Background The joint committee of the Japanese Society of Intensive Care Medicine/Japanese Respiratory Society/Japanese Society of Respiratory Care Medicine on ARDS Clinical Practice Guideline has created and released the ARDS Clinical Practice Guideline 2021. Methods The 2016 edition of the Clinical Practice Guideline covered clinical questions (CQs) that targeted only adults, but the present guideline includes 15 CQs for children in addition to 46 CQs for adults. As with the previous edition, we used a systematic review method with the Grading of Recommendations Assessment Development and Evaluation (GRADE) system as well as a degree of recommendation determination method. We also conducted systematic reviews that used meta-analyses of diagnostic accuracy and network meta-analyses as a new method. Results Recommendations for adult patients with ARDS are described: we suggest against using serum C-reactive protein and procalcitonin levels to identify bacterial pneumonia as the underlying disease (GRADE 2D); we recommend limiting tidal volume to 4–8 mL/kg for mechanical ventilation (GRADE 1D); we recommend against managements targeting an excessively low SpO2 (PaO2) (GRADE 2D); we suggest against using transpulmonary pressure as a routine basis in positive end-expiratory pressure settings (GRADE 2B); we suggest implementing extracorporeal membrane oxygenation for those with severe ARDS (GRADE 2B); we suggest against using high-dose steroids (GRADE 2C); and we recommend using low-dose steroids (GRADE 1B). The recommendations for pediatric patients with ARDS are as follows: we suggest against using non-invasive respiratory support (non-invasive positive pressure ventilation/high-flow nasal cannula oxygen therapy) (GRADE 2D), we suggest placing pediatric patients with moderate ARDS in the prone position (GRADE 2D), we suggest against routinely implementing NO inhalation therapy (GRADE 2C), and we suggest against implementing daily sedation interruption for pediatric patients with respiratory failure (GRADE 2D). Vascular endothelial growth factor (VEGF) signaling plays a central role in vascular development and maintenance of vascular homeostasis. In endothelial cells (ECs), VEGF activates the gene expression of angiogenic transcription factors (TFs), followed by induction of downstream angiogenic responsive genes. Recent findings support that histone modification dynamics contribute to the transcriptional control of genes that are important for EC functions. Lysine demethylase 2B (KDM2B) demethylates histone H3K4me3 and H3K36me2/3 and mediates the monoubiquitination of histone H2AK119. KDM2B functions as a transcriptional repressor in somatic cell reprogramming and tumor development. However, the role of KDM2B in VEGF signaling remains to be elucidated. Here, we show that KDM2B knockdown enhances VEGF-induced angiogenesis in cultured human ECs via increased migration and proliferation. In contrast, ectopic expression of KDM2B inhibits angiogenesis. The function of KDM2B may depend on its catalytic Jumonji C domain. Genome-wide analysis further reveals that KDM2B selectively controls the transcription of VEGF-induced angiogenic TFs that are associated with increased H3K4me3/H3K36me3 and decreased H2AK119ub. These findings suggest an essential role of KDM2B in VEGF signaling in ECs. As dysregulation of VEGF signaling in ECs is involved in various diseases, including cancer, KDM2B may be a potential therapeutic target in VEGF-mediated vasculopathic diseases.

Kounis syndrome is an anaphylactic reaction leading to acute coronary syndrome. The acute treatment of anaphylaxis is epinephrine; however, epinephrine may cause coronary vasoconstriction, reduce coronary blood flow, increase myocardial oxygen demand, and worsen myocardial ischemia. On the other hand, coronary vasodilation, a treatment for acute coronary syndrome, can aggravate hypotension in patients with anaphylaxis. Herein, the authors report a case of type II Kounis syndrome, with vasospasm in a patient with coronary disease, requiring the administration of epinephrine and a coronary vasodilator for resuscitation. The authors administered intravenous epinephrine continuously from lower dosages and performed delicate titration. The coronary vasodilator nicorandil, which has little effect on hemodynamics, also was administered. These treatments improved hemodynamics without complications. Circulatory management that considers both anaphylaxis and coronary lesions is crucial to improve prognosis in this syndrome.

Background Intestinal ischemia and enterocyte injury are significant causes of death after cardiac surgery. Hemodialysis is a well-known risk factor for intestinal ischemia. However, the relationship between enterocyte injury and mortality is unclear. This exploratory study assessed the association between intestinal fatty acid–binding protein (I-FABP), a specific marker of enterocyte injury, at intensive care unit (ICU) admission and in-hospital mortality in patients on hemodialysis who underwent cardiac surgery with cardiopulmonary bypass. Materials and methods Forty-seven consecutive patients on long-term hemodialysis who underwent elective cardiac surgery (median age, 70 y; men, 27 [57%]) were prospectively enrolled. The association between serum I-FABP levels at ICU admission and in-hospital mortality was compared with the associations between serum I-FABP levels and prognostic severity scores, vasoactive-inotropic scores, and lactate levels. Results Only I-FABP levels at ICU admission were significantly related to in-hospital mortality (odds ratio, 5.54; 95% confidence interval [CI], 1.08-28.43) in the simple logistic regression analysis. Univariate and multiple linear regression analyses indicated prolonged cardiopulmonary bypass (ρ, 0.49; 95% CI, 0.15-0.83), higher mean norepinephrine dose (ρ, 0.07; 95% CI, 0.02-0.12), lower mean dopamine dose (ρ, −0.51; 95% CI, −0.94 to −0.08), and intra-aortic balloon pump use (ρ, 3.63; 95% CI, 1.68-5.59) were significant risk factors for high I-FABP levels. Conclusions Enterocyte injury at ICU admission was associated with in-hospital mortality after cardiac surgery for patients on hemodialysis. Intraoperative hidden hypoperfusion of the intestine may impact prognoses. Enterocyte injury prevention, early diagnosis, and intervention for intestinal ischemia might be required to improve outcomes.

A high voltage DC power supply for the cathode of the 8.56 GHz CW klystron has been set up for electron cyclotron heating (ECH) in steady state tokamak operation on QUEST spherical tokamak. The power supply is equipped with an IGBT array and a reactor for fast shutoff of the voltage in 10 μs, where the influx of electric energy at the short circuit is limited to 5 J. AC switches also have been installed in the three-phase power lines. High voltage relays are useful to save electric energy consumption. Fast three-phase AC switching by IGBT-stack is applicable to reduce the electric load of the components of the klystron power supply.

New polarizer and launcher systems on a 28-GHz electron-cyclotron (EC) heating and current drive (HCD) system have been developed for non-inductive second-harmonic EC-plasma-current ramp-up in the QUEST spherical tokamak. A launcher system with two quasi-optical mirrors providing beam steering capability was designed to focus the incident beam to a small-sized waist of ∼0.05 m at the second-harmonic EC resonance layer. A relatively large focusing mirror was designed based on a Kirchhoff integral code developed to derive wave solutions. The focusing property of the launched beam was first confirmed with a 3-dimensional (3D) electromagnetic-wave simulator. Focusing characteristics were also checked at low-power test facilities, together with the steering capability. The performance of this launcher system was demonstrated to work as designed, and assembled in the QUEST device. The system was applied to the non-inductive second-harmonic EC plasma ramp-up experiments with no optimization required regarding the incident polarization. The results obtained for the non-inductive plasma ramp-up are also presented.

Peripartum cardiomyopathy (PPCM) is a life-threatening heart disease arising in the peripartum period. 16 kDa-prolactin (16K PRL) and its subproduct microRNA (miR)-146a-5p are candidate biomarkers for PPCM. However, we showed that miR-146a-5p is not well upregulated in the sera of PPCM patients. Here, we found that miR-320a/b were the most specific miRNAs that suppressed the expression of protein kinase RNA-like endoplasmic reticulum kinase (PERK). We transfected neonatal rat cardiomyocytes with plasmids containing full-length prolactin (23K PRL) or 16K PRL in vitro. We found that 23K PRL, but not 16K PRL, upregulated PERK signaling, and hypoxia promoted this effect. During the perinatal period, cardiomyocyte-specific PERK homogenous knockout (CM-KO) mice showed PPCM phenotypes after consecutive deliveries. Downregulation of PERK or JAK/STAT signaling and upregulation of apoptosis were observed in CM-KO mouse hearts. These results demonstrated that interruption of 23K PRL-PERK interaction by miR-320a/b could be a therapeutic strategy for PPCM. Summary: Endothelial cells (ECs) are phenotypically heterogeneous, mainly due to their dynamic response to the tissue microenvironment. Vascular endothelial cell growth factor (VEGF), the best-known angiogenic factor, activates calcium-nuclear factor of activated T cells (NFAT) signaling following acute angiogenic gene transcription. Here, we evaluate the global mapping of VEGF-mediated dynamic transcriptional events, focusing on major histone-code profiles using chromatin immunoprecipitation sequencing (ChIP-seq). Remarkably, the gene loci of immediate-early angiogenic transcription factors (TFs) exclusively acquire bivalent H3K4me3-H3K27me3 double-positive histone marks after the VEGF stimulus. Moreover, NFAT-associated Pax transactivation domain-interacting protein (PTIP) directs bivalently marked TF genes transcription through a limited polymerase II running. The non-canonical polycomb1 variant PRC1.3 specifically binds to and allows the transactivation of PRC2-enriched bivalent angiogenic TFs until conventional PRC1-mediated gene silencing is achieved. Knockdown of these genes abrogates post-natal aberrant neovessel formation via the selective inhibition of indispensable bivalent angiogenic TF gene transcription. Collectively, the reported dynamic histone mark landscape may uncover the importance of immediate-early genes and the development of advanced anti-angiogenic strategies.

Textural properties play an important role in the appreciation and acceptance of fresh tomatoes by consumers. This study aimed to clarify whether the effect of salt stress (50 and 100 mM NaCl) on texture profiles depends on the cultivars and/or tissue types of cherry tomatoes and visually identify the interactions of texture with organoleptic and consumer sensory properties using correlation network analysis. The effect of salt stress on texture profiles differed according to the cherry tomato cultivar, stress intensity, truss, and tissue (outer pericarp, locule, septa). The cohesiveness, gumminess, and chewiness of the outer pericarp and the cohesiveness of the locule were higher in salt-stressed fruit than in control fruit, but the effect was cultivar-dependent. Sensory properties such as overall taste, sweetness, and skin hardness and several organoleptic properties were 1.2–1.7-fold higher in salt-stressed fruit than in control fruit. Although no textural properties interacted with sensory properties in the control fruit network in correlation network analysis, the connectivity of textural properties such as cohesiveness with other traits in salt-stressed fruit increased with increasing salt stress intensity. In the network combining all treatments, the most connected node was cohesiveness, and cohesiveness was the key factor that enhanced the sensory and organoleptic properties of salt-stressed tomatoes, although the effects were cultivar- and tissue-specific. Thus, correlation network analysis provided visual insights into the interactions of textural properties with organoleptic and sensory properties.

In tokamaks, the temperature of the plasma-facing wall is an important parameter for achieving particle balance and therefore steady-state operation. QUEST, which is a middle-sized spherical tokamak, has hot walls that act as plasma-facing walls. They can be actively heated with sheath heaters and actively cooled with water. To control the wall temperature, heating and cooling systems have been developed. These systems adjust the power of the sheath heaters and the motor valves of the cooling system, respectively. The two systems communicate via Ethernet through UDP and control the hot-wall temperature cooperatively. The plasma control system (PCS) in QUEST has also been modified, especially with respect to gas fueling, in order to enable long-duration plasma sustainment. A feedback controller has been installed in the PCS, together with a mass flow controller, allowing Hα emission from the plasma which is used as a reference signal, to be well controlled. Plasma density calculations using a field-programmable gate array are proposed for the feedback control system.

It has been long debated among comparativists whether democratic states advance human development. By regressing infant mortality rates on a cumulative score of democracy, recent studies have suggested that a long tradition of democracy is of greater importance than the present degree of democracy to explain human development. This approach, however, faces several issues and also fails to pinpoint the effect of a democratic reform at a certain point in time on future human welfare. We argue that political liberalization encourages policymakers to adopt poverty-alleviating policies, but such a policy change is more likely to bear substantive fruit over the relatively long run. Using newly collected panel data on infant mortality rates from 1800 to 2015 from 172 countries, we test our theoretical expectations. Applying error correction models to this extensive time-series cross-sectional data, we find that political liberalization is more likely to increase equal access to public services immediately but that infant mortality rates are reduced in the long run. These results suggest that democratic reforms do have a positive effect on human welfare, but the effect might appear with a passage of time as well as shrinks over time. Our research implies that policymakers may need to make tireless investments in the advancement of political liberalization to continue improving human well-being in the developing world.

Purpose We sought to evaluate the levels of intestinal fatty acid-binding protein (I-FABP), a biomarker of enterocyte injury, as a predictor of 28-day mortality and bowel ischemia in septic shock patients. Material and methods In this preliminary prospective observational study, 57 adult septic shock patients under mechanical ventilation were enrolled. Serum I-FABP levels and prognostic biomarkers were recorded upon intensive care unit (ICU) admission. Results The overall 28-day mortality rate of participants was 23% (13/57). Non-survivors displayed significantly higher lactate ( p = 0.009), I-FABP ( p = 0.012), and N-terminal pro-B-type natriuretic peptide ( p = 0.039) levels compared to survivors. Only I-FABP was associated with 28-day mortality (odds ratio, 1.036; 95% confidence interval, 1.003–1.069; p = 0.031) in a multiple logistic regression analysis adjusted for the Acute Physiology and Chronic Health Evaluation II score. When divided into low and high I-FABP groups based on the optimum cut-off value of 19.0 ng/mL for predicting 28-day mortality, high-I-FABP patients had a significantly higher incidence of non-occlusive mesenteric ischemia (NOMI) (2% [1/43] vs 29% [4/14]; p = 0.011). Conclusions I-FABP level at ICU admission can serve as a predictor of 28-day mortality in septic shock patients and is associated with the incidence of NOMI.
