There is currently great need for high-quality, low-cost, point-of-care diagnostics that can benefit patients in resource-limited settings and correspondingly growing interest in the diagnostic utility of microfluidic platforms that are based on paper. We describe the development, early clinical testing, and potential clinical impact of a novel paper-based, multiplexed microfluidic assay designed for rapid, semiquantitative measurement of aspartate aminotransferase and alanine aminotransferase in a fingerstick specimen. This device ultimately holds promise for providing universal access to affordable point-of-care screening for drug-induced liver injury in resource-limited settings and opens the door to development of similar point-of-care clinical assays for other important analytes.

Background Unvaccinated children may live in households with limited access to other primary health care (PHC) services, and routine vaccination services may provide the opportunity to bring caregivers into contact with the health system. We aimed to investigate the overlap between not being vaccinated and failing to receive other PHC services in low- and middle-income countries (LMICs). Methods Using Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys (MICS) data between 2010-2019 from 92 LMICs, we analysed six vaccination indicators based on the bacille Calmette-Guerin (BCG), polio, diphtheria-pertussis-tetanus (DPT) and measles vaccines and their overlap with four other PHC indicators - at least four antenatal care (ANC) visits, institutional delivery, careseeking for common childhood illnesses or symptoms and place for handwashing in the home - in 211,141 children aged 12-23 months. Analyses were stratified according to wealth quintiles and World Bank income levels. Findings Unvaccinated children and their mothers were systematically less likely to receive the other PHC interventions. These associations were particularly marked for 4+ ANC visits and institutional delivery and modest for careseeking behaviour. Our stratified analyses confirm a systematic disadvantage of unvaccinated children and their families with respect to obtaining other health services in all levels of household wealth and country income. Interpretation We suggested that lack of vaccination goes hand in hand with missing out on other health interventions. This represents an opportunity for integrated delivery strategies that may more efficiently reduce inequalities in health service coverage. Interburns is a charity that was set up with the aim of working to reduce the disparity in the number of cases of burns as well as the outcomes for patients in LMICs. This paper provides an overview of a cyclical framework for quality improvement in burn care for use in LMICs that has been developed using an iterative process over the last 10 years. Each phase of the process is outlined together with a description of the tools used to conduct a gap analysis within the service, which is then used to frame a programme of capacity enhancement. Recent externally reviewed projects have demonstrated sustained improvement with the use of this comprehensive and integrated approach over a three-year cycle. This overview paper will be supported by further publications that present these results in detail.

With Universal Health Coverage (UHC) now entrenched among the top global development priorities, questions arise as to the conditions under which politicians commit to UHC and why certain strategies for health financing and access are favoured over others. The Ethiopian government has been piloting and scaling-up Community-Based Health Insurance (CBHI) for the informal sector since 2010 and is establishing Social Health Insurance for formal sector workers as a means of achieving UHC. CBHI covers 11 million people making it one of the largest health insurance schemes in Africa. This paper employs a process tracing methodology to examine the political drivers of the adoption and evolution of state health insurance based on 28 key informant interviews conducted between 2015 and 2018 with politicians, policymakers and donor officials. The paper highlights the inadequacy of existing theories—focusing on interest group mobilisation, electoral competition and bureaucratic actors—for explaining the Ethiopian case. Instead the paper proposes an ‘Adapted Political Settlements’ framework that explains the state's push to expand CBHI and stalled progress on SHI. This framework highlights the interests and ideas of the ruling coalition as important drivers of reform. In a context of ruling party dominance and minimal threat from electoral competition, the ruling coalition has sought to build political legitimacy through the delivery of socioeconomic progress, including health services. The policy idea of health insurance, meanwhile, has secured elite commitment due to its fit with deeply held ideas within the ruling coalition concerning the importance of self-reliance and resource mobilisation for development. Finally, the centralisation of power within the ruling coalition prior to 2012 enabled the emergence of a long-term developmental vision and the marginalisation of opposition to health insurance, while fragmentation of the ruling coalition since 2012 has led SHI to stall.

Summary Implementation research is important in global health because it addresses the challenges of the know–do gap in real-world settings and the practicalities of achieving national and global health goals. Implementation research is an integrated concept that links research and practice to accelerate the development and delivery of public health approaches. Implementation research involves the creation and application of knowledge to improve the implementation of health policies, programmes, and practices. This type of research uses multiple disciplines and methods and emphasises partnerships between community members, implementers, researchers, and policy makers. Implementation research focuses on practical approaches to improve implementation and to enhance equity, efficiency, scale-up, and sustainability, and ultimately to improve people's health. There is growing interest in the principles of implementation research and a range of perspectives on its purposes and appropriate methods. However, limited efforts have been made to systematically document and review learning from the practice of implementation research across different countries and technical areas. Drawing on an expert review process, this Health Policy paper presents purposively selected case studies to illustrate the essential characteristics of implementation research and its application in low-income and middle-income countries. The case studies are organised into four categories related to the purposes of using implementation research, including improving people's health, informing policy design and implementation, strengthening health service delivery, and empowering communities and beneficiaries. Each of the case studies addresses implementation problems, involves partnerships to co-create solutions, uses tacit knowledge and research, and is based on a shared commitment towards improving health outcomes. The case studies reveal the complex adaptive nature of health systems, emphasise the importance of understanding context, and highlight the role of multidisciplinary, rigorous, and adaptive processes that allow for course correction to ensure interventions have an impact. This Health Policy paper is part of a call to action to increase the use of implementation research in global health, build the field of implementation research inclusive of research utilisation efforts, and accelerate efforts to bridge the gap between research, policy, and practice to improve health outcomes.

The purpose of this article is to comprehensively describe the impact of sustainability strategic planning and management (SSPM) on organisational sustainable performance in a developing country context. A mixed method approach was adopted; data were collected using an interview and a survey targeting 126 organisations operating in the foremost polluting Palestinian manufacturing private sectors. The analyses were conducted employing thematic analysis and Partial Least Squares Structural Equation Modelling (PLS-SEM). The results from data analysis confirm that SSPM had a positive effect on the three dimensions of organisational sustainable performance (i.e. social, ecological and economic). Furthermore, the paper has developed a framework to facilitate integrating SSPM into the business model of manufacturing organisations in a developing country context. This paper is considered among the very few studies exploring the ‘questioned’ impacts of SSPM on organisational sustainable performance in a developing country.

In high-income countries, there is an increased tendency to replace inactivated seasonal trivalent influenza (TIV) vaccines with quadrivalent (QIV) vaccines as these are considered to give a greater public health benefit. In addition, several recent studies from the USA and Europe indicate that replacement with QIV might also be cost-effective; however, the situation in low- and middle-income countries (LMIC) is less clear as few studies have investigated this aspect. The paper by de Boer et al. (2008) describes a dynamic modelling study commissioned by WHO that suggests that in LMICs, under certain conditions, QIV might also be more cost-effective than TIV. In this commentary, we discuss some important aspects that policymakers in LMICs might wish to take into account when considering replacing TIV by QIV. Indeed, from the data presented in the paper by de Boer et al. it can be inferred that replacing QIV for TIV would mean a 25-29% budget increase for seasonal influenza vaccination in South Africa and Vietnam, resulting in an incremental influenza-related health impact reduction of only 7-8% when a 10% symptomatic attack rate is assumed. We argue that national health budget considerations in LMIC might lead decision-makers to choose other investments with higher health impact for a budget equivalent to roughly a quarter of the yearly TIV immunization costs. In addition to an increased annual cost that would be associated with a decision to replace TIV with QIV, there would be an increased pressure on manufacturers to produce QIV in time for the influenza season requiring manufacturers to produce some components of the seasonal vaccine at risk prior to the WHO recommendations for influenza vaccines. Unless the current uncertainties, impracticalities and increased costs associated with QIVs are resolved, TIVs are likely to remain the more attractive option for many LMICs. Each country should establish its context-specific process for decision-making based on national data on disease burden and costs in order to determine whether the health gains out-weigh the additional cost of moving to QIV. For example, immunizing more people in the population, especially those in higher risk groups, with TIV might not only provide better value for money but also deliver better health outcomes in LMICs. Countries with local influenza vaccine manufacturing capacity should include in their seasonal influenza vaccine procurement process an analysis of the pros- and cons- of TIV versus QIV, to ensure both feasibility and sustainability of local manufacturing.

Aim To review the literature on psychosocial interventions for addiction affected family members in Low and Middle Income Countries (LMIC). Methods A systematic review with a detailed search strategy focussing on psychosocial interventions directed towards people affected by addiction without any gender, year or language specifications was conducted. Identified titles and abstracts were screened; where needed full papers retrieved, and then independently reviewed. Data was extracted based on the aims of the study, to describe the modalities, acceptability, feasibility and effectiveness of the interventions. Results Four papers met our selection criteria. They were published between 2003 and 2014; the total sample size was 137 participants, and two studies were from Mexico and one each from Vietnam and Malaysia. The predominantly female participants comprised of parents, spouses and siblings. The common components of all the interventions included providing information regarding addiction, teaching coping skills, and providing support. Though preliminary these small studies suggests a positive effect on affected family members (AFM). There was lowering of psychological and physical distress, along with a better understanding of addictive behaviour. The interventions led to better coping; with improvements in self-esteem and assertive behaviour. The interventions, mostly delivered in group settings, were largely acceptable. Conclusions The limited evidence does suggest positive benefits to AFMs. The scope of research needs to be extended to other addictions, and family members other than spouse and female relatives. Indigenous and locally adapted interventions are needed to address this issue keeping in mind the limited resources of LMIC. This is a field indeed in its infancy and this under recognised and under-served group needs urgent attention of researchers and policy makers.

Refereed/Peer-reviewed Using a comprehensive dataset on unlisted firms in 22 developing countries this paper analyses the relationship between gender, microfinance and access to bank credit. Using heckprobit to control sample selection bias, we reveal evidence of discrimination against female-owned and female-led firms. Also, we find that a mechanism for microfinance to empower women is facilitating loan applications and loan approvals for female-owned firms. Our paper highlights the importance of addressing gender discrimination in the credit market in order to facilitate the development of female-owned firms.

Given many developing economies depend on primary commodities, the fluctuations of commodity prices may imply significant effects for the wellbeing of children. To investigate, this paper examines the relationship between child mortality and commodity price movements as reflected by country-specific commodity terms-of-trade. Employing a panel of 69 low and lower-middle income countries over the period 1970-2010, we show that commodity terms-of-trade volatility increases child mortality in highly commodity-dependent importers suggesting a type of 'scarce' resource curse. Strikingly however, good institutions appear able to mitigate the negative impact of volatility. The paper concludes by highlighting this tripartite relationship between child mortality, volatility and good institutions and posits that an effective approach to improving child wellbeing in low to lower-middle income countries will combine hedging, import diversification and improvement of institutional quality.

Background and goal The Municipal Solid Waste Management (MSWM) sector represents a major challenge for low-and middle-income countries due to significant environmental and socioeconomic issues involving rapid urbanization, their MSWM systems, and the existence of the informal waste sector. Recognizing its role, several countries have implemented various formalization measures, aiming to address the social problems linked to this sector. However, regardless of these initiatives, not all attempts at formalization have proved successful due to the existence of barriers preventing their implementation in the long term. Along with this, there is a frequent lack of knowledge or understanding regarding these barriers and the kind of measures that may enable formalization, thereby attaining a win-win situation for all the stakeholders involved. In this context, policy- and decision-makers in the public and private sectors are frequently confronted with the dilemma of finding workable approaches to formalization, adjusted to their particular MSWM contexts. Building on the review of frequently implemented approaches to formalization, including an analysis of the barriers to and enabling measures for formalization, this paper aims to address this gap by explaining to policy- and decision-makers, and to waste managers in the private sector, certain dynamics that can be observed and that should be taken into account when designing formalization strategies that are adapted to their particular socioeconomic and political-institutional context. This includes possible links between formalization approaches and barriers, the kinds of barriers that need to be removed, and enabling measures leading to successful formalization in the long term. Method This paper involved a literature review of common approaches to formalization, which were classified into three categories: (1) informal waste workers organized in associations or cooperatives; (2) organized in CBOs or MSEs; and (3) contracted as individual workers by the formal waste sector. This was followed by the identification and subsequent classification of measures for removing common barriers to formalization into five categories: policy/legal, institutional/organizational, technical, social, and economic/financial. The approaches to formalization, as well as the barrier categories, were validated through the assessment of twenty case studies of formalization. Building on the assessment, the paper discussed possible links between formalization approaches and barriers, the ‘persistent’ challenges that represent barriers to formalization, as well as key enabling factors improving the likelihood of successful formalization. Results Regardless of the type of approach adopted to formalization, the review identifies measures to remove barriers in all five categories, with a stronger link between the approaches 1 and 2 and the existence of measures in the policy, institutional, and financial categories. Regarding persistent barriers, the review identified ones arising from the absence of measures to address a particular issue before formalization or due to specific country- or sector-related conditions, and their interaction with the MSWM context. 75% of the case studies had persistent barriers in respect of policy/legal issues, 50% of institutional/organizational, 45% of financial/economic, and 40%, and 35% of social and technical issues respectively. Conclusions This paper concludes that independently of the formalization approach, the lack of interventions or measures in any of the five categories of barriers may lead formalization initiatives to fail, as unaddressed barriers become ‘persistent’ after formalization is implemented. Furthermore, ‘persistent barriers’ may also appear due to unfavorable country-specific conditions. The success of a formalization initiative does not depend on a specific approach, but most likely on the inclusion of country-appropriate measures at the policy, economic and institutional levels. The empowerment of informal waste-workers is again confirmed as a further key success factor for their formalization.

Background Interpretive-naturalist methodologies, including Grounded Theory are increasingly being used in health research in Ghana however, none of the studies that have used Grounded Theory in the setting has used the methodology in full. Aim The main aim of this paper is to describe the experiences, the strengths and limitations of using Glaserian Grounded Theory methodology to understand the barriers to Ghanaian midwives’ ability to provide quality maternal and neonatal care. Design Glaserian Grounded Theory methodological principles were adhered to in this study in relation to the use of literature, participant recruitment, data collection and analysis, and theory development. Data were collected through semi-structured interviews and non-participant observation. The study population comprised 33 participants, made up of 29 midwives and four other workers whose work was relevant to the functioning of the midwives that took part in the study. The midwives were recruited from 10 health facilities in seven districts in the Greater Accra Region of Ghana. Glaserian Grounded Theory was used in this study therefore constant comparison was employed in the data analysis. Findings The study led to the discovery of a middle range grounded theory – “Doing magic with very little”, that explains the factors that affect the midwives’ ability to provide quality care to women and neonates. Conclusion This paper contributes to the body of work on methodological knowledge. We provide new information about the factors that researchers planning to use Glaserian Grounded Theory in similar contexts could consider.

In light of the on-going pandemic, it has been recommended that in-patient visits for cancer patients should be substituted by virtual visits and patients should be advised to proceed directly for infusion treatment. Telemedicine and tele-health based interventions have emerged as reasonably practical solutions to these impediments in the delivery of care to cancer patients. Technological advancements have resolved the issue of connectivity for telemedicine even to the remotest places. Teleconsultation is becoming an acceptable alternative for patients and health care providers in this era of information technology. Albeit the challenges that we are facing are diverse and therefore cannot have a singular full proof answer, telemedicine and tele-health based interventions seem to offer promise in effectively complementing our efforts in that direction. Telemedicine is beneficial for both patients and doctors in term to provide quality care without shifting to physical location.

This paper provides an international overview of the most recent estimates of the social costs of road crashes: total costs, value per casualty and breakdown in cost components. The analysis is based on publications about the national costs of road crashes of 17 countries, of which ten high income countries (HICs) and seven low and middle income countries (LMICs). Costs are expressed as a proportion of the gross domestic product (GDP). Differences between countries are described and explained. These are partly a consequence of differences in the road safety level, but there are also methodological explanations. Countries may or may not correct for underreporting of road crashes, they may or may not use the internationally recommended willingness to pay (WTP)-method for estimating human costs, and there are methodological differences regarding the calculation of some other cost components. The analysis shows that the social costs of road crashes in HICs range from 0.5% to 6.0% of the GDP with an average of 2.7%. Excluding countries that do not use a WTP- method for estimating human costs and countries that do not correct for underreporting, results in average costs of 3.3% of GDP. For LMICs that do correct for underreporting the share in GDP ranges from 1.1% to 2.9%. However, none of the LMICs included has performed a WTP study of the human costs. A major part of the costs is related to injuries: an average share of 50% for both HICs and LMICs. The average share of fatalities in the costs is 23% and 30% respectively. Prevention of injuries is thus important to bring down the socio-economic burden of road crashes. The paper shows that there are methodological differences between countries regarding cost components that are taken into account and regarding the methods used to estimate specific cost components. In order to be able to make sound comparisons of the costs of road crashes across countries, (further) harmonization of cost studies is recommended. This can be achieved by updating and improving international guidelines and applying them in future cost studies. The information regarding some cost components, particularly human costs and property damage, is poor and more research into these cost components is recommended.

Highlights • We model 5q0 decline with cross-country heterogeneity in technical progress. • Estimated income elasticity of 5q0 is reduced from around −0.35 to around −0.12. • We estimate a high impact of technical progress of around 80% of recent 5q0 decline. • Geographical context and health systems have strong effects. • Female education is more important in open than closed economies., Controlling for socioeconomic and geographic factors, under-5 mortality (5q0) in developing countries has been declining at about 2.7% per year, a high rate of ‘technical progress’. This paper adduces theoretical and empirical reasons for rejecting the usual specification of homogeneous technical progress across countries and uses a panel of 95 developing countries for the period 1970–2000 to explore the consequences of heterogeneity. Allowing country-specific rates of technical progress sharply reduces the estimated income elasticity of 5q0 and points to country variation in technical progress as the principal source of the (large) cross-country variation in 5q0 decline. Education levels and physician coverage also contribute and are less affected than income of allowing country variation in technical progress. The paper concludes by decomposing 1970–2000 5q0 decline into its different sources for each country.

Population growth particularly in countries with developing economies will result in a need to increase food production by 70% by the year 2050. Biotechnology has been utilized to produce genetically modified (GM) crops for insect and weed control with benefits including increased crop yield and will also be used in emerging countries. A multicomponent safety assessment paradigm has been applied to individual GM crops to determine whether they as safe as foods from non-GM crops. This paper reviews methods to assess the safety of foods from GM crops for safe consumption from the first generation of GM crops. The methods can readily be applied to new products developed within country and this paper will emphasize the concept of data portability; that safety data produced in one geographic location is suitable for safety assessment regardless of where it is utilized.

Plastic particles are small micro and nano range materials internationally produced for industrial applications or as a result of the breakdown of larger plastics. The accumulation of plastic particles varies spatially and temporally, with locations, hydrodynamic conditions, environmental pressure, time, and anthropogenic factors accounting for the disparities in dispersal and distribution. The accelerating global release of plastic particles into the environment seems unending, with long-term consequences for water quality, biodiversity, biological conservation, and human health. In this paper, we review the formation, presence, and toxicity of plastic particles in water, land, and air. Plastic wastes and plastic particles are increasing in abundance every year with increased consumption and rise in the global population. These materials are easily transported over long distances and become ubiquitous in their distribution on land, water, and air with unknown and unproven ecological and human health risks. Notwithstanding the potential threats to ecological health, literary records of plastic particle contamination of environmental media in Africa are very scanty. Managing environmental waste plastics and plastic particles and ensuring effective intervention policies and practices requires an understanding of plastic particles sources, transport, and fate, through regional and international collaboration, and interdisciplinary research. In consideration of the potential threats of plastic particles to ecological functions and human health risks, we recommend specific regulation of microplastics in consumer products. The information contained in this paper adds to the growing body of knowledge in the study of micro- and nano-plastic particles worldwide but with particular reference to developing countries.

This paper makes an attempt to explore whether intelligence of nations is related to gender inequality, measured by Social Institutions and Gender Index (SIGI), in developing countries. Related literature robustly links intelligence to economic development, poverty, quality of institutions and informal economic activity. Controlling for conventional antecedents of gender inequality (i.e. religion, political regime, legal origins and trade openness), this paper finds that, on average, a 10-point increase in national IQ scores in the developing world is associated with an 8.2 point reduction in SIGI, ceteris paribus. To test the robustness of our findings we apply instrumental variables (IV) and robust regression methods. We also test whether our results are sensitive to the choice of control variables and heterogeneity of nations in our sample. The negative association of intelligence with gender inequality remains statistically significant and intact in all cases.

Although the differences between developed and developing countries have been extensively studied in the context of globalization strategies, few studies have so far been conducted on the relationship between country development status and the possession by countries of a favorable (or unfavorable) product country image (PCI). Moreover, the results of such studies to date have been inconclusive. The purpose of this paper is to investigate the moderating role of country developmental status on PCI coupled with two antecedents of PCI, namely consumer ethnocentrism and cosmopolitanism. The paper also distinguishes between the PCI of the home and foreign country images of respondents. We test a new model that incorporates these constructs with a sample of 2655 younger generation consumers. The results show that country development status moderates some relationships but does not moderate others. These findings have significant implications for international companies from both developed and developing countries when developing global strategy.

In developing countries, wastes are usually not separated before being disposed of in solid-waste landfills, most of which are open dumps without adequate measures to prevent environmental pollution. To understand the leaching behavior of brominated flame retardants (BFRs) from waste consumer products in landfills, we have been conducting a long-term landfill lysimeter experiment since 2006 under conditions designed to mimic three types of landfill conditions in developing countries: aerobic, semi-aerobic, and anaerobic. Pilot-scale lysimeters (60-cm i.d.) were filled with a 400-cm layer of mixed wastes consisting of 35 wt% food, 20 wt% paper, 20 wt% paper pulp, 13 wt% plastic, 10 wt% wood chips, 1 wt% glass, and 1 wt% metals, proportions that are typical of unsorted municipal solid waste in Asian developing countries. In the present study, we determined the concentrations of polybrominated diphenyl ethers, tetrabromobisphenol A, tribromophenols, and hexabromocyclododecanes in leachate samples collected from the lysimeters during the first 3.5 years of the experiment, to evaluate BFR elution behavior in early-stage landfills. Under all three conditions, BFR elution started at the beginning of the experiment. The BFR concentrations in the leachates from the aerobic lysimeter tended to be lower than those from the anaerobic lysimeter, suggesting that the presence of air inside landfills considerably reduces BFR elution to the surrounding environment. During the 3.5-year experiment, BFR outflow from the lysimeters was only 0.001–0.58% of the total BFRs in the loaded waste; that is, most of the BFRs in the waste remained in the lysimeters.

This paper examines the different internal and external institutional factors that affect the decision of businesses in developing countries to adopt international certification (IC). Past studies focus on pressure from international laws, the role of multinationals, and businesses mimicking practices of their counterparts in developed countries. This paper finds that, in addition to these external factors, internal factors may have a significant role. Even though environmental regulation is weak in developing countries, governments do not ignore industrial pollution and casualties. They respond by increasing bureaucratic regulations for businesses and this can affect the decision to adopt IC. Furthermore, internal pressure may come from workers' unions that push for a safe and healthy working environment.

Start-ups are counted as ‘engine’ for innovation, especially in developing countries. However, in developing countries, bureaucratic barriers and costs are main impediments for start-ups. In this paper, the main barriers and costs are examined within panel data analysis framework in the developing countries indicating whether there are barriers and costs for start-ups between 2004 and 2012. Panel data analysis which has corrected robust standard errors for heteroscedascity and autocorrelation is used in this paper for researching start-ups condition in 61 developing countries. The results were obtained show that there are bureaucratic barriers and costs are main impediments for start-ups and innovation in the developing countries.

This paper aims to provide inspiration for how risk transfer tools such as insurance can facilitate approaches to address loss and damage associated with the impacts of climate change including slow-onset events. In particular it aims to show how insurance can be used in conjunction with a wide set of climate risk management tools to bolster societal resilience. The paper offers an overview of slow-onset climatic processes, provides a description on the current innovative tools and approaches to help reduce loss and damage associated with slow onset events provides case studies, and discussed some of the gaps and challenges related to implementation and enabling environment needed to manage climate risks in developing countries.

There is a strong correlation between electrification and rise in human development, but in developing countries access to electricity is often unreliable, unavailable, or unobtainable, especially from a centralized electric grid. Portable electrification using batteries provides an affordable energy source while sidestepping costs and regulations associated with physical transmission lines. In this paper, an anaerobic digestion system consumes organic waste and produces biogas that fuels an electric generator for battery charging. The paper presents a detailed approach for conducting an engineering economic analysis of the system. Two candidate business models to evaluate the potential of portable energy storage device distribution services of different sizes to return profit are proposed. The results offer a promising starting point for local entrepreneurs to implement similar business models, particularly at a small scale with a modest capital investment of about $1500, with revenue of about $100 per week, and a payback period of 18 months on the investment.

Implementation science is a useful tool to consider ways in which we can introduce improvements to burn services in low-and-middle income countries (LMICs), where the majority of the burden of burn injury is now experienced. This paper outlines the development of the Delivery Assessment Tool (DAT), a method for facilitating quality improvement in burn services in LMICs. We used a participatory approach that ensured that local clinicians and experts were fully involved in piloting the tool. The DAT is based on internationally agreed operational standards for burn care service delivery and has undergone an iterative process of improvement and refinement through an initial three-year project in Nepal and Bangladesh. The DAT, a 50-item tool organised into 10 subsections, is used to assess a service through a participatory focus group discussion (FGD) with a mixed, multidisciplinary team of staff working at the burn service, typically 6-10 participants. This usually lasts 2 to 3hours. The staff in the unit then select priority areas for quality improvement programmes that are within their control to achieve, which starts a cycle of audit and review. The final version of the tool was used in a further three-year project to evaluate 11 hospitals in Nepal and Bangladesh. Education and training are key components of this work; both were provided by Interburns as part of on-going support for the clinical teams. At the end of the project a >19% improvement in scores was demonstrated using the final version of the DAT in both Nepal and Bangladesh; this achievement is remarkable given the continued difficulties in service provision where patient numbers far outstrip the resources available to care for them. As a result of this work, we have made a digital version of the tool available free of charge.

The changing direction of foreign direct investment (FDI) from developed countries towards developing ones, especially after the crisis, has started to attract more attention in the literature. In this paper, the link between FDI and “ease of doing business” indicators, as one possible source of the changing direction of FDI, is investigated. The data source is the World Bank's Doing Business Database. The study covers the years from 2004 to 2010. Because the paper includes the years right before the economic and financial global crisis, as well as the crisis period, the impact of changing “ease of doing business” on the changing direction of FDI towards developing countries can be better evaluated. The initial results show that countries which have better records of “doing business” tend to attract more FDI. The improvement in “ease of doing business” indicators in developing countries can have a partial explanatory power in determining higher FDI flows to these countries.

The advent of eLearning has seen online discussion forums widely used in both undergraduate and postgraduate nursing education. This paper reports an Australian university experience of design, delivery and redevelopment of a distance education module developed for Vietnamese nurse academics. The teaching experience of Vietnamese nurse academics is mixed and frequently limited. It was decided that the distance module should attempt to utilise the experience of senior Vietnamese nurse academics — asynchronous online discussion groups were used to facilitate this. Online discussion occurred in both Vietnamese and English and was moderated by an Australian academic working alongside a Vietnamese translator. This paper will discuss the design of an online learning environment for foreign correspondents, the resources and translation required to maximise the success of asynchronous online discussion groups, as well as the rationale of delivering complex content in a foreign language. While specifically addressing the first iteration of the first distance module designed, this paper will also address subsequent changes made for the second iteration of the module and comment on their success. While a translator is clearly a key component of success, the elements of simplicity and clarity combined with supportive online moderation must not be overlooked.

At Copenhagen, the developed countries agreed to provide up to $100 bn per year to finance climate change mitigation and adaptation by developing countries. Projects aimed at cutting greenhouse gas (GHG) emissions will need to be evaluated against dual criteria: from the viewpoint of the developed countries they must cut emissions of GHGs at reasonable cost, while host countries will assess their contribution to development, or simply their overall economic benefits. Co-benefits of some types of project will also be of interest to host countries: for example some projects will contribute to reducing air pollution, thus improving the health of the local population.This paper uses a simple damage function methodology to quantify some of the health co-benefits of replacing coal-fired generation with wind or small hydro in China. We estimate the monetary value of these co-benefits and find that it is probably small compared to the added costs. We have not made a full cost-benefit analysis of renewable energy in China as some likely co-benefits are omitted from our calculations. Our results are subject to considerable uncertainty however, after careful consideration of their likely accuracy and comparisons with other studies, we believe that they provide a good first cut estimate of co-benefits and are sufficiently robust to stand as a guide for policy makers.In addition to these empirical results, a key contribution made by the paper is to demonstrate a simple and reasonably accurate methodology for health benefits estimation that applies the most recent academic research in the field to the solution of an increasingly important problem. Small Medium Enterprises (SMEs) have significant role in employment creation and growth of gross domestic products of developing country. In the case of Indonesia, SMEs account for more than 90 percents of all company and in employment [1]. However, in order to grow and contribute more significantly to the economy, SMEs face some constraints. One of the main constraints faced by SMEs is the lack of finance. Islamic bank financing products may help to solve this problem. The Islamic participatory schemes, such as mudarabah and musyarakah , integrate assets of lender and borrowers; therefore, they allow Islamic banks to lend on a longer-term basis to projects with higher risk-return profiles and, thus, to support economic growth. However, as Islamic banks try to avoid uncertainties, the mentioned schemes are not widely used. Therefore, support from government and academia needed to create innovation in the participatory financing scheme so that all related parties can share mutual benefits. Using Indonesia data, the paper analyzes data from Indonesia's National Agency of Statistics and Central Bank of Indonesia and reviews key literature and secondary data on Indonesian SMEs and Islamic banks. In the end, the paper offers a framework in which Islamic financing scheme could be used to solve financing problem faced by Small and Medium Enterprises (SMEs) in the context of developing country.

Depending on their nature and severity, disasters can create large volumes of debris and waste. The waste can overwhelm existing solid waste management facilities and impact on other emergency response and recovery activities. If poorly managed, the waste can have significant environmental and public health impacts and can affect the overall recovery process. This paper presents a system overview of disaster waste management based on existing literature. The main literature available to date comprises disaster waste management plans or guidelines and isolated case studies. There is ample discussion on technical management options such as temporary storage sites, recycling, disposal, etc.; however, there is little or no guidance on how these various management options are selected post-disaster. The literature does not specifically address the impact or appropriateness of existing legislation, organisational structures and funding mechanisms on disaster waste management programmes, nor does it satisfactorily cover the social impact of disaster waste management programmes. It is envisaged that the discussion presented in this paper, and the literature gaps identified, will form a basis for future comprehensive and cohesive research on disaster waste management. In turn, research will lead to better preparedness and response to disaster waste management problems.

The public health burden of tobacco use is shifting to the developing world, and the tobacco industry may apply some of its successful marketing tactics, such as allaying health concerns with product modifications. This study used standard smoking machine tests to examine the extent to which the industry is introducing engineering features that reduce tar and nicotine to cigarettes sold in middle- and low-income countries.Multicountry observational study.Cigarettes from 10 different countries were purchased in 2005 and 2007 with low-, middle- and high-income countries identified using the World Bank's per capita gross national income metric. Physical measurements of each brand were tested, and tobacco moisture and weight, paper porosity, filter ventilation and pressure drop were analysed. Tar, nicotine and carbon monoxide emission levels were determined for each brand using International Organization for Standardization and Canadian Intensive methods. Statistical analyses were performed using Statistical Package for the Social Sciences.Among cigarette brands with filters, more brands were ventilated in high-income countries compared with middle- and low-income countries [χ(2)(4)=25.92, P0.001]. Low-income brands differed from high- and middle-income brands in engineering features such as filter density, ventilation and paper porosity, while tobacco weight and density measures separated the middle- and high-income groups. Smoke emissions differed across income groups, but these differences were largely negated when one accounted for design features.This study showed that as a country's income level increases, cigarettes become more highly engineered and the emissions levels decrease. In order to reduce the burden of tobacco-related disease and further effective product regulation, health officials must understand cigarette design and function within and between countries.

There is by now quite a substantial body of literature discussing the impact of an ageing population in developed countries on travel needs and required changes to transport policy. As many newly developed and developing countries are following demographic trends of “first world” countries, but offset by some decades, the problem is, however, not limited to the industrialised nations. The focus of this paper is on Metro Manila and analyses travel patterns by those aged 60 or over. Trip frequency and tour complexity are analysed with ordered probit regression, separating the effects of socio-demographic characteristics as well as land-use patterns. The results are compared to observations made for cities in developed countries, in particular London as an example for a city in a first world country. We show that there is a more pronounced decrease in total trips made with increasing age in Manila. However, analysing for specific trip purposes we find, similarly to trends in developed countries, that the number of recreational trips is fairly constant in all age groups. Recreational activities also seem to take more time per day than average for younger old, possibly indicating the advent of similar active ageing trends as in industrialised nations. The paper concludes by discussing some implications given future economic trends and advocates that better datasets from developing and newly developed countries are required for urban planning in developing countries.

Background: Perinatal mental health disorders are recognised as an important public health issue in low-income countries as well as in developed countries. This paper reviews evidence on the prevalence and risk factors of maternal mental health disorders in African women living in Africa. Methods: A systematic review of the literature was conducted. Studies were mainly located through computerised databases, and additionally through hand searching references of identified articles and reviews. Thirty-five studies, with a total of 10,880 participants, were identified that reported prevalence rates of maternal psychological health in eight African countries. Results: Depression was the most commonly assessed disorder with a weighted mean prevalence of 11.3% (95% CI 9.5%-13.1%) during pregnancy and 18.3% (95% CI 17.6%-19.1%) after birth. Only a small number of studies assessed other psychological disorders. Prevalence rates of pre- and postnatal anxiety were 14.8% (95% CI 12.3%-17.4%) and 14.0% (95% CI 12.9%-15.2%), respectively; and one study reported the prevalence of PTSD as 5.9% (95% CI 4.4%-7.4%) following childbirth. Lack of support and marital/family conflict were associated with poorer mental health. Evidence relating sociodemographic and obstetric variables to mental health was inconclusive. Limitations: Most studies included in this review were cross-sectional and measures of mental health varied considerably. Conclusions: This paper demonstrates that maternal mental health disorders are prevalent in African women, and highlights the importance of maternal mental health care being integrated into future maternal and infant health policies in African countries. 2009 Elsevier B.V. This paper critically appraises this contention by conducting a vigorous empirical approach using panel data for 107 developing countries. The long-run and dynamic estimation results based on McKinnon’s theoretical model are supportive of the hypothesis. However, when the investment model is conditioned by factors such as financial development, different income levels across developing countries, external inflows, public finance, and trade constraints, the credibility of the hypothesis is undermined.

In developing countries like India, the traffic on roads is highly heterogeneous in nature, with vehicles of widely varying static and dynamic characteristics. In this type of traffic, vehicles do not follow lane discipline and they move freely over the entire width of roadway based on availability of space. To study this type of complex traffic flow and associated vehicular interactions, simulation is considered as an effective tool. An object-oriented methodology (OOM) for heterogeneous traffic simulation is proposed in this paper with focus on mid-block and intersection flow modeling. The paper presents the basics and advanced features of object-oriented programming (OOP) in detail in the context of traffic flow. The sample C++ code is discussed in detail to demonstrate the implementation of OOP features, such as encapsulation, inheritance and polymorphism. The contribution of this research work is the development of software objects for various components such as vehicle, traffic, link and node. To point out the direction and focus of future international climate negotiations, this paper discusses how far developing country commitments can go in an immediate post-2012 climate regime. The paper argues that developing country commitments are most unlikely to go beyond the defined polices and measures in this timeframe. On this basis, the paper suggests that, rather than attempting the unrealistic goal, international climate negotiations may instead need to initially frame the post-2012 developing country participation in terms of certain policies and policies that I envisioned a decade ago. This conclusion does not change, as Barack Obama becomes the U.S. President and the Democrats have regained control over both U.S. House of Representatives and Senate. However, it should be emphasized that his stance on climate issues and how ambitious U.S. commitments would be under his administration are going to be critical for developing countries to take bold steps themselves and to even agree to reflect those national commitments in a global deal.

During the last decades of the 20th century it became increasingly apparent that the inter-relationship between globalisation and health is extremely complex. This complexity is highlighted in debates surrounding the re-emergence of infectious diseases, where it is recognised that the processes of globalisation have combined to create the conditions where once localised, microbial hazards have come to pose a threat to many western nations. By contrast, in an emerging literature relating to the epidemic of non-communicable diseases, and reflected in the WHO 'Global strategy on diet, physical activity and health', it is the so-called 'western lifestyle' that has been cast as the main threat to a population's health. This paper explores critically global responses to this development. Building on our interest in questions of governance and the ethical management of the healthy body, we examine whether the global strategy, in seeking to contain the influence of a 'western lifestyle', also promotes contemporary 'western-inspired' approaches to public health practices. The paper indicates that a partial reading of the WHO strategy suggests that certain countries, especially those outside the West, are being captured or 'enframed' by the integrative ambitions of a western 'imperial' vision of global health. However, when interpreted critically through a postcolonial lens, we argue that 'integration' is more complex, and that the subtle and dynamic relations of power that exist between countries of the West/non-West, are exposed.

This paper analyzes the practices adopted and difficulties experienced by Small and Medium-sized Enterprises (SMEs) to transfer socially responsible behaviors to suppliers that operate in developing countries. In particular, a multiple case study was conducted on five Italian socially responsible SMEs. It was found that companies use different strategies as well as diverse management systems and tools to address Corporate Social Responsibility (CSR) issues along their supply chains (SCs). The paper is innovative mainly since few are the existing studies that investigate the CSR practices adopted by SMEs in the SC. Furthermore, it is argued that our research can be helpful to SME managers willing to deal with CSR issues along their SCs, especially when developing countries are involved.

This paper argues that ergonomics is more sorely needed, easier to implement, and potentially far more effective in industrially developing countries (IDCs) than where its efforts are presently most concentrated in the less populated, more affluent, technologically advanced world. The reasoning is a simple extension of the principle of diminishing returns in which the further from optimal a situation is, the greater the beneficial effect of any implemented improvement. The paper draws attention to the gap between ‘have’ and ‘have not’ cultures, plus the necessity for, and relative ease with which a sustainable ergonomics ethos can be engendered in IDCs. This requires a need to consider network causality, investigating both micro-problems (basic interaction between task and worker) and macro-conditions of the overall scenario (including managerial organisation, planning and responsibility). The two-pronged symbiosis of micro- and macro-ergonomics intervention has the potential to achieve both effective and sustainable development within small, medium and large enterprises.

A black spot treatment program is one type of safety improvement program that can deal with accident occurrences reactively. However, implementation of such program requires relevant accident data, which are normally unavailable or limited in developing countries. Thus, this paper proposes a supportive approach (public participation approach) to overcome this hindrance. The objective is to demonstrate how a public participation approach can be used to assist in identifying black spot locations through the framework of an Accident Public Participation Program (APPP). The real world applications of such a program are presented through a selected case study in Khon Kaen City, Thailand. By contrasting the findings obtained from the proposed framework with those obtained from the classical approach in this field, feasibility, validity, and effectiveness of the program are revealed. The findings indicate statistically significant agreements between the two datasets. This implies that residence can identify locations where accidents occurrences are unusually high and their input is potentially useful for the identification process. In addition, besides the indirect benefits to creating public awareness, the proposed methodology is potentially useful as a means for both speeding up and economizing the black spot locations identification process. The paper is unique because the merits of several professionals in fields such as social science, engineering, and medicine are harmonized to achieve success in a safety improvement program.

Digital, Background Separate studies suggest that the risks from smoking might vary between high-income (HICs), middle-income (MICs), and low-income (LICs) countries, but this has not yet been systematically examined within a single study using standardised approaches. We examined the variations in risks from smoking across different country income groups and some of their potential reasons. Methods We analysed data from 134 909 participants from 21 countries followed up for a median of 11·3 years in the Prospective Urban Rural Epidemiology (PURE) cohort study; 9711 participants with myocardial infarction and 11 362 controls from 52 countries in the INTERHEART case-control study; and 11 580 participants with stroke and 11 331 controls from 32 countries in the INTERSTROKE case-control study. In PURE, all-cause mortality, major cardiovascular disease, cancers, respiratory diseases, and their composite were the primary outcomes for this analysis. Biochemical verification of urinary total nicotine equivalent was done in a substudy of 1000 participants in PURE. This paper critically reviews an experience of health cooperation in an hospital of a rural area of Ivory Coast. This particular situation is analysed in the more general frame of health problems in low-income countries and may suggest priorities for international health cooperation. The analysis of the main causes of avoidable death in poor countries does indicate targets and tools of intervention. In this case, the target was the reduction of infant mortality from anaemia of different origin and from HIV-1 mother-to-infant transmission. The major tool for intervention was the partnership between an Italian teaching and research hospital and the African hospital, with the catalyst of a non-governmental organisation. This paper analyses the different levels at which cooperation developed in this project, from sheer economic support to the implementation of disease-oriented twinning programs that can improve health care and strengthen research capacity on both sides. Besides, medical, ethical and social implications of the ongoing cooperation program are discussed, with particular reference to the problems of preventing mortality from severe anaemia (diet fortification in children and pregnancy and transfusional guidelines in severe malaria) and of preventing mother-to-child neonatal transmission of HIV-1 infection (counselling and testing pregnant women for HIV-1, nevirapine administering to the mother and the baby and breast-feeding).

This paper analyzes the effect of inflation targeting on private domestic investment in developing countries. Using propensity scores matching methods, thus mitigating traditional self-selection problems, we find that inflation targeting has led to a 2.05 -2.53 percentage point increase in domestic investment in targeting countries compared to nontargeting ones. Estimates are economically meaningful and robust to various checks, notably sample changes, additional controls, alternative estimation methods, and falsification tests. A few heterogeneity features of the treatment effect are further highlighted, depending on some factors. Exposure assessment, a component of risk assessment, links sources of pollution with health effects. Exposure models are scientific tools used to gain insights into the processes affecting exposure assessment. The purpose of this paper is to review the process and methodology of estimating inhalation exposure to particulate matter (PM) using various types of models. Three types of models are discussed in the paper. Indirect type of models are physical models that employ inventories of outdoor and indoor sources and their emission rates to identify major sources contributing to exposure to PM, and use fate and transport and indoor air quality models to estimate PM concentrations at receptor sites. PM concentrations and time spent by a subject at each receptor site are input variables to the conventional exposure model that estimates the desired exposure levels. Direct type models use measured exposure or exposure concentrations in conjunction with information obtained from questionnaires to formulate exposure regression models. Stochastic models use exposure measurements, estimates can also be used, to formulate exposure population distributions and investigate associated uncertainty and variability. Since models developed using databases from western countries are not necessarily applicable in developing countries, the difference in requirements among western and developing countries is highlighted in the paper. Employment of exposure modeling methods in developing countries requires development of local information. Such information includes local outdoor and indoor source inventories, local or regional meteorological conditions, adjustment of indoor models to reflect local building construction conditions, and use of questionnaires to obtain local time budget and activity patterns of the subject population.

Iron deficiency anemia (IDA) depresses human productivity, but policymakers do not generally view this effect as an impediment to sustained economic growth. Economic logic should be enfolded in public advocacy for increased investment in the prevention and control of IDA. This paper argues that integrated strategies are required, with each intervention clearly related to particular groups at risk, and benefits carefully calibrated with costs. Protecting women's lives through supplementation has the first call on public resources, but the most productive investments reduce population prevalence at least cost, and these lie with food-based approaches within the competence of the processed food and seed industries. The public and private sectors must embark on modernization of the food industry in developing countries and reorientation of the international agricultural research complex so that iron-enriched essential foods will be affordable and accessible to the poor, especially children. The costs of IDA, the availability of cost-effective strategies and the benefits of sustained iron nutrition improvement to individuals, families and nations are reviewed. The roles of iron supplementation, food fortification, plant breeding and biotechnology, both actual and imminent, are described. The paper concludes that a recast Green Revolution directed toward dietary quality may be the key to enhancing the learning and earning capacity of young people in the developing world. J. Hybrid PV/diesel generator/batteries systems are applied for many off-grid homes overall the world. Most of the existing studies neglect the effect of loading level of diesel generators on the fuel consumption and/or system efficiency. In addition to, the existing experience of smart home load management isn’t considered in such recent off-grid system energy management research. Therefore, a new offline smart isolated home load scheduling scheme is proposed in this paper to suit developing countries homes characteristics. In this work, detailed models of smart homes sources and appliances are presented. Daily load scheduling scheme is proposed that consider users’ weekly and daily plans to satisfy home occupant’s desires. It also extends batteries lifetime by controlling charging/discharging processes. Diesel generator loading levels are optimized to minimize the total fuel consumption. The proposed scheme is applied on the well-known Mixed Integer Linear Programming MILP optimization technique to introduce a simple and cheap load scheduling scheme for such remote unsophisticated communities. The proposed scheme reduces the fuel consumption to 20% with relatively comfortable lifestyle for home users.

Growth development theorists such as Rowstow have averred that third world countries (TWCs) need foreign Aid to stimulate development and move them into the 'take-off stage' which is characterised by growth and development. However, after decades of aid inflow into TWCs, the majority of TWCs are still struggling to achieve their development objectives. Therefore, it is doubtful if aids alone can jumpstart development in TWCs. This reality has given rise to a plethora of alternate propositions aimed at realising the development objectives of TWCs. The central argument of this paper is that Aid alone cannot improve the status of TWCs. It is further argued in the paper that Aid needs to be complemented by stimulants (initiated within TWCs) such as Remittance, Taxation, and Public-Private Partnership (PPP) to solve their economic development problems. To this end, it is suggested that an approach which combines indigenous options with Aid will significantly advance the development objectives of TWCs.

This paper analyzes the economic impact of unions on productivity in the manufacturing sector across six Latin American countries: Argentina, Bolivia, Chile, Mexico, Uruguay, and Panama. Using an augmented Cobb-Douglas production function, the paper finds that unions have positive, but mostly small, effects on productivity, with the exception of Argentina, with a large negative effect, and Bolivia, with no effect. An analysis on profitability shows that, in most cases, the positive productivity effects barely offset higher union compensation, and that unions are negatively related to investment in capital and R & D. Different explanations for these effects are discussed., Este artículo analiza el impacto económico de los sindicatos sobre la productividad en el sector manufacturero en seis países de América Latina: Argentina, Bolivia, Chile, México, Uruguay y Panamá. Utilizando una función de producción aumentada de Cobb-Douglas, el documento encuentra que los sindicatos tienen efectos positivos, pero sobre todo pequeños, sobre la productividad, con la excepción de Argentina, con un gran efecto negativo, y Bolivia, sin efecto. Un análisis de la rentabilidad muestra que, en la mayoría de los casos, los efectos positivos de la productividad apenas compensan una indemnización sindical más alta, y que los sindicatos están relacionados negativamente con la inversión en capital y en I + D. Low-income and middle-income countries (LMICs) have a disproportionately high burden of cancer and cancer mortality. The unique barriers to optimum cancer care in these regions necessitate context-specific research. The conduct of research in LMICs has several challenges, not least of which is a paucity of formal training in research methods. Building capacity by training early career researchers is essential to improve research output and cancer outcomes in LMICs. The International Collaboration for Research methods Development in Oncology (CReDO) workshop is an initiative by the Tata Memorial Centre and the National Cancer Grid of India to address gaps in research training and increase capacity in oncology research. Since 2015, there have been five CReDO workshops, which have trained more than 250 oncologists from India and other countries in clinical research methods and protocol development. Participants from all oncology and allied fields were represented at these workshops. Protocols developed included clinical trials, comparative effectiveness studies, health services research, and observational studies, and many of these protocols were particularly relevant to cancer management in LMICs. A follow-up of these participants in 2020 elicited an 88% response rate and showed that 42% of participants had made progress with their CReDO protocols, and 73% had initiated other research protocols and published papers. In this Policy Review, we describe the challenges to research in LMICs, as well as the evolution, structure, and impact of CReDO and other similar workshops on global oncology research.

This paper studies the pollution control problem faced by an imperfectly informed supranational governmental authority (SNGA) which wishes to design an international environmental agreement (IEA) for developing countries (DC). The SNGA cannot contract directly with polluting firms in the various DCs; it must deal with such firms through their national governments. Further, owing to national sovereignty, the SNGA is unable to either monitor the actions of DC governments and firms or enforce the terms of the IEA in the event of a contractual breach. In this setting, the properties of equitable IEAs in which similar DCs are held to similar environmental standards are studied. In particular, two cases are focused on. In the first case, governments and firms within individual DCs do not collude among themselves, and in the second case, they do. It is shown that when the private information of firms and governments across the two DCs is perfectly correlated, whether or not there is collusion, the SNGA can always implement the full information IEA in a Bayes-Nash equilibrium. My analysis tells us (i) that the significance of the monitoring and enforcement problem in such international settings has been exaggerated, and (ii) that technological similarities between DCs have a far greater bearing on the design problem than do the potentially deleterious effects of sovereignty. Indeed, there are a number of situations in which Pareto efficient IEAs can be designed by the SNGA.
