PURPOSE: Cerebral small vessel disease (cSVD) involves several pathologies affecting the small vessels, including blood-brain barrier (BBB) impairment. Dynamic susceptibility contrast (DSC) MRI is sensitive to both blood perfusion and BBB leakage, and correction methods may be crucial for obtaining reliable perfusion measures. These methods might also be applicable to detect BBB leakage itself. This study investigated to what extent DSC-MRI can measure subtle BBB leakage in a clinical feasibility setting.METHODS: In vivo DCE and DSC data were collected from fifteen cSVD patients (71 (±10) years, 6F/9M) and twelve elderly controls (71 (±10) years, 4F/8M). DSC-derived leakage fractions were obtained using the Boxerman-Schmainda-Weisskoff method (K2). K2 was compared with the DCE-derived leakage rate Ki, obtained from Patlak analysis. Subsequently, differences were assessed between white matter hyperintensities (WMH), cortical gray matter (CGM), and normal-appearing white matter (NAWM). Additionally, computer simulations were performed to assess the sensitivity of DSC-MRI to BBB leakage.RESULTS: K2 showed significant differences between tissue regions (P < 0.001 for CGM-NAWM and CGM-WMH, and P = 0.001 for NAWM-WMH). Conversely, according to the computer simulations the DSC sensitivity was insufficient to measure subtle BBB leakage, as the K2 values were below the derived limit of quantification (4∙10-3 min-1). As expected, Ki was elevated in the WMH compared to CGM and NAWM (P < 0.001).CONCLUSIONS: Although clinical DSC-MRI seems capable to detect subtle BBB leakage differences between WMH and normal-appearing brain tissue it is not recommended. K2 as a direct measure for subtle BBB leakage remains ambiguous as its signal effects are due to mixed T1- and T2∗-weighting. Further research is warranted to better disentangle perfusion from leakage effects.

The lenticulostriate artery (LSA) is a vital perforating cerebral artery, whose occlusion often leads to lacunar infarction. Currently, digital subtraction angiography is mainly used to visualize the LSA in the clinical setting; however, its invasiveness is an important limiting factor. Studies have shown that time-of-flight (TOF) sequencing using a high-field magnetic resonance system (7 T) can better image the LSA. However, the diameter of the LSA is extremely small (approximately 0.3-0.7 mm) with relatively slow blood flow velocity; therefore, imaging the LSA with a 3-T magnetic resonance imaging (MRI) scanner remains challenging. This study aimed to visualize the LSA using 3-dimensional-TOF magnetic resonance angiography (MRA) with compressed sensing using a 3-T system and compare the length and number of the LSAs between patients with infarction and normal controls. The scan times of 3D-TOF MRA with and without compressed sensing were 7 min, and 8 min 44 s, respectively. VR displayed the LSA clearly under both conditions. The total number (p 0.05) and length (p 0.05) of the LSAs did not differ significantly between 3D-TOF MRA with and without compressed sensing. However, the total length and number of visualized LSAs was significantly lower (p 0.05) in the infarction group compared to the control group for both TOF MRA and TOF MRA with compressed sensing. TOF MRA combined with compressed sensing is clinically valuable for analyzing the morphological characteristics of the LSA, and shortens the imaging time to 7 min. This combined technique can meet the requirements of shorter scanning times in clinical settings.

Diffusion Weighted Imaging (DWI) can be used to differentiate benign and malignant pulmonary nodules or masses, while T2WI is also of great value in the differential diagnosis of them. For example, T2WI can be used to differentiate abscess from lung cancer. The study aims to quantitatively evaluate the efficacy of free-breathing BLADE fat-suppressed T2 weighted turbo spin echo sequence (BLADE T2WI) for differentiating lung cancer (LC) and benign pulmonary nodule or mass (BPNM).A total of 291 patients with LC (197 males, 94 females; mean age 63.2 years) and 74 BPNM patients (53 males, 21 females; mean age 62.8 years) who underwent BLADE T2WI at 3-T MRI between November 2016 and May 2022were included in this retrospective study. Two radiologists independently blinded observed the MR images and measured the T2 contrast ratio (T2CR). Mann-Whitney U test was used to compare T2CR values between the two groups, ROC curves were used to evaluate the diagnostic efficacy of BLADE T2WI.The two radiologists had good inter-observer consistency for T2CR (ICC = 0.958). The T2CR of BPNM was significantly higher than LC (all p 0.001); the cut-off value of T2CR was 2.135, and the sensitivity, specificity, and accuracy of diagnosis were 75.6%, 63.5%, and 73.2%, respectively. Moreover, T2CR correctly diagnosed 220 LC cases (220/291 = 75.6%) and 47 BPNM cases (47/74 = 63.5%).The T2CR value of MR non-enhanced BLADE T2WI can be easily obtained and can quantitatively distinguish BPNM from LC, thus avoiding misdiagnosis caused by lack of work experience.

Lamellar bone, compactly and ingeniously organized in the hierarchical pattern with 6 ordered scales, is the structural motif of mature bone. Each hierarchical scale exerts an essential role in determining physiological behavior and osteogenic bioactivity of bone. Engineering lamellar bone with full-scale hierarchy remains a longstanding challenge. Herein, using bioskiving and mineralization, we attempt to engineer compact constructs resembling full-scale hierarchy of lamellar bone. Through systematically investigating the effect of mineralization on physicochemical properties and bioactivities of multi-sheeted collagen matrix fabricated by bioskiving, the hierarchical mimicry and hierarchy-property relationship are elucidated. With prolongation of mineralization, hierarchical mimicry and osteogenic bioactivity of constructs are performed in a bidirectional manner, i.e. first rising and then descending, which is supposed to be related with transformation of mineralization mechanism from nonclassical to classical crystallization. Construct mineralized 9 days can accurately mimic each hierarchical scale and efficiently promote osteogenesis. Bioinformatic analysis further reveals that this construct potently activates integrin α5-PI3K/AKT signaling pathway through mechanical and biophysical cues, and thereby repairing critical-sized bone defect. The present study provides a bioinspired strategy for completely resembling complex hierarchy of compact mineralized tissue, and offers a critical research model for in-depth studying the structure-function relationship of bone.

Intrauterine adhesion (IUA) is the fibrosis within the uterine cavity. It is the second most common cause of female infertility, significantly affecting women's physical and mental health. Current treatment strategies fail to provide a satisfactory therapeutic outcome for IUA patients, leaving an enormous challenge for reproductive science. A self-healing adhesive hydrogel with antioxidant properties will be highly helpful in IUA prevention. In this work, we prepare a series of self-healing hydrogels (P10G15, P10G20, and P10G25) with antioxidant and adhesive properties. Those hydrogels exhibit good self-healing properties and can adapt themselves to different structures. They possess good injectability and fit the shape of the human uterus. Moreover, the hydrogels exhibit good tissue adhesiveness, which is desirable for stable retention and therapeutic efficacy. The in vitro experiments using P10G20 show that the adhesive effectively scavenges ABTS+, DPPH, and hydroxyl radicals, rescuing cells from oxidative stress. In addition, P10G20 offers good hemocompatibility and in vitro and in vivo biocompatibility. Furthermore, P10G20 lowers down the in vivo oxidative stress and prevents IUA with less fibrotic tissue and better endometrial regeneration in the animal model. It can effectively downregulate fibrosis-related transforming growth factor beta 1 (TGF-β1) and vascular endothelial growth factor (VEGF). Altogether, these adhesives may be a good alternative for the clinical treatment of intrauterine adhesion.
