Abstract

BACKGROUND: Adequate maternal thyroid function during pregnancy is important for an uncomplicated pregnancy. Although multiple observational studies have evaluated the association of thyroid dysfunction with hypertensive disorders of pregnancy, the methods and definitions of thyroid function test abnormalities were heterogeneous, and the results were conflicting. We hypothesized that maternal thyroid dysfunction as a risk factor in pregnancy could be due to an association between thyroid dysfunction and hypertensive disorders of pregnancy such as gestational hypertension and preeclampsia. We performed a systematic review and individual participant data meta-analysis to assess whether thyroid function test abnormalities were associated with gestational hypertension and preeclampsia. METHODS: We searched MEDLINE (Ovid), Embase, Scopus, and the Cochrane Database of Systematic Reviews from inception to December 27, 2019, for prospective cohort studies with data on maternal thyroid-stimulating hormone (TSH), free thyroxine (FT4), and/or thyroid peroxidase (TPO) antibody concentrations and gestational hypertension and/or preeclampsia, and we issued open invitations to study authors to participate in the Consortium on Thyroid and Pregnancy and share the individual participant data. We excluded participants who had preexisting thyroid disease, were taking medications which affect thyroid function, or had multifetal pregnancy. The primary outcomes were documented gestational hypertension and preeclampsia. Individual participant data were analyzed using logistic mixed-effects regression models adjusting for maternal age, body mass index, smoking, parity, ethnicity, and gestational age at blood sampling. The study protocol was registered at the International Prospective Register of Systematic Reviews, CRD42019128585. FINDINGS: We identified 1 539 published studies, of which 33 cohorts met the inclusion criteria and 19 cohorts were included after the authors agreed to participate. Our study population comprised 46 528 pregnant women, of whom 39 826 women had sufficient data (TSH and FT4 concentrations and TPO antibody status) to be classified according to their thyroid function status. Of those, 1 275 (3.2%) had subclinical hypothyroidism, 933 (2.3%) had isolated hypothyroxinemia, 619 (1.6%) had subclinical hyperthyroidism, and 377 (0.9%) had overt hyperthyroidism. Subclinical hypothyroidism was associated with a higher risk of preeclampsia (3.6% vs 2.1%; OR, 1.53 [95%CI, 1.09 to 2.15]) compared to euthyroidism. Subclinical hyperthyroidism, isolated hypothyroxinemia, or TPO antibody positivity were not associated with gestational hypertension or preeclampsia. In continuous analyses, both a higher and a lower TSH concentration were associated with a higher risk of preeclampsia (P=0.0001). The FT4 concentration was not associated with the outcomes measured. INTERPRETATION: Subclinical hypothyroidism during pregnancy was associated with a higher risk of preeclampsia. There was a U-shaped association of TSH with preeclampsia. These results quantify the risks of gestational hypertension or preeclampsia in women with thyroid function test abnormalities, adding to the total body of evidence on the risk of adverse maternofetal outcomes of thyroid dysfunction during pregnancy. These findings have potential implications for defining the optimal treatment target in women treated with levothyroxine during pregnancy, which needs to be assessed in future interventional studies.

Abstract

An experimental study has been performed on the effect of exposure to noise (N), vibration (V), and combined noise and vibration (NV), on the distortion product otoacoustic emission (DPOAE) level. The aim of the study is investigating the possible synergistic effect of noise and vibration, increasing the risk of damage to the hearing function due to noise only. Twelve normal-hearing volunteers were randomly exposed to hand arm vibration (HAV) only, noise only, and HAV and noise simultaneously. The exposure consisted of five consecutive sessions interspersed with quiet periods. DPOAEs were recorded during the quiet intervals following each exposure session, and during recovery, and compared to their baseline level, and among different test treatments. An ad hoc acquisition system developed in Labview was used to record high frequency-resolution DPOAE spectra in a suitably short time. Time-frequency filtering, based on the wavelet transform, was used to separate the DPOAE components coming from different generation mechanisms. The three test treatments, N, V, and NV, yielded significantly different results. The condition affecting most significantly all DPOAE component levels was the exposure to vibration only, causing a statistically significant increase of the signal level during all the exposure periods, with a decreasing trend during recovery. Also the exposure to noise only, N, caused a DPOAE level increase, statistically significant for the long latency (LL) DPOAE component. Only when exposure to noise was in combination with vibration, the zero-latency (ZL) component was clearly suppressed during the exposure. The results suggest a synergistic adverse effect of noise and vibration on the cochlear function. Relevance to industry The simultaneous exposure to noise and hand arm vibration is a very common industrial scenario. The risk that exposure to mild noise levels could interfere with the exposure to vibration, enhancing the adverse effect on the hearing function, should be considered to optimize prevention strategies at the workplace.

Abstract

Ectopy is the most common embryogenetic defect of the thyroid gland, representing between 48 and 61% of all thyroid dysgeneses. Persistence of thyroid tissue in the context of a thyroglossal duct remnant and lingual thyroid tissue are the most common defects. Although most cases of ectopic thyroid are asymptomatic, any disease affecting the thyroid may potentially involve the ectopic tissue, including malignancies. The prevalence of differentiated thyroid carcinoma in lingual thyroid and thyroglossal duct cyst is around 1% of patients affected with the above thyroid ectopies. We here review the current literature concerning primary thyroid carcinomas originating from thyroid tissue on thyroglossal duct cysts and lingual thyroid.

Abstract

The cryogenic resonant gravitational wave detectors NAUTILUS and EXPLORER, made of an aluminum alloy bar, can detect cosmic ray showers. At temperatures above 1 K, when the material is in the normal conducting state, the measured signals are in good agreement with the values expected based on the cosmic rays data and on the thermo-acoustic model. When NAUTILUS was operated at the temperature of 0.14 K, in superconductive state, large signals produced by cosmic ray interactions, more energetic than expected, were recorded. The NAUTILUS data in this case are in agreement with the measurements done by a dedicated experiment on a particle beam. The biggest recorded event was in EXPLORER and excited the first longitudinal mode to a vibrational energy of about 670 K, corresponding to about 360 TeV absorbed in the bar. Cosmic rays can be an important background in future acoustic detectors of improved sensitivity. Abstract

The interaction between cosmic rays and the gravitational wave bar detector NAUTILUS is experimentally studied with the aluminum bar at temperature of T=1.5 K. The results are compared with those obtained in the previous runs when the bar was at T=0.14 K. The results of the run at T = 1.5 K are in agreement with the thermo-acoustic model; no large signals at unexpected rate are noticed, unlike the data taken in the run at T = 0.14 K. The observations suggest a larger efficiency in the mechanism of conversion of the particle energy into vibrational mode energy when the aluminum bar is in the superconductive status., Comment: 7 pages, 3 figures, 2 tables. Abstract

Cosmic ray showers interacting with the resonant mass gravitational wave antenna NAUTILUS have been detected. The experimental results show large signals at a rate much greater than expected. The largest signal corresponds to an energy release in NAUTILUS of 87 TeV. We remark that a resonant mass gravitational wave detector used as particle detector has characteristics different from the usual particle detectors, and it could detect new features of cosmic rays. Among several possibilities, one can invoke unexpected behaviour of superconducting Aluminium as particle detector, producing enhanced signals, the excitation of non-elastic modes with large energy release or anomalies in cosmic rays (for instance, the showers might include exotic particles as nuclearites or Q-balls).
