Abstract

Introduction Photobiomodulation therapy with the use of light-emitting diodes (LEDs) is a fast growing therapeutic technique with a wide range of dermatologic indications. Recently it has been suggested that LED therapy could be beneficial in scarring alopecia. Aim Assessment of the efficacy of novel superluminescent diodes (sLED) with a “soft- start” as an adjuvant treatment in selected types of scarring alopecia. Methods This pilot study included 16 female patients: 8 with frontal fibrosing alopecia (FFA) and 8 with lichen planopilaris (LPP), aged 41–76 years with a stable treatment, either topical or systemic. In all patients, sLED irradiations were performed once a week for a 10-week period. The sLEDs’ effectiveness was assessed clinically and using trichoscopy. Results The therapy was well tolerated. Both Lichen Planopilaris Activity Index (LPPAI) and Frontal Fibrosing Alopecia Severity Score (FFASS) were significantly reduced after the therapy (p = 0.012, p = 0.017 respectively). Within the treated area, the number of thick hairs significantly increased after the therapy (p = 0.009), whereas the number of medium-sized hairs and thin hairs did not change significantly (p = 0.836, p = 0.675 respectively). Conclusions Irradiations with sLEDs were demonstrated to be safe and well tolerated. This noninvasive therapy leads to the reduction of subjective symptoms, and improve the outcome both in FFA and in LPP. This effective novel light source can be used as an adjuvant therapy in patients with chronic LPP or FFA. However, further studies including a larger study group and a control group are needed to evaluate the long-term effects of the therapy.

Abstract

Background Superluminescent diodes (sLED) appear to be an innovative and promising light source in photodynamic therapy (PDT), especially in actinic keratosis (AK) lesions treatment. Aim Assessment of tolerability and efficacy of sLED in topical 5-aminolevulinic acid (ALA) PDT of AK lesions. Methods 27 patients received ALA PDT with the use of sLED with “soft starter”. Results Tolerability of sLED lamp depended on the treated lesions field (extension) rather than on their thickness. In contrast sLED lamp efficacy depended on AK lesions thickness. Conclusion sLED PDT is highly effective in the treatment of grade I and II AK lesions. Grade III AK lesions require further treatment.

Abstract

Background Psoriasis is a chronic autoimmune hyperproliferative skin disease of varying severity affecting approximately 2–3% of the general population in the USA and Europe. Although the pathogenesis of psoriasis has not been fully elucidated, an immunologic–genetic relationship is likely. Cutaneous and systemic overexpression of various proinflammatory cytokines (TNF, interleukins, interferon-gamma) has been demonstrated in psoriatic patients. Methods We reviewed the current database literature and summarized the involvement of cytokines and their receptors in the pathogenesis and treatment of psoriasis. Results Although many cytokine/anti-cytokine therapies have been conducted, TNF antagonists in the treatment of both psoriasis arthropatica and vulgaris appear to be the most widely used clinically. Interestingly, the efficacy and tolerability of some cytokines (rhIL-11 or ABX-IL-8,) were found to be much lower than expected. Conclusions Preliminary results obtained with cytokine and anti-cytokine therapies appear promising and as such continued research is clearly indicated.

Abstract

Selective seritonin reuptake inhibitors (SSRIs) are widely used antidepressants that are often safer than alternatives, but may produce a variety of cutaneous reactions including spontaneous bruising, pruritus, urticaria, angioedema, erythema multiforme, Stevens-Johnson syndrome, toxic epidermal necrolysis, erythema nodosum, alopecia, hypertrichosis, leukocytoclastic vasculitis, and an acneiform eruption. We review this category of medications and its side effects. Many cutaneous alterations seen in association with SSRIs can be serious, some even life threatening. Because there appears to be cross-reactions between SSRIs, even though they have different chemical structures, it is advisable to use another family of antidepressants if an SSRI is linked with a serious skin eruption.

Abstract

The purpose of the study was to explore serum pancreatic lipase activity and the serum lipid profile in relation to peripheral blood dendritic cell subsets and disease severity in males with psoriasis. Material and methods The study population consisted of 22 normolipidemic males with psoriasis and 12 aged-matched and body mass index (BMI)-matched healthy males. The percentages of peripheral blood dendritic cell (DC) subsets were evaluated using appropriate monoclonal antibodies and flow cytometry. The serum pancreatic lipase activity and the lipid profile were determined using standard enzymatic and colorimetric techniques. Results Pancreatic lipase activity was increased ( p = 0.56421), high-density lipoprotein (HDL)-cholesterol concentration ( p = 0.00584) was significantly decreased, triglyceride ( p = 0.00766) and VLDL-cholesterol ( p = 0.00765) levels were significantly increased in serum of psoriatic patients compared to controls. The serum pancreatic lipase activity showed significant correlation with serum triglyceride ( r = 0.42; p = 0.04721) and serum VLDL-cholesterol levels ( r = 0.42; p = 0.04721) in psoriatic individuals. In psoriatic patients the percentage of myeloid DCs was increased ( p = 0.54932), the percentage of lymphoid DCs was decreased ( p = 0.14210) and myeloid DC/lymphoid DC ratio was significantly increased ( p = 0.03569) compared to healthy individuals. Conclusion The direct cause of the abnormal lipid profile in psoriasis and its relationship with the immune system disturbances remains unclear. The reciprocal relationship between serum pancreatic activity and serum triglyceride level appears to confirm the hypothesis about abnormal lipid metabolism in psoriasis.

Abstract

The first example of the condensation of (ferrocene)- N -benzhydrylamino-methanephosphonous acid ( 1 ) with α-amino acids 2a – d and several model dipeptides 4a – d and the tripeptide dl -alanyl- dl -leucinyl-glycine ( 4e ) in the presence of DCC resulted in pseudo-phosphono-dipeptides 3a – d and pseudo-phosphono-oligopeptides 5a – d . The probable chiral assistance of the incoming amino acid or peptide in the formation of the new chiral center on phosphorus was also a feature of this method.
