Abstract

BACKGROUND: Depression is a heterogeneous disease. Identification of latent depression subgroups and differential associations across these putative groups and sociodemographic and health-related factors might pave the way toward targeted treatment of individuals.METHODS: We used model-based clustering to identify relevant subgroups of 2900 individuals with moderate to severe depression (defined as scores ≥10 on the PHQ-9 instrument) from the NHANES cross-sectional survey. We used ANOVA and chi-squared tests to assess associations between cluster membership and sociodemographics, health-related variables, and prescription medication use.RESULTS: We identified six latent clusters of individuals, three based on depression severity and three differentially loaded by somatic and mental components of the PHQ-9. The Severe mental depression cluster had the most individuals with low education and income (P LIMITATIONS: Due to the cross-sectional design we cannot make conclusions about causal relationships. We used self-reported data. We did not have access to a replication cohort.CONCLUSIONS: We show that socioeconomic factors, somatic diseases, and prescription medication use are differentially associated with distinct and clinically relevant clusters of individuals with moderate to severe depression.

19. Complementary and alternative medicine use during the COVID-19 pandemic: Community pharmacists’ knowledge, attitudes, and practices

Abstract

The challenges in COVID-19 treatment have driven patients to seek Complementary and Alternative Medicine (CAM) for the prevention and/or treatment of the disease. Pharmacists are uniquely positioned to promote the effective and safe use of CAM.Assess the beliefs, practices, and knowledge of community pharmacists in Lebanon about existing evidence for the use of CAM for the prevention and/or treatment of COVID-19.A national cross-sectional survey was conducted among community pharmacists during the COVID-19 pandemic in Lebanon (n = 310 respondents). Participants completed an online questionnaire addressing, in addition to the socio-demographic characteristics; the beliefs, practices, and knowledge of pharmacists regarding the existing evidence of CAM use for COVID-19 prevention and/or treatment.Pharmacists (70%) received frequent inquiries about CAM for COVID-19 prevention or treatment. Only 25.5% of the pharmacists reported having enough time to get information about CAM and COVID-19. Almost all participants believed that pharmacists have a major role in the fight against COVID-19 (98%) and that they have enough information to counsel patients in that regard (75%). For practices, the majority of pharmacists reported counseling their patients on proper use (98%) and possible adverse reactions (93%) of CAM, but only 51% reported toxic or undesirable effects. Regarding knowledge, although most pharmacists answered correctly the questions on the role of CAM in preventing COVID-19, few knew about its role in treating the infection. Pharmacists who trust the WHO website as a credible source had a higher knowledge score (β = 0.52, 95%CI: 0.05-0.986).The positive beliefs and practices of pharmacists towards CAM during the pandemic were coupled with a few gaps, especially in their knowledge about existing evidence for CAM use. Concerted efforts ought to be dedicated to supporting pharmacists in playing a more prominent role during health crises such as the COVID-19 pandemic.

24. COVID-19 perceived stigma among survivors: A cross-sectional study of prevalence and predictors

Abstract

Perceived stigma related to infectious diseases is of public health importance and can adversely impact patients' physical and mental health. This study aims to identify the level of perceived stigma among COVID-19 survivors in Qatar and investigate its predictors.An analytical cross-sectional design was employed. Four hundred and four participants who had a positive COVID-19 PCR test were randomly selected from medical records. The selected participants were interviewed to collect sociodemographic and health-related information. Perceived stigma was assessed using the COVID-19 perceived stigma scale-22 (CPSS-22) that was developed by the researchers. A descriptive analysis followed by a bivariate analysis investigated possible associations between the perceived stigma levels and independent variables. A multivariable analysis was performed using logistic regression to identify any significant associations with perceived stigma. The validity and reliability of the developed tool were also tested.The prevalence of COVID-19 perceived stigma was twenty-six percent (n=107, 26.4%) at 95% CI [22.4-30.4]. Factors associated with higher COVID-19 perceived stigma were male gender, being a manual worker, non-Arabic ethnicity, low educational level, living alone, and being isolated outside the home. However, only occupation, ethnicity, and low educational level predicted COVID-19 perceived stigma in multivariable analysis. The CPSS-22 showed excellent reliability (Cronbach's alpha 0.92).Perceived stigma was relatively common among participants. Designing programs and interventions targeting male manual workers and those of low-educational levels may assist policymakers in mitigating the stigma related to COVID-19.

Abstract

The misuse of and addiction to opioids are a national public health crisis. The complexity of delivering patient care in emergency departments exposes nurses to stressful work situations with complex patient loads and increasing levels of compassion fatigue. Emergency nurses were asked about their feelings of compassion fatigue while caring for patients with opioid use and/or substance use disorders.Twenty-four focus groups with emergency nurses (N = 53) at a level I trauma center were conducted in late 2019 and early 2020 are used in this qualitative study using thematic analysis that identified 1 main theme of compassion fatigue with 3 subthemes (nurse frustration with addicted patients, emotional responses, and job satisfaction).Findings highlight that emergency nurses working with patients with opioid use and/or substance use disorders are dealing with a number of negative emotional stressors and frustrations, which in turn has increased their levels of compassion fatigue. These nurses repeatedly expressed feelings of increasing frustration with addicted patients, negative emotional responses, and decreasing levels of job satisfaction as components of their compassion fatigue.These emergency nurses identified 3 areas to improve their compassion: improved management support with encouragement across all work shifts, debriefing opportunities, and more education. Fostering a high level of self-awareness and understanding of how the work environment influences personal well-being are necessary strategies to avoid the frustrations and negative emotional responses associated with compassion fatigue.

Abstract

BACKGROUND: Data concerning the long-term risk of heart failure (HF) in patients with takotsubo syndrome (TTS) are sparse. We compared the rates of death and hospitalization due to HF with matched individuals from the background population and patients with ST-segment elevation myocardial infarction (STEMI).METHODS: In this nationwide observational cohort study, all patients with first-time TTS (2011-2018) who were alive at discharge were identified by using data from Danish nationwide registries. These were matched for age and sex with individuals from the background population (1:4 matching) and with patients with STEMI who were alive at discharge (1:3 matching).RESULTS: A total of 881 patients with TTS who were alive at discharge were identified (median age 70 years; 89.4% men). During a mean follow-up of 2.9 years, the incidence rates of death, HF hospitalization, and TTS recurrence in survivors of TTS were 6.9, 0.9 and 1.1 events per 100 person-years. The corresponding absolute 3-year risks were 9.3%, 1.8% and 2.5%, respectively. Survivors of TTS had higher associated rates of death compared with the background population (hazard ratio [HR] 2.05 [95% CI, 1.62-2.60]) and survivors of STEMI (HR 1.69 [1.34-2.13]). Similarly, survivors of TTS had higher associated rates of hospitalization due to HF compared with the background population (HR 4.24 [1.88-9.53]), but lower rates compared with survivors of STEMI (HR 0.34 [0.20-0.56]). Propensity-score matched analyses yielded similar results.CONCLUSIONS: Survivors of TTS had significantly higher associated mortality rates than the background population and survivors of STEMI. Survivors of TTS had lower HF hospitalization rates than survivors of STEMI, but the rates were higher than those of the background population.

Abstract

Zinc oxide-hydroxyapatite composites were prepared using wet impregnation method. Firstly, a natural phosphate ore rich in silica and calcium phosphate was sieved to separate silica phase from phosphate phase. Then, through a chemical precipitation method, a pure hydroxyapatite (HAP) was obtained, which was used as a support for ZnO immobilization and applied for the photodegradation of two toxic contaminants: a transparent molecule (caffeine) and dye molecule (rhodamine B). During the present work two weight ratio percentages of zinc oxide were used: 25 wt.% and 50 wt.% of ZnO relative to HAP. The samples were characterized by X-ray diffraction (XRD), Fourier Transform Infrared (FTIR), X-ray Fluorescence (XRF), BET surface area (SBET), Scanning Electron Microscopy (SEM-EDS) and by Transmission Electron Microscopy (TEM-STEM). The immobilization of ZnO on HAP surface followed by thermal treatment at 400 °C for 2 h to get a homogenous dispersion of ZnO on the hydroxyapatite support. At high ZnO impregnation percentage, photodegradation performances of ZnO-HAP under UV illumination were fast and superior than the ZnO photocatalyst alone. The results showed that due to the presence of HAP, the conversion of both molecules became faster and greater, since it promotes the synergic phenomena of adsorption and photocatalysis. The toxicity of the treated substrate solutions obtained in the corn kernels germination test indicated a low toxicity after the photodegradation processes, probably due to a high mineralization degree.

Abstract

Introduction Since the beginning of the Coronavirus disease 2019 (COVID-19) pandemic, there has been a marked increase in the use of diets and dietary supplements (DDS) for the prevention and treatment of this emerging disease. While DDS are generally regarded as harmless, little evidence exists on the safety and efficacy of their use for COVID-19. Objectives To investigate the pattern and determinants of DDS use among the United Arab Emirates (UAE) population for the prevention and treatment of COVID-19. Design Cross-sectional web-based survey. Setting Adults residing in the UAE. Participants Participants (n = 2,060) residing in the UAE were recruited from databases of the Supreme Council for Family Affairs - Sharjah networks in the various Emirates. Primary and Secondary Outcomes Prevalence and determinants for the use of different DDS for the prevention and treatment of COVID-19 in the UAE, and sources of information for DDS use. Results The majority of participants reported using a form of the DDS understudy, with special foods being the most common (95.5%), followed by intake of citrus fruits (62.1%), supplements use (56.6%), increased water intake (50%), and herbal teas (38.4%). Only 20% of participants reported the main source of information on DDS to be health care practitioners, with the majority relying on either social media (40.4%) or family and friends (28.7%). After adjustment, female gender, older age (>40 years), and Asian ethnicities were characteristics associated with higher odds of using most of the DDS modalities and were also correlates of reporting health care practitioners as the main source of information for their DDS use. Conclusions The findings showed widespread use of DDS for the prevention and treatment of COVID-19. The use of DDS in this study was mainly guided by social media with a marginal role of health care practitioners. These findings call for a more integrative approach towards DDS to ensure its proper and safe use.

Abstract

Summary: Background: Children born into disadvantaged socioeconomic circumstances are more likely to experience both adversity during childhood and premature mortality. The aim of this study was to investigate how much of the parental education gradient in early adult mortality is explained by exposure to childhood adversity. Methods: We used data from the nationwide register-based Danish Life Course cohort study. Our sample consisted of all individuals born between Jan 1, 1980, and Dec 31, 2001, who did not emigrate or die before age 16 years, and for whom information on parental education level was available. These individuals were followed up for mortality from age 16 years until Dec 31, 2018. Highest attained parental education level at birth was divided into low (≤9 years), medium (10–12 years), and high (>12 years) according to years in education. Individuals were assigned to one of five childhood adversity trajectory groups based on their annual exposure between age 0 and 16 years to a broad selection of adversities in three dimensions: material deprivation, loss or threat of loss in the family, and family dynamics. Childhood abuse was not included. The association between parental education level and mortality was assessed with a Cox proportional hazards model. To assess the magnitude of mediation of this association by childhood adversity, we used counterfactual mediation analysis and an Aalen additive hazards model. Analyses were unadjusted and adjusted for parental origin and parental ages at birth. Findings: Our sample consisted of 1 278 156 individuals followed up from birth until age 16–38 years. The sample comprised 655 633 (51·3%) men and 622 523 (48·7%) women, and 1 243 981 (97·3%) participants were of European descent. During follow-up, 5387 deaths were registered. Compared with the high parental education group, we calculated a total effect equal to 8·7 additional deaths (95% CI 6·6–10·9) per 100 000 person-years in the medium parental education group and 31·9 (28·5 to 35·2) per 100 000 person-years in the low parental education group. Mediation through childhood adversity trajectories accounted for 41·5% (95% CI 8·0–67·5) of the additional deaths in the medium parental education group and 46·4% (32·9–58·8) of the additional deaths in the low parental education group. The results were similar when adjusting the analyses for sociodemographic factors. Interpretation: The experience of childhood adversity seems to be an important mediator of the association between parental education and mortality in early adulthood. Interventions reducing the exposure to childhood adversity might thus reduce the parental education gradient in early adult mortality. Abstract

This article presents an extensive review of scientific publications focused on how the impacts of bauxite mining and refining have been investigated. This includes results on which types of impacts have been addressed in scientific publications and details concerning social impacts in the years 2010‒2020. The analysis reveals that while environmental impacts are most prominent in academic publications, social impacts have been increasingly investigated in recent years. Between 2011 and 2020, there have been 14 publications covering social impacts, compared to eight publications from 1981‒2010. Focusing on social impacts, the present study finds a range of impacts including access to resources, impacts on landscape, economic impacts (e.g., job creation, local support programmes and infrastructure creation) and public unrest. The review also emphasises the importance of considering the context of social impacts and how impacts are distributed between social groups. These findings are useful for consideration in future impact assessment practices as well as furthering research in the intersection between mining and impact assessment.

Abstract

BackgroundChildren who are exposed to adversities might be more susceptible to disease development during childhood and in later life due to impaired physiological and mental development. To explore this hypothesis, we assessed hospitalisation patterns through childhood and into adult life among those exposed to different trajectories of adversities during childhood.MethodsFor this population-based cohort study, we used annually updated data from Danish nationwide registers covering more than half a million children (aged 0–15 years) born between 1994 and 2001. Children who were alive and resident in Denmark on their 16th birthday were included in the analysis. Cluster analysis was used to divide children into five distinct trajectories according to their experience of childhood adversities, including poverty and material deprivation, loss or threat of loss within the family, and aspects of family dynamics. To describe comprehensively the disease patterns experienced by these groups of children, we assessed the associations of each adversity trajectory with hospital admission patterns according to the entire spectrum of disease diagnoses in the International Classification of Diseases 10th edition, from birth to 24 years of age, using survival models.Findings508 168 children born between Jan 1, 1994, and Dec 31, 2001, were followed up until Dec 31, 2018, capturing more than 3·8 million hospital admissions from birth to early adulthood. Hospitalisation rates were consistently higher in all four adversity groups compared with the low adversity group. The high adversity group (14 577 children, 3%), who were exposed to adversities of deprivation, family loss, and negative family dynamics, had a markedly higher rate of hospitalisations across all ages. For example, we observed 243 additional hospital admissions per 1000 person-years (95% CI 238–248) in the high versus low adversity group for those aged 16–24 years. These associations were particularly strong for diagnoses related to injuries, unspecified symptoms, and factors influencing health service contacts (eg, health screening and observation). They also covered a considerable burden of respiratory and infectious diseases, congenital malformations, diseases of the nervous system (especially in early life), mental and behavioural diagnoses, and diagnoses related to pregnancy and childbirth in early adult life.InterpretationThe close linkage between childhood adversities and poor lifelong health outcomes highlights a need for public health and policy attention on improving the socioeconomic circumstances children are born into to prevent the early emergence of health inequalities.
