The American Academy of Orthopaedic Surgeons and the American Society for Surgery of the Hand released updated Clinical Practice Guidelines in 2020 on the evaluation and treatment of acute distal radius fractures. Following a rigorous methodology designed and implemented through the AAOS, 7 guidelines based upon the best available evidence were released to assist surgeons and physicians managing distal radius fractures. These guidelines can serve as a reference for surgeons when managing patients with distal radius fractures. We review the evidence behind each guideline and highlight the practical implications of each guideline on care.

The evolving healthcare landscape creates unique challenges for private practitioners. They experience the same issues that face physicians in general - including increased regulatory/documentation burdens and downward financial pressures - but without the safety nets that exist in larger healthcare systems. Costs are born more directly, as there are fewer providers over which to spread expenditures. Financial reserves are less robust, making margins thinner to maintain profitability. Guaranteed referral streams are absent, requiring additional effort and creative solutions to maintain patient volume. As hospital systems expand, private practitioners must remain nimble, while maintaining excellent service and outcomes, in order to stay ahead.

A validated solid phase extraction (SPE) cleanup procedure utilizing Bond Elut NEXUS co-polymeric cartridges has been applied to soil and swab samples containing pre- and post-blast residues of nitro-organic explosives. In addition, simulated samples were also processed by SPE to evaluate matrix rejection and interferences. The matrices were extracted with acetone, diluted with deionized water, and processed by SPE. The final extracts were then analyzed by gas chromatography with electron capture detection (GC/ECD) to screen for the compounds of interest, which included ethylene glycol dinitrate (EGDN), dimethyl dinitrobutane (DMDNB), 4-nitrotoluene (4-NT), nitroglycerin (NG), 2,4-dinitrotoluene (DNT), 2,4,6-trinitrotoluene (TNT), pentaerythritol tetra-nitrate (PETN), cyclotrimethylene trinitramine (RDX), 2,4,6-trinitrophenylmethylnitramine (tetryl), cyclotetra-methylene tetranitramine (HMX), erythritol tetranitrate (ETN), and cyclotrimethylene trinitrosoamine (R-salt). The expected explosives were detected in 97% of cases after processing through SPE and analysis by GC/ECD. Approximately one-third of the samples that screened positive by ECD were subjected to confirmation testing by gas or liquid chromatography/mass spectrometry (GC/MS or LC/MS). The results from these matrices were compared to results obtained by syringe filtration. SPE produced equal or better results than syringe filtration in both the ECD screening and MS confirmation tests, including the ability to detect higher signals for more explosives in the post-blast residue samples. The confirmation results also show that all of the expected explosives could be confirmed after processing by SPE, whereas only 83% of them were confirmed after syringe filtration. This study demonstrates the successful application of a validated method to the cleanup of organic explosive residues in complex matrices.

The focus of this work is the experimental investigation of soot formation in coflow flames formed of two fatty acid methyl esters (FAMEs) by employing the light extinction/scattering technique. Three different sets of experiments were conducted in this study. In the first set, radial soot volume fraction ( f v ) profiles of flames of vaporized neat canola methyl ester (B100CME) and neat soy methyl ester (B100SME) fuels both using air as the oxidizer were obtained. In the second set of experiments, the effect of oxygen content in the oxidizer stream on soot formation was studied in both FAME formed flames by increasing the oxygen content in the oxidizer stream from 21% to 35%, 50% and 80%. In the third set of experiments, the effect of fuel blending on the formation of soot particulates was studied in flames formed using CME blended with No. 2 diesel. The blends consisted of 80% biodiesel/20% diesel (B80) and 50% biodiesel/50% diesel (B50). The flames were scanned in the radial direction at various heights above the burner (HAB). For the B100CME-air flame the measured soot volume fraction f v peak was 4.04 ppm and was located at the symmetry axis at a HAB of 16.25 mm. For B100SME-air, the f v peak was measured to be 4.22 ppm at approximately the same flame height as in the CME-air flame. For the B100CME oxygen enriched-air flames the peak values at 35%, 50% and 80% were 6.50, 5.82 and 3.22 ppm, respectively. It was observed that by increasing the oxygen content in the B100CME flame from 21% to 35% oxygen, the f v peak increases by approximately 61%. However, a further increase in oxygen content in the oxidizer stream suppressed soot formation. A similar trend in the f v was observed for B100SME oxygen-enhanced flames. Furthermore, the increase of diesel fuel in the blending of B50CME resulted in significantly higher f v values compared to the B80CME. The addition of oxygen content in the oxidizer stream in these blended fuel flames (from air to 35%) resulted in an increase in the f v peak of approximately 47% and 71%, respectively. Centerline temperatures were measured at various HAB for selected flames.

Soot particles derived from canola methyl ester (CME), soybean methyl ester (SME), a 50% mixture composed of soybean methyl ester (SME) and animal fats (AF), and diesel fuel have been studied using a wick-generated open-air laminar diffusion flame operating at atmospheric pressure. The particles were collected directly from inside the flame volume at different axial locations using the thermophoretic sampling technique and were analyzed using transmission electron microscopy (TEM) and high resolution TEM (HR-TEM). Interestingly, the results show that carbon particulates produced from several tested biodiesels have diameters significantly smaller than those produced in diesel fuel under the same experimental conditions. Within the tested biofuels, the soot from SME had the largest primary particle diameter followed by B50 SME/B50 AF, with CME having the smallest primary particle diameter. Further studies of the soot particle nanostructure were also conducted using a HR-TEM imaging technique. It was found that the soot derived from biodiesel has a highly graphitic shell–core arrangement compared to diesel soot which exhibits far less graphitic structure as it consists of short, disconnected and not concentrically oriented graphene segments.

One reason the de Haas van Alphen (dHvA) effect plays a central role in heavy fermion physics is that the temperature dependence of quantum oscillations can be used to measure effective masses on a Fermi surface specific basis. We present a simple picture of the physics behind this temperature dependence, and discuss the observation of non-Fermi-liquid properties at low millikelvin temperatures via the dHvA effect.

We discuss spin-dependent mass enhancements associated with field-induced quantum critical points in heavy-fermion systems. We have recently observed this phenomenon on a branch of the Fermi surface of CeRu 2 Si 2 above its metamagnetic transition, complementing earlier work. In CeCoIn 5 , at high fields above a field-induced quantum critical point, we see a strong spin-dependence of the effective mass on the thermodynamically dominant sheets of the Fermi surface. These observations reinforce the suggestion that ‘missing mass’ in some cerium-based heavy-fermion systems will be found on heavy spin-polarised branches of the Fermi surface. In all cases where this phenomenon is observed the linear coefficient of specific heat is field dependent; however, CeCoIn 5 seems to be the first such heavy-fermion system in which the f-electrons are definitely contributing to the Fermi volume, which puts it beyond the existing theory intended for metamagnetic systems.

Prasugrel and ticagrelor are potent P2Y12 ADP receptor inhibitors that have greater clinical efficacy compared with clopidogrel, but more non-CABG bleeding. There are no direct comparisons of clinical outcomes between the drugs with follow-up through 1 year. This retrospective study compared 1 year

The Neonatal Behavioral Assessment Scale (NBAS) (Brazelton, 1973, 1984) is a 27-item scale representing individual differences in the newborn infants' behavioral repertoire. The NBAS items are typically collapsed into six clusters (Lester, 1984). Although the NBAS has been examined using exploratory factor analysis methods, no confirmatory factor analysis study has been published in which the behavior structure of full-term, healthy infants is examined. In the current research, competing models assessing variations of the Lester behavior clusters were performed using confirmatory factor analysis. Models comparing six independent factors, six correlated factors, and models estimating a general adaptiveness factor plus six factors were assessed. Examination of the results suggests continued investigations of neonatal behaviors are needed.

Lester (1984) proposed a set of six clusters for the standard 27 items of the National Behavioral Assessment Scale (NBAS). Nine of these items were recorded to change the scale values for the categories. This analysis examined the impact of recording on the psychometric properties of the NBAS. First, correlations were compared between recorded and nonrecorded items within the clusters. Second, iteritem consistencies were computed within each cluster to determine the extent to which the variables measured a common behavioral dimension. Third, a principal components analysis was done to determine the extent to which recoding makes the clusters more distinguishable. The results indicated that the original scale resulted in greater within-cluster correlations and higher interitem consistencies. The recoding method did not result in more distinguisable behavioral clusters. Overall, the original scale provided more optimal psychometric properties.

The present study compared 57 primary seven children (11 year olds) who live in an area of high political violence to 63 comparable children from an area of low political violence on the Birleson depression questionnaire. No significant difference was, however, obtained between the two schools. As with previous work, the present study is not able to provide evidence that children are affected by the political violence in Northern Ireland. In addition, we report preliminary data on the role of denial and support.
