Myelin, multilayered lipid-rich membrane extensions formed by oligodendrocytes around neuronal axons, is essential for fast and efficient action potential propagation in the central nervous system. Initially thought to be a static and immutable process, myelination is now appreciated to be a dynamic process capable of responding to and modulating neuronal function throughout life. While the importance of this type of plasticity, called adaptive myelination, is now well accepted, we are only beginning to understand the underlying cellular and molecular mechanisms by which neurons communicate experience-driven circuit activation to oligodendroglia and precisely how changes in oligodendrocytes and their myelin refine neuronal function. Here, we review recent findings addressing this reciprocal relationship in which neurons alter oligodendroglial form and oligodendrocytes conversely modulate neuronal function.

Metabolic acidosis induces osteoclastic bone resorption and inhibits osteoblastic bone formation. Previously we found that mice with a global deletion of the proton receptor OGR1 had increased bone density although both osteoblast and osteoclast activity were increased. To test whether direct effects on osteoclast OGR1 are critical for metabolic acidosis stimulated bone resorption, we generated knockout mice with an osteoclast-specific deletion of OGR1 (knockout mice). We studied bones from three-month old female mice and the differentiated osteoclasts derived from bone marrow of femurs from these knockout and wild type mice. MicroCT demonstrated increased density in tibiae and femurs but not in vertebrae of the knockout mice. Tartrate resistant acid phosphatase staining of tibia indicated a decrease in osteoclast number and surface area/bone surface from knockout compared to wild type mice. Osteoclasts derived from the marrow of knockout mice demonstrated decreased pit formation, osteoclast staining and osteoclast-specific gene expression compared to those from wild type mice. In response to metabolic acidosis, osteoclasts from knockout mice had decreased nuclear translocation of NFATc1, a transcriptional regulator of differentiation, and no increase in size or number compared to osteoclasts from wild type mice. Thus, loss of osteoclast OGR1 decreased both basal and metabolic acidosis-induced osteoclast activity indicating osteoclast OGR1 is important in mediating metabolic acidosis-induced bone resorption. Understanding the role of OGR1 in metabolic acidosis-induced bone resorption will provide insight into bone loss in acidotic patients with chronic kidney disease.

BackgroundPrevious research has shown that, in comparison with non-pregnant women of reproductive age, pregnant women with COVID-19 are more likely to be admitted to critical care, receive invasive ventilation, and die. At present there are limited data in relation to outcomes and healthcare utilisation following hospital discharge of pregnant and recently pregnant women admitted to critical care.MethodsA national cohort study of pregnant and recently pregnant women who were admitted to critical care in Scotland with confirmed or suspected COVID-19. We examined hospital outcomes as well as hospital re-admission rates.ResultsBetween March 2020 and March 2022, 75 pregnant or recently pregnant women with laboratory-confirmed COVID-19 were admitted to 24 Intensive Care Units across Scotland. Almost two thirds (n=49, 65%) were from the most deprived socio-economic areas. Complete 90-day acute hospital re-admission data were available for 74 (99%) patients. Nine (12%) women required an emergency non-obstetric hospital re-admission within 90 days. Less than 5% of the cohort had received any form of vaccination.ConclusionsThis national cohort study has demonstrated that pregnant or recently pregnant women admitted to critical care with COVID-19 were more likely to reside in areas of socio-economic deprivation, and fewer than 5% of the cohort had received any form of vaccination. More targeted public health campaigning across the socio-economic gradient is urgently required.

Background High systolic blood pressure (SBP) increases cardiac afterload, whereas low diastolic blood pressure (DBP) may lead to impaired coronary perfusion. Thus, wide pulse pressure (high systolic, low diastolic [HSLD]) may contribute to myocardial ischemia and also be a predictor of adverse cardiovascular events. Objectives The purpose of this study was to determine the relationship between pre-procedural blood pressure and long-term outcome following percutaneous coronary intervention (PCI). Methods The study included 10,876 consecutive patients between August 2009 and December 2016 from the Melbourne Interventional Group registry undergoing PCI with pre-procedural blood pressure recorded. Patients with ST-segment elevation myocardial infarction, cardiogenic shock, and out-of-hospital cardiac arrest were excluded. Patients were divided into 4 groups according to SBP (high ≥120 mm Hg, low 70 mm Hg, low ≤70 mm Hg). Results Mean pulse pressure was 60 ± 21 mm Hg. Patients with HSLD were older and more frequently women, with higher rates of hypercholesterolemia, renal impairment, diabetes, and multivessel and left main disease (all p ≤ 0.0001). There was no difference in 30-day major adverse cardiac events, but at 12 months the HSLD group had a greater incidence of myocardial infarction (p = 0.018) and stroke (p = 0.013). Long-term mortality was highest for HSLD (7.9%) and lowest for low systolic, high diastolic (narrow pulse pressure) at 2.1% (p = 0.0002). Cox regression analysis demonstrated significantly lower long-term mortality in the low systolic, high diastolic cohort (hazard ratio: 0.50; 99% confidence interval: 0.25 to 0.98; p = 0.04). Conclusions Pulse pressure at the time of index PCI is associated with long-term outcomes following PCI. A wide pulse pressure may serve as a surrogate marker for risk following PCI and represents a potential target for future therapies.

Objectives: Despite the recognized importance of neuromuscular exercises, there is currently no widely accepted clinical outcome measure focused on neuromuscular control for patients with knee osteoarthritis (OA). The purposes of the present study were to investigate the test-retest reliability, concurrent validity and longitudinal validity of the star excursion balance test (SEBT) in patients with knee OA. Design: 74 patients performed the SEBT on two sessions within 7 days, and on a third session after completing 12 weeks of a home exercise program focused on neuromuscular control. A subgroup of 37 performed the SEBT while in the field of view of a motion capture system to estimate concurrent validity. The SEBT was recorded in cm and also normalized to leg length (LL). Participants also completed the 40 m fast-paced walk test and patient-reported outcomes before and after the exercise program. Results: Intraclass correlation coefficients (95% confidence intervals) were 0.94 (0.91 to 0.96) and 0.93 (0.89 to 0.96) and standard errors of measurement were +/- 2.68 cm and +/- 3.05% LL for raw and normalized composite scores, respectively. The minimum detectable change at the 95% confidence level for the composite score was 7.44 cm and 8.45% LL. Correlations between observer and motion capture measures were very high (Pearson r > 0.96). There was a significant increase in SEBT following the exercise program (standardized response mean = 0.74). The change in SEBT had low correlations with changes in 40 m walk times (r = 0.26) and pain (r = 0.28). Conclusion: The SEBT has suitable measurement properties for use in patients with knee OA. Brown adipose tissue (BAT) is a specialized metabolic organ responsible for non-shivering thermogenesis. Recently, its activity has been shown to be critical in systemic metabolic health through its utilization and consumption of macronutrients. In the face of energetically demanding states, metabolic flexibility and systemic coordination of nutrient partitioning is requisite for health and survival. In this study, we elucidate BAT's differential transcriptional adaptations in response to multiple nutrient challenges and demonstrate its context-dependent prioritization of lipid, glucose, and amino acid metabolism. We show that the transcription factor Krüppel-like factor 15 (KLF15) plays a critical role in BAT metabolic flexibility. BAT-specific loss of KLF15 results in widespread changes in circulating metabolites and severely compromised thermogenesis in response to high energy demands, indicative of impaired nutrient utilization and metabolic flexibility. Together, our data demonstrate KLF15 in BAT plays an indispensable role in partitioning resources to maintain homeostasis and ensure survival.

Multiple sclerosis (MS) is a demyelinating, autoimmune disease of the central nervous system. While work has focused on myelin and axon loss in MS, less is known about mechanisms underlying synaptic changes. Using postmortem human MS tissue, a preclinical nonhuman primate model of MS, and two rodent models of demyelinating disease, we investigated synapse changes in the visual system. Similar to other neurodegenerative diseases, microglial synaptic engulfment and profound synapse loss were observed. In mice, synapse loss occurred independently of local demyelination and neuronal degeneration, but coincided with gliosis and increased complement component C3, but not C1q, at synapses. Viral overexpression of the complement inhibitor Crry at C3-bound synapses decreased microglial engulfment of synapses and protected visual function. These results indicate that microglia eliminate synapses through the alternative complement cascade in demyelinating disease and identify a strategy to prevent synapse loss that may be broadly applicable to other neurodegenerative diseases.

Most women develop MS before menopause. Menopausal hot flashes can worsen MS symptoms, and could be relieved with hormone therapy. Our objective was to evaluate feasibility, tolerability and symptom response of Duavee® (bazedoxifene + conjugated estrogen) in a Phase Ib/IIa double-blind randomized controlled clinical trial.We randomized 24 peri/postmenopausal women with MS and symptomatic hot flashes 1:1 to Duavee® versus placebo. Evaluations occurred at baseline and 2 months.Groups were balanced for age (mean 51.2 ± 3.6 years), EDSS [median 3 (IQR:2.5, 4.5)], and MS duration. 21/24 participants completed the study.Enrollment was protracted (34 months), partially due to concerns about hormone therapy safety.treatment group participants reported greater satisfaction and fewer missed doses; one participant (placebo) developed new MRI lesions; liver function testing remained normal for all patients.Hot Flash Related Daily Interference scale at 2 months was lower in treatment vs. placebo group [median (IQR) of 4 (0.5, 14) vs. 9 (0, 33)]. Between-group differences were not statistically significant.Despite perceived benefits in MS, estrogens have perceived risks that represent a hurdle to enrollment. With appropriate education and screening of participants, the favorable study retention (87%) and treatment satisfaction observed in the current study support the feasibility of a longer, powered trial to evaluate whether a proven treatment for menopausal symptoms, Duavee®, could also improve MS-related function in menopausal women with MS.

Wastewaters contaminated with nitrogenous pollutants, derived from anthropogenic activities, have exacerbated our ecosystems sparking environmental problems, such as eutrophication and acidification of water reservoirs, emission of greenhouse gases, death of aquatic organisms, among others. Wastewater treatment facilities (WWTF) combining nitrification and denitrification, and lately partial nitrification coupled to anaerobic ammonium oxidation (anammox), have traditionally been applied for the removal of nitrogen from wastewaters. The present work provides a comprehensive review of the recent biotechnologies developed in which nitrogen-removing processes are relevant for the treatment of both wastewaters and gas emissions. These novel processes include the anammox process with alternative electron acceptors, such as sulfate (sulfammox), ferric iron (feammox), and anodes in microbial electrolysis cells (anodic anammox). New technologies that couple nitrate/nitrite reduction with the oxidation of methane, H2S, volatile methyl siloxanes, and other volatile organic compounds are also described. The potential of these processes for (i) minimizing greenhouse gas emissions from WWTF, (ii) biogas purification, and (iii) air pollution control is critically discussed considering the factors that might trigger N2O release during nitrate/nitrite reduction. Moreover, this review provides a discussion on the main challenges to tackle towards the consolidation of these novel biotechnologies.

Percutaneous coronary intervention (PCI) continues to evolve with shifting patient demographics, treatments, and outcomes. We sought to document the specific changes observed over a 9-year period in a contemporary Australian PCI cohort. The Melbourne Interventional Group is an established multicenter PCI registry in Melbourne, Australia. Data were collected prospectively with 30-day and 12-month follow-ups. Demographic, procedural, and outcome data for all consecutive patients were analyzed with a year-to-year comparison from 2005 to 2013. National Death Index linkage was performed for long-term mortality analysis; 19,858 procedures were captured over 9 years. Patient complexity and acuity increased with a higher proportion of traditional risk factors and more elderly patients who underwent PCI. Angiographic lesion complexity increased with more multivessel coronary artery disease and more American College of Cardiology/American Heart Association type B2/C lesions proceeding to PCI. The 30-day rate of death, myocardial infarction, or target vessel revascularization has not changed nor has 12-month mortality, myocardial infarction, or major adverse cardiovascular event rates. As one special type of sustainable energy source, radioisotopes have extremely high energy densities. Electrical generators using the decay heat from these radioisotopes can provide long-lasting power when other types of energy, such as solar or chemical energy, are not available. Starting from the 1960s, radioisotope generators were safely used in numerous space and terrestrial missions, and have powered systems from interstellar spacecraft to lighthouses along the arctic coast. This work reviewed the generator development and critical components that constitute a functional system. Started with a brief generator history, the paper focused on technological aspects including fuel manufacturing, energy conversion elements, thermal insulation, structural materials, prototype measurements, future improvements, and concluded with the safety aspects for both space and terrestrial applications. Radioisotope generators are one example of using nuclear energy in a peaceful way to benefit human be-ings. Plutonium fuel productions were resumed and accelerated in the United States and americium fuel research was actively sponsored by the European Space Agency. Rationale & objective Creation of an arteriovenous fistula (AVF), compared to an AV graft (AVG), is associated with longer initial catheter dependence after starting hemodialysis but longer access survival and lower long-term catheter dependence. The extent of these potential long-term benefits in elderly patients is unknown. We assessed catheter dependence after AVF or AVG placement among elderly patients who initiated HD without a permanent access in place. Study design Retrospective cohort study. Setting& participants Patients ≥67 years of age identified in the United States Renal Data System (USRDS) who had a first AVF (n=14,532) or AVG (n=3391) placed within 1 year after HD initiation between May 2012 and May 2017. Exposure AVF versus AVG placement in first year of hemodialysis. Outcome Catheter dependence after AVF or AVG placement assessed using CROWNWeb data. ANALYTICAL APPROACH: - Generalized estimating equations and negative binomial regression for catheter use over time and Cox proportional hazards models for mortality. Results Creation of an AVF versus AVG was associated with greater catheter dependence at one month (95.6% vs. 92.5%) and at three months (82.8% vs. 41.2%), but lower catheter dependence at 12 months (14.2% vs. 15.8%) and at 36 months (8.2% vs. 15.0%). Creation of an AVF, however, remained significantly associated with greater cumulative catheter-dependent days (80.1 vs 54.6 days per person-year) and a lower proportion of catheter-free survival time (78.1% vs 85.1%) after three years of follow-up. Limitations Potential for unmeasured confounding and analyses limited to elderly patients. Conclusions Creation of an AVF was associated with significantly greater cumulative catheter dependence than placement of an AVG in an elderly population initiating HD without a permanent access. As the long-term benefits in terms of catheter dependency of an AVF are not realized in many elderly patients, specific patient characteristics should be considered when making decisions regarding vascular access.

This work describes RTPV-PhC-1, an initial prototype for a radioisotope thermophotovoltaic (RTPV) system using a two-dimensional photonic crystal emitter and low bandgap thermophotovoltaic (TPV) cell to realize spectral control. We validated a system simulation using the measurements of RTPV-PhC-1 and its comparison setup RTPV-FlatTa-1 with the same configuration except a polished tantalum emitter. The emitter of RTPV-PhC-1 powered by an electric heater providing energy equivalent to one plutonia fuel pellet reached 950 °C with 52 W of thermal input power and produced 208 mW output power from 1 cm2 TPV cell. We compared the system performance using a photonic crystal emitter to a polished flat tantalum emitter and found that spectral control with the photonic crystal was four times more efficient. Based on the simulation, with more cell areas, better TPV cells, and improved insulation design, the system powered by a fuel pellet equivalent heat source is expected to reach an efficiency of 7.8%.

During development oligodendrocyte precursor cells (OPCs) rapidly proliferate and migrate throughout the central nervous system. The mobilization of OPCs is followed by terminal differentiation into mature oligodendrocytes and the subsequent myelination of axons. Differentiation of OPCs is CNS-wide and robust, and yet spatially and temporally restricted. What factors control this precise and coordinated differentiation effort? We discuss evidence for both intrinsic and extrinsic cues in regulating OPC differentiation and gather that extrinsic cues play the leading role in regulating the differentiation of OPCs into mature oligodendrocytes.

In humans, high level of collagen remodeling is seen during normal physiological events such as bone renewal, as well as in pathological conditions, such as arthritis, tumor growth and other chronic wounds. Our lab recently discovered that collagen mimetic peptide (CMP) is able to hybridize with denatured collagens at these collagen remodeling sites with high affinity. Here, we show that the CMP's high binding affinity to denatured collagens can be utilized to deliver angiogenic signals to scaffolds composed of heat-denatured collagens (gelatins). We first demonstrate hybridization between denatured collagens and QKCMP, a CMP with pro-angiogenic QK domain. We show that high levels of QKCMP can be immobilized to a new artificial matrix containing both fibrous type I collagen and heat denatured collagen through triple helix hybridization, and that the QKCMP is able to stimulate early angiogenic response of endothelial cells (ECs). We also show that the QKCMP can bind to excised tissues from burn injuries in cutaneous mouse model, suggesting its potential for promoting neovascularization of burn wounds.

This paper presents the effect of chip stacking in a 3D integrated circuit package during plastic encapsulation. An experiment was conducted on four stacked chips with bumps in a perimeter array. The flow front advancement and chip displacement in the experiment were validated by using FLUENT 6.3 and ABAQUS 6.9, respectively. A total of four models, which consist of two, three, four, and five stacked chips with through-silicon vias, were studied numerically. A simultaneous or direct solution procedure was employed to solve the variables of the fluid/structural domain. This approach provides better visualization of the actual plastic encapsulation process by considering the fluid-structure interaction phenomenon during the process. A constant ratio of inlet and outlet gate heights was applied to create a more uniform flow front advancement among the models. Results indicate that the highest displacement occurred in Model 4, which contains the most stacked chips. The highest von Mises stress was also detected in Model 4. Therefore, unfavorable deformation is anticipated when more stacked chips are employed. The experimental and numerical studies provide useful information in understanding the fluid flow of epoxy resin and subsequent structural deformation under the effect of chip stacking.

a b s t r a c t Building envelope airtightness is important for residential energy use, occupant health and comfort. We analyzed the air leakage measurements of 134,000 single-family detached homes in US, using normalized leakage (NL) as the metric. Weatherization assistance programs (WAPs) and residential energy efficiency programs contributed most of the data. We performed regression analyses to examine the relationship between NL and various house characteristics. Explanatory variables that are correlated with NL include year built, climate zone, floor area, house height, and whether homes participated in WAPs or if they are energy efficiency rated homes. Foundation type and whether ducts are located outside or inside the conditioned space are also found to be useful parameters for predicting NL. We developed a regression model that explains approximately 68% of the observed variability across US homes. Of these variables considered, year built and climate zone are the two that have the largest influence on NL. The regression model can be used to predict air leakage values for individual homes, and distributions for groups of homes, based on their characteristics. Using RECS 2009 data, the regression model predicts 90% of US houses have NL between 0.22 and 1.95, with a median of 0.67.

An analogue of the anticancer drug cisplatin (mtPt) was delivered to mitochondria of human cells using a peptide specifically targeting this organelle. mtPt induces apoptosis without damaging nuclear DNA, indicating that mtDNA damage is sufficient to mediate the activity of a platinum-based chemotherapeutic. This study is the first to demonstrate specific delivery of a platinum drug to mitochondria and to investigate the effects of directing this agent outside the nucleus.

In this paper, an efficient numerical approach has been adopted to analyze the fluid/structure interaction (FSI) implications to 3D IC with through-silicon via (TSV) structures during plastic encapsulation process, under the effect of different EMC rheological properties. A simultaneous or direct solution procedure is employed to solve the solution variables of fluid/structural domain together. Therefore, this approach can provide better visualization of the actual plastic encapsulation process by considering FSI phenomenon during the process. Different epoxy molding compound (EMC) rheological properties have resulted dissimilar fluid flow characteristics, pressure distribution, air void entrapment level, structural displacement and von Mises stress, which are discussed in the paper. Unfavorable results are obtained when higher viscous flow takes place. EMC flow front prediction has been validated with the experimental results from literature. The numerical approach and results presented in this paper are able to provide useful information for the works in optimizing plastic encapsulation process.

Bacterial pneumonia remains a significant burden worldwide. Although an inflammatory response in the lung is required to fight the causative agent, persistent tissue-resident neutrophils in non-resolving pneumonia can induce collateral tissue damage and precipitate acute lung injury. However, little is known about mechanisms orchestrated in the lung tissue that remove apoptotic neutrophils to restore tissue homeostasis. In mice infected with Klebsiella pneumoniae, a bacterium commonly associated with hospital-acquired pneumonia, we show that interleukin (IL)-10 is essential for resolution of lung inflammation and recovery of mice after infection. Although IL-10(-/-) mice cleared bacteria, they displayed increased morbidity with progressive weight loss and persistent lung inflammation in the later phase after infection. A source of tissue IL-10 was found to be resident CD11b(+)Gr1(int)F4/80(+) cells resembling myeloid-derived suppressor cells (MDSCs) that appeared with a delayed kinetics after infection. These cells efficiently efferocytosed apoptotic neutrophils, which was aided by IL-10. The lung neutrophil burden was attenuated in infected signal transducer and activator of transcription 1 (STAT1)(-/-) mice with concomitant increase in the frequency of the MDSC-like cells and lung IL-10 levels. Thus, inhibiting STAT1 in combination with antibiotics may be a novel therapeutic strategy to address inefficient resolution of bacterial pneumonia.

Establishment of oligodendrocyte identity is crucial for subsequent events of myelination in the central nervous system (CNS). Here, we demonstrate that activation of ATP-dependent SWI/SNF chromatin-remodeling enzyme Smarca4/Brg1 at the differentiation onset is necessary and sufficient to initiate and promote oligodendrocyte lineage progression and maturation. Genome-wide multistage studies by ChIP-seq reveal that oligodendrocyte-lineage determination factor Olig2 functions as a pre-patterning factor to direct Smarca4/Brg1 to oligodendrocyte-specific enhancers. Recruitment of Smarca4/Brg1 to distinct subsets of myelination regulatory genes is developmentally regulated. Functional analyses of Smarca4/Brg1 and Olig2 co-occupancy relative to chromatin epigenetic marking uncover novel stage-specific cis-regulatory elements that predict sets of transcriptional regulators controlling oligodendrocyte differentiation. Together, our results demonstrate that regulation of the functional specificity and activity of a Smarca4/Brg1-dependent chromatin-remodeling complex by Olig2, coupled with transcriptionally-linked chromatin modifications, is critical to precisely initiate and establish the transcriptional program that promotes oligodendrocyte differentiation and subsequent myelination of the CNS.

This paper presented the study of encapsulation process in 3D stacked chips with different microbump arrays. An experiment was carried out on two-stacked chips with bumps in perimeter array, and validated with numerical simulation done in FLUENT 6.3. In the numerical study, three different microbump arrays, namely full, semi-full and perimeter arrays, were studied. It was found that perimeter array provided the easiest route for epoxy molding compound (EMC) flow front advancement with the least EMC conversion and microbump impediment. Therefore, the air entrapment level was the lowest in perimeter array. For full and semi-full arrays, higher level of air entrapment was observed as there were more significant EMC conversion, microbump impediment and imbalanced EMC flow fronts. The data presentation in this paper provides a good understanding on EMC flow behavior, especially in various microbump arrays during the encapsulation process.

Deep sequencing of the 16S rRNA gene provides a comprehensive view of bacterial communities in a particular environment and has expanded our ability to study the impact of the microflora on human health and disease. Current analysis methods rely on comparisons of the sequences generated with an expanding but limited set of annotated 16S rRNA sequences or phylogenic clustering of sequences based on arbitrary similarity cutoffs. We describe a novel approach to characterize bacterial composition using deep sequencing of 16S rRNA gene. Our method defines operational taxonomic units based on phylogenetic tree reconstruction and dynamic clustering of sequences using solely sequencing data. These OTUs can be used to identify differences in bacteria abundance between environments. This approach can perform better than previous phylogenetic methods and will significantly improve our understanding of the microfloral role on human diseases by providing a comprehensive analysis of the microbial composition from various bacterial communities.

To aid in the design of low-temperature high-efficiency thermophotovoltaic (TPV) systems, based on low-bandgap photovoltaic (PV) diodes and selective thermal emitters, we developed a detailed model of GaSb and GaInAsSb PV diodes. An equivalent circuit model describes the electrical behavior of the diode as a function of the operating point defined by a photocurrent and a junction temperature. The single diode equivalent circuit model has five variable parameters: photocurrent, dark current, ideality, and series and shunt resistance. The model accurately predicts the circuit parameters for a given operating point. In addition, parameterized quantum efficiency is used to calculate photocurrent while accounting for temperature induced bandgap narrowing. Models show very good agreement with experimental results obtained with calibrated blackbody source. Although we focus on TPV systems, these methods and results are applicable to solar-thermophotovotaic and concentrator photovoltaic systems.

Despite aggressive efforts to increase autogenous fistula prevalence primarily from recommendations by the NKF and the Fistula First National Vascular Access Improvement Initiative, catheters remain an essential access modality for a large percentage of the hemodialysis population. Tunneled dialysis catheters or chronic catheters are associated with a multitude of complications including infections, stenosis, thrombosis, and increased morbidity and mortality even after adjustment for potential confounding variables. Also, given the blood flow rates of catheters, dialysis adequacy is compromised as compared with arteriovenous fistulae and arteriovenous grafts. This review endeavors to provide an update on catheter outcomes in the Fistula First and Kidney Disease Outcomes Quality Initiative era in relation to the increasing fistula prevalence and decline in graft placements. The conflicting view of whether catheters are increasing concurrently with the increase in fistula prevalence is discussed. The management of tunneled dialysis catheter-related complications is considered with a review of the most recent data. Future research strategies and innovations in catheter design are also addressed. This review provides a comprehensive update of tunneled hemodialysis catheters, their necessity and pitfalls, and novel directions for future investigation.

Atypical hemolytic uremic syndrome, or the nondiarrheal form of hemolytic uremic syndrome, is a rare disorder typically classified as familial or sporadic. Recent literature has suggested that approximately 50% of patients have mutations in factor H (CFH), factor I (CFI), or membrane cofactor protein (encoded by CD46). Importantly, results of renal transplantation in patients with mutations in either CFH or CFI are dismal, with recurrent disease leading to graft loss in the majority of cases. We describe an adult renal transplant recipient who developed recurrent hemolytic uremic syndrome 1 month after transplantation. Bidirectional sequencing of CFH, CFI, and CD46 confirmed that the patient was heterozygous for a novel missense mutation, a substitution of a serine reside for a tyrosine residue at amino acid 369, in CFI. This report reemphasizes the importance of screening patients with atypical hemolytic uremic syndrome for mutations in these genes before renal transplantation and shows the challenges in the management of these patients.

We have fabricated and tested the performance of sub-50nm gate nMOSFETs to assess their suitability for mixed signal applications in the super high frequency (SHF) band, i.e. 3-30GHz. For a 30nm×40 μm×2 device, we found f(T) =465GHz at V(ds)=2V, V(g)=0.67V, which is the highest cut-off frequency reported for a MOSFET produced on bulk silicon substrate so far. However, our measurements of f(max) and noise figure indicate that parasitics impose limitations on SHF operation. We also present a high-frequency ac model appropriate to sub-50nm gate length nanotransistors, which incorporates the effects of the parasitics. The model accurately accounts for measurements of the S and Y parameters in the frequency range from 1 to 50GHz.

In the event of a short-term, large-scale toxic chemical release to the atmosphere, shelterin-place (SIP) may be used as an emergency response to protect public health. We modeled hypothetical releases using realistic, empirical parameters to explore how key factors influence SIP effectiveness for single-family dwellings in a residential district. Four classes of factors were evaluated in this case-study: (a) time scales associated with release duration, SIP implementation delay, and SIP termination; (b) building air-exchange rates, including air infiltration and ventilation; (c) the degree of sorption of toxic chemicals to indoor surfaces; and (d) the shape of the dose-response relationship for acute adverse health effects. Houses with lower air leakage are more effective shelters, and thus variability in the air leakage of dwellings is associated with varying degrees of SIP protection in a community. Sorption on indoor surfaces improves SIP effectiveness by lowering the peak indoor concentrations and reducing the amount of contamination in the indoor air. Nonlinear dose-response relationships imply substantial reduction in adverse health effects from lowering the peak exposure concentration. However, if the scenario is unfavorable for sheltering (e.g. sheltering in leaky houses for protection against a nonsorbing chemical with a linear dose-response), the community must implement SIP without delay and exit from shelter when it first becomes safe to do so. Otherwise, the community can be subjected to even greater risk than if they did not take shelter indoors.

For a large-scale, unanticipated release of a toxic chemical into the atmosphere, it is recommended for nearby populations to shelter indoors. Two new metrics to quantify the community-scale effectiveness of shelter-in-place (SIP) are introduced. The casualty reduction factor (CRF) quantifies the expected reduction in casualties if SIP is performed. The safety-factor multiplier (SFM) quantifies the extent of toxic-load reduction for individuals in each exposed building. In this paper, idealized models are combined to explore the relationships among important input parameters and the SIP-effectiveness metrics. A Gaussian plume model predicts ambient concentrations for a hypothetical release event. A box model predicts indoor concentrations in buildings. A toxic-load model links exposure to health consequences. SIP effectiveness varies significantly with the toxic-load exponent, m, which characterizes the dose–response relationship. Another influential variable is a dimensionless time scale, ξ, equal to the release duration multiplied by the building air-exchange rate. Other factors that influence SIP effectiveness include the magnitude of the release relative to the toxicity of the pollutant, atmospheric transport and dispersion rates, and punctual termination of SIP once the toxic cloud has passed. Using matrix assisted laser desorption ionisation mass spectrum (MALDI-MS), this study reports the observations of the fouling distribution and composition along the membrane channel after the membranes were subjected to ultrafiltration of protein mixture solution in a crossflow module with and without spacer. In the fouling layer on a fully retentive membrane, the protein components with high molecular weight has higher presentation after 2 h of filtration and the presentation reduced to be lower than the smaller components after 6 h of filtration due to protein exchange and displacement phenomena in deposition layer caused by the differences in structure and diffusivity of different components. The protein exchange and replacement in the deposition layer was also observed on partial retentive membrane using a sequential fouling procedure. Fouling distribution along the membrane channel with spacer inserted in the module was more uniform and the flux was higher than that without spacer despite higher protein deposition observed in some cases.

Hard red spring wheat (Triticum aestivum cv Butte86) was grown under controlled environmental conditions and grain produced under 24/17 °C, 37/17 °C or 37/28 °C day/night regimens with or without post-anthesis N supplied as NPK. Flour proteins were analyzed and quantified by differential fractionation and RP-HPLC, and endosperm proteins were assessed by two-dimensional gel electrophoresis (2-DE). High temperature or NPK during grain fill increased protein percentage and altered the proportions of S-rich and S-poor proteins. Addition of NPK increased protein accumulation per grain under the 24/17 °C but not the 37/28 °C regimen. However, flour protein composition was similar for grain produced with NPK at 24/17 °C or 37/28 °C. 2-DE of gluten proteins during grain development revealed that NPK or high temperature increased the accumulation rate for S-poor proteins more than for S-rich proteins. Flour S content did not indicate S-deficiency, however, and addition of post-anthesis S had no effect on protein composition. Although, high-protein flour from grain produced under the 37/28 °C regimen with or without NPK had loaf volumes comparable to flour produced at 24/17 °C with NPK, mixing tolerance was decreased by the high temperature regimen.

The varied and exotic shapes of new nanoscale organic and inorganic building blocks provide new opportunities to engineer materials possessing specific functionality and physical properties dictated by the unique packings of these particles. We briefly review some of the current strategies for inducing the self-assembly of these building blocks focusing on one strategy in particular—the attachment of tethers to the building blocks at precise locations to create tethered nanoparticle “shape amphiphiles”. We use computer simulation to demonstrate that the resulting anisotropy imparted to nanocrystals or nanocolloids by the tethers can be used to encode simple design rules into the building blocks that ultimately result in a unique self-assembled structure. We present a general classification scheme for tethered nanoparticles wherein the anisotropy of a shape amphiphile is described by a vector comprised of one or more axes each describing a measure of anisotropy.
