright &y& Elsevier]

The internet has become a place of increased risk of abuse, including sexual abuse, for young people (YP). One potential risk factor to online abuse and exploitation is the ability to mentalise. We developed the i-Minds app, a mentalisation-based digital health intervention (DHI) for YP who have experienced technology assisted sexual abuse (TASA), which we tested in a clinical feasibility trial. Nested within the trial was a qualitative implementation study with clinicians who referred to the trial. To explore the barriers and enablers to the future integration of i-Minds into clinical practice. Twelve HCPs were recruited from across two trial recruitment sites (Manchester and Edinburgh). Semi-structured interviews were informed by Normalisation Process Theory (NPT). Framework analysis was used; transcripts were coded deductively to NPT constructs. Practitioners were positive about the need for, and added value of, the i-Minds app over existing interventions, including other DHIs. While they felt confident with the app, concerns remained around the safety of using the app without practitioner support. i-Minds promoted changes in practitioners' work and impacted online behaviour of YP. There was an identified need for further training and organisational support. Practitioners are aware of TASA but have limited knowledge, skills and tools to work with TASA in clinical practice with YP. There is a need for awareness raising and education about TASA and DHI. i-Minds offers a theory-informed DHI for working with YP exposed to TASA that is acceptable to practitioners and YP. To evaluate the effectiveness of Shop Smart Tennessee (SSTN) in engaging small food retailers in promoting healthy food items and impacting consumer purchasing Research has shown the food retail environment, including accessibility and affordability of foods, is an important component of dietary choices. As a strategy to increase consumers' ability and motivation to purchase healthier foods, food retail work is a critical component of a multipronged approach to address chronic disease prevention (CDC, 2014). Using the Theory of Planned Behavior and reciprocal determinism, SSTN works with small food retailers - mostly rural - to implement strategies to encourage healthy food choices. Additionally, by engaging consumers, SSTN seeks to increase demand of healthier foods such as low-calorie beverages, whole grains, lean proteins, and fruit. Target audiences included small food retailers and shoppers of SNAP/EBT accepting stores in low-income census tracts from 2021-2023. SSTN is an eight-week SNAP-Ed food retail program designed to encourage consumers to make healthy food selections. SSTN uses weekly taste testing, educational materials, behavioral economics approaches, and social media to achieve program goals. Additionally, SSTN educators work with retailers to implement store environmental changes and stocking enhancements designed to promote healthy choices. Evaluation data were collected through a standardized store assessment conducted at the start of the intervention, post-intervention retailer interviews, and paper surveys distributed to consumers during final sessions. Since 2021, SSTN has been implemented in 31 stores reaching over 9,100 consumers. 370 adults completed surveys. Fifty-six percent (n=209) of respondents reported they purchased promoted foods as a result of the in-store taste testings. Twenty store managers completed surveys. During program delivery, 40% (n=8) managers started stocking new healthy items and 42% (n=9) increased their stock of current healthy items. SSTN effectively engaged food retailers and positively impacted consumer demand for healthier foods demonstrating the ability of small food retail interventions to benefit both customers and store owners. The study of quality of life (QOL) in patients with asymptomatic diseases receiving interventional treatment provides an essential metric for the assessment of procedural benefits in the surgical patient population. In this study, we analyzed QOL data collected from patients with unruptured intracranial aneurysms (UIAs) before and after endovascular coiling in the HEAT Trial, alongside a systematic review on QOL in unruptured brain aneurysms. HEAT was a randomized controlled trial comparing recurrence rates in aneurysms treated with either bare platinum coils or hydrogel coils. Patients enrolled in this trial completed a short form−36 (SF-36) QOL questionnaire before treatment and at the 3- to 12- and 18- to 24-month follow-ups. The change in QOL before and after treatment was assessed. Regression analysis evaluated the effect of select baseline characteristics on QOL change. A total of 270 patients were eligible for analysis. There was an increase in the role physical (P = 0.043), vitality (P = 0.022), and emotional well-being (P < 0.001) QOL components at the 18- to 24-month follow-up compared with baseline scores. Regression analysis showed that age younger than 60 and absence of serious adverse events were associated with improved social functioning and vitality. The literature review showed a mixed effect of intervention on QOL in patients with UIAs. Our analysis has revealed that patients with 3- to 14-mm UIAs had improvements in some physical and emotional components of QOL at 18–24 months following aneurysm coiling in the HEAT study. The literature remains indeterminate on this issue. Further studies are needed to better understand the effects of the diagnosis of UIAs and their treatment on QOL. [ABSTRACT FROM AUTHOR]

Abstract: A series of 2,3,6-pyrazine Rho Kinase inhibitors were optimized for in vivo activity for topical ocular dosing. Modifications of the 2-(piperazin-1-yl)pyrazine derivatives produced compounds with improved solubility and physicochemical properties. Modifications of the 6-pyrazine substituent led to improvements in in vitro potency. Compound 9 had the best in vitro and in vivo potency of EC50 =260nM with a 30% reduction of IOP in a non-human primate model at a dose of 0.33%. [Copyright &y& Elsevier]

Summary: Objectives: This study aimed to evaluate the stain removing/controlling properties of a whitening dentifrice and a stain controlling system, in a parallel group, multiple use study. Methods: Subjects (63), were stratified according to baseline stain scores and randomly allocated to a group, A (standard dentifrice), B (whitening/stain removing dentifrice) or C (whitening/stain removing dentifrice, tooth polish, tooth polisher and whitening/stain removing mouthwash). At each attendance, extrinsic stain was scored blind by a trained and calibrated examiner using the Shaw and Murray Stain Index, under constant lighting conditions. In addition, whiteness was measured using a colorimeter. Results: The toothpaste regimens were well balanced at baseline (p=0.811). At 4 weeks, Regimens B and C had significantly less stain than A (p&#60;0.05). Lower stain values were maintained/reduced for regimens B and C by week 6, with changes from baseline significantly greater than for Regimen A. When lightness was assessed by colorimeter, Regimen C was also found to be significantly whiter than Regimen A (p&#60;0.05). Conclusions: It was concluded that both whitening regimens had a significant effect on reducing tooth staining and improving whiteness over the 6 week time period. [Copyright &y& Elsevier]

To determine the effectiveness of adapting healthy food retail components for online delivery. Small food retail stores that accept SNAP/EBT are important food sources for low-income Americans, especially in areas where traditional grocery stores are difficult to access. SNAP-Ed recognizes healthy food retail interventions as an approach to encourage healthy eating and support direct nutrition education. Small food retail stores that accept SNAP/EBT in low-income census tracts were targeted. Shop Smart Tennessee is an 8-week small food retail program that uses taste testing, educational handouts, and in-store marketing to encourage healthy customer selections. In response to COVID-19, in-store program components were modified for the pilot to be delivered on social media. Food demonstration videos were developed to replace taste testings, and educational handouts were translated to social media posts. Educators posted videos and educational content to the partnering store's Facebook page or tagged the store in posts. To assess the effectiveness of online delivery methods we used participant surveys linked to food demonstration video posts, social media metrics, and nutrition educator feedback. Five stores participated in this pilot. Thirty-seven videos were posted to Facebook with 4,320 views and 863 engagements. Forty-three people completed online participant surveys, with 89% (n = 38) responding they would likely make the recipe at home. Nutrition educator feedback suggested store managers were supportive of online delivery and that it was an acceptable alternative to in-store programming. This pilot demonstrated that components of food retail programming can be adapted for social media. This method was well received by participants, store managers and nutrition educators. It allowed us to reach store customers from a different angle and can be used to inform future food retail work involving social media.
