Abstract

PURPOSE/OBJECTIVE(S): stereotactic ablative radiotherapy (SABR) has been shown to increase survival rates in oligometastatic disease (OMD), but local control of colorectal metastases still remains poor. We aimed to identify potential predictive factors of SBRT response through a multicenter large retrospective database and to investigate how lung SBRT can impact on the progression to the polymetastatic disease (PMD). MATERIALS/METHODS: the study involved 22 centers, and was approved by the Ethical Committee (Prot. Negrar 2019-ZT). 1023 lung metastases treated with SBRT in 622 patients were reported. The median BED was 105 Gy10. Lesion diameter GTV, PTV volume, dose, fractionations, and site of primary tumor were evaluated as potential predictive marker for SBRT response for the primary end-point local progression-free survival (LPFS). EGFR, KRAS, NRAS, BRAF, and MSI were also evaluated. Secondary end-point was the time to the polymetastatic conversion (ttPMC). RESULTS: the median follow-up was 26 months (range 3-117 months). The median lesion diameter was 13 mm (range 4-58 mm). The 2- and 3-year LPFS were 75.6% and 71%, respectively. At the univariate analysis, BED ≥125Gy10 was associated with improved LPFS (2-year: 94.1% versus 72.6%; P = < 0.0001), single fraction SBRT correlated with better LPFS in the overall population (2-year: 80.6% versus 73.7%; P = 0.03), but no significant difference was observed when considering the population treated with BED > 100 Gy10. Lesion diameter ≤19 mm correlated with improved LPFS (2-year 80% versus 60%; P = < 0.0001). The median ttPMC was 26 months, and the 2-year ttPMC was 54.5%. The median PFS was 11.3 months. After SABR, 36% patients had polymetastatic relapse, 39.5% patients had an oligometastatic relapse, and 24.5% patients had no further relapse. CONCLUSION: the present is the largest series of lung colorectal metastases treated with SABR. The results support the use of SBRT in lung oligometastatic colorectal cancer patients as it might delay the transition to PMD or offer relatively long disease-free period in selected cases. Several biological and clinical predictive factors were identified to assure the highest local control, on the basis of which a decisional algorithm will be derived.

Abstract

Until the middle of the 20th century, embryogenesis patterns were considered as based on a rigid, unidirectional ontogenetic development, whose nuclear programming yields an irreversibility feature for cellular determination. Further empirical pieces of evidence have provided new insights about a certain reversibility to cellular determination, finding new biomolecular mechanisms (nuclear reprogramming, dedifferentiation, transdifferentiation) which have clearly shown that such a reversibility exists, warranting a certain cellular plasticity inside cell cycle; moreover, they seem mainly ruled by epigenetic factors. In this framework, evolution can be viewed as a systemic transformation of the spatiotemporal epigenetic organization, and the maintenance of the stable final adult stage includes a possibility of dedifferentiation at the particular points of ontogenetic development leading to the achievement of the final stage though the alternate sets of epigenetic trajectories. This paper is aimed to briefly outline historically the main aspects which have led to define the mechanisms of cellular plasticity, highlighting the chief empirical facts supporting it and the related still unresolved problematic issues.

Abstract

Post-license advanced driver training (ADT) addresses different categories of road users such as: novice drivers, professional drivers, company employers and recidivists. These training courses can be carried out on-track or on the road. On-track courses allow participants to gain knowledge on driving physics and experience limits in a safe road environment. On-road courses are more focused on hazard perception and situation awareness. Although extensive research has been done in this field, knowledge of the effects of these courses on road accident risk remains unclear. Previous evaluation of on-track courses did not always show a positive effect on crash rate. For example, post-license training focused on mastery of driving skills can lead to an increase of accident risk, especially on young males. However, research identified several factors that may enhance the effectiveness of driving training. In Europe a new framework for driver education and training has been proposed based on a safe driver hierarchical model (the GADGET model) and the development of a strategy for continuous learning. According to this framework, an evaluation study of on-track post-license advanced driver training has been undertaken in Italy with the main goal of assessing the safety effects of these courses and identifying training aspects to be improved. Besides crash rate, the study aims at assessing also driver behavior, knowledge of risks, self-evaluation and training quality. This paper presents the results of the possible effects of advanced driver training on driving behavior, considering in particular the number and type of violations. For each driver, data on age, gender and driving violations history were extracted from the platform and the national violations database. Three cases were addressed through a before-after analysis with control group. Case 1 considers all drivers who attended an ADT course. Case 2 aimed at understanding the effects of the courses on a specific target group: the traffic violators. Case 3 is similar to Case 2, however the control group was selected in a way that drivers characteristics and the violation rate was similar to the violation rate of the treatment group in the before period.The significance of the differences highlighted was assessed through appropriate statistical tests (i.e. paired t-test and the Wilcoxon signed-rank test). The study showed in general a higher propensity to commit traffic violations after attending an ADT course. These results are in contrast to what expected and show the necessity to diversify the training classes according to the different needs of participants.

Abstract

Background Panic disorder (PD) is characterized by sudden episodes of acute anxiety occurring without any apparent reason. PD is the most disabling anxiety disorder and it affects twice as many women as men. The heritability of PD is estimated to be up to 48% and epidemiological studies show that both cumulative and specific life events are risk factors for the development of PD. Therefore, the investigation of genetic factors and gene-environment interaction is of high importance for understanding the pathophysiology of PD. As such, examining epigenetic differences in PD patients is of great interest given that environmental factors, in combination with genetic variation, can influence DNA methylation. Methods We conducted an Epigenome-Wide Association Study (EWAS) comparing non-medicated PD patients (49 females, 40 males) with healthy controls (48 females, 28 males). Replication was sought in an independent sample (131 cases, 190 controls) and further confirmed with a meta-analysis across both samples (220 cases, 266 controls). DNA methylation levels were assessed in whole blood using the Infinium HumanMethylation450 BeadChip. Failed probes were excluded based on a detection p-value larger than 0.01 in >50% of the samples. X chromosome, Y chromosome and non-specific binding probes were removed. The data were normalized with functional normalization in Minfi and batch-corrected using ComBat. Regression analyses accounting for cellular heterogeneity, sex and age were performed to test for associations between PD status and DNA methylation. To further assess functionality of the significant CpG, gene expression profiles (Illumina HumanHT-12 v3.0 array) in peripheral blood before and after exposure to the glucocorticoid receptor agonist dexamethasone were tested for association with DNA methylation in another independent female sample (N=71). Results No genome-wide associations were observed in PD patients compared to controls in the whole sample. Interestingly when stratified by sex, only the comparison of female PD patients with controls yielded one genome-wide association surviving FDR of 5% (P= 1.094 x 10-7 , P-adj=0.046). Specifically, cg07308824, located in the promoter of the HECA gene, was hypermethylated in female PD patients (N=49) compared to controls (N=48). The same CpG was also hypermethylated in female PD patients in the replication sample (P=0.035) and the genome-wide significant association was confirmed in the meta-analysis (P-adj=0.007). Methylation at this CpG site was associated with mRNA expression of HECA both at baseline (P= 0.046) and after induction by dexamethasone (P= 0.029). Pathway analyses using Web Gestalt investigating the top 50 associated CpG sites showed enrichment in female-specific pathways, e.g. female infertility pathways. Discussion Our study is the first to examine epigenome wide differences in peripheral blood for PD patients. Interestingly, our results point to possible sex-specific and functional methylation changes in PD. Further studies are needed to examine a possible contribution of the HECA gene methylation to female specific factors in PD.
