To assess whether having a pet in the home is a risk factor for community-acquired urinary tract infections associated with extended spectrum β-lactamase (ESBL)- or AmpC β-lactamase (ACBL)- producing Enterobacterales.An unmatched case-control study was conducted between August 2015 and September 2017. Cases (n=141) were people with community-acquired urinary tract infection (UTI) caused by ESBL- or ACBL- producing Enterobacterales. Controls (n=525) were recruited from the community. A telephone questionnaire on pet ownership, and other factors was administered, and associations were assessed using logistic regression.Pet ownership was not associated with ESBL- or ACBL-producing Enterobacterales related human UTIs. A positive association was observed for recent antimicrobial treatment, travel to Asia in the previous year, and a doctor's visit in the previous six months. Among isolates with an ESBL-/ACBL-producing phenotype 126/134 (94%) were Escherichia coli, with sequence type (ST) 131 being the most common (47/126).Companion animals in the home were not found to be associated with ESBL- or ACBL-producing Enterobacterales related community-acquired UTI in New Zealand. Risk factors included overseas travel, recent antibiotic use, and doctor visits.

Acute rheumatic fever (ARF) and rheumatic heart disease (RHD) are rare in high-income countries; however, in Aotearoa New Zealand ARF and RHD disproportionately affect Indigenous Māori and Pacific Peoples. This narrative review explores the evidence regarding non-surgical management of patients with clinically significant valve disease or heart failure due to RHD.Medline, EMBASE and Scopus databases were searched, and additional publications were identified through cross-referencing. Included were 28 publications from 1980 onwards.Of the available interventions, improved anticoagulation management and a national RHD register could improve RHD outcomes in New Zealand. Where community pharmacy anticoagulant management services (CPAMS) are available good anticoagulation control can be achieved with a time in the therapeutic range (TTR) of more than 70%, which is above the internationally recommended level of 60%. The use of pharmacists in anticoagulation control is cost-effective, acceptable to patients, pharmacists, and primary care practitioners. There is a lack of local data available to fully assess other interventions; including optimal therapy for heart failure, equitable access to specialist RHD care, prevention, and management of endocarditis.As RHD continues to disproportionately affect Indigenous and minority groups, pro-equity tertiary prevention interventions should be fully evaluated to ensure they are reducing disease burden and improving outcomes in patients with RHD.

International audience; There is increasing evidence that elimination strategies have resulted in better outcomes for public health, the economy, and civil liberties than have mitigation strategies throughout the first year of the COVID-19 pandemic. With vaccines that offer high protection against severe forms of COVID-19, and increasing vaccination coverage, policy makers have had to reassess the trade-offs between different options. The desirability and feasibility of eliminating SARS-CoV-2 compared with other strategies should also be re-evaluated from the perspective of different fields, including epidemiology, public health, and economics. To end the pandemic as soon as possible-be it through elimination or reaching an acceptable endemic level-several key topics have emerged centring around coordination, both locally and internationally, and vaccine distribution. Without coordination it is difficult if not impossible to sustain elimination, which is particularly relevant in highly connected regions, such as Europe. Regarding vaccination, concerns remain with respect to equitable distribution, and the risk of the emergence of new variants of concern. Looking forward, it is crucial to overcome the dichotomy between elimination and mitigation, and to jointly define a long-term objective that can accommodate different political and societal realities.

Summary Background As with many Indigenous populations internationally, Māori in New Zealand suffer health inequity. We aimed to assess the rate of fall injuries at home with and without home modifications in houses with Māori occupants. Methods We did a single-blind randomised controlled trial in the Wellington and Taranaki regions of New Zealand and enrolled owner-occupied households with at least one Māori occupant. Only households who stated they intended to live at that address for the subsequent 3 years were eligible for participation. We randomly assigned (1:1) households to either the intervention group, who received home modifications (handrails for outside steps and internal stairs, grab rails for bathrooms, outside lighting, repairs to window catches, high-visibility and slip-resistant edging for outside steps, fixing of lifted edges of carpets and mats, non-slip bath mats, and slip-resistant surfacing for outside areas such as decks) immediately, or the control group, who received the modifications 3 years later. Data on home injuries were obtained from the Accident Compensation Corporation and coded by study team members, who were masked to study group allocation. The primary outcome was the rate of medically treated fall injuries at home per household per year, analysed according to intention to treat. This Māori Home Injury Prevention Intervention (MHIPI) trial is now completed, and is registered with the Australian New Zealand Clinical Trials Registry, ACTRN12613000148774. Findings Between Sept 3, 2013, and Oct 1, 2014, 824 households were assessed for eligibility and 254 were enrolled, of which 126 (50%) were assigned to the intervention group and 128 (50%) were assigned to the control group. After adjustment for previous falls and geographical region, there was an estimated 31% reduction in the rate of fall injuries at home per year exposed to the intervention compared with households in the control group (adjusted relative rate 0·69 [95% CI 0·47–1·00]). Interpretation Low-cost home modifications and repairs can be an effective means to reduce injury disparities. The high prevalence of modifiable safety issues in Māori homes merits considerable policy and community effort. Funding Health Research Council of New Zealand.

BackgroundIn response to the COVID-19 pandemic, some countries in the Asia-Pacific Region used very intensive control measures, and one of these, New Zealand (NZ), adopted a clear “elimination strategy”. We therefore aimed to compare key health and economic outcomes of NZ relative to OECD countries as of mid-June 2021.MethodsThis analysis compared health outcomes (cumulative death rates from COVID-19 and “excess death” rates) and economic measures (quarterly GDP and unemployment levels) across OECD countries.ResultsNZ had the lowest cumulative COVID-19 death rate in the OECD at 242 times lower than the 38-OECD-country average: 5·2 vs 1256 per million population. When considering “excess deaths”, NZ had the largest negative value in the OECD, equivalent to around 2000 fewer deaths than expected. When considering the average GDP change over the five quarters of 2020 to 2021-Q1, NZ was the sixth best performer (at 0·5% vs -0·3% for the OECD average). The increase in unemployment in NZ was also less than the OECD average (1·1 percentage points to a peak of 5·2%, vs 3·3 points to 8·6%, respectively).ConclusionsNew Zealand’s elimination strategy response to COVID-19 produced the best mortality protection outcomes in the OECD. In economic terms it also performed better than the OECD average in terms of adverse impacts on GDP and employment. Nevertheless, a fuller accounting of the benefits and costs needs to be done once the population is vaccinated and longer-term health and economic outcomes are considered.

Background: For countries with strong border control and an epidemic that is not yet advanced, there is an opportunity to eliminate SARS-CoV-2 that is causing the COVID-19 pandemic. We show how public health policies and their effects can be modelled to estimate the probability of elimination of SARS-COV-2 under i) strict physical distancing policies implemented in New Zealand and Australia on the 26th and 28th March, respectively, continuing until such time that elimination is achieved; and ii) the same policy, but with physical distancing decaying over 60 days to 26th and 28th of May. Methods: We developed an agent-based SEIR model that simulated key aspects of both country’s populations, disease, spatial and behavioural dynamics, as well as the mechanism and effect of public health policy responses on the transmission of SARS-CoV-2. Findings: Under maintained strong physical distancing, we estimated a median elimination date of July 12th for Australia (95% SI: June 5th to August 28th) and June 3rd for New Zealand (95% SI May 4th to June 27th). A 90% probability of elimination is achievable in Australia on August 17th (95% SI: August 8th – August 30th) and on June 14th (95% SI: June 17th – June 29th) in New Zealand. However, under our scenario of decaying adherence to physical distancing from implementation to 60 days post restrictions, a rebound in SARS-CoV-2 infections (i.e., a second wave) was likely in Australia and possibly in New Zealand. At 100 days, the probability of elimination was reduced to 10% in Australia and 68% in New Zealand. Interpretation: The findings suggest that with the effective implementation and maintenance of public health policies limiting physical interaction, it was possible to estimate a pattern the elimination of SARS-CoV-2 transmission in both countries. It seems highly likely NZ will have successfully eliminated as of 8 June, but not in Australia; the latter is consistent with our lower probabilities that modelled less stringent restrictions applied in Australia.

Approaches to preventing or mitigating the impact of the COVID-19 pandemic have varied markedly between nations. We examined the approach up to July 2020 taken by two countries which had successfully eliminated COVID-19: Taiwan and New Zealand (NZ). Both jurisdictions experienced the COVID-19 pandemic in the first half of 2020. Taiwan reported a lower COVID-19 incidence rate (19.0 cases per million) compared with NZ (235.5 per million). Extensive public health infrastructure established in Taiwan pre-COVID-19 enabled a fast coordinated response, particularly in the domains of early screening, effective methods for isolation/quarantine, and digital technologies for identifying potential cases. This timely and vigorous response allowed Taiwan to avoid the national lockdown used by NZ and the majority of high-income countries. Many of Taiwan’s pandemic control components could potentially be adopted by other high-income jurisdictions.

Background: For countries with strong border control and an epidemic that is not yet advanced, there is an opportunity to eliminate community transmission of SARS-CoV-2. We show how public health policies and their effects can be modelled to estimate the likely progression of SARS-COV-2 under i) strict physical distancing policies implemented in New Zealand and Australia on the 26 th and 28 th March, 2020 respectively, and ii) from September 1st, 2020 in Victoria, Australia as this state planned its exit from a significant second wave of infections. Methods: We developed a dynamic, public health policy model using an agent-based framework that simulated key aspects of both country’s populations, disease, spatial and behavioural dynamics, as well as the mechanism and effect of public health policy responses on the transmission of SARS-CoV-2. We demonstrate the model’s development and application through the first wave of infections in New Zealand, and first and second wave of infections experienced in Australia. Findings: Under maintained strong physical distancing, we originally estimated a median elimination date of July 12th for Australia (95% SI: June 5th to August 28th) and June 3rd for New Zealand (95% SI May 4th to June 27th). We estimated a 90% probability of elimination was achievable in Australia on August 17th (95% SI: August 8th – August 30th) and on June 14th (95% SI: June 17th – June 29th) in New Zealand. However, under our scenario of decaying adherence to physical distancing from implementation to 60 days post restrictions, we predicted a resurgence in SARS-CoV-2 infections (i.e., a second wave) was likely in Australia and possibly in New Zealand. At 100 days, the probability of elimination was reduced to 10% in Australia and 68% in New Zealand. Having demonstrated the utility of our model above, the subsequent adaptation of the model in Victoria, Australia estimated that a threshold of less than 5 new cases per day over 14 days (from a peak of ~500 in August) could be achieved on or around the 17th of October. This would enable Victoria to move to, and sustain, lower levels of restrictions to movement and trade than had been possible under its strict lockdown policy. Interpretation: The findings suggest that during Australia and NZ’s first wave of infections, elimination of community transmission of SARS-CoV-2 in both countries through effective implementation and maintenance of public health interventions was possible. Subsequently, the structure and function of the agent-based policy model developed to analyse the effect of policies during wave 1 has been instrumental in supporting decision-making among public health authorities during Australia’s current second wave of infections. Hotel-based Managed Isolation and Quarantine (MIQ) is a key public health intervention in Aotearoa New Zealand's (NZ) COVID-19 border control strategy for returning citizens and permanent residents. We aimed to investigate the experience of transiting through MIQ in NZ, to inform future refinements of this type of system. A qualitative thematic analysis method was utilised to explore experiences in depth with seventy-five individuals who had undergone MIQ in NZ between April 2020 and July 2021. Participants were interviewed by telephone or Zoom or completed an online qualitative questionnaire. Interviews were audio recorded, transcribed and coded; questionnaire responses were sorted and coded. All data were subjected to thematic analysis. Three main themes described the key elements of the participants' experience of MIQ that influenced their overall experiences: 1) The MIQ process, 2) MIQ Hotels, and 3) Individual experience. The variation in participants' overall experience of MIQ was strongly influenced by their perceptions of how well the MIQ process was managed (including communication, flexibility, and compliance with disease prevention and control measures); and the quality of the hotels they were allocated to (in particular hotel staff, meals and information). This valuable insight into the experience of individuals in NZ MIQ hotels can inform better planning, management and implementation of the MIQ process for NZ and adds to the literature of countries utilising such strategies to minimise the transmission of COVID-19, whilst protecting the wellbeing of those using the system.

From 2014 to 2017, the World Health Organization convened a working group to evaluate influenza disease burden and vaccine efficacy to inform estimates of maternal influenza immunization program impact. The group evaluated existing systematic reviews and relevant primary studies, and conducted four new systematic reviews. There was strong evidence that maternal influenza immunization prevented influenza illness in pregnant women and their infants, although data on severe illness prevention were lacking. The limited number of studies reporting influenza incidence in pregnant women and infants under six months had highly variable estimates and underrepresented low- and middle-income countries. The evidence that maternal influenza immunization reduces the risk of adverse birth outcomes was conflicting, and many observational studies were subject to substantial bias. The lack of scientific clarity regarding disease burden or magnitude of vaccine efficacy against severe illness poses challenges for robust estimation of the potential impact of maternal influenza immunization programs.KeywordsInfluenzaImmunizationPregnancyInfants

Background/aims Over the last decade and following international trends, cases of mosquito-borne arboviral infections, notably dengue fever, chikungunya and Zika, have increased among travellers arriving in New Zealand, but no locally acquired cases have been identified. Imported cases are characterised and examined to identify trends and features that might assist in reducing transmission risk from travellers. Methods Information on traveller arrivals, notified cases and risk factors for disease acquisition were obtained from national sources. Trends in importation rates, seasonality are described and relationships of notifications with traveller arrivals were examined with a negative binomial regression model. Results There was a significant increase in dengue notifications combined with the emergence of Zika and chikungunya. Most notifications were from arrivals in Auckland from Pacific Islands during summer and early autumn. Conclusion/implications Overseas travel from New Zealand, particularly to the Pacific Islands and Southeast Asia, involves a risk of arboviral infection. The repeated introduction of arboviruses to New Zealand also increases the risk of local transmission in a country that has vector capable and vector potential mosquitoes, as well as an increasingly suitable climate for new vectors to establish.

Background Group A Streptococcus (GAS) can trigger an immune-mediated response resulting in acute rheumatic fever (ARF). Historically, ARF has been considered a consequence of preceding GAS pharyngitis, but increasing evidence suggests that GAS skin infections may be a driver. Data on the primary care burden of GAS skin infection are limited. This paper aims to describe and compare the prevalence and distribution of GAS detection in skin swabs and ARF rates in the Auckland population. Methods This cross-sectional study used all laboratory skin swab data from people who had a skin swab taken as a result of a consultation with a health professional in the Auckland region (2010–2016). Initial primary hospitalisations for ARF were identified and all data were linked using unique patient identifiers to patient's age, prioritised ethnicity, sex, and socio-economic status. Findings 377,410 skin swabs from 239,494 individuals were included. 12·8% of swabs were GAS positive, an annual incidence of 4·8 per 1,000 person-years. Māori and Pacific Peoples under 20 years of age had markedly higher GAS detection in skin swabs (RR 4·0; 95% CI 3·9–4·2: RR 6·8; 95% CI 6·6–7·0) and significantly higher ARF rates (RR 30·3; 95% CI 19·5–46·9: RR 69·7 95% CI 45·8–106·1) compared with European/Other ethnicities. Interpretation The observation that GAS detection was markedly higher in Māori and Pacific Peoples provides a potential explanation for the marked ethnic differences in ARF. These findings support a greater focus on addressing the burden of skin infection in NZ, including as ARF prevention. Background We aimed to estimate the protection afforded by inactivated influenza vaccine, in both community and hospital settings, in a well characterised urban population in Auckland during 2014. Methods We used two different comparison groups, all patients who tested negative for influenza and only those patients who tested negative for influenza and had a non-influenza respiratory virus detected, to calculate the vaccine effectiveness in a test negative study design. Estimates were made separately for general practice outpatient consultations and hospitalised patients, stratified by age group and by influenza type and subtype. Vaccine status was confirmed by electronic record for general practice patients and all respiratory viruses were detected by real time polymerase chain reaction. Results 1039 hospitalised and 1154 general practice outpatient consultations met all the study inclusion criteria and had a respiratory sample tested for influenza and other respiratory viruses. Compared to general practice patients, hospitalised patients were more likely to be very young or very old, to be Māori or Pacific Islander, to have a low income and to suffer from chronic disease. Vaccine effectiveness (VE) adjusted for age and other participant characteristics using all influenza negative controls was 42% (95% CI: 16 to 60%) for hospitalised and 56% (95% CI: 35 to 70%) for general practice patients. The vaccine appeared to be most effective against the influenza A(H1N1)pdm09 strain with an adjusted VE of 62% (95% CI:38 to 77%) for hospitalised and 59% (95% CI:36 to 74%) for general practice patients, using influenza virus negative controls. Similar results found when patients testing positive for a non-influenza respiratory virus were used as the control group. Conclusion This study contributes to validation of the test negative design and confirms that inactivated influenza vaccines continue to provide modest but significant protection against laboratory-confirmed influenza.

Background Influenza has a substantially but poorly measured impact on population health. Estimating its true contribution to hospitalisations remains a challenge. Methods We used simple and comprehensive negative binomial regression models with weekly counts of hospitalisations and isolates of influenza A, B and respiratory syncytial virus for the period 1994– 2008. Results The estimated annual national average number of hospitalisations attributable to influenza was 822.1(95% CI: 815.3, 828.9) for pneumonia and influenza, 1861.3 (95% CI: 1842.9, 1879.7) for respiratory illness, 12.1 (95% CI: 2.6, 21.6) for circulatory illness, 2260.0 (95% CI: 2212.2, 2307.8) for all medical illness and 2419.9 (95% CI: 2356.4, 2483.4) for all causes. The contribution of influenza to total hospitalisations was about nine times larger than indicated by routine discharge data. New Zealanders 80 years of age and older had the highest annual excess rates of influenza-related hospitalisations (327.8 per 100,000); followed by infants under 1 year (244.5 per 100,000). Estimated influenza hospitalisation rates were also markedly higher in Pacific (83.3 per 100,000) and Māori (80.0 per 100,000) compared with European/Others (58.1 per 100,000). Respiratory illness was the major contributor to all cause hospitalisations attributed to influenza accounting for 77%. Influenza hospitalisations included only a negligible contribution from circulatory illness. Conclusion These findings support efforts to reduce the impact of influenza, particularly for the most vulnerable population groups highlighted here. Analysis of the cost-effectiveness of such interventions needs to consider these higher modelled estimates of disease impact.

Highlights • A symptom screening tool was used at the border to predict which travellers were more likely to have respiratory virus infections. • The most frequently identified viruses were rhinoviruses, enteroviruses and influenza B. • The positive predictive value (PPV) of any symptom for any respiratory virus infection was low at 26%. • There was a substantial overlap in the symptom profiles between the respiratory viruses found in the study participants. • It is unlikely that clinical symptoms alone can be used to predict infections with specific respiratory viruses., Background There is very little known about the prevalence and distribution of respiratory viruses, other than influenza, in international air travellers and whether symptom screening would aid in the prediction of which travellers are more likely to be infected with specific respiratory viruses. Objectives In this study, we investigate whether, the use of a respiratory symptom screening tool at the border would aid in predicting which travellers are more likely to be infected with specific respiratory viruses. Study design Data were collected from travellers arriving at Christchurch International Airport, New Zealand, during the winter 2008, via a symptom questionnaire, temperature testing, and respiratory sampling. Results Respiratory viruses were detected in 342 (26.0%) of 1313 samples obtained from 2714 symptomatic travellers. The most frequently identified viruses were rhinoviruses (128), enteroviruses (77) and influenza B (48). The most frequently reported symptoms were stuffy or runny nose (60%), cough (47%), sore throat (27%) and sneezing (24%). Influenza B infections were associated with the highest number of symptoms (mean of 3.4) followed by rhinoviruses (mean of 2.2) and enteroviruses (mean of 1.9). The positive predictive value (PPV) of any symptom for any respiratory virus infection was low at 26%. Conclusions The high prevalence of respiratory virus infections caused by viruses other than influenza in this study, many with overlapping symptotology to influenza, has important implications for any screening strategies for the prediction of influenza in airline travellers.

To analyse the incidence, demographics and molecular epidemiology of invasive group A streptococcal (GAS) disease in New Zealand between 2002 and 2012.Using laboratory-based surveillance data, invasive GAS isolates were identified from the Institute of Environmental Science and Research, New Zealand. Hospitalization and mortality data were obtained from the New Zealand Ministry of Health. Molecular typing was performed by sequence analysis of the emm gene.The incidence of invasive GAS infections increased from 3.9 per 100,000 population in 2002 to 7.9 per 100,000 population (P 0.001) in 2012. The incidence was highest in the over 75-year age group, and in Pacific peoples. There was temporal variation in emm types associated with invasive GAS disease, with emm1 being the overall predominant emm type. The diversity of emm types varied significantly according to ethnicity. Overall, 59% of GAS isolates were theoretically covered by an experimental M-protein vaccine.Our study provides valuable data on the epidemiology of invasive GAS disease in New Zealand, and represents one of the few studies to assess such longitudinal data across an entire nation. The increase in invasive GAS disease is concerning, and reasons for this should be explored further.

Few studies report the effectiveness of trivalent inactivated influenza vaccine (TIV) in preventing hospitalisation for influenza-confirmed respiratory infections. Using a prospective surveillance platform, this study reports the first such estimate from a well-defined ethnically diverse population in New Zealand (NZ).A case test-negative design was used to estimate propensity adjusted vaccine effectiveness. Patients with a severe acute respiratory infection (SARI), defined as a patient of any age requiring hospitalisation with a history of a fever or a measured temperature ≥38°C and cough and onset within the past 7 days, admitted to public hospitals in South and Central Auckland were eligible for inclusion in the study. Cases were SARI patients who tested positive for influenza, while non-cases (controls) were SARI patients who tested negative. Results were adjusted for the propensity to be vaccinated and the timing of the influenza season.The propensity and season adjusted vaccine effectiveness (VE) was estimated as 39% (95% CI 16;56). The VE point estimate against influenza A (H1N1) was lower than for influenza B or influenza A (H3N2) but confidence intervals were wide and overlapping. Estimated VE was 59% (95% CI 26;77) in patients aged 45-64 years but only 8% (-78;53) in those aged 65 years and above.Prospective surveillance for SARI has been successfully established in NZ. This study for the first year, the 2012 influenza season, has shown low to moderate protection by TIV against influenza positive hospitalisation.

a b s t r a c t Home injuries are a substantial health burden worldwide, with the home setting being at least as important as the road for injury. Focusing on common injury hazards presented by the home environment, we sought to examine the justification for significant expenditure on safety-related repairs to the housing stock. Trained inspectors assessed 961 New Zealand houses for injury hazards. Using administrative data from the Accident Compensation Corporation (the national injury insurance agency), 1328 home injuries were identified amongst the 1612 occu- pants of these houses over the 2006-2009 period. Telephone interviews gathered data on the location and nature of these injuries, and the attitudes of those injured to potential injury hazards in their homes. Commonly occurring injury hazards that could be repaired at modest cost were identified based on their prevalence estimated by the housing inspection, and their location with respect to the areas of the home where the injuries occurred (identified during the telephone interviews). About 38% of the home injuries studied were potentially related to a structural aspect of the home environment. Common safety hazards included the lack of working smoke detectors (65% of the sample), inadequately fenced driveways (55%), hot water temperatures measured at over 60 ◦ (49%) and poorly lit access to the house (34%). A protocol for identifying and repairing important common hazards was designed. The actual safety effects of this protocol are in the process of being examined in a randomised controlled trial.

An ecological study was conducted in New Zealand to examine the relationship between incidence of human cryptosporidiosis and indicators of community drinking-water supply quality, using routinely collected data from June 1996 to August 1998. A total of 790 cases of cryptosporidiosis lived in communities with a registered drinking-water supply. Within each category of the grading system there was a wide range of rates of cryptosporidiosis. The lowest mean rates of notified cryptosporidiosis were observed in communities served by drinking water supplies with completely satisfactory public health gradings, and the highest rates in communities served by ungraded drinking- water supplies. Mean rates of notified cryptosporidiosis in drinking-water zones that complied with New Zealand drinking-water standards were lower than rates in zones that did not comply with the standards. Current methods of assessing the quality of New Zealand drinking-water supplies reflect in broad terms the risk to human health from Cryptosporidium parvum. Data limitations and uncontrolled confounding mean that the excess risk due to poor quality of the drinking water supply cannot be quantified, but improvements in drinking water quality are likely to be associated with health benefits at the population level. On Feb 1, 2002, inactivated poliomyelitis vaccines replaced live-attenuated oral poliovirus vaccine (OPV) in New Zealand's immunisation schedule, allowing systematic monitoring of OPV virus circulation. Findings of paediatric-inpatient surveillance indicate that 7% of children excreted polioviruses before this switch, but none did so 1 month afterwards. Acute flaccid paralysis surveillance detected no poliovirus during and after the switch, whereas enterovirus surveillance detected poliovirus only once during the switch. Environmental surveillance identified polioviruses in sewage samples until May, 2002, after which they were detected infrequently. Intratypic differentiation and sequencing showed that all polioviruses were Sabin-like. Multiple surveillance methods hence showed that OPV strains did not persist for extended periods after a vaccine switch in a developed country with a temperate climate. Sequence homology with Sabin vaccine parent strains indicated that polioviruses detected more than 4 months after the switch were of recent origin, consistent with importation from OPV-using countries.

Alzheimer's disease (AD) is characterized by a pathological process with specific predilection for association neocortex and the mesial temporal lobes. Recently developed high-resolution positron emission tomographs (PET) are able to quantitate regional cerebral metabolic rates for glucose (rCMRglc) in these brain regions and map the distribution of the metabolic consequences of Alzheimer pathology. In order to evaluate the relative involvement of mesial and neocortical temporal lobe brain regions in AD, we studied 22 AD patients, 11 of whom were mildly demented and 11 of whom were moderately demented in comparison to 8 age-matched control subjects. We used a PET instrument with 2.6 mm in-plane resolution and quantitated rCMRglc in anterior, middle, and posterior temporal neocortex, visual association cortex, primary visual cortex, and mesial temporal cortex. Although the moderately demented AD patients showed significantly lower metabolic rates than controls in visual association cortex and all temporal lobe regions except right anterior temporal neocortex, the mildly demented patients were different from the controls in only middle temporal neocortex. Considerable variability was found in the relative involvement of mesial temporal lobes and temporal neocortex in the AD patients, however, as shown by greater variance of a ratio of mesial temporal lobe rCMRglc to temporal neocortical rCMRglc (MES/NEO ratio) in the AD patients than the controls. A series of stepwise multiple regressions showed that this ratio was related to patient cognitive symptomatology, with more severely memory-impaired patients showing lower MES/NEO ratios, while patients with visuospatial disturbances showed higher MES/NEO ratios. In addition, the only biological variable that was related to this ratio was patient age, with older patients showing lower MES/NEO ratios. These results indicate that mesial temporal lobe structures are not invariably the earliest nor the most severely metabolically involved brain regions in AD and that the relative involvement of the mesial and neocortical temporal lobe is related to the patient's cognitive symptoms and age.
