Abstract

Calidad de vida; Sarcoma de tejido blando localmente avanzado; Radioterapia Qualitat de vida; Sarcoma de teixit tou localment avançat; Radioteràpia Quality of life; Locally advanced soft-tissue sarcoma; Radiation therapy Purpose Act.In.Sarc (NCT02379845) demonstrated that the first-in-class radioenhancer NBTXR3, activated by preoperative radiation therapy (RT), doubled the rate of pathologic complete response after resection compared with preoperative RT alone in adult patients with locally advanced soft tissue sarcoma of the extremity or trunk wall (16.1% vs 7.9%, P = .045), and more patients achieved R0 resections (77.0% vs 64.0%, P = .042). These are the toxicity and health-related quality of life (HRQoL) results. Methods and Materials Act.In.Sarc randomized eligible patients 1:1 to either NBTXR3 (single intratumoral injection, volume equivalent to 10% of baseline tumor volume, at 53.3 g/L) activated by external-beam RT (arm A) or external-beam RT alone (arm B) (50 Gy in 25 fractions), followed by surgery in both arms. Here, we report the safety analyses in the all-treated population with a long-term follow-up of at least 2 years, and HRQoL in the intention-to-treat full analysis set. Results During the on-treatment period, serious adverse events (SAEs) of all grades related to NBTXR3 occurred in 10.1% (9/89) of patients (arm A), and SAEs related to RT occurred in 5.6% (5/89) (arm A) versus 5.6% (5/90) (arm B); postsurgery hospitalization owing to SAEs occurred in 15.7% (14/89) (arm A) versus 24.4% (22/90) (arm B). During the follow-up period, posttreatment SAEs (regardless of relationship) occurred in 13.5% (12/89) (arm A) versus 24.4% (22/90) (arm B). NBTXR3 did not negatively affect HRQoL; during the follow-up period, there was an improvement in most mean Toronto extremity salvage, EuroQoL 5-dimension (EQ-5D), EQ5D02-EQ visual analog scale, reintegration to normal living index, and musculoskeletal tumor rating scale scores. Conclusions NBTXR3 did not negatively affect safety or HRQoL. Long-term safety results reinforce the favorable benefit–risk ratio of NBTXR3 plus RT.

Abstract

Despite various international regulatory initiatives over the last 20 years, many challenges remain in the field of paediatric drug development and evaluation. Indeed, drug research and development is still focused essentially on adult indications, thereby excluding many paediatric patients, limiting the feasibility of trials and favouring competing developments. Off-label prescribing persists and the development of age-appropriate dosage forms for children remains limited. Against this background, the members of this panel (TR) recommend the launch of multi-partner exchange forums on specific topics in order to focus new drug research and development on the real, unmet medical needs of children and adolescents, and in keeping with the underlying mechanisms of action. Scientific information sharing and cooperation between stakeholders are also essential for defining reference evaluation methods in each medical field. These forums can be organised through existing paediatric facilities and research networks at the French and European level. The latter are specifically dedicated to paediatric research and can facilitate clinical trial implementation and patient enrolment. Moreover, specific grants and public/private partnerships are still needed to support studies on the repositioning of drugs in paediatric indications, and pharmacokinetic studies aimed at defining appropriate dosages. The development of new pharmaceutical forms, better suited for paediatric use, and the promotion of resulting innovations will stimulate future investments. Initiatives to gather observational safety and efficacy data following off-label and/or derogatory early access should also be encouraged to compensate for the lack of information available in these situations. Finally, the creation of Ethics Committees (EC) with a specific "mother-child" advisory expertise should be promoted to ensure that the current regulation (Jardé law in France) is implemented whilst also taking into account the paediatric specificities in medical trials.

Abstract

Understanding the interaction between sediment production on and export from shallow-water areas of platforms and slopes is primordial when assessing sedimentary processes on a carbonate-platform scale. In this manuscript we explore variations in facies, sediment export, sediment deposition and reorganisation, hydroacoustic- and small-scale sedimentary structures, but also assess the variability in current systems as observed during the Quaternary for the north-facing margin of Little Bahama Bank (LBB) and compare those characteristics with features observed on other slopes surrounding LBB and Great Bahama Bank (GBB). Over the past decade, the northern margin of LBB was explored during a series of oceanographic cruises of the CARAMBAR project, which included the collection of 24.270 km2 of bathymetry data, 6.398 km of very high-resolution seismic profiles, and 42 cores covering water depths ranging from 177 m to 4873 m. This study evaluates the results obtained from the analysis of sediment cores retrieved in the Great Abaco Canyon area (GAC), located between the lower northern LBB slope, which is connected to the abyssal plain. The analysis of the shallower parts of the LBB slope relies on earlier studies and are complementary to our data, and allow for a detailed analysis of the sedimentary processes acting along the entire LBB slope. The data reveal that Quaternary sediment distribution differs when moving from the north-eastern to the north-western LBB slope. The entire LBB slope is dissected by numerous canyons. Gravity processes enriched in coarse platform components occur infrequently and are concentrated within lobes in the east. Only coarse-grained rich bank facies can concentrate coarse grains on this margin. The western LBB slope is mostly influenced by fine-grained platform export and current circulation. Abstract

The Radiation Induced Sarcoma (RIS) is a rare but serious adverse event following radiotherapy (RT). Current RT techniques are more precise, but irradiate a larger volume at a low dose. This study aimed to describe radiation characteristics in a large series of patients suffering from RIS.Patient-representative voxel-based anthropomorphic phantoms were used to reconstruct patient-specific RT fields for 125 patients diagnosed with RIS after primary breast cancer. For each patient, the location of the RIS onset site was determined and transferred onto the phantom as a contour. Using a treatment planning system (TPS), the dose distribution on the RIS in the phantom was calculated.The mean dose (Dmean) received in the area where RIS subsequently developed was 47.8 ± 11.6 Gy. The median dose in the zones where RIS later developed ranged from 11 Gy to 58.8 Gy. The median time from RT to RIS development was 8 years (range 2-32 years). Analysis for predictors of time to radiation-induced sarcoma development highlighted a significant impact of age of patient during the RT whereas in multivariable analysis chemotherapy and hormonotherapy for primary breast cancer were not associated with a significant difference in time to diagnosis of RIS.This study highlights that the dose received by the tissue in which the RIS developed was almost 47 Gy. These results are encouraging for the use of new RT techniques increasing volumes receiving low doses, without fear of an excess of RIS over the next 10 years.

Resume Objectifs de l’etude Nous presentons les resultats du PHRC Tridicol, etude prospective francaise de phase II dont l’objectif etait d’augmenter la dose delivree au volume cible lors de la curietherapie des cancers du col de l’uterus localement evolues. Materiel et methodes Huit centres ont inclus 48 patientes, prises en charge par chimioradiotherapie concomitante, puis curietherapie uterovaginale. Resultats Le suivi median etait de 63 mois. La dose mediane de curietherapie delivree en equivalent biologique d’une irradiation delivree par fractions de 2 Gy (EQD2) dans 90% du volume cible anatomoclinique a haut risque (D90 CTV HR) etait de 80 Gy. Le taux de controle local a 5 ans etait de 84 %, proche de l’hypothese de 86,7 %. Le taux de complications severes (de grade 3–4) etait de 23 % a 5 ans. La dose recue par le rectum etait correlee au risque de complications severes. Conclusion La dose dans le CTV HR a ete inferieure a l’objectif (85 Gy) du fait d’une faible utilisation des applicateurs avec aiguilles interstitielles parametriales, les centres participants ne disposant pas toujours d’un applicateur adequat, ou parce qu’ils etaient a l’epoque au debut de leur courbe d’apprentissage. Le taux de controle local a 5 ans etait ameliore par rapport a celui du groupe comparable du STIC PDR (programme de soutien aux techniques innovantes couteuses curietherapie de debit pulse) ([78 %] mais inferieur a la cohorte retroEMBRACE du GEC ESTRO (Groupe Europeen de Curietherapie -European Society for Radiotherapy) (89 %). Le taux de complications etait plus eleve que dans le groupe comparable du STIC PDR mais proche de celui de retroEMBRACE (intErnational study on MRI-guided BRachytherapy in locally Advanced CErvical cancer). La formation des equipes de curietherapie a l’implantation interstitielle ou l’adressage des patientes a des centres de recours doit permettre d’ameliorer l’indice therapeutique des cancers du col de l’uterus.

Abstract

To obtain a national overview of the epidemiology and management of invasive fungal infections (IFIs) in France for severely immunocompromised children who were treated for acute leukemia or had undergone allogeneic hematopoietic stem cell transplantation (a-HSCT).We performed a national multicenter retrospective study to collect epidemiologic data for proven and probable IFIs in children with acute leukemia under first- line or relapse treatment or who had undergone a-HSCT. We also conducted a prospective practice survey to provide a national overview of IFI management in pediatric hematology units.From January 2014 to December 2017, 144 cases of IFI were diagnosed (5.3%) in 2721 patients, including 61 cases of candidiasis, 60 cases of aspergillosis, and 23 cases of infection with "emergent" fungi, including 10 cases of mucormycosis and 6 cases of fusariosis. The IFI rate was higher in patients with acute myelogenous leukemia (12.9%) (OR, 3.24; 95% CI, 2.15-4.81; P.0001) compared with the rest of the cohort. Patients undergoing a-HSCT had an IFI rate of only 4.3%. In these patients, the use of primary antifungal prophylaxis (principally fluconazole) was associated with a lower IFI rate (OR, 0.28; 95% CI, 0.14-0.60; P = 4.90 ×10The emerging fungi and new antifungal resistance profiles uncovered in this study should be considered in IFI management in immunocompromised children.

Abstract

Radiation-induced myelopathy is a devastating late effect of radiotherapy. Fortunately, this late effect is exceptional. The clinical presentation of radiation myelopathy is aspecific, typically occurring between 6 to 24 months after radiotherapy, and radiation-induced myelopathy remains a diagnosis of exclusion. Magnetic resonance imaging is the most commonly used imaging tool. Radiation oncologists must be extremely cautious to the spinal cord dose, particularly in stereotactic radiotherapy and reirradiation. Conventionally, a maximum dose of 50Gy is tolerated in normofractionated radiotherapy (1.8 to 2Gy per fraction). Repeat radiotherapies lead to consider cumulative doses above this recommendation to offer individualized reirradiation. Several factors increase the risk of radiation-induced myelopathy, such as concomitant or neurotoxic chemotherapy. The development of predictive algorithms to prevent the risk of radiation-induced myelopathy is promising. However, radiotherapy prescription should be cautious, regarding to ALARA principle (as low as reasonably achievable). As the advent of immunotherapy has improved patient survival data and the concept of oligometastatic cancer is increasing in daily practice, stereotactic treatments and reirradiations will be increasingly frequent indications. Predict the risk of radiation-induced myelopathy is therefore a major issue in the following years, and remains a daily challenge for radiation oncologists.

Abstract

International audience; Contexte scientifique: Malgré des avancées majeures en oncologie depuis vingt ans, 20 % des enfants et adolescents atteints d'un cancer en France décèdent de leur maladie et parmi ceux qui survivent, beaucoup gardent pendant toute leur vie des séquelles plus ou moins invalidantes consécutives à leur traitement. C'est pourquoi en 2017, les chercheurs, cliniciens, biologistes et étudiants français travaillant dans le domaine de la recherche fondamentale et translationnelle en oncologie pédiatrique se sont organisés en réseau sous le nom de REACT4KIDS (https:// react4kids.apps-dev.io/). Ce réseau a pour objectif principal de mieux coordonner les efforts des équipes de recherche françaises (une trentaine) travaillant sur les cancers de l'enfant afin de faire émerger de nouvelles pistes thérapeutiques. Chaque année, REACT4KIDS organise un colloque afin que ces équipes puissent se rencontrer, présenter leurs travaux, partager leurs données et mettre en place de nouvelles collaborations. Cette année, ce colloque scientifique était également tourné vers l'international, avec la venue de deux conférenciers étrangers de renommée mondiale, et vers la société, avec la participation de nombreuses associations de patients et le grand public. Après la tenue des deux premiers colloques en oncologie pédiatrique au centre Léon-Bérard à Lyon, la ville de Bordeaux a été choisie pour organiser la 3 e édition de cette manifestation.

Abstract

Brachytherapy is part of the treatment of locally advanced cervical cancers, accounting for about half of the total delivered dose. The benefit of dose escalation is the most important in advanced cases or if the tumor has responded poorly. The use of interstitial implantations makes it possible to reach doses of the order of 85 to 90Gy (including external beam radiotherapy contribution) in most patients, through image-guided approaches. Brachytherapy delivery is one of the quality criteria for patient care. To date, no data allow us to consider as an alternative the use of external boost through intensity-modulated or stereotactic body radiotherapy. Indeed, the doses delivered to the tumor and the capacity to spare normal tissues remains lower, as compared to what is permitted by brachytherapy. It is therefore appropriate for centers that do not have access to the technique to establish networks with centers where brachytherapy is performed, to allow each patient to have access to the technique. It is also necessary to promote brachytherapy teaching. The issue of reimbursement will be crucial in the coming years to maintain expertise that is today insufficiently valued in its financial aspects, but has a very high added value for patients.
